Chemotherapy induced nausea and vomiting (CINV) is the most common adverse reaction in cancer patients during chemotherapy. 5-HT3 receptor antagonists are recommended as the first-line drugs against CINV. However, the commonly used clinical dosage forms of 5-HT3 receptor antagonists are mainly oral and intravenous administration, among which intravenous administration is easy to produce adverse reactions and requires professional medical operation. Oral administration has first-pass effect, especially for patients with severe vomiting after chemotherapy, so the commonly used clinical dosage forms have certain defects and limited clinical application. Transdermal drug delivery system has the advantages of avoiding gastrointestinal first-pass effect, less toxic and side effects, and good patient compliance, so the transdermal drug delivery system of anti-tumor nausea and vomiting drugs has been widely concerned and studied. In this paper, the research status of transdermal delivery of 5-HT3 receptor antagonists at home and abroad is reviewed, which provides reference for the research and development of new preparations and new delivery routes of these drugs.

化疗所致恶心呕吐(chemotherapy induced nausea and vomiting, CINV)是肿瘤患者化疗过程中最常见的不良反应。5-HT3受体拮抗剂类药物被推荐为抗CINV的一线用药。但5-HT3受体拮抗剂药物目前临床常用剂型以口服和静脉给药为主,其中静脉注射给药易产生不良反应且需要专业医护操作,口服给药有首过效应,尤其是化疗后严重呕吐患者无法口服用药,所以目前临床常用剂型具有一定的缺陷,临床应用受到限制。经皮给药系统具有避免胃肠道首过效应、毒副作用小、病人依从性好等优点,所以抗肿瘤所致恶心呕吐药物的经皮给药系统受到人们的广泛关注和研究。本研究就5-HT3受体拮抗剂类药物的经皮给药制剂国内外研究现状进行综述,为该类药物的新制剂和新给药途径研发提供借鉴和参考。

化疗所致恶心呕吐(chemotherapy induced nausea and vomiting, CINV)是肿瘤患者化疗过程中最常见的不良反应。5-HT3受体拮抗剂类药物被推荐为抗CINV的一线用药。但5-HT3受体拮抗剂药物目前临床常用剂型以口服和静脉给药为主,其中静脉注射给药易产生不良反应且需要专业医护操作,口服给药有首过效应,尤其是化疗后严重呕吐患者无法口服用药,所以目前临床常用剂型具有一定的缺陷,临床应用受到限制。经皮给药系统具有避免胃肠道首过效应、毒副作用小、病人依从性好等优点,所以抗肿瘤所致恶心呕吐药物的经皮给药系统受到人们的广泛关注和研究。本研究就5-HT3受体拮抗剂类药物的经皮给药制剂国内外研究现状进行综述,为该类药物的新制剂和新给药途径研发提供借鉴和参考。

Chemotherapy induced nausea and vomiting (CINV) is the most common adverse reaction in cancer patients during chemotherapy. 5-HT3 receptor antagonists are recommended as the first-line drugs against CINV. However, the commonly used clinical dosage forms of 5-HT3 receptor antagonists are mainly oral and intravenous administration, among which intravenous administration is easy to produce adverse reactions and requires professional medical operation. Oral administration has first-pass effect, especially for patients with severe vomiting after chemotherapy, so the commonly used clinical dosage forms have certain defects and limited clinical application. Transdermal drug delivery system has the advantages of avoiding gastrointestinal first-pass effect, less toxic and side effects, and good patient compliance, so the transdermal drug delivery system of anti-tumor nausea and vomiting drugs has been widely concerned and studied. In this paper, the research status of transdermal delivery of 5-HT3 receptor antagonists at home and abroad is reviewed, which provides reference for the research and development of new preparations and new delivery routes of these drugs.

化疗所致恶心呕吐(chemotherapy induced nausea and vomiting,CINV)是肿瘤患者化疗过程中最常见且最令人畏惧的不良反应,据统计,如果在化疗过程中不给予预防呕吐用药,70%~80%的患者会出现恶心呕吐,导致脱水、代谢紊乱、自理能力下降、营养缺乏、厌食、体力下降、伤口裂开及食管黏膜撕裂等不良后果,显著影响患者的生活质量,进而降低患者后续抗肿瘤治疗的依从性,甚至导致患者终止治疗[1-3]。5-羟色胺3(5-hydroxytryptamine 3,5-HT3)与5-HT3受体高选择性结合,拮抗中枢化学感受区及外周迷走神经末梢的5-HT3受体,从而抑制恶心、呕吐的发生,在多类型呕吐治疗中发挥重要作用,现已被推荐为肿瘤化疗后恶心呕吐的一线用药[4-5]
目前,5-HT3受体拮抗剂药物临床常用剂型以口服和静脉注射为主[6-7],其中静脉注射给药易产生不良反应且需要专业医护人员操作,且病人依从性差,而口服可能有一定的首过效应,且化疗后严重呕吐患者无法口服用药,所以目前临床常用剂型具有显著缺陷,临床应用受到严重限制。因此,开发5-HT3受体拮抗剂类药物的新剂型和新的给药途径,对于提高CINV控制率至关重要。与口服和注射等给药方式相比,经皮给药系统具有避免胃肠道首过效应、毒副作用小、病人依从性好、克服患者由于呕吐不能口服药物问题等许多优点,是继口服、注射后的第三大给药系统,在防治疾病上发挥着越来越重要的作用[8-11]。所以,抗肿瘤所致恶心呕吐的药物经皮给药系统受到人们的广泛关注和研究。本研究就5-HT3受体拮抗剂类药物的经皮给药制剂国内外研究现状进行综述,以期为该类药物的新制剂和新的给药途径研发提供借鉴和参考。
呕吐是由大脑控制的多步骤反射过程,由化学感受器触发区、咽和胃肠道的迷走神经传入纤维以及大脑皮层向位于延髓的呕吐中枢传入冲动而触发,再将传出信号传递到不同的器官和组织,诱导呕吐。按照发生时间及治疗效果,CINV通常可分为急性、延迟性、预期性、爆发性和难治性5种类型,其分类依据见表1[3]
目前使用的预防呕吐的药物包括5-HT3受体拮抗剂、NK-1受体拮抗剂、多巴胺受体拮抗剂和糖皮质激素等,其中5-HT3受体拮抗剂目前已被推荐为肿瘤化疗后恶心呕吐的一线用药[4],其代表性药物有昂丹司琼、格拉司琼、雷莫司琼、多拉司琼、阿扎司琼及帕洛诺司琼等[5]。临床常用的5-HT3受体拮抗剂代表性药物及用量见表2[3]
可见,目前5-HT3受体拮抗剂药物在临床上的剂型主要为静脉注射和口服[12],这些剂型存在如下缺陷:有呕吐反应、食道梗阻等情况较严重的患者在化疗后无法口服用药,且口服可能存在首过效应的问题;静脉制剂仍存在药物稳定性差、毒副作用大、需要专业护理操作、患者依从性差等问题,临床应用也受到了严重限制。
经皮给药系统(transdermal drug delivery systems,TDDS)是指在皮肤表面给药,通过皮肤以固定剂量和可调节速率递送药物,经毛细血管吸收进入体循环, 产生全身或局部治疗作用的给药系统[8]。相较于口服、注射等给药方式,经皮给药系统具有许多优点[11],包括:能避免胃肠道首过效应,维持恒定的血药浓度,从而避免口服给药等引起的血药浓度峰谷现象,降低毒副作用和减少用药次数,尤其对于化疗药物导致严重恶心呕吐的肿瘤病人,皮肤给药能避免患者由于呕吐不能口服药物的问题,提高病人的依从性和抗呕吐治疗效果。将5-HT3受体拮抗剂药物制成TDDS,将有助于解决目前临床制剂的缺陷,但目前皮肤给药制剂仅有格雷司琼透皮贴片。由于皮肤角质层巨大的屏障作用,许多药物难以透过皮肤吸收,适合经皮给药的药物通常需要具备以下性质[10]:①剂量低;②相对分子质量低,通常要低于500;③logP值介于1~3之间;④熔点低,一般低于200 ℃。5-HT3受体拮抗剂药物的理化性质见表3
除多拉司琼剂量较大,不适合制备成经皮给药系统外,其他大多数5-HT3受体拮抗剂均满足制备成经皮给药的条件。目前关于司琼类药物的经皮给药系统研究较多,包括传统的透皮贴剂、凝胶剂、凝胶膏剂及纳米给药制剂等,现分别进行介绍。
贴剂是指原料药物与适宜的材料制成的供粘贴在皮肤上可产生局部或全身作用的一种薄片状制剂,贴剂一般由背衬层、含药基质、压敏胶和防黏层组成。透皮贴剂大致可分为骨架型和储库型系统,其中骨架型贴剂是将药物及经皮吸收促进剂直接分散在与皮肤接触的压敏胶材料中,形成压敏胶骨架,铺于背衬膜上,加保护膜而成,压敏胶充当其中骨架材料和与皮肤黏合的双重功能。 其具有生产工艺简单、成本低廉、使用方便等优点,是目前透皮贴剂的主流[15]。目前有多种5-HT3受体拮抗剂被制备成贴剂。
格拉司琼能以氧和氮原子的静电形式与5-HT3受体结合,从而阻止5-HT3与受体结合,是一种强效、高选择性的外周和中枢神经系统5-HT3受体拮抗剂,临床上用于预防和治疗化疗引起的恶心呕吐。格拉司琼存在严重的肝首过效应,生物利用度为60%左右[16],半衰期较短(3~4 h),口服给药需要频繁服用,血药浓度波动大,所以将其制成经皮制剂能避免该问题。格拉司琼透皮贴片(商品名:Sancuso,中文名:善可舒)于2008年被美国FDA批准用于接受中度和重度致吐性化疗方案的患者,成为首个获批上市的5-HT3受体拮抗剂透皮贴片产品,随后欧洲药品管理局也于2013年批准其上市,我国国家药品监督管理局于2018年批准其在国内上市,至今该制剂仍是市面上唯一被用于治疗CINV的贴剂[17],但该产品价格昂贵。
目前国内也有关于格拉司琼透皮贴剂的相关研究报道。Zhao等[18]制备了格拉司琼游离碱压敏胶分散性透皮贴,以药物的经皮累积透过量和稳态透过速率为主要评价指标,采用单因素考察法对格拉司琼贴剂的处方进行筛选,确定贴剂最优处方中各成分及质量百分数为:格拉司琼游离碱5%;促渗透剂薄荷醇庚酸酯5%;促渗透剂肉豆蔻酸异丙酯5%;载药基质DURO-TAKR87-9301压敏胶含量为85%。巴马香猪体内的药动学结果表明,与静脉注射相比,该贴剂血浆中药物的ρmax显著下降,平均驻留时间(MRT)显著延长,说明格拉司琼贴剂能显著延长药物在巴马香猪体内的滞留时间,获得较平稳而持久的血药浓度。Li[16]制备了格拉司琼贴剂,采用含有羧基基团的DURO-TAKR87-2677压敏胶作为载药压敏胶基质,丙二醇/肉豆蔻酸异丙酯为复合促渗透剂,优选处方在人尸皮肤体外渗透实验结果推算,52 cm2贴剂24 h累计透过量达到3 mg,该贴剂体外透皮渗透速率与Sancuso贴剂中的格拉司琼的透皮渗透速率相当。
帕洛诺司琼是第二代5-HT3受体拮抗剂,是FDA在2003年批准唯一用于治疗延迟性恶心呕吐的药物。与第一代药物相比,帕洛诺司琼有更长半衰期(40 h)[19]和更高亲和性等特点,在预防急性和延迟性呕吐方面具有明显优势。帕洛诺司琼较低的相对分子质量(296)、合适LogP值(2.7)以及较低的熔点(91 ℃)等药物基本性质符合开发经皮给药系统的基本要求。Liu等[20]在兔皮肤体外渗透研究中重点考察了压敏胶黏剂对帕洛诺司琼皮肤通透性的影响,采用傅里叶变换红外光谱仪和热分析方法分别研究了药物-压敏胶相互作用以及压敏胶的热力学活性,结果表明,胶黏剂DURO-TAKR87-2516具有较高的药物透皮量,该贴剂在家兔体内的绝对生物利用度为43%,且具有良好的体内外相关性。为促进帕诺洛司琼的皮肤渗透性,Nair等[21]研究了各种化学促渗透剂对帕洛诺司琼透皮通透性的影响,筛选出DURO-TAKR87-9301为最佳压敏胶黏剂,体外Franz扩散池法考察了促渗透剂丙二醇、二乙二醇单乙醚、聚山梨酯80和油酸对帕洛诺司琼透皮通透性的影响,其中丙二醇(质量分数5%)的渗透率最高。另有较多文献[14,22]和专利[23]对帕洛诺司琼透皮贴剂的研发进行报道。尽管关于帕洛诺司琼贴剂的研究报道较多,但目前尚未见产品上市。
昂丹司琼与5-HT3有相似的母核,其与5-HT3受体的亲和力是与其他受体的250~500倍[24],是首个上市的高选择性5-HT3 受体拮抗剂,对癌症放化疗所引起的恶心呕吐具有高效的预防治疗作用,尤其用于接受顺铂治疗患者及对常规止吐剂(胃复安,地塞米松)治疗无效或不耐受的病人。1991年,美国FDA批准昂丹司琼注射液(枢复宁,Zofran)上市,次年昂丹司琼片剂批准上市。目前临床上使用的昂丹司琼剂型有片剂、注射剂、胶囊剂、糖浆及口溶膜等,通过口服或静脉注射给药。对于术后卧床患者、接受癌症化疗的患者及老年患者,常存在吞咽困难或服用不便的情况。近年来发现高剂量静脉注射昂丹司琼会增加患者心律失常的风险,FDA于2011年9月16日发布的安全警示中指出,昂丹司琼高剂量静脉注射可能有延长QT间期的风险[25],引发尖端扭转型室性心动过速。2012 年由英国药品和健康产品管理局发布的安全信息中指出成人单剂量注射昂丹司琼不可高于16 mg。2012年12月4日,FDA将32 mg的静脉用昂丹司琼注射液撤市。以上信息提示临床上使用昂丹司琼时应该小剂量多次给药。开发昂丹司琼的新型给药系统,尤其是血药浓度平稳、毒副作用小、患者用药顺应性好的剂型对于昂丹司琼的临床应用具有重要意义。
昂丹司琼口服给药后吸收良好,由于具有首过效应,绝对口服生物利用度平均为60%,且生物半衰期较短(3~5 h)[26],传统的口服剂型频繁给药会产生较大血药浓度波动,从而产生不良的副作用。同时由于昂丹司琼的油水分配系数适中(logP 2.07)、相对分子质量较小(293),理想的理化性质也使其适合经皮给药,且同时能避免口服首过效应并降低血药浓度波动。Krishnaiah等[26]研究了盐酸昂丹司琼膜控透皮治疗系统(TTS),考察了聚乙二醇6000(PEG6000)用量对乙烯醋酸乙烯酯共聚物膜(EVA1802)体内外透皮作用的影响,体外渗透研究表明,不同含量的PEG6000(0,5%,10%和15%)显著影响盐酸昂丹司琼透皮能力,其中PEG6000含量为0和5%时促渗透效果差,含量为15%时具有突释现象,而含量为10%的促渗透效果最好。并比较了口服盐酸昂丹司琼速释片(剂量8 mg,给药间隔8 h)和应用膜控透皮贴片(剂量60 mg)后志愿者体内的平均血药浓度,结果表明,该透皮贴剂与多剂量口服片剂的平均稳态血药浓度相当,但血药浓度波动较小(图1)。
阿扎司琼为第一代5-HT3受体阻断药,对5-HT3受体具有很高的亲和性、强效性、选择性,没有多巴胺受体拮抗作用,通过阻断腹部迷走神经向心性纤维上存在的5-HT3受体,对顺铂等抗恶性肿瘤药诱发的恶心及呕吐能够发挥明显的抑制作用。阿扎司琼1994年在日本上市, 1997年经国家卫生部药政局批准进口[27]
Sun等[28] 制备了阿扎司琼贴剂,采用双室扩散池法体外考察不同压敏胶、渗透促进剂及贴剂中阿扎司琼用量对阿扎司琼经兔皮肤渗透的影响,最终以DURO-TAK 87-9301作为载药压敏胶基质,以肉豆蔻酸异丙酯为促渗透剂,最佳贴剂配方在体内可持续释放216 h,且贴片在巴马小型猪体内吸收曲线与体外渗透曲线具有良好的相关性。
托烷司琼主要是通过对中枢神经系统和外周神经5-HT3受体的抑制作用阻断呕吐感应过程中化学神经递质的传递,达到抑制化疗和放疗所致恶心呕吐目的,对托烷司琼的临床药效学、不良反应等药理学研究显示,其抑制肿瘤化疗引起的恶心呕吐效果显著,其效果优于昂丹司琼和甲氧氯普胺等药物[29-30]
Jeong等[31]对含有托烷司琼的新型透皮贴片在大鼠体内抗肿瘤药物诱导的呕吐效果进行了评价。应用托烷司琼贴剂后,顺铂等抗癌剂诱导的高岭土消耗增加显著减少,降低一半水平,其效果与托烷司琼注射剂相当。托烷司琼贴剂血药浓度在12 h内达到30~40 ng·mL-1的最高水平,并在贴剂去除前能维持较高血药浓度,而静脉注射剂药物在体内迅速消除,半衰期仅为2.21 h(图2)。
凝胶剂系指药物与能形成凝胶的辅料制成溶液、混悬或者乳状液型的稠厚液体或者半固体制剂。凝胶剂的基质可分为水性基质与油性基质,水性基质一般由水、甘油或丙二醇与纤维素衍生物、卡波姆、海藻酸盐等构成,水性凝胶基质大多在水中溶胀成水性凝胶而不溶解,具有无油腻感、易于涂展、易于洗除、稠度小、利于药物释放等优点[32]。水性基质的缺点是润滑作用较差,易失水和霉变,常需添加保湿剂和防腐剂。油性凝胶基质则由液状石蜡与聚氧乙烯、脂肪油、胶体硅、铝皂及锌皂等构成,必要时可加入附加剂来促进保湿、防腐、抗氧化及透皮吸收等。目前5-HT3受体拮抗剂经皮凝胶剂的研究主要集中在昂丹司琼,其次是格拉司琼。
Patel等[33]设计了一种昂丹司琼经皮凝胶。并用Franz 扩散池测试了经皮凝胶在猪耳皮肤的体外渗透性,结果与对照凝胶相比,2.5%的樟脑或肉豆蔻酸异丙酯(IPM)分别使皮肤累积渗透量提高了1.2倍和2.5倍。为促进凝胶剂的经皮吸收,进一步采用了微针技术对皮肤进行了处理,然后分别对昂丹司琼注射剂和昂丹司琼凝胶皮肤给药在家兔体内的血浆浓度进行了比较。结果表明,微针能使樟脑凝胶血浆ρmax和AUC分别增加(10.47±1.68)倍和(9.31±4.91)倍,使IPM凝胶的ρmax和AUC分别增加(2.31±0.53)倍和(1.59±0.38)倍。所以,微针对皮肤预处理可轻微改善IPM凝胶的给药效果,而显著增加樟脑凝胶的透皮给药效果。
Can等[34]开发了以高相对分子质量壳聚糖为基质的新型盐酸昂丹司琼透皮凝胶。该研究考察了柠檬烯、橙花醇和桉油精3种不同类型的萜烯对昂丹司琼体外皮肤渗透的影响,采用Franz扩散池研究了壳聚糖透皮凝胶中昂丹司琼的皮肤渗透性,结果表明,以桉油精为促渗剂的壳聚糖凝胶对昂丹司琼的渗透量显著高于其他2种含橙花酚或柠檬烯的制剂(P<0.05)。
Obata等[35]采用Box-Behnken设计对昂丹司琼水凝胶的处方工艺进行了优化,并对其在小鼠的皮肤渗透和药理作用进行了评价。以羟乙基纤维素和羟丙基纤维素为凝胶基质,L-薄荷醇为促渗剂,异丙醇(IPA)、n-甲基-2-吡罗烷酮(NMP)和水为溶剂。以L-薄荷醇、IPA、NMP的用量作为变量制备了15种不同处方的昂丹司琼水凝胶,再通过体外皮肤渗透实验和皮肤刺激实验研究了15种水凝胶的渗透性和皮肤刺激性,所得结果表明了昂丹司琼具有经皮给药的可行性。
凝胶膏剂(gel plasters)原称为巴布剂,系指药物与适宜的亲水性基质混匀后,涂布于背衬材料上制成的贴膏剂。凝胶膏剂具有与皮肤的生物相容性好、载药量大、软化角质层利于皮肤吸收及使用方便等优点,因此在传统皮肤给药制剂中很受欢迎[36]。基质的配方是凝胶膏剂研究的核心内容,对凝胶膏剂的成型有很大影响。但是在抗化疗引起呕吐的药物中,通过制备成凝胶膏剂经皮给药的相关研究在国内外还较少。
Liu等[37]采用正交设计法对盐酸格拉司琼巴布膏剂基质处方进行优化,以氮酮作为渗透促进剂,优化筛选出的基质配方为部分中和聚丙烯酸钠(NP-700)-氢氧化钙-聚乙烯吡咯烷酮-甘油=1∶0.15∶4∶4。该凝胶膏剂具有良好的涂展性和保湿性,且黏着力适中。根据《中国药典》2020年版释放度测定第三法对巴布膏剂进行体外释放度的测定,其释放规律符合Higuchi方程。采用改良扩散池进行体外透皮试验,经皮渗透符合零级释放过程。进一步选用抗肿瘤药物顺铂为诱发剂建立大鼠异食癖模型,对凝胶膏剂进行药效学研究,结果显示,不同剂量巴布膏剂对顺铂引起的大鼠异食癖行为有显著的抑制作用,说明其有抗呕吐作用。SD大鼠体内药动学实验表明,MRT比静注组显著延长,说明该巴布膏剂具有缓释效果,其绝对生物利用度为64.16%。
近年来,越来越多的纳米制剂被用作经皮给药的载体,与传统皮肤外用制剂相比,纳米制剂可显著提高载药量,药物透皮吸收的效果也更为理想,常见的纳米载体有纳米乳、脂质体和固体脂质纳米粒等[38]。由于纳米制剂技术在促皮肤渗透方面的显著优势,目前采用纳米制剂技术促进5-HT3受体拮抗剂透皮吸收的研究报道较多。
纳米乳也称为微乳,是由水相、油相、乳化剂和助乳化剂在适当配比下自发形成的一种透明或半透明的热力学和动力学稳定的分散体系,其粒径在10~100 nm之间。由于纳米乳具有较低的表面张力、易于湿润皮肤、可使角质层结构发生变化、增加脂质双分子层流动性、破坏水性通道以及降低其屏障作用而被广泛研究,且纳米乳为小液滴尺寸,有良好的流动性,与皮肤细胞产生显著的相互作用可有效促进小分子药物的透皮吸收[39]
格拉司琼:Zheng等[40]以肉豆酸异丙酯(IPM)为油相、聚山梨酯85(Tween 85)为表面活性剂、乙醇为助表面活性剂制备格拉司琼纳米乳,采用伪三元相图法确定了各组分的浓度范围,最佳处方为盐酸格拉司琼2.5%,IPM 4%,Tween 85/乙醇40%(1∶1)。结果表明,纳米乳的粒径分布均匀,平均粒径为(50.4±1.5)~(82.4±0.9)nm,加入10%皮肤渗透剂n-甲基吡咯烷酮(NMP)后,与格拉司琼胶束比较,药物通透性增强了4.1倍,为盐酸格拉司琼的透皮给药系统提供了一种有前途的载体。
Zhang等[41]研制了盐酸格拉司琼水包油型微乳,经伪三元相图和正交实验,利用体外透皮评价筛选出微乳最佳处方为油相-KM=1∶6(KM为卵磷脂PC+聚山梨酯80+无水乙醇);水相用量61.7%;载药量占5%。采用Franz扩散池法考察在兔离体皮肤中的透皮效果,结果格拉司琼微乳的累积渗透量均显著大于格拉司琼混悬液、凝胶剂和溶液3个对照组(P<0.01)(图3),说明微乳较其他制剂有更好的透皮效果。
昂丹司琼:Al Abood等[42]研究了昂丹司琼经皮给药微乳制剂,用于治疗化疗引起的恶心和呕吐,考察药物在大鼠离体皮肤中的渗透性,采用伪三元相图法优化微乳配方,最优配方为:昂丹司琼含量为0.5%(ω),油酸含量为5%(ω),聚山梨酯20和PEG 400(比例2∶1)混合物含量为30%(ω),并进一步将微乳制备成卡波姆934水凝胶经皮制剂。对该制剂进行了大鼠体内药动学研究,与口服常规糖浆剂相比,微乳剂中昂丹司琼的生物利用度增加了6.03倍,与常规经皮凝胶相比增加了9.66倍(图4)。
Malakar等[43]制备了盐酸昂丹司琼微乳液并评价其外观、稳定性以及在离体猪皮中的渗透性能。最优比例为:10%肉豆蔻酸异丙酯作为油相,8%水相,82%表面活性剂(聚山梨酯80-异丙醇=3∶1),此比例制备的微乳具有较高的渗透率[最高渗透通量为(0.284±0.003)μg·cm-2·h-1],平均液滴直径为427.53 nm,Zeta电位为-11.80 mV,多分散指数为0.35。这些结果表明,盐酸昂丹司琼微乳有经皮给药的可行性,和进一步研究开发经皮制剂的前景。
脂质体是指将有效成分包封于类脂质双分子层内而形成的微型球状载体。脂质体在经皮给药系统中具有以下特点[44]:对药物有增溶的效果,可作为难溶性药物的载体;脂质体与皮肤角质层脂质高度相似,既能增加药物在皮肤局部的积累,起到持续药物释放的效果,又可以增强药物进入角质层或表皮的作用。所以,脂质体是促进药物透皮吸收的重要方法。目前已有多种5-HT3受体拮抗剂被制备成脂质体的研究报道。
格拉司琼:Aggarwal等[45]以蒸发-超声法制备了格拉司琼脂质体经皮凝胶,以脱氧胆酸钠为表面活性剂。采用Box-Behnken实验设计方法三因素和三水平优化制剂处方,选择的自变量为脂质(X1)、表面活性剂(X2)和超声时间(X3),因变量为包封率(Y1)、粒径(Y2)和透皮通量(Y3)。筛选的最优处方为磷脂200 mg,表面活性剂脱氧胆酸钠25 mg,超声处理30 min。格拉司琼脂质体粒径为(127.7±1.08)nm,包封率为(83.0±3.22)%,电位为-24 mV,大鼠皮肤透皮通量为(20.0±1.88)μg·cm-2·h-1。Deng[46]采用正交设计法优化盐酸格拉司琼脂质体的处方工艺,并将脂质体进一步制备成凝胶剂,对该制剂进行了体内外评价。大鼠体内药动学结果为,格拉司琼脂质体凝胶剂的tmax9.16 h,ρmax33.92 ng·mL-1;格拉司琼普通凝胶的tmax为11 h,ρmax为24.00 ng·mL-1。格拉司琼脂质体凝胶剂的绝对生物利用度为53.75%,格拉司琼普通凝胶剂绝对生物利用度为38.28%。以上结果表明,格拉司琼脂质体凝胶剂较普通凝胶的生物利用度更高,且吸收更快。
昂丹司琼:Ammar等[47]采用薄膜水化技术制备了昂丹司琼脂质体。以多因素实验设计(factorial experimental design)法对处方工艺进行筛选,以粒径、多分散指数、Zeta电位、药物包封率(EE%)和体外渗透对制剂进行表征,优化出的最优处方为:以Span®60为表面活性剂,Span®60-胆固醇摩尔比为7∶1,脱氧胆酸钠质量浓度为25 mg·mL-1。差示扫描量热法(DSC)和粉末X射线衍射(PXRD)研究表明,昂丹司琼以无定形状态存在。体内药动学实验表明,昂丹司琼口服溶液峰浓度ρmax(153.773±34.57)ng·mL-1,达峰时间0.5 h,脂质体经皮制剂的ρmax为(85.972±144.72)ng·mL-1,达峰时间为1.5 h,与口服溶液相比,其相对生物利用度为164%,说明所制备的脂质体生物利用度较口服高,且吸收更加缓慢。
托烷司琼:Zhang等[48]采用乙醇注入法制备不同载药量的盐酸托烷司琼醇质体,并表征所得制品的粒径、ζ电位、形态、包封率和体外经皮渗透性等性质。进一步将醇质体制备成凝胶贴膏,解决了醇质体用药不便且难以持续递送药物的问题。体外经皮渗透试验结果表明,载药量0.36%的托烷司琼醇质体凝胶贴膏的经皮渗透率可达到21.66%,24 h累积透皮量为(92.45±6.85)μg·cm-2。进一步比较了该醇质体凝胶贴膏与同浓度水溶液凝胶贴膏的体外释放速率和体外经皮渗透量,结果显示,醇质体凝胶贴膏中托烷司琼的释放速度慢于水溶液凝胶贴膏,但其经皮渗透量却显著高于水溶液凝胶贴膏(P<0.05),能够起到缓释且促进渗透的作用。
为弥补口服和静脉注射给药的不足,5-HT3受体拮抗剂经皮制剂的研究具有重要的临床意义和市场前景,受到人们高度重视,相关研究报道也较多。经皮制剂的研究主要集中在贴剂和凝胶剂,其次是纳米促吸收经皮制剂的研究。对于纳米制剂的研究,目前的工作主要集中在纳米乳、纳米脂质体等领域,制剂类型单一,在纳米混悬体、纳米胶束等领域尚未见报道;另外,目前纳米乳、纳米脂质体的研究主要集中在制剂处方筛选、透皮性验证等方面,关于纳米制剂自身的毒性和不稳定性,以及进入皮肤后的代谢产物是否会产生不良反应等研究报道较少,需要进一步深入研究。
由于皮肤角质层的存在,经皮制剂的生物利用度是该给药途径面临的最大难题。大部分文献都采用了各种化学手段促进吸收,包括采用各种皮肤吸收促渗透剂和纳米制剂手段。微针等物理技术是近年来蓬勃发展的一种新型促进药物经皮吸收的有效手段[49-51],已在许多药物的经皮吸收研究中取得了长足进展,微针技术和化学促吸收技术的联合应用将更有利于促进药物的经皮吸收,所以,可以将该技术引入5-HT3受体拮抗剂经皮制剂的研究中,以有效促进5-HT3受体拮抗剂的研究开发。
目前关于5-HT3受体拮抗剂经皮给药上市制剂仅格拉司琼透皮贴片(善可舒)一个品种,大多数研究处于体外研究和临床前药动学初步研究阶段,所以,在今后应加快进行经皮制剂的临床前药效学、安全性和临床研究,以推进该类药物的经皮制剂研发和上市进程,让广大的肿瘤化疗所致恶心呕吐患者受益。
