OBJECTIVE To ensure the scientificalness and rationality of proficiency testing and measurement audit projects in the field of authenticity identification of Chinese material medica and decoction pieces, and to provide new ideas for high quality development in this field. METHODS The key technical points were analyzed in the proficiency testing and measurement audit projects of authenticity identification of Chinese material medica and decoction pieces. RESULTS Proficiency testing and measurement audit projects of authenticity identification of Chinese material medica and decoction pieces are insufficient, which cannot meet the needs of market supervision. CONCLUSION Proficiency testing providers should seriously summarize and think about the key technical points in the proficiency testing and measurement audit projects of authenticity identification of Chinese material medica and decoction pieces. By improving capabilities and service, actively organizing proficiency testing projects, and establishing measurement audit sample bank, proficiency testing providers can constantly improve the quality of the proficiency testing and measurement audit projects in the field of authenticity identification of Chinese material medica and decoction pieces, and the shortcomings of external quality control in this field will be complemented which can promote the high quality development of traditional Chinese medicine(TCM) and TCM industry.

目的 保障中药材及饮片真伪鉴别能力验证/测量审核项目的科学性、严谨性,为该领域高质量发展提供新的思路和方法。方法 剖析中药材及饮片真伪鉴别能力验证/测量审核项目组织中技术要点和要求并提出建议。结果 目前国内缺乏中药材及饮片真伪鉴别能力验证/测量审核项目,无法满足市场监督需求。结论 能力验证提供者应认真总结中药材及饮片真伪鉴别领域能力验证/测量审核项目技术要点和要求,通过提升自身能力和服务水平、积极组织开展能力验证活动、建立测量审核样品库等方式,补齐我国在该领域外部质控不足的短板,促进中药质量提升和产业高质量发展。

OBJECTIVE To ensure the scientificalness and rationality of proficiency testing and measurement audit projects in the field of authenticity identification of Chinese material medica and decoction pieces, and to provide new ideas for high quality development in this field. METHODS The key technical points were analyzed in the proficiency testing and measurement audit projects of authenticity identification of Chinese material medica and decoction pieces. RESULTS Proficiency testing and measurement audit projects of authenticity identification of Chinese material medica and decoction pieces are insufficient, which cannot meet the needs of market supervision. CONCLUSION Proficiency testing providers should seriously summarize and think about the key technical points in the proficiency testing and measurement audit projects of authenticity identification of Chinese material medica and decoction pieces. By improving capabilities and service, actively organizing proficiency testing projects, and establishing measurement audit sample bank, proficiency testing providers can constantly improve the quality of the proficiency testing and measurement audit projects in the field of authenticity identification of Chinese material medica and decoction pieces, and the shortcomings of external quality control in this field will be complemented which can promote the high quality development of traditional Chinese medicine(TCM) and TCM industry.

目前中药材及饮片市场仍存在较多的质量问题,主要表现为以伪品冒充正品、不同药材品种混用、使用非药用部分,以及掺泥沙或其他物质增重等[1]。这些问题严重影响中药材及饮片的质量安全,危害公众健康。中药材及饮片的真伪鉴别可有效控制其质量,确保其临床疗效和安全性。但中药材经过浸润切制、加工炮制等工艺,外观性状受到部分破坏,致使真伪鉴别难度加大,这对药品检验机构的中药质量评价能力提出了更高的要求。药品检验机构应重视自身能力建设,积极参加外部质量控制活动以维持和提升检测能力,为中药质量安全提供有效技术支撑。
能力验证是实验室参加外部质量活动和监测自身能力水平、寻求改进方向的重要途径之一[2]。2023年全国共有6家中国合格评定国家认可委员会(China National Accreditation Service for Conformity Assessment,CNAS)认可的能力验证提供者(Proficiency Testing Provider,PTP),发布中药领域能力验证计划35个,涉及中药材及饮片真伪鉴别能力验证项目3个;7家CNAS PTP机构发布中药领域测量审核计划共84个,涉及中药材及饮片真伪鉴别测量审核项目6个。能力评价内容主要集中在水分、总灰分、农药残留、重金属及有害元素等,中药材及饮片真伪鉴别仅占7.56%,无法满足中药实验室参加外部质量控制活动以监测、维持、提升自身中药材及饮片真伪鉴别能力的需求。
本研究探讨中药材及饮片真伪鉴别能力验证及测量审核项目组织的技术要点和要求,以保证能力验证及测量审核项目的科学性、严谨性,对开展中药材与中药饮片真假鉴别能力验证及测量审核活动具有重要的指导意义,为如何提升中药材及饮片真伪鉴别能力验证及测量审核服务拓展了思路和方法。
能力验证及测量审核活动涉及样品选择、制备、均稳性检验、数据统计及结果评价等过程,对组织实施机构的人员、设施设备、检测能力有很高的技术要求,一般由行业内权威性高、技术能力强的检测实验室组织实施。因此,能力验证及测量审核组织机构也应严格按能力验证提供者质量管理体系运行,遵守CNAS-CL03:2010《能力验证提供者认可准则》、CNAS-RL06:2018《能力验证提供者认可规则》等相关要求,保证项目的科学性、严谨性。
样品的选择应坚持以实际问题为导向的原则,综合研判市场对能力验证及测量审核的需求。首先,组织机构要结合上年度能力验证及测量审核实施情况及实验室具体实际决定次年计划,如各项目参加者数量、不满意率、样品是否可获得。再者,组织方应及时关注中药材及饮片问题品种的动态变化,为制定实施能力验证及测量审核计划提供依据,以满足不同实验室的实际需求。具体可参考药监部门组织的中药材及饮片监督检验、评价性抽验和专项抽验数据,并充分调研药材市场、药材种植养殖基地、中药生产经营企业,以及医疗机构、零售药店、网络销售平台等使用单位的中药质量情况。
质量合格可控的样品是保证能力验证及测量审核活动成功实施的关键因素之一。中药产业链长,涉及环节多,如种子种苗、栽培种植、采收、产地加工、炮制、包装、运输及贮藏等,每个环节均对中药质量产生影响[3]。为严格保证样品质量,使其满足能力验证及测量审核活动要求,应尽可能选用来源清晰且可追溯的中药材及饮片作为能力验证及测量审核样品。按其制备过程具体分为3类:一是从道地产区、中药材交易市场和种植基地采收或采购合格中药材或者对应的伪品药材,尽可能选用符合《中药材生产质量管理规范》(GAP)的道地药材,按2020年版 《中国药典》或其他炮制规范进行炮制加工;二是直接采购、收集具有全链条质量控制追溯体系的中药材及饮片或对应的伪品;三是直接使用标准样品—对照药材作为能力验证及测量审核样品。
从人们将中药用于预防、治疗疾病时起,中药真伪问题便开始出现,中药材及饮片真伪鉴别是一个不断改进、完善的过程。中药鉴别方法多种多样,不同品种间所适用的鉴别方法不同,因此能力验证及测量审核组织者在选定了样品品种后,应为各品种匹配最优、最具代表性的鉴别方法,以此为原则确定项目研究方法。
为如实反映参加者的中药材及饮片真伪鉴别能力、保证参加实验室检验数据的科学性准确性,建议优先选择2020年版 《中国药典》中收录的鉴别方法。具体包括经验鉴别,显微鉴别、理化鉴别及DNA 分子鉴定等[4]。其中,经验鉴别是指广大中医药工作者在长期生产实践和临床实践中,总结出的简便、易行、有效的传统方法,以此来观察与判断药材和饮片“性状”真伪或优劣,主要包括眼看、鼻闻、口尝、手摸、水试(黏性、颜色变化与浮沉情况)、火试(爆鸣、火焰、烟等特征)、划痕,以及前述多法的组合等[5]。其中最具有代表性的是许多鉴别术语,形象直观地概括了真伪鉴别特征,具有这些特征的药材或饮片一般为正品,而伪品不具备这些特征。如天麻一端有红棕色的芽茎残留,俗称“鹦哥嘴”;何首乌横切面花纹如云锦状,俗称“云锦花纹”;羚羊角对光透视,上半段中央有一条隐约可辨的细孔道直通角尖,俗称“通天眼”。显微鉴别是指利用显微镜对药材或饮片切片、粉末、表面、解离组织或磨片制片,以及含有饮片粉末的制剂观察,并根据组织、细胞或内含物等特征进行鉴别的一种方法[6]。理化鉴别是利用物理、化学或仪器分析方法,分析中药中所含的主要化学成分,以此来鉴别真伪,主要包括一般鉴别法、光谱法、色谱法[7],具有代表性的有微量升华法鉴别大黄药材,泡沫指数和溶血指数用于鉴别含有皂苷类成分的药材或饮片。 中药 DNA 分子鉴定方法是基于不同生物固有的特异性 DNA 序列而建立的、用于识别中药样本种属来源的方法,与传统中药鉴别方法比较,中药分子鉴别从遗传物质的结构层次出发,具有准确性高、灵敏度好的特点[8]。蕲蛇和乌梢蛇分子鉴定作为第一个中药分子鉴别方法被收录于2010年版《中国药典》,标志着中药分子鉴别技术已经成熟应用于中药鉴别中[9]。中国食品药品检定研究院于2021年成功组织了聚合酶链式反应(PCR)法检测蕲蛇的能力验证项目,并于2022年组织了该项目的测量审核活动,为各参加实验室提供了一个衡量和验证自身蕲蛇聚合酶链式反应-限制性片段长度多态性(PCR-RFLP)鉴别能力的平台。
准确的指定值是测量审核活动成功实施的前提。中药材及饮片真伪鉴别能力验证及测量审核活动属于一种定性能力验证,检测结果只有“正品”或“伪品”两个值。此外,药材基原的准确性和丰富的经验鉴别对最终的鉴别结果至关重要。综上,可采用3种不同的方法确定能力验证及测量审核样品指定值:一是依据准确翔实的基原、产地、采收、加工及炮制等样品来源或制备信息确定指定值;二是直接将标准样品作为能力验证及测量审核样品,可将参照值或有证值作为指定值,如将中国食品药品检定研究院的对照药材作为能力验证及测量审核样品。需要注意的是,使用该方法时应尽可能隐藏标准样品的特征,以避免参加实验室熟悉标准样品而获满意结果的情况发生;三是对于缺乏来源、制备等信息或无法判断追溯信息准确性的样品,可召集中药鉴别领域的专家组成专家组,经充分讨论形成一致意见后确定指定值。此外,为确保专家意见的一致性,组织机构可以选择不同的中药鉴别专家组轮流对样品进行“盲审”,并定期评估专家组之间的一致性[10]
同一能力验证及测量审核计划中,分配到所有参加实验室的样品必须均匀一致、稳定可靠,才能够确保能力验证及测量审核活动中出现任何可疑结果或不满意结果与样品之间或样品本身的变异无关,仅是实验室间的能力差异所引起的。因此能力验证及测量审核组织机构必须严格按ISO/IEC 17043:2023《合格评定能力验证提供者的通用要求》质量管理体系运行,遵守CNAS-CL03:2010《能力验证提供者认可准则》、CNAS-RL06:2018《能力验证提供者认可规则》等要求,对样品开展均匀性和稳定性检验。
中药材受生长特性、气候环境等因素影响,部分药材品种不同部位的指标性成分含量、性状往往有较大差异,如厚朴药材依据植物形态学分类,由下往上依次为根朴、蔸朴、筒朴和枝朴,各不同规格厚朴药材中的厚朴酚含量逐渐减少,其中根朴与枝朴中总酚含量的差异可达10倍以上[11]。由不同规格的厚朴药材炮制而成的净厚朴或姜厚朴饮片,其厚度、色泽、质地、断面及气味等性状特征具有明显差异[12]。而性状是中药材及饮片经验鉴别中的最重要特征,因此,在中药材及饮片真伪鉴别测量审核活动中,应确保测量审核样品性状均匀,具有一致的鉴别特征。
不同品种的中药材及饮片的真伪鉴别特征存在差异,应依据不同品种开展针对不同鉴别指标的均匀性检验。如:谷精草与华南谷精草的主要鉴别特征为显微结构中的腺毛、种皮表皮细胞以及花茎横切面[13];而北柴胡、南柴胡及藏柴胡性状中的形态、根茎特征、质地及断面具有明显差异,可以直接用肉眼识别,辅助薄层色谱定性分析及柴胡皂苷定量分析,可快速、准确地鉴别3种不同基原的柴胡[14]。总的来说,真伪鉴别均匀性检验应重点关注所有鉴别特征的均匀性,确保每份样品的一致性。具体操作方法为对样品库中的每份样品进行唯一性编号,采用随机取样方式,从样品库中抽取大于等于10的样品数量,平行鉴别2次。需要注意的是,均匀性检验不以得到真伪结果为目的,应确保在检验过程中,能够观察到每份样品中全部的真伪鉴别特征。如采用显微法鉴别谷精草与华南谷精草,待测的每份样品应均具有能够区分正品和伪品的腺毛、种皮表皮细胞以及花茎横切面等显微特征。
中药材及饮片在储存运输过程中,温度、湿度、日光、空气、虫害及真菌等对质量均有较大影响,不当的环境条件,可能导致中药材或饮片虫蛀、发霉、泛油、变色、风化、潮解、粘连、腐烂以及气味散失等,影响参加实验室识别鉴别特征,从而导致能力验证及测量审核能力评定结果无法真实反映参加者自身的能力水平,最终造成能力验证及测量审核活动失败。因此,需关注环境条件在储存及运输过程中对样品的待测特性量的影响。此外,不同品种药材和饮片的稳定性影响因素具有差异性,应根据不同品种设计侧重点不同的稳定性检验方案,以此确定不同品种的储存、包装与运输条件。譬如富含挥发油的药材或饮片,如当归、白术、丁香、薄荷等,应重点关注贮藏时环境温度的影响,若温度太高,易散失香气或泛油;而含糖量高的药材或饮片,如大枣、地黄、知母、黄精等,对环境湿度有较高要求,湿度较大容易吸潮而糖化发黏,导致霉烂变质。综上,针对不同品种可开展不同的影响因素试验、加速试验与长期试验进行稳定性检验。同时部分易变质的品种应开展运输稳定性考察试验,运输稳定性试验的测量指标也需尽可能选择对试验条件敏感的真伪鉴别特征。
中药材及饮品真伪鉴别只存在“正品”或“伪品”两种结果。能力验证及测量审核组织机构可采用两种方式进行能力评定:一是比较测定结果与指定值的符合性,将参加实验室鉴别结果与指定值比较,如果两者相同,则判定参加者的结果为“满意”,否则判定为“不满意”;二是由专家意见确定,当无法采用第一种方法或能力评价结果存在争议时,可组织专家组对参加者的测试结果进行评定。当采用此方法时,应确保专家组所有成员无法辨认出参加者的任何信息,以保证能力评定结果客观合理、公平公正[10]。此外,能力验证及测量审核组织机构还应重点关注参加者的原始记录,核对试验结果所依据的鉴别特征,避免实验室误判而获正确的真伪鉴别结果。
面对我国在中药材及饮片真伪鉴别能力验证及测量审核方面供需失衡的境遇,能力验证提供者应直面挑战,抓住机遇。一方面作为参加者的“指导老师”应有过硬专业能力,要重视自身建设,不断提高检测能力和服务水平。能力验证及测量审核服务是组织机构与参加客户双向选择的过程,对于不同机构组织的类似项目,参加者倾向于选择与更权威、更专业的能力验证机构合作来证明自己在该领域的水平。再者,CNAS PTP能够更多组织中药材及饮片真伪鉴别能力验证及测量审核项目,解决我国在该领域项目不足的问题。需要注意的是,能力验证提供者在根据市场需求确定中药材及饮片品种后,应按不同品种选择最适鉴别方法的原则综合考量,最终确定具体项目计划。
开展能力验证活动一般需经过方案设计、集中发样、收集测定结果、数据统计、能力评定、报告审核与发放等过程,往往周期较长,一般需要数月时间,不能很好满足实验室对中药材及饮片真伪鉴别领域能力验证的需求。测量审核具有灵活性、常态性和及时性等显著优势,可以作为能力验证的重要补充[15]。在中药领域,每年因资源、价格或天气条件等原因,市场对不同品种中药材及饮片的需求波动较大,容易出现问题的中药材及饮片品种往往动态变化。因此,能够为中药检测实验室持续提供能力评价服务显得尤为重要。
建立中药材及饮片真伪鉴别测量审核样品库不仅能够有效解决中药领域能力验证及测量审核项目不足的问题,还能长期、广泛、科学、高效地向中药检测实验室提供中药材及饮片真伪鉴别测量审核服务。能力验证提供者需要认真回顾分析每次能力验证及测量审核项目中样品选择与制备、均匀性和稳定性及结果评价等关键技术,在积累了丰富的经验后,建立中药材及饮片真伪鉴别测量审核样品库,样品库应严格按能力验证提供者的质量管理体系要求运行。
在经济全球化浪潮中, 我国中药行业面临着前所未有的机遇和挑战,越来越多的新技术、新方法应用于全面监控中药质量[16-17],我国药品检验机构尤其是中药实验室需全面增强检验、检测能力,提升中药材及饮片真伪鉴别等基本检验、检测能力,满足中药行业高质量发展需要。开展中药材及饮片真伪鉴别能力验证及测量审核服务,能够考核各类中药质控实验室的能力水平,帮助其识别与同行实验室之间的差异,发现问题,提升水平。本研究对中药材及饮片真伪鉴别能力验证及测量审核项目技术要点和要求逐条剖析并提出建议,希望为该领域能力验证及测量审核项目后续高质量发展提供借鉴。
