This article provides an overview of the current application status and existing problems of pharmaceutical excipients for pediatric medicines in China. In-depth research was conducted on the application and collection of standards of pharmaceutical excipients for pediatric medicines in China, and suggestions were put forward for improving the quality standard system. A scientific standard system of pharmaceutical excipients for pediatric medicines can ensure the safety and standardization of products, technically support the evaluation and approval process, and have important significance for the quality control of pediatric medicines.

儿童用药是当前医药行业备受关注的热点,药用辅料在儿童用药中的安全合理应用对儿童用药产品质量具有决定性作用。本文概述了我国儿童用药辅料的应用现状及存在的问题,着重对我国儿童用药辅料质量标准应用和收载情况进行深入调研,并对其质量标准体系的完善提出相关建议。科学的儿童用药辅料标准体系可为产品的安全性和规范性应用提供技术保障,充分发挥国家标准对审评审批工作的技术支撑作用,对儿童用药产品质量控制有重要意义。

Pediatric medicines are the hot topic in the pharmaceutical industry currently, and the safe and reasonable application of pharmaceutical excipients for pediatric medicines plays a decisive role in the quality control of products. This article provides an overview of the current application status and existing problems of pharmaceutical excipients for pediatric medicines in China. In-depth research was conducted on the application and collection of standards of pharmaceutical excipients for pediatric medicines in China, and suggestions were put forward for improving the quality standard system. A scientific standard system of pharmaceutical excipients for pediatric medicines can ensure the safety and standardization of products, technically support the evaluation and approval process, and have important significance for the quality control of pediatric medicines.

儿童是人类的未来,是社会可持续发展的重要保障。保证儿童的用药安全和健康,对于全面提高中华民族素质,建设人力资源强国具有重要战略意义。近年来,国家频繁出台一系列政策促进儿童药的发展,2019年,国务院修订《中华人民共和国药品管理法》,提出鼓励儿童药物的研发以及优先审评审批制度[1];2020年以来,国家药监局相继颁布了《药品注册管理办法》《药品上市许可优先审评审批工作程序(试行)》以及《儿童用药(化学药品) 药学开发指导原则(试行)》,对儿童用药的优先审评审批条件进行了明确[2-4]。这一系列政策足以体现国家保障儿童用药需求,促进儿童健康成长的决心。
儿童用药是给儿童使用的,起到预防、治疗等作用的药品。儿童用药辅料的安全合理应用是影响儿童用药产品质量的关键因素。本文概述了我国儿童用药辅料的应用现状及存在的问题,着重对我国儿童用药辅料质量标准应用和收载情况进行深入调研,并对其质量标准体系的完善提出相关建议,以助力我国儿童用药产品质量的整体提升,从而保障我国儿童用药安全。
药用辅料系指生产药品和调配处方时使用的赋形剂和附加剂,是除活性成分或前体以外,在安全性方面已进行合理的评估,一般包含在药物制剂中的物质。儿童用药是专用于儿童这一特殊群体,由于儿童处于特殊的生理、生长发育阶段,儿童用药风险具有长期性、复杂性、社会敏感性等特点[5-7],使得儿童用药日渐备受关注,儿童用药中辅料的应用也不应被忽视。
国内药用辅料行业起步晚,制药行业一直存在“重原料,轻辅料”的现象,这也导致药用辅料一直缺乏重视,儿童用药辅料的关注度也不够。在当前,我国儿童用药中辅料应用存在的问题主要体现如下:①儿童用药中辅料的选择仍与成人保持一致,由于儿童身体脏器功能等与成人差异较大,其药动学特征与成人也有很大不同[8-10],儿童对药用辅料更加敏感,药用辅料的品种、用量等可能都会对儿童用药安全性产生影响[11-14];②儿童用药多选择能满足给药灵活性、服药顺应性和安全性的制剂,对剂型的要求较高,尤其是便于服用、方便贮存的儿童口服固体制剂。相较于国外新型儿童口服固体制剂的研发[15],国内对儿童用药辅料在新剂型开发中的关键作用认识尚不充分,对儿童用药辅料的科学合理应用有待加强;③芳香剂、甜味剂、着色剂、助悬剂等为儿童用药的关键特色辅料,这些类别的辅料有着其各自的特点。在当前儿童用药中,对其中掩味和着色等类别辅料的监管还有待加强,尤其是此类辅料的质量标准体系有待完善。
儿童服药特点为吞服困难、对味道敏感,再加上胃肠及肝肾功能尚未发育完全、免疫功能脆弱,对药物及剂型、剂量、口感、外观等均有特殊需求。国家药品监督管理局批准的儿童用药,以溶液剂、颗粒剂和片剂为主,占比超过50%,而美国则多为溶液剂、糖浆剂、颗粒剂(混悬颗粒剂和干混悬剂等)、片剂(分散片和泡腾片等)[16],在我国儿童用药的适宜剂型和规格相对缺乏。据相关文献和调研结果可知,当前儿童用药适宜剂型通常为片剂、颗粒剂以及液体制剂等,用于儿童用药中的辅料常包括:填充剂、黏合剂、润湿剂、崩解剂、助流剂、润滑剂、溶剂、助悬剂、甜味剂、包合剂、芳香剂、增溶剂、抗氧剂、抑菌剂、矫味剂、着色剂等[17-18]。基于儿童用药的特点,尤其要关注其中的掩味矫味、助悬稳定以及着色等类别辅料,这些都是儿童用药的关键特色辅料,应结合辅料本身的功能特性,进行科学合理应用。
口服儿童用药的味道会对其可接受性产生巨大的影响,而可接受的适口性会关系到儿童用药的依从性。许多原料药具有令人不快的味道,因此有必要在处方中掩盖这一特性,故有必要对苦味或具其他不良气味的药物进行掩味或矫味处理。掩味和矫味类药用辅料是指制剂中用于改善或屏蔽药物不良气味,使得药物的苦味难以察觉或者减轻的药用辅料。目前,在药物制剂中加入适当的矫味掩味药用辅料是最常用的技术手段,另外还有包衣掩味技术、包合物材料掩味技术以及离子交换树脂掩味技术等。常用掩味矫味类药用辅料通常包含芳香剂、甜味剂、味蕾麻痹剂、苦味阻滞剂、包衣材料、包合剂和离子交换树脂等。常用的掩味矫味类辅料主要包括:①芳香剂通常为香料或者香精,主要是天然植物挥发油香料、用芳香油调配的芳香水剂和人工合成的香精香料。经调研,儿童用药最常用的复合香精主要为苹果香精、葡萄香精、草莓香精、香蕉香精、柠檬香精、西柚香精、甜橙香精、混合莓香精、牛奶香精等;②甜味剂是指添加后在一定程度上能增加对甜味的感应,掩盖药物苦味的辅料。一般分为天然甜味剂和合成甜味剂。常用甜味剂包括麦芽糖、葡萄糖、果葡糖浆、安赛蜜、糖精钠、甜蜜素、赤藓糖醇、纽甜、甘草酸铵、异麦芽等;③味蕾麻痹剂是口腔麻痹技术的关键,由于味觉细胞被暂时性麻痹,因此对苦味的感应降低,使得苦味阈值增高,从而达到掩盖药物不良味道的目的。据报道常用的味蕾麻痹剂有酸味剂如枸橼酸、枸橼酸钠、酒石酸、苹果酸等;清凉剂如薄荷脑、薄荷油;温热剂如丁香油、冬青油等;以及泡腾剂(如二氧化碳)等[19];④苦味阻滞剂通过与苦味物质竞争性地与鸟苷酸结合蛋白偶联受体部位结合,阻滞味觉信息从口内传至脑部,干扰味觉传导而发挥矫味作用。不同传导机制的苦味要通过不同的阻滞剂掩味。目前文献报道的苦味阻滞剂有单磷酸腺苷及其衍生物、阿魏酸和咖啡酸及苯乙烯酸衍生物、磷脂酸等[20];⑤包衣材料在儿童用药中常用于对药物进行包裹隔离,通过形成物理屏障阻隔或减少药物在吞服过程中的溶解释放,实现阻滞药物释放和掩盖不良气味等目的。包衣材料是一种较传统的掩味技术,很多种包衣预混辅料都具有掩味功能;⑥包合剂通常具有特殊的中空筒状结构,通过将药物包入包合剂分子内部空腔并形成稳定的包合物,降低药物在口中的溶解,从而减少药物与味蕾的直接接触,达到掩味的目的。包合剂主要指环糊精类,以及胆酸盐类辅料;⑦离子交换树脂主要通过离子交换过程吸附可解离药物,形成的复合物制成液体混悬剂,降低了药物与味蕾的直接接触,从而达到掩味目的。常见的有苯乙烯/二乙烯基苯共聚物、聚苯乙烯磺酸钠、聚克立林钾、交联聚丙烯酸骨、消胆胺树脂等[21]
口服液体制剂是最常用的儿童用药剂型,其中口服混悬剂是较重要的类别。混悬剂是指固体微粒分散在液体介质中的非均相体系,可有利于提高药物在溶液中的稳定性、便于服用、可掩盖不良气味等优势,使其作为儿童用药的适宜剂型具有较大优势。但混悬剂同时有一定的缺点,如在贮存中药物的物理形态如晶型或粒径容易发生改变,混悬液中药物分布不均,剂量不易准确等问题。要解决此类问题,除了适当的生产工艺外,助悬稳定类辅料的使用必不可少[17]。常用的稳定助悬类辅料主要包括:①助悬剂可增加分散介质的密度及黏度,减慢微粒的沉降速度,从而有效改善混悬液的稳定性与均一性。助悬剂稳定分散体系或增稠效应有很多机制,例如增加黏度、形成凝胶或立体稳定性等,这些均是助悬类辅料流变特性的表观体现;②常用润湿剂主要为表面活性剂,可通过表面活性剂降低界面张力,减少微粒与分散介质之间的接触角,适当的润湿剂可有效阻止结晶长大及粒度分布变化,起到絮凝和反絮凝等作用;③抑菌剂及抗氧剂为药用辅料的重要类别,可延长制剂的保存期限,是保障药品安全的重要手段之一。在药物制剂中加入氧化电位低的还原性辅料,可延缓或者防止药物制剂发生氧化反应,这类辅料称为抗氧剂。在药物制剂中起防止或抑制微生物发育生长的辅料称为抑菌剂,抑菌剂有利于保持制剂的稳定状态,防止微生物对制剂的污染。
为使药品有悦目的外观,易于识别,或为使某些药品的色泽一致,常常在药剂制备时加入使之着色的物质,这类物质多为色素,通常称为着色剂[22]。着色剂用于药品时,应对人体无害,理化性能稳定,且应考虑患者对颜色的心理状态,以及色差辨识度等因素。在儿童用药中,常会加入着色剂类辅料,可提高儿童用药的依从性,使儿童易于接受用药。着色类辅料多为色素类物质。色素可分为天然色素与合成色素。天然色素主要来自植物中提取、加工,也可来自于有色的矿物质。如天然植物色素有叶绿素、矿物质有赭石、朱砂等都为天然色素。合成色素按化学结构可分为偶氮类、三芳基甲烷类、氧杂蒽类、喹啉酮类等。也可将色素分为有机色素和无机色素,有机色素按来源一般可分为天然有机色素和人工合成色素,无机色素为天然矿石染料及其提纯加工品,多含金属元素,如氧化铁类、二氧化钛等。此外,为解决可溶性色素迁移而引起的色斑现象等问题,工艺中常应用色淀。色淀通常是用氧化铝、不含石棉的滑石粉或硫酸钡粉等作吸附剂,将水溶性色素沉淀并永久吸附在吸附剂上,成为具有覆盖力的不溶性着色辅料。
由于儿童用药的特殊性,各国家和地区对儿童用药辅料品种的科学合理应用以及该类辅料的标准收载也不尽相同。为了解市场需求与现行药典标准体系之间的差异,以及市场上儿童药用辅料的使用品种在国内外药典的收载情况,对我国677个相关厂家和1 109个已上市儿童制剂品种及部分在研儿童制剂产品进行了初步调研和统计。相关厂家在儿童用药中使用的辅料品种共计203个,其中112个收载于Ch.P 2020年版四部或二部,91个尚未被Ch.P收载,未收载品种占比45%[23]。可见儿童用药中所应用的辅料品种有近半数质量保障还有待进一步加强。其中未收载的品种中,约55%为香精或甜味剂,17%为着色剂,9%为包衣材料,4%为助悬剂,其他为保护剂、黏合剂、抑菌剂、溶剂等。我国儿童用药辅料品种在Ch.P 2020年版收载情况及辅料品种类别分布见图1~2
由于调研范围有限,已进行的市场调研存在一定的局限性。为此,本研究进一步比较和梳理儿童用药中受关注类别辅料在Ch.P 2020年版与国外药典之间收载情况的差异。相关类别辅料品种的标准情况,通过进一步交叉查询,列出了儿童用药中常用的关键特色辅料类别及其标准收载状况。
①芳香: 市场上芳香剂(香料或香精)主要来自国标《GB2760-2014》中收载允许使用的1 477个品种的合成香料,经过企业调研,市场最常用的香精涉及80种最常用的香料成分。使用的调配香精均来源于《GB2760-2014》中收载的表B.3允许使用的食品用合成香料名单,考虑部分香精香料存在赋型的需要,增加《GB30616-2020》中附表B.1溶剂及载体,附表B.2其他辅料,交叉筛选查询[24-26],其中食品添加剂使用标准的1 586个品种的合成香料,Ch.P收载83个,占比5.2%,《欧洲药典》(EP)11.0版、《英国药典》(BP)2022年版收载89个,《美国药典》(USP)2022年版收载107个[27-29]。即使囊括Ch.PUSPEP收载的全部合成香料品种,国标允许使用的香料依然有90%以上未有药典标准,可见香精香料品种在各国药典标准体系中,仅零星收载。而食品用香精中允许使用的溶剂及载体,Ch.P收载70.0%,其他允许的辅料Ch.P收载50.7% ;②甜味剂:通过市场调研,常用的麦芽糖、安赛蜜、糖精钠等甜味剂基本包含在《GB2760-2014》收载的常用甜味剂范围内,允许使用的甜味剂共17种,Ch.P收载其中7种,USP收载12种,EPBP收载10种;③味蕾麻痹剂:常用味蕾麻痹剂有酸味剂如枸橼酸、枸橼酸钠、酒石酸、苹果酸等;清凉剂如薄荷脑、温热剂如丁香油等;以及泡腾剂如二氧化碳等标准在各国药典中均有收载;④苦味阻滞剂:常用的苦味阻滞剂有单磷酸腺苷及其衍生物、苯乙烯酸衍生物和磷脂酸等,这些品种的标准在各国药典暂未收载。在BPEP中有收载阿魏酸和咖啡酸的标准;⑤ 包衣材料 对国标和各国药典的包衣剂品种进行统计后发现,食品国家标准中涉及相关的品种较少,但各国标准多有收载,对USPCh.P收载的包衣剂进行梳理,发现USP收载47个包衣剂品种,其中21个收载于Ch.P,BPEP收载了其中的27个品种;⑥包合剂 常用的包合剂环糊精类辅料如阿尔法环糊精、倍他环糊精、伽马环糊精等均已收载各国药典中,而胆酸盐类品种在各国药典均未收载;⑦离子交换树脂Ch.P暂未收载离子交换树脂品种,USP有收载聚苯乙烯磺酸钠、聚克立林钾、消胆胺树脂等3个品种,EP仅收载了聚苯乙烯磺酸钠。掩味矫味类辅料在各国药典的具体收载情况见表1
①助悬剂:《GB2760-2014》有收载部分助悬剂,共41个品种,其中18个收载于Ch.P,占43.9%。考虑到食品助悬剂在品类上不如药物制剂丰富,对USP中的助悬剂进行梳理统计,USP收载助悬剂96个品种,其中的57个品种收载于Ch.P,占比59.4%。②润湿剂:润湿剂在食品标准中较少,但在各国药典中多有收载,交叉查询《GB30616-2020》《GB2760-2014》以及各国药典,可发现USP收载润湿剂品种最多,共收载41个品种。③抑菌剂及抗氧剂:《GB2760-2014》收载的抑菌剂抗氧剂总数为61个品种,其中Ch.P收载15个品种,占比24.6%,USP收载24个品种,占比39.3%,EP收载14个品种,占比23.0%。助悬稳定类辅料在各国药典的具体收载情况见表2
经对部分着色剂生产企业调研发现:着色剂常用品种45种,其中无机色素5种,合成色素36种,天然色素4种。其中的5个无机色素品种均收载于各国药典中,分别为红氧化铁、黄氧化铁、黑氧化铁、紫氧化铁和二氧化钛。天然色素4种及合成色素10种具有国家食品标准,对于剩余的26种合成色素,企业按FDA 联邦规章典籍21条(Code of Federal Regulations Title 21, CFR 21)要求进行控制。可见Ch.P 2020年版对着色剂类辅料品种的收载也有待进一步完善。
由调研统计结果可知,儿童用药的特色关键辅料主要包括掩味矫味类、助悬稳定类辅料,以及着色类辅料,尤其是掩味矫味类辅料,品种繁多,容易疏于规范管理。由于儿童群体的特殊性,对于儿童用药辅料要着重关注产品的安全性。基于儿童用药辅料的特性,考虑到各类辅料标准收载和应用的状况,并结合Ch.P标准体系的特点,拟对儿童用药辅料质量标准体系建设提出以下建议,分阶段、有重点按步骤完成标准体系建设。
掩味矫味类辅料的品种较多,各种类的标准收载状况差异较大,应按不同种类科学开展标准研究。掩味矫味类辅料中,芳香剂(香精香料)占比最大,此类辅料涉及品种数目巨大,要求药典要对该类辅料品种逐一收载不太现实,但会造成该类辅料的很多品种无相应药用辅料标准执行的问题。此外,香精香料的管理与其他辅料存在一定的区别,《国务院关于改革药品医疗器械审评审批制度的意见》并未将香精香料纳入关联审评审批的药用辅料目录[30]。并且由于香精香料行业市场竞争的特殊性,香精配方作为企业的核心竞争力,绝大多数香精公司拒绝向下游药用企业提供香精配方,使得药物制剂企业对香精成分了解甚少,这样会影响制剂企业对产品质量的控制,从而可能留下安全隐患。而这类辅料用于儿童制剂时,其安全性是一个无法回避和忽略的问题,因此建议起草儿童用药中香精香料类辅料的安全性指导原则,对该类辅料在儿童用药中的安全应用进行引导和规范。尤其要关注高致敏类和醛类物质的香精香料在儿童制剂中的安全应用,因为高致敏物质在人体内存在安全性隐患,醛类物质对API的稳定性可能存在影响。建议尽可能制订高致敏类和醛类辅料品种的质量标准,并对其在儿童用药中的安全性进行充分评估,同时加强对该类辅料的标签标示管理,确保使用的安全。对于甜味剂、苦味阻滞剂、包合剂以及离子交换树脂等类别,涉及品种相对较少,但目前各国药典还未做到全覆盖收载。可以根据品种在儿童用药中的使用情况,逐步开展标准研究工作,尽可能做到充分收载。尤其是离子交换树脂,现行版中国药典还未收载这类品种的标准,建议开展此类品种的标准研究工作,填补此类品种标准收载的空白。
着色剂类辅料在儿童用药中的使用频次也较高,其管理与常用辅料有所不同,此类辅料并未纳入实施关联审评审批的药用辅料目录[30]。着色剂类辅料的使用有着其特殊性,通常根据不同品种和厂家的需求,与其它辅料订制成预混辅料使用于制剂中。以上各种原因都可能导致该类辅料的管理易被疏忽,可能会导致产品质量隐患。故应关注此类辅料在儿童用药中的安全应用,对常用的品种,建议对标准进行深入研究,适当增订入药典中。由于该类辅料品种较多,且常预混订制使用,品种标准的收载很难覆盖充分,建议开展相关的指导原则研究,拟订质量控制相关的指导原则,引导和规范儿童用药中着色剂类辅料的安全合理应用。
用于儿童用药辅料的数量众多,难以在短时间内对这些辅料的每个品种制订标准。建议根据对各类相关辅料质量标准的梳理情况,首先可从企业常用辅料品种入手,比对筛选各品种在国内外各国药典的收载情况,并根据市场使用情况和标准收载状况,分阶段对各类儿童用药辅料品种的质量标准进行研究和制订,逐步增加和补充儿童用药辅料质量标准以及相关通则或指导原则在Ch.P中的收载,完善儿童用药辅料质量标准体系的建设。
药用辅料在儿童用药中应关注其有效性和安全性,有效性可增加儿童用药的依从性,安全性是指应考虑儿童的特殊体质,辅料应足够安全。儿童用药辅料本身的质量问题是儿童用药产品质量全生命周期管理中不可忽视的环节,其可能给儿童用药的质量带来一定的隐患。当前,各国尚未对儿童用药辅料的使用和管理有特殊、明确及系统的规定,药用辅料在成人与儿童制剂中不区分管理和应用会给儿童用药带来一定的潜在风险,应予以重视。各国家已逐渐意识到该问题并有所举措,如多国和地区要求儿童用药产品包装或说明书应对所用辅料进行标示等管理[31]。欧美创建了儿科辅料安全性和毒性网络数据库(STEP),其收载了近40种儿童用药辅料的安全性、毒性数据,为儿童尤其是新生儿药物制剂辅料的合理应用具有重要贡献[32-33],安全性数据库对品种质量标准的研究和制订提供了依据和补充。迄今为止,尚未有聚焦儿童用药辅料标准体系建设的相关措施,儿童用药辅料科学的标准体系在一定程度上可确保产品的安全性和规范性,对儿童用药产品质量控制有重要意义,相信随着儿童用药辅料标准体系的逐步建设,也可充分发挥国家标准对审评审批工作的技术支撑作用,推动儿童用药行业的有序发展,保证儿童用药安全。
