OBJECTIVE To evaluate the distribution and clearance of tumor infiltrating lymphocytes (TIL) in NOG mice. METHODS Eighty-four NOG mice were used, divided into control group and test group. The mice were given a single tail vein injection. After administration blood was collected and organs were taken at different time points, flow cytometry and real-time quantitative PCR (qPCR) were used to detect the distribution of cells in the blood and different organs. RESULTS After 1-2 d of administration, the number of CD3+, CD3+CD4+, and CD3+CD8+cells in the blood reached their peaks. At the same time, qPCR showed that TIL was mainly distributed in tissues such as lungs, blood, and liver, with less distribution in other tissues. On day 42 after administration, gene copies of most tissues were below the lower limit of quantitation. CONCLUSION Single tail vein injection of TIL in NOG mice is mainly distributed in the lungs, blood, and liver, and its duration in vivo does not exceed 42 d.

目的 评估肿瘤浸润淋巴细胞(tumor infiltrating lymphocyte,TIL)在NOG小鼠体内的分布及清除情况。方法 研究共使用84只NOG小鼠,分为溶媒对照组和受试物组,单次尾静脉注射给予小鼠TIL,在给药后不同时间点采血和解剖,取脏器,采用流式细胞术和实时定量聚合酶链反应(real-time quantitative PCR,qPCR)方法检测细胞在血液和不同脏器的分布情况。结果 给药后1~2 d,血液中CD3+、CD3+CD4+、CD3+CD8+细胞数量达到峰值,同时qPCR检测结果显示,TIL主要分布于肺、血液、肝脏等组织,其他组织分布较少,给药后42 d,大部分组织基因拷贝低于定量下限。结论 NOG小鼠单次尾静脉注射TIL,主要在肺脏、血液、肝脏中分布,并且在体内存续时间不超过42 d。

目的 评估肿瘤浸润淋巴细胞(tumor infiltrating lymphocyte,TIL)在NOG小鼠体内的分布及清除情况。方法 研究共使用84只NOG小鼠,分为溶媒对照组和受试物组,单次尾静脉注射给予小鼠TIL,在给药后不同时间点采血和解剖,取脏器,采用流式细胞术和实时定量聚合酶链反应(real-time quantitative PCR,qPCR)方法检测细胞在血液和不同脏器的分布情况。结果 给药后1~2 d,血液中CD3+、CD3+CD4+、CD3+CD8+细胞数量达到峰值,同时qPCR检测结果显示,TIL主要分布于肺、血液、肝脏等组织,其他组织分布较少,给药后42 d,大部分组织基因拷贝低于定量下限。结论 NOG小鼠单次尾静脉注射TIL,主要在肺脏、血液、肝脏中分布,并且在体内存续时间不超过42 d。

OBJECTIVE To evaluate the distribution and clearance of tumor infiltrating lymphocytes (TIL) in NOG mice. METHODS Eighty-four NOG mice were used, divided into control group and test group. The mice were given a single tail vein injection. After administration blood was collected and organs were taken at different time points, flow cytometry and real-time quantitative PCR (qPCR) were used to detect the distribution of cells in the blood and different organs. RESULTS After 1-2 d of administration, the number of CD3+, CD3+CD4+, and CD3+CD8+cells in the blood reached their peaks. At the same time, qPCR showed that TIL was mainly distributed in tissues such as lungs, blood, and liver, with less distribution in other tissues. On day 42 after administration, gene copies of most tissues were below the lower limit of quantitation. CONCLUSION Single tail vein injection of TIL in NOG mice is mainly distributed in the lungs, blood, and liver, and its duration in vivo does not exceed 42 d.

肿瘤在包含多种细胞的复杂组织微环境中发展,包括先天免疫细胞和适应性免疫细胞,肿瘤细胞和免疫系统的细胞之间存在动态的相互作用,有效地抗肿瘤免疫才能清除肿瘤。然而,在某些情况下,肿瘤细胞可能会逃避免疫监测,同时不受控制地增殖。过继细胞疗法(adoptive cellular therapy,ACT)可克服免疫系统对肿瘤的低反应性并增强抗肿瘤免疫,其中肿瘤浸润淋巴细胞为自然浸润实体肿瘤的异质性淋巴细胞群体,在黑色素瘤、乳腺癌等疾病中展现出较好的临床效果,是一种非常有前景的过继细胞疗法[1-2]。肿瘤微环境中存在免疫抑制代谢产物(如腺苷、乳酸)、细胞因子(转化生长因子-β)和各类细胞(如调节性T细胞、肿瘤相关巨噬细胞等),能够共同增强肿瘤抗原特异性CD4+和CD8+T细胞的功能。从新鲜肿瘤样本中分离出自然产生的肿瘤浸润淋巴细胞(tumor-infiltrating lymphocyte,TIL),并在体外扩增和活化,通过改善T细胞表型、功能和肿瘤反应特性,重新激活T细胞,最后回输到病人体内用于杀伤肿瘤细胞[3-4]。与其他ACT相比,TIL疗法具有独特的优势,包括能够识别一系列肿瘤抗原的多种T细胞克隆性、较好的肿瘤归巢能力、低脱靶毒性、克服免疫抑制障碍诱导较强的免疫介导的肿瘤消退等优点,此外,肿瘤组织中较高水平的TIL,尤其是CD8+ T细胞,与多种实体瘤的抗肿瘤效果改善有关[5-7]
2024年2月,Lovance Biotherapeutics公司产品Lifileucel(LN-144)获得美国食品药品监督管理局(FDA)加速批准上市,用于治疗PD-1抗体治疗后进展的晚期黑色素瘤,这是全球首款批准上市的TIL产品,具有里程碑的意义。该产品的2期临床研究数据显示,晚期黑色素瘤患者接受TIL治疗后,客观缓解率为31.4%,其中8例完全缓解,40例部分缓解,中位总生存期为13.9个月,疾病控制率为79.3%,从注射药物到最佳疗效的中位时间为1.5个月,显示出非常好的治疗效果[8]。目前,国内多家公司研发的TIL产品(如BST02、GC101、GT101等)也逐步进入临床阶段,竞争逐渐白热化。本研究产品为北京循生生物医学研究有限公司研发,为加速该产品临床转化,对其进行了体内生物分布的研究。本研究尾静脉给予NOG小鼠TIL细胞,采用流式细胞术和实时定量聚合酶链反应(qPCR)的方法,检测TIL在小鼠不同脏器内的分布和存续情况,为本产品进入临床试验提供数据参考。

组织和血液基因组DNA提取试剂盒[天根生化科技(北京)有限公司,货号:DP304-03];2×T5 Fast qPCR MIX(Probe)(北京擎科生物科技股份有限公司,货号:TSE301);异氟烷(河北一品制药有限公司)。探针、上游引物、下游引物[通用生物(安徽)股份有限公司];GAPDH质粒标准品(北京擎科生物科技股份有限公司合成);Ph PerCP mouse anti-human CD3(批号:020548)、Ph PE mouse anti-human CD4(批号:9172628)、PE FITC mouse anti-human(批号:0104976)(美国BD公司);注射用人白介素-2(IL-2)(批号:20210316-2,北京四环生物制药有限公司)。
本研究使用细胞注射液和溶媒由北京循生免疫医学转化研究院提供,细胞注射液主要成分为人肿瘤组织浸润淋巴细胞、体积分数1%人血白蛋白和体积分数0.9%氯化钠注射液,溶媒为体积分数1%人血白蛋白。在每次给药当天,用细胞注射液将TIL配制为每毫升5×107 mL-1,并在6 h内完成给药。
研究共使用84只SPF级NOG小鼠,雌雄各半,6~7周龄,购自北京维通利华实验动物技术有限公司,购入时动物体质量:雄性:18.2~21.1 g;雌性:15.2~17.3 g。在温度20~26 ℃、湿度40%~70%、12 h照明条件下饲养。提供经钴60放射灭菌的鼠全价颗粒饲料,自由摄食及饮水。本试验方案实施前已经动物管理与使用委员会伦理审查批准,动物的使用符合3R原则(Reduction,Replacement,Refinement),研究中所有实验动物相关的操作严格按照国际实验室动物伦理行为准则“Guide for the Care and Use of Laboratory Animals”实施(伦理编号:IACUC-2022-014)。
研究使用84只NOG小鼠(雌雄各半),根据动物体质量使用TOXSTA 2006软件进行随机分组,分为溶媒对照组和受试物组,溶媒对照组和受试物组均为42只小鼠(7个解剖时间点,每个时间点雌雄各3只小鼠),溶媒对照组动物尾静脉注射给予溶媒1次;受试物组动物尾静脉注射给予细胞注射液1次,后连续7 d腹腔给予IL-2(每只动物给予4 000 IU)。于给药后30 min、1 h、3 h、1 d、4 d眼后静脉丛采血,采用流式细胞术对CD3+、CD3+CD4+、CD3+CD8+细胞进行分析。在给药后3 h、2、7、14、28、42、59 d等7个时间点对溶媒组以及受试物组小鼠使用异氟烷麻醉后,解剖取心脏、肝脏、脾脏、肺、肾脏、脑、睾丸、附睾、子宫、卵巢、胃、十二指肠、结肠、骨髓、脂肪、骨骼肌,脏器-80 ℃冻存,留待后续进行qPCR试验,另取部分血液进行流式细胞分析,以研究受试物在外周血以及上述组织器官中的分布情况。
本试验使用探针法检测产品中细胞的GAPDH基因拷贝数,该拷贝数可反映人源细胞的基因拷贝数。采用绝对定量的方法,以已知浓度的含有人GAPDH基因片段的质粒为标准品,建立标准曲线,计算待测样本中的GAPDH基因拷贝数试验使用引物信息见表1
参考《中国药典》2020年版第四部通则9012——生物样品定量分析方法验证指导原则和人用药品技术要求国际协调理事会(ICH)《M10:生物分析方法验证及样本分析》,在正式样本检测前,对qPCR进行充分的方法学验证,包括选择性、特异性、标准曲线(线性和扩增效率)、精密度和准确度、基因组DNA储存稳定性和质粒储存稳定性、标准曲线样本稳定性、方法适用性等。
①小鼠混合背景基因组DNA:提取空白NOG小鼠肝脏、脾脏、肾脏、十二指肠的混合DNA(m-gDNA),作为标准品、质控样本等反应体系中的背景DNA使用。m-gDNA于-80 ℃冰箱储存,备用,避免反复冻融。②小鼠个体基因组DNA:取空白小鼠心脏、肝脏、脾脏、肺、肾脏、脑、子宫、睾丸、卵巢、附睾、胃、十二指肠、脂肪、骨骼肌、结肠、骨髓、血液样本,进行DNA提取。将提取后DNA添加不同浓度的质粒标准品,用于选择性研究。③细胞基因组DNA:受试细胞进行DNA提取,-80 ℃冰箱储存,避免反复冻融,用于特异性研究。④标准曲线的制备:将质粒DNA添加到小鼠基因组背景中制备而成。取1×109 copies·μL-1质粒作为工作液,梯度稀释,制备标准曲线范围为:12.5~1.25×109 copies·μL-1。⑤质控品制备:取1×109 copies·μL-1质粒作为工作液,梯度稀释,制备定量上限、高质控、中质控、低质控、定量下限分别为1.25×106、6.25×105、6.25×103、6.25×101和12.5 copies·μL-1
取小鼠组织的基因组DNA(心、肝、脾、肺、肾、脑、子宫、睾丸、卵巢、附睾、胃、十二指肠、脂肪、骨骼肌、结肠、骨髓、全血),以提取后DNA为背景,配制高浓度(6.25×105 copies·μL-1)、低浓度(6.25×101 copies·μL-1)质控品及不添加质控品的阴性对照,检测人GAPDH基因,考察不同组织基因组DNA对检测的干扰作用。可接受标准:80%以上的高、低浓度质粒质控的回收率(RE%)在50%~150%内,同时不添加质控品的阴性对照无扩增。
考察该分析方法对扩增某一特定基因的能力。样本制备同“2.2.1”项下操作,并增加对细胞注射液细胞扩增的特异性考察,接受标准:①小鼠个体基因组背景的高、低质控均有GAPDH基因的特异性扩增;②小鼠个体基因组背景的阴性对照样本,无扩增或检测低于定量下限;③细胞样本,检测值高于定量下限。
线性和扩增效率通过将模板稀释成一系列浓度梯度进行PCR反应,来考察反应的线性范围和不同拷贝数的初始模板是否具有相同的扩增效率,以此判断方法是否为最优化。以m-gDNA基因组为背景,以含人GAPDH基因的质粒为标准品,配制6个浓度12.5~1.25×106 copies·μL-1)GAPDH基因进行检测。结果以copies· μg-1 DNA为横坐标,以Cq值为纵坐标作图,绘制标准曲线。计算扩增效率、线性以及各点回收率。分析批数量:10。可接受标准:①扩增效率和线性:标准曲线扩增效率均值满足90%~110%,r2>0.980;②准确度:质粒标准曲线定量范围内各点至少有75%的校正标样的RE%介于50%~150%。
①批内精密度和准确度:设置3套质控,每套质控包括定量上限(1.25×106 copies·μL-1)、高质控(6.25×105 copies·μL-1)、中质控(6.25×103 copies·μL-1)、低质控(6.25×101 copies·μL-1)、定量下限(12.5 copies·μL-1),每套质控随行标准曲线样本,设置6个分析批。可接受标准是各浓度质控样本平均拷贝数的准确度介于50%~150%和各浓度质控样本平均拷贝数的精密度小于50%。②批间精密度和准确度:上述5个浓度质控进行3 d检测,可接受标准同①。
提取细胞基因组DNA与小鼠混合基因组DNA分别按高、低比例进行混合,进行不做处理、冻融5次、室温4 h、-60 ℃以下长期放置3个月的稳定性考察,每个条件重复3个样本,将处理后DNA样本进行GAPDH基因扩增。可接受标准:67%以上不同条件处理后样本与未处理样本相比,拷贝数偏差在±50%内。
将质粒工作液(1×109 copies·μL-1)进行室温4 h和-60 ℃以下长期放置4.5个月的稳定性考察。处理完毕后,进行凝胶电泳检测纯度,重复3个样本。可接受标准:不同条件处理后质粒与未处理相比,凝胶电泳图显示质粒无明显降解。
以小鼠混合基因组为背景,以含人GAPDH基因的质粒为标准品,配制标准曲线。将标曲样本均分为2份,一份进行实验,另一份储存于4 ℃冰箱,12 h以上后再进行人GAPDH基因扩增。可接受标准:①4 ℃存放标曲样本的扩增效率均值满足90%~110%,r2>0.980。②4 ℃存放标曲定量范围内各点至少有75%的校正标样的回收率(RE%)介于50%~150%。③将4 ℃存放标曲样本的各浓度点的值代入新鲜标曲计算拷贝数,要求至少75%的校正标样的回收率(RE%)介于50%~150%。
使用小鼠混合基因组DNA将受试细胞基因组DNA梯度稀释5个点,对人GAPDH基因进行扩增,计算扩增效率和线性。可接受标准:扩增效率均值满足90%~110%,r2>0.980。
按照组织和血液基因组DNA提取试剂盒操作说明进行DNA提取。
使用NanoReady测定提取的DNA的浓度及纯度。样本质量浓度需≥25 μg·mL-1,OD 260/280为1.6~2.0。
采用二步法进行荧光定量PCR,反应条件程序为预变性:95 ℃,30 s;PCR 反应:95 ℃,5 s,60 ℃,30 s,循环数40。
每轮定量PCR检测时,均于96孔反应板设置1套标准曲线,2套3水平的质控样本(低、中、高),每样本3复孔。每分批标准曲线扩增效率均值满足90%~110%,r2≥0.980。
使用Bio-Rad CFX Manager 3.1软件进行Q-PCR数据分析。以理论copies· μg-1 DNA为横坐标,以检测Cq值为纵坐标,进行线性拟合,绘制标准曲线。根据公式(1)计算样本浓度。

所有个体空白DNA样本添加高、低质控,均满足RE%在50%~150%内,同时不添加质控品的阴性对照无扩增。
含小鼠个体基因组背景的高、低质控均有GAPDH基因的特异性扩增;含小鼠个体基因组背景的阴性对照样本,无扩增或检测低于定量下限;细胞样本检测拷贝数(104 401.7 copies·μg-1 DNA)高于定量下限。
10个分析批,质粒标准品在12.5~1.25×106 copies·μL-1内,线性关系良好,r2介于0.998~1.000之间,扩增效率为95.93%~102.59%,满足线性和扩增效率要求。各浓度准确度介于84.5%~121.2%之间。
批内:定量上限、高质控、中质控、低质控、定量下限样品批内准确度分别介于81.5%~111.7%、93.3%~116.4%、82.1%~111.7%、64.3%~110.7%、59.9%~107.5%之间;批内精密度分别介于3.5%~17.3%、7.8%~15.3%、6.1%~18.8%、0.8%~10.5%、2.8%~17.3%之间;批间:定量上限、高质控、中质控、低质控、定量下限样品批间准确度分别为100.3%、102.0%、98.0%、87.6%、85.4%,批间精密度分别为14.4%、13.0%、14.9%、16.8%、19.2%。
与未处理样本相比,所有处理后DNA样本拷贝数偏差在±50%内。
对质粒标准品工作液于室温4 h和-60 ℃以下长期放置4.5个月处理样本进行琼脂糖凝胶电泳,结果见图1,质粒样本无明显降解。
配制后立即检测标曲样本的扩增效率为98.0%,r2值为0.999;4 ℃存放标曲样本的扩增效率为102.1%,r2值为0.999;4 ℃存放标曲定量范围内各点RE%介于91.6%~115.8%之间;将4 ℃存放标曲样本的各浓度点的值代入新鲜标曲计算拷贝数,各浓度点RE%介于76.4%~110.8%之间,满足验证要求。
梯度稀释后标准曲线扩增效率为91.57%,r2为0.991,满足方法适用性验证要求,表明该方法适用于该品种GAPDH基因拷贝数检测。
TIL在小鼠体内的生物分布情况见表2,根据42 d检测结果,59 d动物组织不再检测。NOG小鼠尾静脉给予细胞后,主要分布于肺、血液、肝脏等组织,其他组织组织分布较少。给药后2 d时,肝脏、血液中目的拷贝数略有升高(基因拷贝数均值分别为2 304.2和6 009.5 copies·μg-1 DNA),随后逐渐下降;给药后3 h时,肺组织中拷贝数达到最高(基因拷贝数均值为6 662.8 copies·μg-1 DNA),随后逐渐下降。至给药14至42 d,仅有个别动物可检测到目的基因拷贝数,多数组织均未检测到目的基因拷贝。肾、脂肪、肌肉、结肠、骨髓样本仅个别时间点检测到少量目的基因拷贝;心脏、脑、生殖器官(子宫、睾丸、卵巢、附睾)、胃、十二指肠于各时间点目的基因拷贝均低于定量下限。
采用流式细胞术检测外周血中5 000个细胞中CD3+、CD3+CD4+、CD3+CD8+细胞数量。在不同时间点检测CD3+、CD3+CD4+、CD3+CD8+细胞,3种细胞变化趋势一致。见图2,给药后30 min,即可检测到3种细胞,但与CD3+和CD3+CD4+细胞相比,CD3+CD8+细胞检出数量较少。与1 h时比,3 h时3种细胞数量降低,给药后1~2 d时达到峰值(达到峰值细胞数量均值分别为400、380和21),后逐渐降低,至给药后28、42和59 d,3种细胞数量接近于零。该结果与qPCR检测结果趋势一致,表2中血液检测结果显示,给药后2 d,基因拷贝数最高,给药后28 d和42 d,基因拷贝数均低于定量下限,提示流式检测结果可靠。
美国国家癌症研究所在20世纪80年代便开始了TIL细胞治疗的开创性研究工作,研究中转移性黑色素瘤患者进行非清髓淋巴细胞耗竭,后给予TIL和IL-2,结果证实了TIL疗法的临床安全性和显著的疗效,高达55%的病人出现肿瘤消退[9-10]。TIL主要由B细胞(CD19+CD20+)、T细胞(T cells TCRγδ+等)和NK细胞组成,肿瘤微环境中的免疫抑制和免疫刺激作用由这种多样化的组成来表现,这是发挥抗肿瘤功能所必须的[11]。有研究通过流式细胞术检测体外扩增的TIL,发现TIL主要是以CD8+ T或者CD4+ T细胞为主,并且回输的CD8+ T细胞大多数为效应记忆细胞,同时也发现含有少量的CD3-CD56+NK细胞[12]。TIL除用于肿瘤治疗外,也被认为是影响多种癌症疾病结果的预后因素,可以用来预测患者的生存率,Shojaei等[13]收集乳腺癌患者肿瘤样本,经过临床组织病理学检查,确认肿瘤组织中淋巴细胞浸润的百分比,当淋巴细胞浸润超过30%,采用免疫组化的方法检测CD8+细胞毒性淋巴细胞,结果发现,CD8+细胞毒性淋巴细胞的存在与患者总生存期的改善密切相关。作为TIL细胞治疗方案的一部分,短疗程的IL-2给药为一种支持性治疗,用来增强回输后T细胞的活性和发育。不同于仅为治疗目的IL-2单药给药,通常在TIL重复给药治疗过程中给予高剂量IL-2,因此治疗过程中极大可能出现与IL-2相关的毒性,如发热、低血压、肺水肿等,临床上需尽早监测和管理可能出现的不良反应[14]
本研究使用产品拟申报开展临床试验,细胞产品的生物分布和存续情况是影响其有效性和安全性的最重要因素,故按照相关指导原则要求,需开展细胞药动学研究,以评估细胞在体内的增殖情况以及分布、迁移、归巢和消除特征[15]。因人源细胞无法在免疫系统功能正常小鼠中存活,故本研究使用重度免疫缺陷小鼠开展试验,并在单次给药后设置6个解剖点,动态观察单次给药后细胞在小鼠体内各个脏器的分布情况。流式细胞术是一种单细胞技术,根据细胞的复杂性和大小在混合悬浮液中分离出异质性细胞群,可以在单细胞水平上快速定量评估多种标志物的表达,能够高速检测6~15个参数[16]。本研究采用流式细胞术检测该产品的主要成分CD3+CD4+、CD3+CD8+细胞,可以反映细胞在血液中动态变化情况,同时使用qPCR方法检测小鼠血液和组织内的人GAPDH基因,经过方法学验证确认qPCR方法适用于本产品的检测。
本试验结果显示,小鼠血液中给药后1~2 d CD3+、CD3+CD4+、CD3+CD8+细胞数量达到峰值,均值分别为400、380和21个,与CD3+、CD3+CD4+细胞相比,CD3+CD8+细胞数量较少,且给药后14 d后,3种细胞数量较低,均值分别为31、29和2个,给药后28~42 d,细胞几乎检测不到。qPCR检测结果也显示,在血液中,给药后第2天基因拷贝数最高,后逐渐降低,28和42 d时,检测结果低于定量下限。在脏器中,除肺脏、血液、肝脏外,其他组织基因拷贝数均较少,且42 d解剖点时仅极个别动物个别脏器检测出目的基因拷贝数(考虑可能是动物个体差异造成的),其他动物脏器基因拷贝数均低于定量下限,提示该产品在重度免疫缺陷小鼠体内存续时间可能不超过42 d。生殖系统(子宫、卵巢、睾丸、附睾)各时间点检测值均低于定量下限,说明该产品出现生殖毒性风险较低。
TIL传统的生产过程需要对特定的肿瘤识别能力进行检测,通常需要6~8周时间,然而TIL在体外长时间培养后容易耗竭,导致不能在病人体内存续较长时间,这可能是本产品在小鼠体内存续时间较短的原因。为提高TIL在体内的存活和功能,减少对高剂量IL-2的依赖,下一代基因修饰TIL正在研发中,通过病毒转导目的基因或者通过CRISPR或TALEN等技术敲除靶基因,例如插入IL-2、IL-12细胞因子或者敲除PD-1等[17]。另外,也有研究表明,在TIL治疗过程中添加免疫检查点抑制剂(如伊匹木单抗和纳武单抗)联合给药,可以降低IL-2使用剂量,同时支持TIL在体外的最佳扩增,从而提高TIL疗法针对多种实体瘤的整体安全性和有效性[18]
综上,本研究采用流式细胞术和qPCR技术评估了TIL在重度免疫缺陷鼠体内的分布特征和存续情况。研究结果提示,TIL在小鼠体内存续时间不超过42 d,研究数据可支持其临床试验申请,目前该产品已通过国家药品监督管理局药品审评中心审评,进入临床试验阶段。
