The chemically induced mouse colitis model is the most commonly used animal model for human inflammatory bowel disease, among which dextran sulfate sodium induced colitis model is the most widely used。In the preparation of a dextran sulfate induced model, different concentrations of dextran sulfate and different strains of mice showed varying degrees of lesion severity in the intestinal segments of different mice. Therefore, it is unreasonable to take a sample of a certain segment of the colon for pathological evaluation. The entire colon should be made into a “Swiss roll” and the pathological changes should be comprehensively evaluated by dividing it into upper, middle and lower segments of the colon.The anal canal lesion is severe, it should be evaluated separately. Only by comprehensively and correctly evaluating the intestinal lesion can the therapeutic effect of drugs be correctly evaluated.

化学诱导小鼠结肠炎模型是人类炎症性肠病最常用的动物模型,其中葡聚糖硫酸钠诱导的结肠炎模型应用最广。在葡聚糖硫酸钠诱导模型制备中,不同浓度的葡聚糖硫酸钠、不同品系的小鼠,在不同小鼠的肠段所呈现的病变严重程度不一样。因此,取某一肠段的样本进行病理学评价是不合理的,应该将整个大肠制成“瑞士卷”,分上段、中段和下段结肠全面评价病理改变。肛管病变严重,应该单独评价。全面正确评价肠道病变,才能正确评价药物的治疗效果。

化学诱导小鼠结肠炎模型是人类炎症性肠病最常用的动物模型,其中葡聚糖硫酸钠诱导的结肠炎模型应用最广。在葡聚糖硫酸钠诱导模型制备中,不同浓度的葡聚糖硫酸钠、不同品系的小鼠,在不同小鼠的肠段所呈现的病变严重程度不一样。因此,取某一肠段的样本进行病理学评价是不合理的,应该将整个大肠制成“瑞士卷”,分上段、中段和下段结肠全面评价病理改变。肛管病变严重,应该单独评价。全面正确评价肠道病变,才能正确评价药物的治疗效果。

The chemically induced mouse colitis model is the most commonly used animal model for human inflammatory bowel disease, among which dextran sulfate sodium induced colitis model is the most widely used。In the preparation of a dextran sulfate induced model, different concentrations of dextran sulfate and different strains of mice showed varying degrees of lesion severity in the intestinal segments of different mice. Therefore, it is unreasonable to take a sample of a certain segment of the colon for pathological evaluation. The entire colon should be made into a “Swiss roll” and the pathological changes should be comprehensively evaluated by dividing it into upper, middle and lower segments of the colon.The anal canal lesion is severe, it should be evaluated separately. Only by comprehensively and correctly evaluating the intestinal lesion can the therapeutic effect of drugs be correctly evaluated.

炎症性肠病(inflammatory bowel disease, IBD)是一种特发性肠道慢性炎症性疾病,肠道黏膜免疫紊乱引起黏膜屏障的破坏是IBD发生的关键环节。IBD发病与遗传因素、肠壁免疫、肠道菌群和食物结构改变等有关。IBD的发病率在持续增长,其包含溃疡性结肠炎(ulcerative colitis,UC)和克罗恩病(Crohn's disease,CD)。我国UC发病率已接近西方发达国家,CD的发病率相对较低。临床目前还没有有效的治疗方法,在疾病诊断后不同阶段分别试用水杨酸制剂、糖皮质激素和免疫抑制剂,但只是缓解症状不能治愈疾病。IBD是一种非恶性的终身疾病但严重影响患者的生活质量。 UC多发于结肠和直肠,而CD多发于回肠和结肠。病理学上CD是肉芽肿性炎症,而UC为慢性非特异性炎症。学界已对IBD进行了长期的探索性研究,从临床实践到体内外实验研究,但进步有限,且迄今没有一个理想的动物模型来复制人类的IBD,因此常常临床前研究有很理想的结果,但到临床就无法显效。本刊特组织相关研究文章辑成“炎症性肠病研究专栏”,目的并非展示研究成果,而是引起大家关注。希望有更多的研究者关注理想的动物模型的研究、确立正确评价实验研究效果的方法、多途径发现有效药物以至发现临床有效的综合治疗方案。当然发病机制的阐明是解决IBD治疗的关键,但疾病发生是个复杂的过程,阐明确切的机制并非易事。希望众志成城,依靠新思路,借助新技术,努力攻克IBD这个顽疾。

炎症性肠病(inflammatory bowel disease, IBD)是一组原因不明的肠道慢性炎症性疾病,主要包括溃疡性结肠炎(ulcerative colitis,UC)和克罗恩病(Crohn's disease, CD)。IBD自百年前出现于西方工业化国家以来,其发病率一直在上升。近年来,我国的IBD发病率也迅速上升[1-2]。基于全国医保数据计算,2016年中国城镇IBD发病率为10.04/10万,这个发病率介于新兴工业化国家和西方国家之间[3]。我国UC发病率达8.95/10万,已与美国、澳大利亚、加拿大等一些西方国家接近,但CD发病率为0.71/10万,远低于西方国家。据估计,2025年我国IBD患者将达到150万人,成为重要的健康问题[4]
IBD的发病涉及遗传、环境、免疫和肠道菌群等多种因素的共同作用,其病因和发病机制不清[5],迄今没有有效缓解和治愈的药物。为了探索IBD发病机制,寻找有效的治疗药物,研究者已建立了近70种动物模型[6]。在动物模型中,以化学诱导小鼠结肠炎模型为最主要的方法,其中葡聚糖硫酸钠(dextran sulfate sodium, DSS)诱导模型应用极为广泛。由于DSS诱导肠炎的一些特点,模型对肠道病变的病理学正确评价很重要。如果不能客观、全面评价模型的病理学改变就无法评价药物的真正疗效。笔者结合阅片的体会,提出了做好病理学评估应该注意的一些问题。
小鼠DSS诱导肠炎模型由Okayasu等[7]于1990年首次建立,后来广泛应用于IBD的研究。Cooper等[8]1993年对模型的病理学改变进行了详细的研究,该工作是DSS诱导肠炎模型病理学研究的经典之作,此后陆续开展的相关研究的病理学评估均以此文评价标准为基础。Cooper组将DSS模型分为急性和慢性,急性肠炎是5% DSS饮用1周后处死小鼠,解剖分析病变。慢性肠炎分以下5种类型:第1种类型是7 d 5% DSS饮用后1周常规饮水;第2种是第1种重复1周期,即(7 d 5% DSS饮用+7 d常规饮水)×2;第3种是第1种重复2周期;第4种是7 d 5% DSS饮用加14 d常规饮水;第5种是7d 5% DSS饮用加21 d常规饮水。到规定时间处死小鼠,解剖进行病理学评价。不同文献DSS浓度不同,饮用时间也有调整。
小鼠DSS诱导肠炎模型可很好建立小鼠慢性肠炎-结直腺瘤-腺癌模型。一般采用氧化偶氮甲烷(azoxymethane,AOM)/DSS法建立[9-10]。该模型结合各种基因修饰,在炎癌转化,结直肠癌发生机制和药物研究中具有重要价值。
人类UC可累及结肠各段,病变是直肠向上蔓延,可见于结肠。病变呈弥漫性、连续分布。典型的病变有隐窝炎和隐窝脓肿,隐窝杯状细胞减少。固有膜可见炎症细胞浸润,严重病例黏膜下及肌层可有炎症细胞浸润,继而有广泛的浅表溃疡(图1)。慢性反复发作的病例,隐窝数量减少和萎缩,纤维组织增生、纤维化,有些肠隐窝可以增生、分支、结构扭曲。严重的病例可以出现异形增生,最终癌变。
人类CD病变以消化道病变为主,可累及消化道以外的器官。病变主要累及回肠末端,其次为结肠、近端回肠和空肠等处。肠道病变呈节段状,病变之间黏膜正常。黏膜面有纵行溃疡并发展为裂隙,重症病例可引起肠穿孔及瘘管形成。组织学上最典型的改变是非干酪性肉芽肿形成,肠壁各层有慢性炎症细胞浸润(图1)。
从上述描述可以看出,尽管UC和CD都归入IBD的“帽子”下,但其病理改变和发病机制不同。
小鼠DSS肠炎模型其病理改变包括肠隐窝不同程度的上皮消失、黏膜糜烂或溃疡、炎症细胞浸润,严重的病变可累及肠壁各层(透壁性炎症)(图2),慢性病例纤维组织增生,甚至纤维化,隐窝扭曲、分支。杯状细胞减少,部分动物腺体出现不典型增生,形成腺瘤样改变(图3)。在急性肠炎模型的少数动物中出现的这种不典型增生大多存在于炎症病灶附近。这种改变是一种修复性上皮增生,并非真正肿瘤性异形增生。在小鼠模型中,人类UC中常见典型的隐窝脓肿,没有肉芽肿样病变。肠道病变并非连续性。从病理学改变的角度,DSS诱导小鼠肠炎模型更符合人类UC的病理变化[11-12]
文献上对DSS诱导肠炎模型形成的重度黏膜损伤性病变有糜烂和溃疡混用的现象[7]。在人体病理学上有严格的界定。黏膜糜烂是指黏膜上皮坏死脱落,导致浅表性缺损,其炎症性病变局限于黏膜肌以内。假如缺损较大,炎症病变累及黏膜下层,甚至肌层就称为溃疡。区别糜烂和溃疡有助于判定病变的严重程度。
DSS诱导时所用DSS浓度不同、小鼠品系不同,在不同肠段的病变严重程度也不一样[5]。因此动物取材仅在肠段的某一部位取1块组织进行固定、切片、苏木精-伊红(HE)染色评判病变严重程度并不能确切地反映动物病变的程度。因此,由此来评价药物治疗的效果并不合理,不同的研究者会取得不同的研究结果。同样取1个点进行病变的各种组学分析,包括单细胞测序,其结果也不能反映病变的真实情况和发生机制。在模型的病理学评估时要注意以下问题。
DSS诱导肠道病变可以通过病鼠体重丢失、便血和大便性状改变综合积分来评价,通常用疾病活动指数(disease activity index,DAI)来反映。文献多以Cooper等[8]提出的计分方法来评判(表1)。只要观察仔细、认真,该计分能很好反映动物肠道病变的总体情况,其数值与病变计分的数值有很好的正相关。该法评判简单,数据稳定性好,报道的文献大多按此方法执行。
DSS诱导肠炎造模的DSS浓度不同,多在1%~5%。因为不同浓度引起的病变轻重不同,因此各研究团队应根据所用动物,摸索最佳的造模浓度,以取得理想的、真实的实验结果。就筛选IBD药物来讲,如果DSS浓度高,病变严重,即使最有效的药物也不能做出药物有效的结果来。严重的透壁性炎症,伴有纤维组织增生,甚至广泛的纤维化,很难有药物在动物应用1周或稍长一点时间能产生明显的效果。笔者工作的体会是:如果比较造模成功后给药治疗与造模的同时给予相同剂量的药物,实验结果往往是造模成功后给药组作用有限,而同时给药组抑制炎症的作用比较明显。这一实验结果可以用上述现象来解释。临床所用药物对人类UC治疗不佳,可能的原因之一就是慢性病变,纤维化对药物没有反应。如果仔细阅读文献,就会知道该研究设计是药物的治疗作用,还是对肠炎形成的预防作用。如果同时给药可以减轻造模动物肠道的炎症,但只是对肠炎形成的预防作用。通过这种模型发现的药物疗效很难在病人身上发挥作用。
不同种系小鼠DSS诱导肠道病变的严重程度不一。Okayasu等[7]采用CBA/J和BALB/C雌鼠,报道病变最重的是左侧结肠。Cooper等[8]应用Swiss-Webster鼠,病变最重的是中段结肠。笔者采用C57BL/6鼠造模,病变最重的也是中段结肠,远端结肠大多是修复性改变,但是肛管的病变非常严重,有透壁性炎症细胞浸润(图4)。因为肛管覆盖的是鳞状上皮,因此一般没有溃疡形成。
基于上述动物造模的真实情况,不主张随意确定某一部位取材进行病理检查,这不能正确反映整个结肠的病变情况。因此建议结肠均分为3段,即上段、中段和下段结肠,肠段做成“瑞士卷”(Swiss-rolled-colon)(图5)[13],全部包埋,病理阅片可以看到整个结肠的全貌。肛管可以卷在下段结肠或分离包埋,单独分析。
在正常结肠黏膜层(黏膜肌以内)可见少数淋巴小结(图6),即派氏斑(Peyer's patches)。派氏斑也可以出现在黏膜下层,甚至到达浆膜下,但这种情况很少。正常淋巴小结中的淋巴细胞相对稀疏,不密集,一般也没有生发中心。如果在造模动物肠道炎症处有淋巴小结,小结中淋巴细胞密集,并有中性粒细胞浸润,大多也没有生发中心。在阅片时需要正确评估淋巴小结以及数量。
目前发表的DSS诱导肠炎模型的病理改变程度评价论文大多数是应用Cooper等[8]的工作,但真正评价要比原始文献的评价方法简单。Okayasu等[7]在其原始文献中,病变严重程度采用0~3级:0代表正常;1代表灶性炎症细胞浸润;2代表腺上皮丢失,隐窝脓肿,炎症细胞浸润;3代表黏膜溃疡。而Cooper方法详细,复杂,实际操作困难,特别是要评价很多动物时,病理人员很难有耐心去认真做好。评估隐窝上皮改变,炎症程度,隐窝扭曲和增生性改变存在等4个内容。每个又分为程度(0~4)和病变范围(0~4)。程度和范围相乘得出1个组织切面的病变程度分数,将所有切片的分数相加,除以切片数得出病变的总分数。
另外,DSS诱导肠炎模型最早的病理改变是隐窝上皮的消失,而不是炎症,炎症是继发于上皮丢失后出现的改变[8]。尽管可以依据上皮丢失的程度分为1~4级,但这些改变在饮用DSS水3 d就已出现,到饮用1周或几周后处死小鼠,再去病理观察是不能见到这种改变的。因此在慢性DSS模型和人类样本中很难见到这种改变,即缺乏炎细胞浸润的隐窝上皮消失,只有在实验开始就每天观察和处死动物,再分析样本才能见到。
为了实际可操作,依据Cooper标准,结合阅片体会,笔者建议采用表2方法评价,肛管单独评估(表3)。
表2~3评分的确定以病变最重的地方为准,不再去评估占了多少面积,实际阅片时也很难去正确评价病变占整个肠段的面积。通过3个肠段和肛门分别评判,可以分别比较不同肠段病变的程度或以总分比较不同动物间病变程度的差异。
造模动物解剖后比较大肠长度的变化,不论急性模型还是慢性模型大肠一般都会缩短。原因是在DSS诱导小鼠的肠壁可以看到肠壁平滑肌有不同程度的增厚[10],平滑肌的收缩使长度缩短。在慢性病例中,炎症,特别是重度炎症的肠壁有纤维组织增生和纤维化,纤维化致肠壁缩短。因此大肠长度的缩短,某种程度上可以反映造模动物肠道病变的严重程度。
DSS诱导小鼠肠炎模型操作简单,造模易成功,也是目前研究IBD选用最多的模型。正确评价病理改变对获得正确的结果非常重要,因此呼吁研究者高度重视。
化学诱导小鼠肠炎模型对阐释人类IBD的机制、发明有效的治疗方法具有很大价值,但迄今没有能够很好地复制人类IBD的理想模型。DSS诱导的肠炎模型以其病理改变类似溃疡性结肠炎、造模成功率高、实验周期短等特点,成为应用最广泛的IBD模型。目前人们在应用该模型研究时关注其病理发展过程不够,因此会影响临床前研究结果的判断。有许多因素会影响模型的病理学发展过程,不同的DSS浓度、同一浓度下不同动物或同一动物不同肠段和在药物干预炎症过程时不同的给药时间等因素都会影响病理学改变及其改变程度。为了正确评价药物的疗效,应该高度重视病理学评价并规范动物试验过程,这样才能保证结果的可靠性和可比较性。从药物研发角度,建议通过多中心合作,制订DSS诱导肠炎模型的基础性规范,包括不同动物建模的DSS来源和浓度,动物肠道处理方法和病变的全面、实际可操作的评价方法,治疗方法(或药物)在造模期间干预(给药)的时间等等。只有正确的病理学评价,才会有可重复的研究结果,并得到转化。
