OBJECTIVE To analyze the intervention effect of prescription pre-review and Six Sigma management model on the qualified rate of outpatient prescriptions to improve the quality of pharmaceutical care and ensure the safety, rationality, and effectiveness of medication in patients. METHODS The qualified rates of outpatient prescriptions in Beijing Hospital from April 2020 to March 2024 were collected. The multi-stage model of interrupted time series analysis (ITSA) was used to analyze the changes in the qualified rate of outpatient prescriptions in three stages: before and after the pre-review system was implemented in the whole department, as well as the implementation of Six Sigma management method based on the pre-review. RESULTS The ITSA showed that the qualified rate of outpatient prescriptions in our hospital was stable around 86.56% before the pre-review was carried out in all departments. After the implementation of pre-review, the qualified rate of prescriptions in that month increased by 9.81%. In November 2021, after the implementation of define measure analyze improve control (DMAIC) management based on prescription pre-review, the qualified rate of prescriptions in the month increased to 98.07%, reaching the target value, and the qualified rate of prescriptions continued to increase with a monthly trend of 0.05%. CONCLUSION The implementation of prescription pre-review and Six Sigma management is conducive to the formation of a closed-loop management model of pre-review, in-process intervention, post-comment, result publicity, and timely communication and feedback, which significantly improves the level of rational drug use in the outpatient department, better standardizes the prescribing behavior of doctors, promotes the communication and cooperation between pharmacists and physicians, and ensures the medication safety of drug use for patients.

目的 分析前置审核系统及六西格玛管理模式对门诊用药医嘱合格率的干预效果,以期提高药学服务质量,保障患者用药安全、合理、有效。方法 收集北京医院2020年4月至2024年3月门诊处方合格率信息,利用中断时间序列分析(interrupted time series analysis, ITSA)的多阶段模型对前置审核系统全科室开展前、开展后,以及在前置审核基础上实施六西格玛管理法3个阶段门诊处方合格率的变化情况进行分析。结果 ITSA结果显示,在全科室开展前置审核之前,我院门诊处方合格率基本稳定,约为86.56%。开展前置审核之后,当月处方合格率较前提高了9.81%,且处方合格率以0.17%的增长率呈逐月上升的趋势。2021年11月,在处方前置审核的基础上结合定义-测量-分析-改进-控制(DMAIC)管理法实施后,当月处方合格率提升至98.07%,达到目标值,处方合格率以每月0.05%的趋势持续提升。。结论 开展处方前置审核,实施六西格玛管理有助于形成事前审核、事中干预、事后点评、结果公示、及时沟通反馈的闭环管理模式,显著提升了门诊合理用药水平,更好地规范了医生处方行为,促进药师与医师的沟通协作,保障患者用药安全。

目的 分析前置审核系统及六西格玛管理模式对门诊用药医嘱合格率的干预效果,以期提高药学服务质量,保障患者用药安全、合理、有效。方法 收集北京医院2020年4月至2024年3月门诊处方合格率信息,利用中断时间序列分析(interrupted time series analysis, ITSA)的多阶段模型对前置审核系统全科室开展前、开展后,以及在前置审核基础上实施六西格玛管理法3个阶段门诊处方合格率的变化情况进行分析。结果 ITSA结果显示,在全科室开展前置审核之前,我院门诊处方合格率基本稳定,约为86.56%。开展前置审核之后,当月处方合格率较前提高了9.81%,且处方合格率以0.17%的增长率呈逐月上升的趋势。2021年11月,在处方前置审核的基础上结合定义-测量-分析-改进-控制(DMAIC)管理法实施后,当月处方合格率提升至98.07%,达到目标值,处方合格率以每月0.05%的趋势持续提升。。结论 开展处方前置审核,实施六西格玛管理有助于形成事前审核、事中干预、事后点评、结果公示、及时沟通反馈的闭环管理模式,显著提升了门诊合理用药水平,更好地规范了医生处方行为,促进药师与医师的沟通协作,保障患者用药安全。

OBJECTIVE To analyze the intervention effect of prescription pre-review and Six Sigma management model on the qualified rate of outpatient prescriptions to improve the quality of pharmaceutical care and ensure the safety, rationality, and effectiveness of medication in patients. METHODS The qualified rates of outpatient prescriptions in Beijing Hospital from April 2020 to March 2024 were collected. The multi-stage model of interrupted time series analysis (ITSA) was used to analyze the changes in the qualified rate of outpatient prescriptions in three stages: before and after the pre-review system was implemented in the whole department, as well as the implementation of Six Sigma management method based on the pre-review. RESULTS The ITSA showed that the qualified rate of outpatient prescriptions in our hospital was stable around 86.56% before the pre-review was carried out in all departments. After the implementation of pre-review, the qualified rate of prescriptions in that month increased by 9.81%. In November 2021, after the implementation of define measure analyze improve control (DMAIC) management based on prescription pre-review, the qualified rate of prescriptions in the month increased to 98.07%, reaching the target value, and the qualified rate of prescriptions continued to increase with a monthly trend of 0.05%. CONCLUSION The implementation of prescription pre-review and Six Sigma management is conducive to the formation of a closed-loop management model of pre-review, in-process intervention, post-comment, result publicity, and timely communication and feedback, which significantly improves the level of rational drug use in the outpatient department, better standardizes the prescribing behavior of doctors, promotes the communication and cooperation between pharmacists and physicians, and ensures the medication safety of drug use for patients.

2017年北京深化医疗改革,2017年7月12日原国家卫生健康委员会发布《关于加强药事管理转变药学服务模式的通知》[1],标志着药学部门职能的拓展。自此,药学部不仅承担药品供应保障的职责,更逐步承担了用药安全保障。积极探索新的药学服务模式和促进药师转型势在必行。2018年7月10日,《医疗机构处方审核规范》发布,明确提出“药师是处方审核第一责任人,所有处方均应当经审核通过后方可进入划价收费和调配环节”[2]。庞大的处方量对单纯的人工审核模式造成了巨大挑战,因此,采用信息化手段,建立高效审方系统能够更好地辅助药师提高工作效率,保障临床安全用药。北京医院(以下称“我院”)药学部以原有的医院信息系统和合理用药系统为依托,建立处方前置审核系统,调整了门诊患者的就医流程,及时、有效地拦截不合理处方,并进行干预,逐步实现了全部门诊处方在患者缴费前审核,大大提高了处方的合格率,但依然存在诸多问题。六西格玛(6б)管理于20世纪90年代末在医疗管理领域开始应用,可广泛用于提高医疗机构服务质量。定义-测量-分析-改进-控制(DMAIC)是6б管理模式的主要工具.通过系统查找工作流程中影响质量的关键因素,并进行分析和整改,从而提高服务质量[3]
本研究以我院门诊处方合格率情况为样本,实施处方前置审核,针对处方点评和前置审核过程中存在的问题,运用6б管理法进行改进,并采用中断时间序列分析(interrupted time series analysis, ITSA)方法评估其干预效果,以期保障患者用药安全、合理、有效。
收集我院2020年4月至2024年3月门诊处方合理率信息。处方合理率由人工点评处方合理条目数除以当月抽取处方点评总条目数得出。每月处方抽样和处方点评均使用医院内置的符合国家卫健委处方审核标准的PASS管理系统处方点评模块。
处方前置审核系统是以说明书、指南等循证医学证据为依托,结合医院需求个性化规则设置对诊疗信息进行多维度点评的模式。2020年我院引入前置审核系统,通过不断与计算机室、信息中心、医生工作站沟通,于2020年12月实现医院全处方前置审核,所有处方经过前置审核后才进入收费和调配环节。处方前置审核工作模式流程图(图1)。
定义:系统学习药品说明书、《处方管理办法》《临床用药须知》《北京市医疗机构处方专项点评指南(试行)》、临床疾病诊疗指南等,作为处方点评的依据。不合理处方的判断分3个方面,分别是用药不适宜处方、不规范处方和超常处方。目前多数医院信息化改造工作发展较快,手写处方已逐步被计算机打印电子处方替代。因此在处方点评过程中主要审核用药不适宜处方和超常处方。在实施前置审核后,我院处方点评合格率虽有大幅提升,但维持在瓶颈阶段,部分科室处方点评合格率较低,无法突破,期望通过DMAIC管理将全院处方合格率再次提升,目标是达到98.00%以上。
测量:逐月查看2020年12月至2021年11月门诊处方点评情况及前置审核情况,收集整理每月处方点评结果统计表、点评结果统计(按处方科室统计)表、点评问题明细表和前置审核系统中不合理问题医嘱清单。
分析:处方点评小组认真分析不合理处方,共同探讨可能导致不合理处方产生的原因,结合测量数据利用鱼骨图进行根因分析[4]。从医师、药师、管理、工具和其他5个要素绘制鱼骨图(图2)。
改进:针对鱼骨图中发现造成不合理处方产生原因提出整改建议,逐步进行干预。建立处方前置审核、处方实时干预、处方点评、结果公示和定期沟通的闭环管理,以确保该方案能够满足质量改进目标。主要改进措施如下:①梳理:梳理所有点评不合理的处方,按科室进行整理,通过线上或线下方式与各临床科室进行沟通,概述不合理问题原因并给出相应建议;②记录:在处方点评的过程中,记录处方点评不合理但前置审核系统自动通过的信息,实时维护药品数据库,尽量避免前置审核过程中出现假阴性结果;③反馈:建立临床被动和主动反馈机制;被动反馈:定期收集前置审核系统中的高频提示信息,通过查看说明书,系统检索国内外数据库,查找相关指南、共识以及其他循证证据,建立共享规则库,分享至各科室征求临床专家建议,并结合药学建议,最终经过审核小组审核通过后,调整前置审核系统规则,减少假阳性结果;主动反馈:制定“前置审核-临床科室建议表”,临床科室根据本科室频繁弹出的超说明书用药,但自认合理的情况填写建议表,交由药学部门审核,审核通过后,调整前置审核系统规则。不断增强与医师的沟通,提高医师对前置审核提示的认可度;④审查:定期审查审核系统中的药品信息,及时进行药品配对维护;⑤培训:加强调剂药师和审方药师的培训和学习;⑥沟通:与医院管理部门进行沟通,定期将不合理处方信息公示,提高医师对处方规范性的重视程度;⑦限制:对部分药品的开具进行科室限制,避免跨科室开药产生用药错误;⑧强制:针对明确不合理的问题,通过系统规则设置成拦截模式,医生必须修改正确才能提交处方。⑨控制:采取上述改进措施后,制定控制模式,对每月产生的不合理问题进行实时监测与持续干预,不断发现问题、解决问题,及时总结,巩固成绩。
北京医院门诊处方合格率改善的2个干预点分别为2020年12月实现医院全部处方前置审核和2021年11月进行DMAIC流程管理。因此,采用ITSA的多阶段模型对前置审核前、前置审核后、前置审核联合DMAIC管理3个阶段的每月处方合理率进行分析,通过深入了解干预实施后处方合理率的瞬时变化和趋势变化,评价处方前置审核和DMAIC管理措施对处方合理率的作用效果,模型见公式1:

其中,Y是在等间隔时间下的观测结果;t为时间序列变量,从研究开始至研究结束;x为哑变量,表示干预情况;p为干预后的时间变量;ε为残差项。当拟合分段线性回归模型后,得到系数β05β0为截距项,表示处方合格率的初始水平;β1为干预前的斜率,表示处方前置审核干预前的合格率变化趋势;β2为干预的即刻效应,表示处方前置审核干预后与假设未进行干预的结果之差;β3为干预的持续效应,表示干预前后合格率变化趋势的斜率之差;β4为二次干预的即刻效应,表示在处方前置审核联合DMAIC管理后合格率的即刻变化量;β5为二次干预的持续效应,表示二次干预前后合格率变化趋势的斜率之差。
采用Excel 2013软件建立处方合理率数据库,利用Stata 16.0软件进行统计分析。选择actest检验验证时间序列是否存在自相关,并确定自相关阶数;使用Newey-West标准误处理自相关及可能的异方差;选用ITSA命令开展多阶段中断时间序列分析[5]
2020年4月至2024年3月北京医院处方合理率数据显示,2020年4月至2020年11月处方合理率为82.17%~88.20%,2020年12月至2021年10月为95%~97.22%,2021年11月至2024年3月为97.77%~99.28%。
通过actest检验发现时间序列存在自相关,最大滞后水平为1,选用ITSA的lag(1)命令的Newey-West模型开展多阶段中断时间序列分析,结果见表1
2020年4月至2020年11月,北京医院门诊的处方合格率水平约为86.56%,每月的变化趋势没有显著差异(P>0.05)。2020年12月,北京医院全科室上线前置处方审核,当月处方合格率较前提高了9.81%(P<0.01,95%CI=[7.62,12.00]),且处方合格率以0.17%(P<0.01,95%CI=[0.11,0.24])的增长率呈逐月上升的趋势。2021年11月,在处方前置审核的基础上结合DMAIC管理法实施后,处方合格率较前瞬时提高了0.30%(P=0.13,95%CI=[-0.10,0.71]),且处方合格率以0.05%(P<0.01,95%CI=[0.04,0.06])的增长率呈逐月上升的趋势。门诊处方合格率2020年4月至2024年3月的变化趋势情况见图3
国家新医改政策对医院药学服务提出了更高的要求,药学信息化辅助策略成为药学服务的重要工具。据全国临床安全用药监测网2018—2021年的数据统计,医生引发用药错误的占比逐年增高[6]。因此,采用信息化手段,建立高效审方系统能够更好地辅助药师提高工作效率,保障临床安全用药。处方前置审核系统是以医院信息系统和合理用药系统为依托,调整了门诊患者的就医流程,在患者缴费前及时、有效地拦截并干预不合理处方,大大提高了处方的合格率。但通过对比处方前置审核和处方点评的结果发现,系统常有假阳性或假阴性的问题发生。假阳性问题的频繁发生会降低医生对前置审核系统的认可度和药师的工作效率,给审方工作带来极大的不便。假阴性问题的持续发生,可能会影响患者的用药安全和处方合格率水平,使处方合格率水平保持在瓶颈阶段无法继续提升。基于此,在处方前置审核的基础上结合6б管理模式是优化完善处方审核规则,提高处方合格率的重要措施。6б管理模式通过DMAIC的5个步骤对不合理用药情况进行干预,从而持续推进处方审核流程优化,提高处方合格率水平,提升药学服务质量,形成事前审核,事中干预,事后点评,结果公示,及时沟通反馈的闭环管理,保障用药安全合理。
本研究应用中断时间序列分析方法,分析处方前置审核及6б管理模式对门诊处方合理率的干预效果。结果发现,在全科室开展处方前置审核干预前,处方合格率维持在86.56%左右,干预后处方合理率大幅提升,提高了9.81%,处方合格率以0.17%的趋势逐月提升。2021年5月至2021年10月,处方合格率一直保持在97.03%~97.22%波动,达到瓶颈期。在处方前置审核的基础上结合DMAIC管理模式,当月处方合格率提升至98.07%,达到预定目标值,并以每月0.046%的趋势提升。2023年1月,新冠感染的甲类防控措施解除,门诊量激增,导致部分处方的诊断书写不全,造成合格率略有下降。但经过一个月的调整,处方合格率恢复至稳步提升状态。近期的处方合格率最高达到99.28%,通过对各科室处方合格率的调查发现,各科室处方合格率水平都已达到95%以上。由此证明,处方前置审核系统结合DMAIC管理模式对处方合格率的提升颇有成效。
DMAIC管理模式是1个动态、持续改进的过程,需要多学科合作,充分利用信息化手段,在前置审核的基础上,加强与临床科室的沟通,提高药师的药学服务技能,完善合理用药管理制度,不断细化、更新合理用药绩效考核指标,有分工、有协作、有监管,处方合格率水平才能持续提升[7]。在DMAIC管理中,查找既往处方前置审核和处方点评等工作中存在的漏洞,并利用鱼骨图的形式对医师、药师、管理、工具和其他5个要素进行根因分析,并采取改进措施。首先,应加强药师的培训学习,提高药师的知识技能,审方药师应熟练掌握临床指南、治疗规范、国内外文献检索等循证证据的收集使用。针对前置审核系统中频繁提示的假阳性问题,建立审方规则库,共享至临床科室,收集临床专家意见并结合药学专家意见,达成共识后修改前置审核规则,以提高前置审核系统的工作效率与临床对该系统的认可度和满意度。这方面工作需要审方药师与医生的共同协作,实际工作中主要通过2条路径完成:①医生主动提交信息,医生在频繁接到处方不合理提示后,可以通过前置审核系统的线上沟通渠道或者再提交处方的“处方理由”处填写相关的证据与审方人员沟通;②审方人员定期收集系统提示不合理,但医生坚持要开具的处方,如“双签复核”后再次提交或进行“双签通过”的处方,根据药品等关键词进行指南、共识、文献检索,发现有权威证据后主动进行前置审核系统的维护。例如,泌尿外科为膀胱过度活动征患者开具托特罗定和米拉贝隆时,频繁弹出重复用药的提示,审方药师在共享规则库中录入该条信息及检索到的相关指南证据,得到临床专家和药学专家的认可后,将该审核规则修改。如,肾内科针对慢性肾衰合并难治性高血压患者,开具2种钙通道阻滞剂时系统频繁弹出重复用药,该科室主动向药学部提交处方前置审核问题建议表,并附相关证据,经药学专家认可后,修改规则。循证医学证据更新较快,处方前置审核系统是否能够及时更新用药证据也成为审方系统面临的巨大挑战之一。在日常工作中,多参考国内外已发表的指南、共识、国外批准用法、临床实践经验等,建立审方规则库,将合理的用药依据转化为规则并及时优化维护系统,可保证前置审核系统高效运行[8]。第二,在处方点评过程中发现的问题,应先追溯前置审核系统的审查结果,假阴性问题应及时弥补系统漏洞,未配对药品及时进行药品维护。由医师的不规范处方行为造成的处方不合理情况,由科室进行整理,通过线上或线下的方式定期与临床科室沟通反馈,并将处方点评结果交由医院管理部门进行公示,大大提高了医师对处方规范性的重视程度,同时提升了处方合理率,如,放疗科在为乳腺癌患者开具维生素B12溶液时,诊断仅写了原发疾病,通过处方点评发现未写明药品相关适应证现象,及时与放疗科进行沟通反馈,建议诊断可调整为原发诊断+放射性皮炎的形式,得到了临床认可,避免了该类问题的发生。第三,发现跨科室开药不规范的现象应及时干预。在处方点评过程中发现,部分西药科室在为患者开具中成药时,常出现多种中药重复使用的现象,如,为感冒患者同时开具连花清瘟、金花清感和复方双花属重复用药,通过前置审核系统规则对该类药物进行限制,避免了跨科室开药产生的错误。前置审核系统中,可能也会出现假阴性的情况。因此,每月对全院各科室处方分别进行事后抽样点评,针对处方点评过程中发现的明确不合理问题,通过前置审核系统设置了拦截模式,医生必须修改后才可顺利提交处方,如,硝苯地平控释片不可掰开服用,从审核规则的用法用量模块入手,对单次给药剂量进行了限定,避免了不合理用药现象。在采用上述改进措施后,定期对不合理问题进行持续干预,及时总结,巩固成果。
本研究的创新之处在于运用了6б管理模式结合前置审核系统对处方合理率进行干预,并利用中断时间序列模型对干预结果进行评价。医院作为特殊的服务机构,药学服务质量直接关乎患者的用药安全。6б管理法重点关注过程管理,通过对产生缺陷的根本进行原因分析,优化工作流程,从而提高服务质量。本研究的不足之处在于未对医师和患者进行满意度调查,药师的服务对象是医师和患者,虽然处方审核具有一定的强制性,但也应评估医师的接受程度并汲取改进建议,以优化完善前置审核系统和DMAIC管理流程,增强药师和医师之间的沟通协作。
综上所述,通过中断时间序列模型发现,我院处方合格率在前置审核系统全面上线后有了显著提高,结合DMAIC管理模式后,突破瓶颈,处方合格率达到目标值98%以上,并呈逐月上升趋势。在DMAIC管理过程中,提高了药学服务水平,增进了医师和药师之间的沟通,降低了不合理用药现象,从而保障患者用药安全有效。为实现与医生工作站、检验工作站等系统对接的多学科合作模式奠定基础,进一步完善了合理用药精细化闭环管理制度。
