OBJECTIVE To investigate the safety of the tetravalent influenza vaccines adjuvanted with PEG6000 and cationic [2,3-dioleoxypropyltrimethylammonium chloride(DOTAP)∶1,2-dioleoyl phosphatidylcholine(DOPC)]-modified liposomes. METHODS Mice were divided into five groups∶ PBS negative control group, blank neutral liposome group, PEG6000 liposome group, cationic 4∶1 liposome group, and vaccine stock solution group. Abnormal toxicity test, acute toxicity test, and long-term toxicity test were performed. The general behavior, body weight, food intaking, viscera index, morphology, and blood indexes of mice were recorded to evaluate the safety of the vaccines adjuvanted with PEG6000 and cationic-modified liposomes. RESULTS Abnormal toxicity test∶ within 7 days after injection, all mice were alive with weight gaining and without abnormal symptoms. Acute toxicity test∶ 0.5 mL (hemagglutinin content 15 μg) was injected intraperitoneally, and no abnormal reaction or death was observed within 14 d. Long-term toxicity test∶ The mice were inoculated with 0.2 mL of hemagglutinin (6 μg) at 0, 14 and 28 d respectively. The mice were executed in the acute phase (3 d after the last immunization) and the recovery phase (14 d after the last immunization). No abnormal conditions and death were observed in the mice during the experiment period, and their body weight increased. Hematological examination and blood biochemical indexes fluctuated within the normal range, no toxic reaction related to drug administration was found, and no pathological changes were found in organs. CONCLUSION The tetravalent influenza vaccine adjuvanted with PEG6000 and cationic (DOTAP∶DOPC mole ratio 4∶1)-modified liposome has good safety.

目的 考察以聚乙二醇6000(PEG6000)和阳离子[2,3-二油氧基丙基三-甲基氯化铵(DOTAP)∶1,2-二油酰基磷脂酰碱(DOPC)摩尔比4∶1]修饰的脂质体为佐剂的四价流感疫苗安全性。方法 将小鼠分为5组:PBS阴性对照组、空白中性脂质体组、PEG6000脂质体组、阳离子4∶1组、疫苗原液组,通过异常毒性实验、急性毒性实验、长期毒性实验,记录小鼠的一般行为、体质量、摄食量、脏器指数与形态、血液指标等对以PEG6000和阳离子修饰的脂质体为佐剂的四价流感疫苗进行安全性研究。结果 异常毒性实验:注射后7 d 内,小鼠全部健存,均未出现异常症状,体质量增加;急性毒性实验:一次性腹腔注射0.5 mL(血凝素含量15 μg),14 d内均未见小鼠的异常反应和死亡,体质量均增加,各项血液指标无异常;长期毒性实验:于0、14、28 d进行3次腹腔免疫0.2 mL(血凝素含量6 μg),分别于急性期(最后一次免疫3 d后)和恢复期(最后1次免疫14 d后)分批处死小鼠,实验期间小鼠未见异常情况和死亡,体质量均增加,血常规和血液生化指标均属正常范围内波动,未发现与给药相关的毒性反应,病理组织检查脏器无病变。。结论 以PEG6000和阳离子4∶1组(DOTAP∶DOPC摩尔比4∶1)修饰的脂质体为佐剂的四价流感疫苗具有较好的安全性。

目的 考察以聚乙二醇6000(PEG6000)和阳离子[2,3-二油氧基丙基三-甲基氯化铵(DOTAP)∶1,2-二油酰基磷脂酰碱(DOPC)摩尔比4∶1]修饰的脂质体为佐剂的四价流感疫苗安全性。方法 将小鼠分为5组:PBS阴性对照组、空白中性脂质体组、PEG6000脂质体组、阳离子4∶1组、疫苗原液组,通过异常毒性实验、急性毒性实验、长期毒性实验,记录小鼠的一般行为、体质量、摄食量、脏器指数与形态、血液指标等对以PEG6000和阳离子修饰的脂质体为佐剂的四价流感疫苗进行安全性研究。结果 异常毒性实验:注射后7 d 内,小鼠全部健存,均未出现异常症状,体质量增加;急性毒性实验:一次性腹腔注射0.5 mL(血凝素含量15 μg),14 d内均未见小鼠的异常反应和死亡,体质量均增加,各项血液指标无异常;长期毒性实验:于0、14、28 d进行3次腹腔免疫0.2 mL(血凝素含量6 μg),分别于急性期(最后一次免疫3 d后)和恢复期(最后1次免疫14 d后)分批处死小鼠,实验期间小鼠未见异常情况和死亡,体质量均增加,血常规和血液生化指标均属正常范围内波动,未发现与给药相关的毒性反应,病理组织检查脏器无病变。。结论 以PEG6000和阳离子4∶1组(DOTAP∶DOPC摩尔比4∶1)修饰的脂质体为佐剂的四价流感疫苗具有较好的安全性。

OBJECTIVE To investigate the safety of the tetravalent influenza vaccines adjuvanted with PEG6000 and cationic [2,3-dioleoxypropyltrimethylammonium chloride(DOTAP)∶1,2-dioleoyl phosphatidylcholine(DOPC)]-modified liposomes. METHODS Mice were divided into five groups∶ PBS negative control group, blank neutral liposome group, PEG6000 liposome group, cationic 4∶1 liposome group, and vaccine stock solution group. Abnormal toxicity test, acute toxicity test, and long-term toxicity test were performed. The general behavior, body weight, food intaking, viscera index, morphology, and blood indexes of mice were recorded to evaluate the safety of the vaccines adjuvanted with PEG6000 and cationic-modified liposomes. RESULTS Abnormal toxicity test∶ within 7 days after injection, all mice were alive with weight gaining and without abnormal symptoms. Acute toxicity test∶ 0.5 mL (hemagglutinin content 15 μg) was injected intraperitoneally, and no abnormal reaction or death was observed within 14 d. Long-term toxicity test∶ The mice were inoculated with 0.2 mL of hemagglutinin (6 μg) at 0, 14 and 28 d respectively. The mice were executed in the acute phase (3 d after the last immunization) and the recovery phase (14 d after the last immunization). No abnormal conditions and death were observed in the mice during the experiment period, and their body weight increased. Hematological examination and blood biochemical indexes fluctuated within the normal range, no toxic reaction related to drug administration was found, and no pathological changes were found in organs. CONCLUSION The tetravalent influenza vaccine adjuvanted with PEG6000 and cationic (DOTAP∶DOPC mole ratio 4∶1)-modified liposome has good safety.

流行性感冒(流感)是一种威胁人们身心健康的急性呼吸道传染病,发病率高,容易引起暴发性流行或大流行。人群对流感病毒普遍易感,预防流感最有效的方法是接种疫苗,但基于目前的生产工艺与方法,很难在流感疫情暴发的短时间内生产出足量的疫苗。研究表明,疫苗的研发制备过程中添加佐剂,能够增强其免疫原性,降低用量[1]。脂质体(liposome, Lip)是近年来较为前沿的一种颗粒型疫苗佐剂和靶向药物载体[2-3]。有相关文献表明,无修饰的中性脂质体作为四价流感疫苗的佐剂具有免疫增强作用[4],但是,普通脂质体极易从体内清除,这大大降低了其作为佐剂和载体的优势[5-6], 聚乙二醇 (polyethylene glycol,PEG)安全性和水溶性高、组织相容性好[7]。PEG修饰的脂质体降低了吞噬细胞识别摄取作用,可在体内达到长循环效果,提高药物的治疗指数和疗效[8]。2,3-二油氧基丙基三-甲基氯化铵(DOTAP)是常用的阳离子[9],1,2-二油酰基磷脂酰碱(DOPC)是一种中性磷脂,不同摩尔比DOTAP和DOPC可以调节脂质体的Zeta电位,使脂质体带有不同强度的正电荷[10]。生物膜一般荷负电,可因静电吸附作用而提高阳离子脂质体摄取率,从而增强其免疫佐剂作用[11]。在对PEG(2000/3000/6000)和不同比例阳离子(DOTAP/DOPC摩尔比为4∶1,3∶2,2∶3,1∶4)修饰的脂质体进行免疫原性研究中发现,以PEG6000和阳离子4∶1组免疫增强作用最好[12-13]。在此基础上,结合相关文献指导,以KM小鼠为实验对象,通过异常毒性实验、急性毒性实验、长期毒性实验,对以PEG6000和阳离子4∶1修饰的脂质体为佐剂的四价流感疫苗进行安全性研究。
流感疫苗原液:H1N1(批号:202107003B),血凝素含量:181.5·mL-3;H3N2(批号:202108002B),血凝素含量: 220.7·mL-3;B(V)(批号:202103007B),血凝素含量:134.8·mL-3;B(Y)(批号:202103004B),血凝素含量:205.2·mL-3(江苏沃森生物技术有限公司)主要试剂:大豆卵磷脂(分析纯,北京美亚斯磷脂技术公司);胆固醇(分析纯,北京鼎国昌盛生物技术优先责任公司);PEG6000(分析纯,美国Sigma公司);DOTAP、DOPC(分析纯,美国Avanti公司)。
SPF级KM小鼠,5周龄,雌雄各半,标准体质量(18~22 g),昆明医科大学实验动物中心提供,实验动物生产许可证号:SCXK(滇)2015-0002。
以大豆卵磷脂、胆固醇、DOTAP和DOPC(DOTAP与DOPC摩尔比为4∶1)为膜材,溶于无水乙醇,溶解后旋转蒸发至成膜状;加入PBS缓冲液30 mL,50℃水化旋转,间歇超声,50℃水浴孵育,冷却至常温,即为阳离子脂质体;准确称取0.461 0 g PEG6000 溶于 10 mL PBS 溶液中,取 2.0 mL 混合液与 28 mL PBS 缓冲液混合均匀制成水化液,以大豆卵磷脂、胆固醇为膜材,溶于无水乙醇,溶解后旋转蒸发至成膜状;加入上述水化液30 mL,50 ℃水化旋转,间歇超声,50 ℃水浴孵育,冷却至常温,即为PEG6000脂质体。
将四价流感疫苗与脂质体按1.4∶1.0的体积分数混合,采用冻融-冻干法将混合液制备为以阳离子和PEG6000脂质体为佐剂的四价流感[14]
将20只雌性KM小鼠分为4组,即PBS组、空白组、阳离子4∶1组、PEG6000组,每组5只。经腹腔注射0.5 mL供试品,连续观察7 d,记录动物的毒性反应、死亡情况、体质量变化。实验期间,观察动物活动情况,有无异常反应,到期每只小鼠称重[15]
将50只KM小鼠随机分为5组:即PBS组、空白组、阳离子4∶1组、PEG6000脂质体组、原液组,每组10只,雌雄各半。一次性腹腔注射0.5 mL(血凝素含量15 μg),相当于一人用接种剂量。给药后4 h内严密观察并记录,并于每天上、下午各观察1次,连续观察14 d,记录小鼠的毒性反应情况、生长情况、死亡情况、体质量变化、摄食量变化等;实验结束后进行大体解剖,取全血进行血液常规检测,并观察脏器病变情况。
疫苗通常需注射1次以上才能达到有效的保护和治疗效果,通过连续反复多次给药观察佐剂对实验动物的长期毒性反应非常重要[16]。将110只KM小鼠随机分为5组:即PBS组、空白组、阳离子4∶1组、PEG6000脂质体组、原液组,每组22只,雌雄各半。于0、14、28 d进行3次腹腔免疫0.2 mL(血凝素含量6 μg),此为前期免疫原性实验得出产生最佳免疫效应的剂量。于最后1次接种后3 d(急性期31 d)和14 d(恢复期42 d)分批处死(急性期12只,恢复期10只)。实验期间记录小鼠的毒性反应情况、生长情况、死亡情况、体质量变化、摄食量变化等;实验结束后进行眼球取血,部分全血进行血液常规指标检测,包括白细胞计数、淋巴细胞绝对数、红细胞计数、血红蛋白量、血小板计数;一部分分离血清进行血液生化指标检测,包括丙氨酸氨基转移酶、天门冬氨酸转移酶、尿素、血糖、总胆固醇、甘油三酯。观察脏器病变情况,进行病理组织学检查[17]
采用SPSS18.0 统计软件进行分析,多组间比较选择单因素方差分析,以P<0.05认为差异有统计学意义,统计结果以(均值±标准差)表示。
腹腔注射给药7 d内,小鼠均全部健存,无异常反应。且7 d后,小鼠体质量均增加(表1)。表明脂质体合格,无异常毒性。
实验期间各组小鼠行为活动无明显异常,尸检未见心、肝、脾、肺、肾、脑、胸腺等组织器官存在异常。5组小鼠第7天、第14天体质量差异无统计学意义(P>0.05);各组摄食量变化趋势大致相同,组间差异不存在统计学意义,实验结束时大体解剖无肉眼可见病变,各重要脏器指数组间差异无统计学意义,各重要脏器指数各组间无显著性差异(P>0.05);血液学检查无用药相关异常改变。
实验结果表明,在0、14、28 d免疫3次后,急性期(31 d)实验期间小鼠无异常死亡,精神状态、食欲及体质量增长均正常,与PBS组无明显差异。将实验小鼠的心脏、肝脏、脾脏、肺、肾脏、脑等脏器进行解离并称重,各脂质体组脏器重及其与体质量比无明显异常,与对照组相比除脾脏外其余脏器差异无统计学意义。PBS组脾脏指数与原液组、PEG6000阳离子4∶1组间存在显著性差异,分析原因是2种佐剂和原液组多次免疫刺激小鼠免疫系统,免疫细胞增殖,造成反应性脾增大(表2)。小鼠各血液学指标、血液生化指标有一定变化但无用药相关差异(图12),病理切片结果显示小鼠脾脏无异常(图3),心脏、脑、肾脏病理切片均未见病理性变化。
恢复期(42 d)实验期间小鼠无异常死亡,精神状态、摄食量、体质量增长均正常,与PBS组无差异。将实验小鼠的心脏、肝脏、脾脏、肺、肾脏、脑等脏器进行解离并称重,各脂质体组脏器重及其与体质量比无明显异常,与PBS组相比,除脾脏、肝脏外其余脏器差异无统计学意义。脾脏指数原液组、PEG6000组和阳离子4∶1组与其余各组间存在显著性差异,分析原因是多次免疫刺激小鼠免疫系统活跃,免疫细胞增殖,造成反应性脾增大。肝脏指数PEG6000组和阳离子4∶1组与其余各组间存在显著性差异(表3)。小鼠各血液学指标、血液生化指标有一定变化但无用药相关差异(图45),病理切片结果显示小鼠肝脏、脾脏无异常(图6),心脏、脑、肾脏病理切片均未见病理性变化[18]
本实验以四价流感疫苗为抗原,探究PEG6000和阳离子4∶1(DOTAP/DOPC4∶1)修饰的脂质体作为免疫佐剂的安全性。异常毒性结果显示,制备脂质体的过程不存在有害物质造成异常毒性;急性毒性试验临床观察未见异常,体质量、进食、脏器指数、血液学检查等无异常改变,小鼠无急性毒性反应;长期毒性实验旨在探究供试品对动物是否存在蓄积毒性以及多次用药引起的严重不良反应,实验结果表明,各实验组临床观察均未见异常,小鼠相关指标均未见与给药相关的规律性改变,组织病理学检查未发现病变。综上所述,以PEG6000和阳离子4∶1修饰的脂质体为佐剂的四价流感疫苗异常毒性、急性毒性和长期毒性实验均无异常,具有良好的安全性。
以脂质体作为免疫佐剂,具有免疫增强作用好,毒性小的优点,并且可以同时刺激机体产生细胞免疫和体液免疫。传统的铝佐剂只能激活体液免疫,对于细胞免疫的刺激作用较小[19]。经PEG6000修饰的脂质体在表面形成水化层能减弱与吞噬细胞的摄取作用,降低药物释放速度,以实现脂质体长循环作用[20]。阳离子脂质体由于其表面带有电荷,可通过静电吸附,提高与细胞的结合率[21],此两者皆有较好的免疫增强作用和安全性,是理想的佐剂及药物递送材料。一种免疫佐剂是否得以应用,安全性与有效性同等重要,立足于课题组前期的免疫原性和无修饰中性脂质体安全性结果,对修饰过的2种脂质体佐剂进行安全性研究,为后续更全面的安全性实验提供基础。
