OBJECTIVE To investigate the effect and mechanism of ginkgo flavone on androgenetic alopecia (AGA) in mice. METHODS AGA model of mice was established by applying 1 mg·mL-1 (mass concentration) testosterone solution after hair removal in blank group, model group, positive drug group and drug administration group. The positive drug group was coated with 50 mg·mL-1 (mass concentration) minoxidil, and the administration group was coated with 5%, 10% and 20% (mass fraction) ginkgo flavone solution, respectively. The hair growth rates of mice in the hair removal area on day 28 were observed. The hair qualities and numbers of hair follicles were observed by scanning electron microscopy (SEM) and HE staining. The levels of vascular endothelial growth factor-A(VEGF-A) and transforming growth factor-β(TGF-β) in skin were detected by immunohistochemistry(IHC) and Western blot(WB), and the levels of testosterone, dihydrotestosterone, estrogen, and ROS in the skin tissue were detected by ELISA. RESULTS Compared with the model group, ginkgo flavone could significantly accelerate the growth rate of hair in AGA model mice, increase the number of hair follicles, and increase the length, diameter and scale size of regenerated hair. The mechanism study found that ginkgo flavone could increase the level of VEGF-A in the skin of mice, reduce the level of hair growth inhibitor TGF-β, and had a certain regulatory effect on the level of sex hormones in the skin of mice, while the model under this experimental condition may not be related to oxidative stress. CONCLUSION Ginkgo flavone can promote hair regeneration and improve the quality of regenerated hair in AGA model mice, and its mechanism may be related to promoting hair follicle regeneration by reducing the level of androgens in the skin, decreasing the expression of TGF-β, and increasing the expression of VEGF-A.

目的 探索银杏黄酮对小鼠雄激素性脱发(androgenetic alopecia, AGA)的作用和机制。方法 设立空白组、模型组、阳性药组及给药组,通过脱毛后涂抹质量浓度1 mg·mL-1睾酮诱导建立小鼠AGA模型,阳性药组外涂质量浓度50 mg·mL-1米诺地尔,给药组分别外涂质量分数5%、10%、20%银杏黄酮,观察28 d内小鼠脱毛区域的毛发生长速度,并通过扫描电子显微镜(scanning electron microscope,SEM)、皮肤组织苏木精-伊红(hematoxylin-eosin,HE)染色观察小鼠毛发质量及毛囊数量,通过免疫组织化学染色(immunohistochemistry,IHC)、蛋白质免疫印迹法(Western blot,WB)检测皮肤中血管内皮生长因子-A(vascular endothelial growth factor-A,VEGF-A)、转化生长因子-β(transforming growth factor-β,TGF-β)水平,酶联免疫吸附实验(enzyme linked immunosorbent assay,ELISA)检测皮肤睾酮、二氢睾酮、雌激素及活性氧(reactive oxygen species,ROS)水平。结果 与模型组相比,银杏黄酮能显著加快AGA模型小鼠毛发新生速度,增加毛囊数量,同时增加新生毛发的长度、直径和毛鳞片大小。机制研究发现,银杏黄酮能够提高小鼠皮肤中的血管生成因子VEGF-A水平、降低毛发生长的抑制因子TGF-β水平,且对小鼠皮肤中的性激素水平有一定的调节作用,但本实验条件下的模型可能与氧化应激无关。结论 银杏黄酮外涂能促进AGA模型小鼠的毛发新生并提高新生毛发质量,其作用机制可能与降低皮肤中的雄激素水平,降低TGF-β表达并增加VEGF-A表达,从而促进皮肤毛囊再生有关。

目的 探索银杏黄酮对小鼠雄激素性脱发(androgenetic alopecia, AGA)的作用和机制。方法 设立空白组、模型组、阳性药组及给药组,通过脱毛后涂抹质量浓度1 mg·mL-1睾酮诱导建立小鼠AGA模型,阳性药组外涂质量浓度50 mg·mL-1米诺地尔,给药组分别外涂质量分数5%、10%、20%银杏黄酮,观察28 d内小鼠脱毛区域的毛发生长速度,并通过扫描电子显微镜(scanning electron microscope,SEM)、皮肤组织苏木精-伊红(hematoxylin-eosin,HE)染色观察小鼠毛发质量及毛囊数量,通过免疫组织化学染色(immunohistochemistry,IHC)、蛋白质免疫印迹法(Western blot,WB)检测皮肤中血管内皮生长因子-A(vascular endothelial growth factor-A,VEGF-A)、转化生长因子-β(transforming growth factor-β,TGF-β)水平,酶联免疫吸附实验(enzyme linked immunosorbent assay,ELISA)检测皮肤睾酮、二氢睾酮、雌激素及活性氧(reactive oxygen species,ROS)水平。结果 与模型组相比,银杏黄酮能显著加快AGA模型小鼠毛发新生速度,增加毛囊数量,同时增加新生毛发的长度、直径和毛鳞片大小。机制研究发现,银杏黄酮能够提高小鼠皮肤中的血管生成因子VEGF-A水平、降低毛发生长的抑制因子TGF-β水平,且对小鼠皮肤中的性激素水平有一定的调节作用,但本实验条件下的模型可能与氧化应激无关。结论 银杏黄酮外涂能促进AGA模型小鼠的毛发新生并提高新生毛发质量,其作用机制可能与降低皮肤中的雄激素水平,降低TGF-β表达并增加VEGF-A表达,从而促进皮肤毛囊再生有关。

OBJECTIVE To investigate the effect and mechanism of ginkgo flavone on androgenetic alopecia (AGA) in mice. METHODS AGA model of mice was established by applying 1 mg·mL-1 (mass concentration) testosterone solution after hair removal in blank group, model group, positive drug group and drug administration group. The positive drug group was coated with 50 mg·mL-1 (mass concentration) minoxidil, and the administration group was coated with 5%, 10% and 20% (mass fraction) ginkgo flavone solution, respectively. The hair growth rates of mice in the hair removal area on day 28 were observed. The hair qualities and numbers of hair follicles were observed by scanning electron microscopy (SEM) and HE staining. The levels of vascular endothelial growth factor-A(VEGF-A) and transforming growth factor-β(TGF-β) in skin were detected by immunohistochemistry(IHC) and Western blot(WB), and the levels of testosterone, dihydrotestosterone, estrogen, and ROS in the skin tissue were detected by ELISA. RESULTS Compared with the model group, ginkgo flavone could significantly accelerate the growth rate of hair in AGA model mice, increase the number of hair follicles, and increase the length, diameter and scale size of regenerated hair. The mechanism study found that ginkgo flavone could increase the level of VEGF-A in the skin of mice, reduce the level of hair growth inhibitor TGF-β, and had a certain regulatory effect on the level of sex hormones in the skin of mice, while the model under this experimental condition may not be related to oxidative stress. CONCLUSION Ginkgo flavone can promote hair regeneration and improve the quality of regenerated hair in AGA model mice, and its mechanism may be related to promoting hair follicle regeneration by reducing the level of androgens in the skin, decreasing the expression of TGF-β, and increasing the expression of VEGF-A.

雄激素性脱发(androgenetic alopecia,AGA)是一种最常见的脱发类型,是起始于青春期或青春后期的一种进行性毛囊微小化的脱发疾病[1]。《中国人雄激素性脱发诊疗指南-2019》中显示,我国男性患病率达到21.3%,女性患病率约为6.0%,且近年来,AGA存在年轻化、女性占比提升的趋势[2]。在目前的医疗方法中,手术价格高昂,口服非那雄胺片并不适宜于女性,米诺地尔临床有效率仅有30%~50%,常出现脱发期,引发头皮瘙痒、脱屑甚至脂溢性皮炎等问题[3-4]。由于AGA是一个进行性加重直至秃发的过程,因此应强调早期治疗和长期治疗的重要性,急需开发安全有效的防脱生发新药物。
中医学认为,“发为血之余”,压力过大、七情内伤导致气滞血瘀,则毛发失养,引起脱发,如清·王清任《医林改错》载“…皮里肉外血瘀,阻塞血路,新血不能养发,故发脱落。无病脱发,亦是血瘀”;清·唐宗海《血证论》载“瘀血在上焦,或发脱不生”均提出血瘀致脱发的观点。而银杏叶归心经、肺经,具有活血化瘀、通络止痛之功。现代研究已发现银杏提取物有黄酮、萜、多糖等多类化合物,其中银杏黄酮具有抗氧化、抗炎、增加局部血流量、舒张血管平滑肌、镇痛等多种药理作用[5-6]。故推测银杏黄酮对AGA可能有一定的防治作用。本研究建立小鼠AGA模型以研究银杏黄酮的防脱生发药效,并进一步探索其可能的药理作用机制。

参照文献[7]并加以改进,建立涂抹睾酮诱导的AGA模型。将小鼠腹腔注射舒泰50进行麻醉后用剃毛器剃除小鼠背脊线两侧略大于1 cm×2 cm矩形区域毛发,再将自制定位1 cm×2 cm的硅胶片放在剃毛区域,用脱毛膏均匀涂满硅胶片空格,停留3.5 min后擦去脱毛膏,并洗净残留的脱毛膏,放回笼内饲养。于脱毛后1 d观察小鼠背部脱毛区域皮肤情况,剔除皮肤有大块黑色素或皮肤破损以至于严重影响后续药效评价的小鼠,随机分为空白组、模型组、米诺地尔组(阳性对照)、质量分数分别为5%、10%、20%的银杏黄酮组,每组6只。每天麻醉小鼠后,空白组于脱毛区域皮肤按0.1 mL·cm-2涂抹体积分数50%乙醇溶液,每天1次,连续28 d;其余各组小鼠均涂抹质量浓度1 mg·mL-1睾酮溶液,每日1次,连续28 d。每天造模30 min后,空白组和模型组小鼠于脱毛区域用棉签外涂基质溶液,各药物组外涂对应药物溶液,给药体积均为每只0.2 mL。
①质量浓度1 mg·mL-1睾酮溶液:称取睾酮粉末,加入体积分数50%乙醇溶液充分溶解混匀,终浓度为1 mg·mL-1;②基质溶液:称取体积分数95%乙醇、丙二醇和纯水按3∶6∶2质量比充分混匀;③质量分数5%、10%、20%银杏黄酮:称取适量银杏叶提取物粉末,分别按1∶19、1∶9、1∶4质量比加入基质溶液溶解混匀。
观察小鼠皮肤颜色变化和毛发生长情况,于第0天、第7天、第14天、第21天、第28天麻醉后拍摄照片,参考文献评分标准[8]并结合实际情况,对每只小鼠的毛发新生情况进行评分,评分标准为:1分表示皮肤粉红色,为刚脱毛时的状态,红色较明显,此时毛囊处于静止期;2分表示皮肤粉白色,红色不明显,但尚未见灰色,此时毛囊开始进入生长早期;3分表示皮肤白色,有明显灰色出现,但尚未见黑色,此时毛囊处于生长早期;4分表示皮肤灰色,且局部变黑,偶见短毛,此时毛囊处于生长中期;5分表示皮肤黑灰色,且覆盖短毛,此时毛囊处于生长中后期;6分表示皮肤被黑色长毛覆盖,此时毛囊处于生长后期。按照以上评分标准,由3个人分别对小鼠毛发生长情况评分后取平均值作为其最终评分。第28天,用Image J测量小鼠脱毛区域的毛发覆盖面积和脱毛区域面积,按公式1计算。

第28天处死小鼠,用镊子随机拔取3处脱毛区域的新生毛发(毛发数量>20根),在白纸上铺开后选择最长的20根,分别用游标卡尺测量后记录小鼠的新生毛发长度。
随机选取脱毛区域的新生毛发,进行扫描电子显微镜(scanning electron microscope, SEM)拍摄,用Image J测量图片中的毛发中端直径以评价毛发粗细,测量毛发中端的毛鳞片长度以评价毛鳞片大小,综合评价小鼠新生毛发的质量。
小鼠处死后剪下实验区域皮肤,置于4%多聚甲醛组织固定液中,固定至少24 h,脱水后进行石蜡包埋、切片、脱水、苏木精-伊红染色、封片,在光学显微镜下观察并记录结果。使用Image J的计数功能,对各组HE染色图片中的毛囊数量进行计数。
皮肤组织蜡块切片后脱蜡、脱水,蒸馏水冲洗确保去除有机溶剂。抗原修复完成后将血清加到的切片中,封闭30 min。切片再与VEGF-A一抗稀释液(1∶200)共孵育过夜,再次用一抗相应的二抗标记组织,二氨基联苯胺(DAB)显色,苏木素复染。脱水后滴加中性树胶封片,晾干,进行图片拍摄。使用Image J计算各组免疫组化染色图片中的VEGF-A阳性面积,并以空白组为基准做按公式2归一化处理。

①样本制备:小鼠皮肤称重,按1∶9的质量体积比加入裂解液,将皮肤组织剪至小于2 mm2的碎片,使用冷冻研磨仪(4 ℃,90 s,65 Hz)充分研磨制成匀浆液,在4 ℃下离心(12 000 r·min-1,30 min),小心避开上层油脂,收集上清液,进行总蛋白的提取,采用BCA定量试剂盒进行总蛋白定量,98 ℃加热5~8 min变性。②Western blot:配制(10~12)%分离胶和5%浓缩胶,进行电泳、全湿转膜,5%脱脂奶粉室温封闭2 h,4 ℃摇床孵育一抗过夜后,室温孵育二抗1 h,曝光显影。采用Image J分析条带的灰度值,按公式3计算。
各组目的蛋白相对表达=各组目的蛋白相对表达量/空白组平均相对表达量
小鼠皮肤称重,按1∶9的质量体积比加入PBS溶液,同“1.5.8”项制备样本,参照ELISA试剂盒的说明测定上清液的睾酮、二氢睾酮、雌二醇和ROS的含量。
所有数据均采用Graphpad Prism软件中的One-way ANOVA进行统计分析,实验结果以Mean±SEM表示,P<0.05表示有显著性差异。
小鼠脱毛区域的毛发生长情况见图1A,对其进行评分后统计结果见图1B,从第14天起,模型组小鼠毛发生长速度与空白组相比减慢,有显著性差异;与模型组相比,阳性药米诺地尔和质量分数5%、10%、20%的银杏黄酮均能加快小鼠的毛发生长速度,其中第28天时质量分数20%银杏黄酮组具有显著性差异,由于小鼠毛发生长的个体差异较大,其他组别差异无统计学意义。文献中也常用毛发覆盖率评价小鼠毛发新生情况[7],本实验统计第28天的小鼠毛发覆盖率结果见图1C,与空白组相比,模型组小鼠毛发覆盖率显著降低,而米诺地尔与质量分数20%银杏黄酮组小鼠毛发覆盖率与模型组相比显著升高,质量分数5%、10%银杏黄酮组明显升高但未见显著性,银杏黄酮的促生发效果呈现一定的剂量相关性。
排除毛发无再生的小鼠,将小鼠的脱毛区域新生毛发长度进行测量统计,见图2,模型组小鼠新生毛发长度与空白组相比明显减少,而各给药组毛发长度与模型组相比均有增加,且显示出一定的剂量相关性。
使用SEM拍摄小鼠的毛发可以直观的观察到毛发直径和毛鳞片的改变,评价新生毛发的质量,新生毛发粗且毛鳞片大说明毛发质量较好[7]。在本实验中,空白组与模型组毛发直径统计未见明显变化,模型组毛鳞片略小于空白组,但无显著性差异;米诺地尔组毛鳞片略有增大但无显著性,质量分数5%、10%、20%银杏黄酮组毛发直径有所增加,毛鳞片显著增大,见图3
脱毛后第28天,模型组小鼠皮肤毛囊数量与空白组相比显著减少;与模型组相比,米诺地尔组和质量分数10%、20%银杏黄酮组皮肤毛囊数量显著增加,质量分数5%银杏黄酮组毛囊数量亦有所增加,但未见显著性差异,见图4。同时,从图4A中可以明显看出,空白组和各给药组皮肤中多数毛囊可见毛球部膨大、黑色素生成,生长深入真皮层,提示已进入生长期,而模型组多数毛囊仍位于皮肤表层,处于静止期,提示银杏黄酮可以促进毛囊从静止期向生长期转变,促进毛囊再生,增加毛囊数量,从而促进毛发新生。
与空白组相比,模型组小鼠皮肤中的睾酮、二氢睾酮、雌二醇含量均显著显著升高,米诺地尔和质量分数5%、10%、20%银杏黄酮均能降低小鼠皮肤中的性激素水平,米诺地尔组和质量分数20%银杏黄酮组DHT含量与模型组相比有显著性差异,但由于个体差异较大,其余指标均未见统计学意义。空白组与模型组小鼠皮肤中的ROS含量无差异,见图5
VEGF-A免疫组化染色结果见图6A-B,与空白组相比,模型组小鼠皮肤中的VEGF-A表达显著降低,与模型组相比,米诺地尔组以及质量分数10%、20%银杏黄酮组VEGF-A表达显著增加,质量分数5%银杏黄酮组也具有增加VEGF表达的趋势。
采用Western blot检测小鼠皮肤中的VEGF-A和TGF-β表达结果见图6C,与空白组相比,模型组小鼠皮肤组织中TGF-β表达显著升高,VEGF-A表达显著降低,与模型组相比,米诺地尔组、质量分数10%、20%银杏黄酮组TGF-β表达显著降低,米诺地尔组和质量分数10%银杏黄酮组VEGF-A表达显著升高,与免疫组化结果趋势相同,质量分数5%和20%银杏黄酮组VEGF-A表达也有所增加,但未见统计学意义。
AGA模型小鼠毛发新生减慢,且新生毛发毛干较短、毛发质量较差[8],与临床AGA患者毛发生长速度缓慢,且毛发长度、直径均小于健康人群的临床症状[9]相似。本研究通过建立局部涂抹睾酮溶液诱导的AGA小鼠模型,较为系统地研究了银杏黄酮外涂的防脱生发药效,并进一步探索其可能的药理作用机制。实验结果显示,银杏黄酮能够显著地提高毛发生长速度,增加毛发覆盖率,提高新生毛发的长度和质量,多方位改善AGA症状。AGA的发病机制非常复杂,目前还未明确,主要认为与基因、激素代谢失调、毛囊周围微环境紊乱等因素有关[10]。在T代谢物DHT的影响下,毛囊发生小型化,毛干缩小,处于静止期的毛囊百分比增加,甚至整个毛囊消失,导致发量减少[11],毛发形态改变,毛干变短。实验结果显示,银杏黄酮能够恢复小鼠皮肤毛囊数量和生长期毛囊比例,且能降低小鼠皮肤中的雄激素水平,尤其是DHT的水平,故银杏黄酮的促毛发再生作用可能与其降低雄激素水平,从而诱导毛囊进入生长期,改善毛囊小型化有关。另外,氧化应激是造成毛囊周围微环境紊乱的主要因素之一,可通过多种来源的ROS诱导发生,包括广泛的细胞代谢(如黑色素生成)、脱毛引起的刺激、毛发周期变化、激素、环境应激源,过量ROS的积累会诱导毛囊细胞的凋亡,并通过调控雄激素等多条信号通路抑制毛囊由静止期进入生长期[12]。但实验结果显示,模型组小鼠皮肤中的ROS含量与空白组无差异,提示本实验条件下的AGA模型可能与氧化应激损伤无关。中药提取物成分复杂,其发挥药效常具有多靶点的特征,银杏黄酮具有明确的抗炎、抗氧化应激作用,但本次实验条件不能模拟临床AGA患者头皮的氧化应激损伤状态,无法探讨银杏黄酮的防脱生发效果是否与其抗氧化应激作用有关,后续可另作研究探讨。
毛发生长的营养需要毛囊周围毛细血管的供给,而VEGF在毛囊周围毛细血管的形成中发挥重要作用。VEGF是一种高度特异性的促血管内皮细胞生长因子,具有促进血管形成,增加血管通透性,促进细胞外基质变性、血管内皮细胞迁移、增殖等作用[13-14],VEGF在毛囊外根鞘角质形成细胞中过表达,可以诱导毛囊周围血管形成,导致脱毛后毛发加速再生、毛囊和毛干尺寸增加、毛发密度增加[15],其中VEGF-A是一种主要的亚型。本实验结果显示,银杏黄酮提取物能够促进AGA模型小鼠皮肤中VEGF-A的表达,且VEGF主要分布在毛囊外根鞘、皮脂腺和表皮细胞。此外,据文献报道,在AGA患者的头皮中,雄激素会刺激真皮乳头细胞中的TGF-β产生和分泌[16],TGF-β是一种受DHT诱导的毛发生长抑制因子,会诱导AGA患者头皮毛囊微血管内皮细胞的凋亡,导致血管退化[17],诱导毛囊纤维化反应,抑制毛发从休止期进入生长期,是雄激素性脱发发病机制中的关键因素之一[18-20]。本实验结果显示,银杏黄酮能显著降低AGA小鼠皮肤中的TGF-β表达,且具有一定的剂量相关性。以上结果提示,银杏黄酮可能一方面通过促进皮肤中的VEGF表达,促进毛囊周围血管形成,另一方面通过降低TGF-β表达,抑制毛囊纤维化和微血管退化,使毛囊获得足够的营养,从而加快毛囊进入生长期,促进毛发生长。
综上所述,本研究发现银杏黄酮外涂具有较好的防脱生发药效,能促进AGA模型小鼠的毛发再生并改善新生毛发的质量,具有一定的剂量相关性,其机制可能是通过降低皮肤中的雄激素水平,降低TGF-β表达同时增加VEGF-A表达,从而促进皮肤毛囊再生,促进毛发新生。本研究初步确认了银杏黄酮对AGA的改善作用,并多方面探讨了其可能的作用机制,挖掘了其作为防脱生发药物的潜力,为银杏叶提取物在AGA的临床应用提供了参考。
