OBJECTIVE To evaluate the testing capability of the vaccine quality control laboratories and vaccine manufacturer laboratories which participated in the proficiency testing of pH, osmotic pressure molar concentration and aluminum content of inactivated COVID-19 vaccine. METHODS Samples of inactivated COVID-19 vaccine were tested for uniformity and stability by F-test and t-test, and then distributed to participating laboratories by cold chain transportation. The abilities of the participating laboratories for determination of pH, osmotic pressure molar concentration, and aluminum content were evaluated by Z ratio. RESULTS A total of 30 laboratories across the country participated in this proficiency testing, and the pH and aluminum content assessment results were all satisfactory, while the osmotic pressure molar concentration results of two laboratories were unsatisfactory. After participating in measurement audits, satisfactory results were obtained. CONCLUSION The provincial vaccine quality control laboratories, and vaccine production enterprises participating in this proficiency testing have good testing capabilities and quality control levels.

目的 通过开展新型冠状病毒灭活疫苗pH值、渗透压摩尔浓度、铝含量的测定能力验证,评价疫苗质控实验室及疫苗生产企业实验室的检测能力。方法 采用F检验和t检验对新型冠状病毒灭活疫苗样品开展均匀性和稳定性考察,样品通过均匀性稳定性检验后冷链运输分发至各参加实验室。以Z比分数评价参加实验室的pH值、渗透压摩尔浓度、铝含量检测能力。结果 全国共有30家实验室参加本次能力验证活动,pH值、铝含量考核结果均为满意,2家实验室渗透压摩尔浓度考核结果为不满意,后经参加测量审核,获得满意结果。结论 参加本次能力验证的省级疫苗质控实验室以及疫苗生产企业具有较好的检测能力和质控水平。

目的 通过开展新型冠状病毒灭活疫苗pH值、渗透压摩尔浓度、铝含量的测定能力验证,评价疫苗质控实验室及疫苗生产企业实验室的检测能力。方法 采用F检验和t检验对新型冠状病毒灭活疫苗样品开展均匀性和稳定性考察,样品通过均匀性稳定性检验后冷链运输分发至各参加实验室。以Z比分数评价参加实验室的pH值、渗透压摩尔浓度、铝含量检测能力。结果 全国共有30家实验室参加本次能力验证活动,pH值、铝含量考核结果均为满意,2家实验室渗透压摩尔浓度考核结果为不满意,后经参加测量审核,获得满意结果。结论 参加本次能力验证的省级疫苗质控实验室以及疫苗生产企业具有较好的检测能力和质控水平。

OBJECTIVE To evaluate the testing capability of the vaccine quality control laboratories and vaccine manufacturer laboratories which participated in the proficiency testing of pH, osmotic pressure molar concentration and aluminum content of inactivated COVID-19 vaccine. METHODS Samples of inactivated COVID-19 vaccine were tested for uniformity and stability by F-test and t-test, and then distributed to participating laboratories by cold chain transportation. The abilities of the participating laboratories for determination of pH, osmotic pressure molar concentration, and aluminum content were evaluated by Z ratio. RESULTS A total of 30 laboratories across the country participated in this proficiency testing, and the pH and aluminum content assessment results were all satisfactory, while the osmotic pressure molar concentration results of two laboratories were unsatisfactory. After participating in measurement audits, satisfactory results were obtained. CONCLUSION The provincial vaccine quality control laboratories, and vaccine production enterprises participating in this proficiency testing have good testing capabilities and quality control levels.

疫苗接种是预防传染病最好的方法之一。我国具备全部疫苗品种的完善供应体系,目前有46家疫苗生产企业,能够生产超过60种疫苗用以预防34种疾病,年产能超过10亿剂[1]。我国已经于2014年、2017年和2022年三次通过了世界卫生组织(WHO) 的疫苗国家监管机构(National Regulatory Authority,NRA)认证,并按照NRA-全球基准评估工具(global benchmarking tool,GBT)的要求[2]对我国疫苗全生命周期进行监管。我国多家企业生产的多种类型新冠疫苗获得国家药品监督管理局附条件批准上市,新冠疫苗种类包括灭活疫苗、腺病毒载体疫苗、重组蛋白疫苗、流感病毒载体疫苗、mRNA疫苗等。新冠灭活疫苗通过了WHO的认证并被列入WHO紧急使用清单,在我国接种人数最多,同时应用在全球多个国家和地区预防和控制新冠疫情。
2020年12月11日,国家市场监督管理总局令第33号公布,修订后的《生物制品批签发管理办法》[3]旨在加强生物制品批签发工作监督管理,规范生物制品批签发行为,保证生物制品安全、有效。要求我国施行生物制品批签发制度对获得上市许可的疫苗采用资料审核、样品检验等方式进行监管,中国食品药品检定研究院(以下简称中检院)承担了疫苗的批签发检验工作。疫苗行业快速发展,为缩短批签发时间,保证疫苗的供应,我国加快省级药检机构的建设,增加批签发授权机构[4]。通过法律规定以及国家药监局的考核授权,逐步形成了以中检院为中心,围绕省级药检机构作为支持的疫苗质控网络实验室。2021年中检院对我国多家省级药检机构进行了新型冠状病毒疫苗检验相关培训,并进行了能力考核。北京市药品检验研究所、上海市食品药品检验研究院、湖北药品监督检验研究院等多家省级药检机构通过了国家药品监督管理局考核并获得批签发授权[5]
能力验证作为国际通行的手段,能够客观地考察参加实验室的质量控制能力。中检院为国家药监局指定的生物制品批签发机构,同时具备中国合格评定国家认可委员会(CNAS)认可的能力验证提供者(proficiency testing provider,PTP) 资质,能够为社会提供结果可信的能力验证计划[6-7]。pH值、渗透压摩尔浓度、铝含量是新型冠状病毒灭活疫苗的重要物理指标,为检验疫苗质控实验室及相关企业的检测质量水平,中检院于2022年组织实施最新一轮新型冠状病毒灭活疫苗pH值、渗透压摩尔浓度、铝含量的测定能力验证(编号:NIFDC-PT-351)。
本能力验证计划依照 ISO/IEC 17043《合格评定能力验证提供者通用能力要求》实施[8]
能力验证使用的样品为某企业提供的新冠灭活疫苗,中检院委托某物流公司冷链运输至各参加实验室。每参加单位发放1套。每套包含新冠疫苗供试品20支,西林瓶包装,为同一样品。
pH值检测参照《中国药典》2020年版三部0631 pH值测定法,建议使用量:5支样品混合后测定3次,报告结果;渗透压摩尔浓度检测参照《中国药典》2020年版三部0632渗透压摩尔浓度测定法,单位mOsmol·kg-1,测定3次,报告结果;铝含量检测参照《中国药典》2020年版三部3106 氢氧化铝(或磷酸铝)测定法,单位mg·m·mL-1[9]。建议使用量:5支样品混合后测定。要求参加实验室在收到样品后10个工作日内完成实验,并在能力验证网络平台上提交结果报告。
由能力验证组织者开展实验,随机取样10个样品,将单支样品按照新型冠状病毒灭活疫苗渗透压摩尔浓度检测方法进行测定,每个样品测定3次,对结果进行单因子方差分析(F检验),显著性水平取0.05。如样品之间无显著性差异,则表明样品均匀。
将样品委托某冷链物流公司,从北京以2~8 ℃冷链运输至广州、哈尔滨后,返回中检院,实验室2~8 ℃保存样品和冷链运输返回后样品均按照新型冠状病毒灭活疫苗渗透压摩尔浓度检测方法进行测定,2~8 ℃保存和运输至广州、哈尔滨后返回的样品各检测5支,每支样品测定3次。对结果进行t检验,显著性水平取0.05。如正常保存和运输后样品之间抗原含量无显著性差异,则表明样品稳定。
依据GB/T 28043-2019/ISO13528:2015和CNAS-GL002-2018,采用中位值和标准化四分位距法对参加实验室报告的结果进行统计分析,参加实验室报告结果的中位值(med)作为本次能力验证计划的指定值(X),标准化四分位距(NIQR)或连续性能力验证计划标准差作为能力评定标准差,计算各实验室结果的Z比分数(Z值)。
对本次能力验证计划参加者的检测结果,按公式1计算Z值:
Z= x - X σ
式中:x为参加实验室测试结果;X为指定值;σ为能力评定标准差。
本计划以下列准则评价样品的结果,即:
|Z|≤2 表明“满意”,无须采取进一步措施;
2<|Z|<3 表明“有问题”,产生警戒信号;
|Z|≥3 表明“不满意”,产生措施信号。
三项检测项目全部合格,结果判定为合格,有合格项和可疑项,结果判定为可疑,有不合格项,结果判定为不合格。
使用SPSS 20和Excel 2016软件对结果进行统计分析。
采用单因子方差分析(F检验)对样品渗透压摩尔浓度均匀性进行评价(表1),F(1.58)<F临界值(3.02),P值大于0.05,无显著性差异。上述结果表明样品均匀性满足能力验证的要求(表2)。
对正常保存样品和冷链运输后样品渗透压摩尔浓度进行比较。样品以2~8 ℃冷链运输至广州或哈尔滨后返回中检院样品渗透压摩尔浓度平均值为290和291 mOsmol·kg-1,正常保存的样品为291 mOsmol·kg-1(表3),冷链运输至广州或哈尔滨后返回中检院样品和正常保存样品渗透压摩尔浓度稳定性检验P值分别为0.391和0.821。样品在运输前后抗原含量均无显著性差异,表明样品稳定性可以满足能力验证的要求(表4~6)。
本次能力验证共有30家实验室报名,共收到30家实验室反馈的结果(2家单位报名2次,32份结果),28家实验室结果为满意,2家实验室渗透压摩尔浓度检测项目偏离中位值,为不满意结果。不满意结果的机构通过整改,参加测量审核,获得测量审核满意结果。各实验室结果见表7
对各实验室报告结果进行汇总分析,各实验室检测结果中位值为7.4,NIQR为0.074 13,以中位值7.4作为指定值,NIQR为标准差σ,各实验室评价结果均为满意。
各实验室检测结果中位值为289 mOsmol·kg-1,NIQR为0.1,以中位值289 mOsmol·kg-1作为指定值,NIQR为标准差σ,2家实验室评价结果在本次统计中存在偏离,为可疑结果。
对各实验室报告结果进行汇总分析,各实验室检测结果中位值为0.47 mg·mL-1,以中位值 0.47 mg·mL-1作为指定值,参考《预防用含铝佐剂疫苗技术指导原则》、GBT27404-2008《实验室质量控制规范》对准确度的要求以及持续性能力验证计划《NIFDC-PT-285疫苗铝佐剂中铝含量测定能力验证计划报告》能力验证标准差,经专家组讨论,本次能力验证铝含量项目标准差σ为0.01,各实验室评价结果均为满意。
本次能力验证共有32家实验室参加并反馈结果32份,包括21家省级疫苗质控实验室和7家药品生产企业和4家其他类型实验室。21家省级疫苗质控实验室结果均为“满意”,表明在省级药检机构,尤其是承担国家疫苗批签发任务的机构,具有良好的检测能力。
两家实验室结果离群,仅代表其所报送的结果在本次统计中存在偏离的情况,并不代表其检测结果一定错误。渗透压摩尔浓度的检测,该项目采用的标准为《中国药典》2020年版三部0632渗透压摩尔浓度测定法,该方法对仪器的准确性要求较高。1家企业实验室和1家其他类型实验室的检测结果偏高于本次能力验证各实验室的中位值,通过不符合分析,1家实验室由于参加机构在进行仪器校准时,所用的校准点离中位值289 mOsmol·kg-1较远,另1家实验室未进行期间核查,造成结果偏离。不满意结果的机构通过加强仪器和标准溶液的期间核查,对比仪器的检定校准结果和标准溶液的指定值,对仪器参数做相应调整,同时,在检测前,使用标准溶液对仪器进行校准的方式提高准确度。2家结果不满意实验室实施整改工作,通过参加渗透压摩尔浓度的测量审核,最终获得满意结果,确定整改有效。
