OBJECTIVE To establish a high performance liquid chromatography (HPLC) method for the detection of related substances in somatostatin for injection. METHODS The determination was performed on a C18 column with mobile phases consisting of potassium dihydrogen phosphate solution and acetonitrile by gradient elution. The detection wavelength was set at 215 nm and the column temperature was maintained at 50 ℃. RESULTS The established method can effectively separate the main components and seven major degradation impurities. The peak area of somatostatin and 7 kinds of impurities had good linear relationships with the concentration (r≥0.999 1, n=8). Relative standard deviation (RSD) of precision, stability and repeatability tests of 7 kinds of impurities were all lower than 3.6%. The average recoveries of 7 kinds of impurities ranged from 100.0% to 104.4% (n=9). The impurity content determined by the established method is higher than that determined by the European Pharmacopoeia(EP) and Chinese Pharmacopoeia(Ch.P). CONCLUSION The method is simple, accurate and stable. It can control the impurities in somatostatin for injection and provide a basis for improving the standard of this drug.

目的 建立注射用生长抑素中有关物质的高效液相色谱检测方法。方法 色谱柱为C18柱;以磷酸二氢钾溶液-乙腈为流动相,梯度洗脱。检测波长为215 nm,柱温为50 ℃。结果 本研究新建方法能够将主成分以及7种主要降解杂质有效分离,生长抑素和7种杂质峰面积与浓度线性关系良好(r≥0.999 1,n=8);7种杂质精密度、稳定性、重复性试验的相对标准偏差(RSD)均低于3.6%;平均回收率在100.0%~104.4%之间(n=9)。应用本研究新建方法测得样品中的杂质含量高于《欧洲药典》和《中国药典》方法所测得结果。结论 该方法操作简便,结果准确,重复性好,可对注射用生长抑素杂质进行质量控制,并为该品种的标准修订提供方法依据。

OBJECTIVE To establish a high performance liquid chromatography (HPLC) method for the detection of related substances in somatostatin for injection. METHODS The determination was performed on a C18 column with mobile phases consisting of potassium dihydrogen phosphate solution and acetonitrile by gradient elution. The detection wavelength was set at 215 nm and the column temperature was maintained at 50 ℃. RESULTS The established method can effectively separate the main components and seven major degradation impurities. The peak area of somatostatin and 7 kinds of impurities had good linear relationships with the concentration (r≥0.999 1, n=8). Relative standard deviation (RSD) of precision, stability and repeatability tests of 7 kinds of impurities were all lower than 3.6%. The average recoveries of 7 kinds of impurities ranged from 100.0% to 104.4% (n=9). The impurity content determined by the established method is higher than that determined by the European Pharmacopoeia(EP) and Chinese Pharmacopoeia(Ch.P). CONCLUSION The method is simple, accurate and stable. It can control the impurities in somatostatin for injection and provide a basis for improving the standard of this drug.

生长抑素为人工合成的环状十四个氨基酸肽,临床主要用于治疗严重急性食管静脉曲张出血、严重急性胃或十二指肠溃疡出血、并发性急性糜烂性胃炎、出血性胃炎以及胰、胆和肠瘘的辅助治疗、胰腺术后并发症的预防和治疗、糖尿病酮症酸中毒的辅助治疗等[1-3]。注射用生长抑素由默克雪兰诺原研,商品名为思他宁(Stilamin),在加拿大、法国、韩国等多个国家上市。
目前,生长抑素原料收载于《中国药典》(Ch.P)[4]和《欧洲药典》(EP)[5],其冻干粉针仅Ch.P有收载。Ch.P在2010年版中首次收载注射用生长抑素,至今未进行过修订,有关物质采用磷酸缓冲液-乙腈为流动相进行梯度洗脱分析,总杂要求不得超过2.0%。Ch.P 2020色谱条件对生长抑素有关物质不能进行很好的分离,Zhang等[6]Ch.P 2020的方法洗脱条件进行了优化,改善了分离效果。EP生长抑素总杂限度与Ch.P一致,采用三氟乙酸溶液-乙腈为流动相进行梯度洗脱,方法灵敏度低于Ch.P[6] 。注射用生长抑素为2023年国家药品抽检品种,本次抽检共涉及12个企业102批样品,其中59批样品执行Ch.P 2020标准,43批样品执行企业注册标准,企业注册标准有关物质检测方法各异,方法的差异导致结果无法反映产品本身存在的质量差异。通过调研生产企业,结合对品种的特性分析,锁定了7种贮存过程中可能产生或增加的杂质,杂质信息见图1。本研究建立了统一的方法对注射用生长抑素中这7种杂质进行分离,并对其他检测到的未知单杂和总杂进行分析,以期该品种的质量标准修订提供依据。
SHIMADZU LC-20AD型高效液相色谱仪(日本岛津公司);METTLER AG245型电子天平(瑞士梅特勒-托利多公司);Sartorius PB-10型酸度计(德国赛多利斯公司);Milli-Q超纯水仪(美国密理博公司) ;Nano ChromCore AR 300 C18色谱柱(4.6 mm×250 mm,5 μm,苏州纳普公司)。
注射用生长抑素来自国内外12家企业;生长抑素对照品(中国食品药品检定研究院,批号:140711-202006,含量:86.5%);杂质A对照品(批号:C1-003a/D2201F01,含量:87.0%)、杂质C对照品(批号:C1-003g/D2201F01,含量:90.2%)、杂质D对照品(批号:C1-003b/D2201F01,含量:89.0%)、杂质E对照品(批号:C1-003e/D2201F01,含量:90.5%)、杂质F对照品(批号:C1-003f/D2201F01,含量:88.8%)、杂质G对照品(批号:C1-003o/D2201F01,含量:85.2%)(成都圣诺生物制药公司);杂质B对照品(批号:SIM-1-20191101-1,含量:86.0%,海南中和药业有限公司);甘露醇、冰乙酸、磷酸二氢钾、磷酸、氢氧化钠、盐酸、过氧化氢均为分析纯;乙腈为色谱纯。
采用Nano ChromCore AR 300 C18色谱柱(4.6 mm×250 mm,5 μm),以0.02 mol·L-1的磷酸二氢钾溶液(用磷酸调节pH值至3.5)为流动相A,以乙腈为流动相B,按表1进行梯度洗脱;流速为1.5 mL·min-1,进样量为20 μL,检测波长为215 nm,柱温为50 ℃。
系统适用性溶液:精密称取杂质A~G与生长抑素对照品各适量,加水溶解并稀释制成每1 mL中含各杂质5 μg与生长抑素1 mg的混合溶液,摇匀。
对照品溶液:精密称取杂质A~G和生长抑素对照品各适量,分别用水溶解并稀释制成1 mg·mL-1的溶液,摇匀,作为各对照品储备液。取对照品储备液1 mL,置200 mL量瓶中,加水稀释至刻度,即得对照品溶液。
供试品溶液:取注射用生长抑素1瓶加水溶解并稀释制成约含生长抑素1 mg·mL-1的溶液。
分别参照Ch.P 2020和EP 11.0各论生长抑素有关物质方法测定“2.2”项下系统适用性溶液,Ch.P 2020和EP 11.0色谱条件下杂质B和主成分形成共出峰,且杂质D、E峰和杂质G峰重合。本研究新建的方法能将主峰和7个杂质完全分离。见图2
辅料干扰试验:按处方比例取甘露醇、乙酸适量,按“2.2”项下供试品溶液浓度制成空白辅料溶液。按“2.1”项下色谱条件进样分析,记录色谱图。结果显示,辅料无干扰,结果见图3
破坏性试验:取1支注射用生长抑素,加3 mL水,混匀,作为未破坏样品溶液。另取同批次样品,按以下条件进行破坏。①酸破坏:取2 mol·L-1盐酸溶液1 mL加入样品中,放置1 h,然后加2 mol·L-1氢氧化钠溶液1 mL,再加入1 mL水,即得酸破坏溶液;②碱破坏:样品加入2 mL水溶解,再加0.05 mol·L-1氢氧化钠溶液0.5 mL,放置15 min后,加0.05 mol·L-1盐酸溶液0.5 mL,即得碱破坏溶液;③高温破坏:取样品1支,置于80 ℃烘箱24 h,加水3 mL溶解,即得高温破坏溶液;④光破坏:取样品1支,置光照箱3 500 lx下,放置24 h,加水3 mL溶解,滤过,即得光破坏溶液;⑤氧化破坏溶液:取样品1支,加30%过氧化氢1 mL,放置1 h 后,加水2 mL,滤过,即得氧化破坏溶液;同法处理各条件下空白辅料溶液。结果表明,各条件下辅料溶液均无干扰,在不同破坏条件下,生长抑素与降解产物峰均能有效分离。其中,酸破坏、碱破坏,高温破坏、氧化破坏条件下杂质明显增多,尤其在碱破坏后主峰急剧减小。但在光照破坏条件下主峰面积与未破坏样品溶液主峰面积相差较小,具有较好的稳定性。结果见图3,图中空白辅料溶液为未破坏的辅料溶液。
精密量取“2.2”项下对照品储备液,稀释制成系列对照品溶液。按“2.1”项下色谱条件进样分析,记录峰面积。以生长抑素和各杂质质量浓度为横坐标(x),峰面积为纵坐标(y)进行线性回归,生长抑素及各杂质的浓度线性范围见表2
精密量取“2.2”项下对照品储备液,加水逐级稀释,按“2.1”项下色谱条件进样分析。以信噪比为3:1计算检出限,信噪比10:1计算定量限。生长抑素和杂质A~G的检出限及定量限见表2
精密量取“2.2”项下对照品溶液,按“2.1”项下色谱条件连续进样 6 次,记录峰面积,生长抑素和各杂质峰面积的相对标准偏差(RSD)在0.2%~0.4%之间,表明仪器精密度良好。
取样品(批号:04221001)6支,按“2.2”项下供试品溶液制备方法平行制备 6 份,按“2.1”项下色谱条件进样分析,记录峰面积,并计算各杂质的含量。结果杂质B、C、G未检出,已知杂质A含量为0.02%,RSD为2.0%;杂质D含量为0.04%,RSD为3.6%;杂质E含量为 0.02%,RSD为2.9%;杂质F含量为0.01%,RSD为3.6%。
取样品(批号:04221001),分别加入对照品储备液,加水稀释,配制成低、中、高 3 个浓度水平的加标供试品溶液,每个浓度水平平行配制 3 份,按“2.1”项下色谱条件进样分析,计算回收率,结果见表3
取注射用生长抑素样品(批号:04221001),按供试品溶液制备方法制备样品,10 ℃下放置21 h,分别测定0、9、12、15、18、21 h各已知杂质及未知杂质的含量,各已知杂质及未知杂质含量的RSD均小于3.6%(n=6),提示本品在21 h内溶液较为稳定。
尝试单独改变流速(±0.1 mL·min-1)和柱温(±5 ℃),更换色谱仪品牌使用安捷伦1260型高效液相色谱仪,系统适用性溶液7个杂质峰与主峰均能完全分离,测定3批样品,总杂结果变化均在0.06%内,说明方法耐用性好。使用月旭Ultimate XB C18柱、安捷伦 Zorbax C18柱和菲罗门Titank C18柱,系统适用性溶液杂质A和主峰不能分离,杂质D、E、F分离度低于1.5。色谱柱对杂质的分离效果影响大,需使用指定型号的色谱柱。
分别参照Ch.P 2020和EP 11.0各论生长抑素有关物质项、文献[6]的方法及本研究新建方法测定样品,本研究新建方法测得的有关物质明显高于EP 11.0和Ch.P 2020方法,与文献[6]方法测得结果相近。具体结果比对见表4
取注射用生长抑素样品,按“2.2”项下供试品溶液制备,按“2.1”项下色谱条件进样分析,测定结果见图4表5。由图4可知,本研究新建方法能较好地分离检测生长抑素的有关物质,大部分企业的主要杂质为杂质D、E、F。企业8的5个抽样来自不同省市,其中两个抽样为同一批号,总杂结果已达到Ch.P规定的上限,样品杂质含量均较高。企业1~5样品原料均购自同一原料生产厂商,企业6和7原料购自另一原料生产厂商,原料来源相同的样品杂质谱峰也有所不同,说明制剂的生产工艺等对杂质也有重要影响。
合成多肽药物的杂质谱一般较为复杂,包含由起始物料引入的杂质、工艺相关的杂质和降解产生的杂质等,这使得多肽药物的杂质分析具有一定困难。采用Ch.P 2020和EP 11.0生长抑素各论有关物质方法,杂质分离效果不好,测得结果远低于杂质真实水平。Zhang等[6]Ch.P 2020方法进行了优化,优化后杂质分离效果变好。使用相同品牌型号的色谱柱,分别采用本研究新建方法与Zhang等[6]建立的方法,测得5批加速稳定性试验样品杂质含量接近,结果见表5,测得色谱图见图5。文献[6]报道的方法杂质出峰时间集中在主峰附近,随着杂质含量增大,部分杂质峰未能与主成分分离,可能导致检出结果偏小。课题组新建方法杂质峰较为分散,已知杂质和未知杂质分离较好,有利于杂质的检出和计算。
本课题组前期使用六氟磷酸钾缓冲溶液-乙腈和硫酸盐缓冲液-乙腈色谱系统,尝试不同的流速、pH值和流动相比例等,发现系统适用性溶液不能分离。对EP 11.0的方法进行优化,改变三氟乙酸浓度和梯度,系统适用性溶液的分离效果不佳。将课题组新建方法的柱温升高至60 ℃,杂质A和主峰的分离度会下降至1.0以下,将柱温降低至40 ℃,杂质D和E分离度低于1.5。生长抑素与各杂质在200~220 nm之间均有较大吸收,且波长越小,色谱峰的响应值越大,但基线波动也会变大,未知杂质检出灵敏度会降低,综合考虑检测波长选择215 nm。
本研究考察比较了不同计算方法(杂质对照品法、自身对照法及面积归一化法)对有关物质结果的影响,发现杂质对照品法和自身对照法计算结果基本一致,峰面积归一化法计算结果偏小。除了二聚体,其余6个杂质校正因子在0.93~1.03之间。考虑到杂质对照品较难获得,选择自身对照法加权校正因子计算,可较为准确得到杂质含量。在所建方法的色谱图中,二聚体出峰时间较晚,企业2、8、10样品色谱图中二聚体峰后面还有杂质峰,用TSK柱测得这3个企业样品中多聚体含量也高于其他企业,推测二聚体后的峰可能为多聚体峰。二聚体的校正因子为2.6,多聚体校正因子可能也较大,如样品中多聚体杂质较多,用自身对照法计算误差则可能较大,因此需对聚合物杂质进行进一步研究。
生长抑素的两个半胱氨酸由二硫键连接成环,根据文献[7]报道,目前国内有企业采用一定浓度的过氧化氢进行氧化环合,可能产生新的氧化杂质。课题组调研了本次抽检涉及的企业,企业9和11使用的原料采用了过氧化氢环合工艺。破坏性实验显示过氧化氢破坏后产生的大部分杂质在主峰前出峰,这两家企业的样品在相同位置杂质峰很小,与其他企业无明显差异,说明此步骤工艺控制较好。将样品置于25 ℃下放置3个月,各杂质变化不明显。将样品置于40 ℃下放置3个月,分别测定0、1、2、3个月杂质情况,杂质D、E、F均明显增加,为制剂贮存过程主要降解杂质,见图6。杂质A、B、C、G均有增加,杂质D峰前的杂质增加也较为明显,2、3个月取样点的杂质G峰后杂质峰明显增多,说明注射用生长抑素对贮存温度较为敏感。《化学合成多肽药物药学研究技术指导原则(试行)》[8]指出,根据多肽药物的稳定性特征和剂型特点,应重点研究制剂的降解杂质(如脱酰胺、氧化和乙酰化杂质等);为降低不良反应发生率,还应对聚合物进行针对性研究。杂质D、E、F为生长抑素乙酰化杂质,杂质B、C为脱酰胺杂质,杂质G为聚合物,均为应重点关注的杂质。
本研究建立了一种注射用生长抑素有关物质的测定方法,能准确、灵敏地测定样品中7种已知杂质。相较于Ch.PEP 11.0的方法,本方法能更全面地测定注射用生长抑素的有关物质,能更真实地反映产品中杂质的水平,后续可为注射用生长抑素的质量标准修订提供新方法,也可为多肽类药物有关物质方法开发提供参考。
