OBJECTIVE To investigate the effect of tea polyphenols (TP) on aspirin-induced injury of human gastric epithelial cells GES-1 and its molecular mechanism. METHODS GES-1 cells were treated with aspirin (19.27 mmol·L-1) and different concentrations of tea polyphenols (50, 100, 200 μmol·L-1) for 24 h. GES-1 cells were randomly divided into control group, aspirin group(19.27 mmol·L-1), aspirin 19.27 mmol·L-1+TP 50 μmol·L-1 group, aspirin 19.27 mmol·L-1+TP 100 μmol·L-1 group, aspirin 19.27 mmol·L-1+TP 200 μmol·L-1 group. Methyl thiazolyl tetrazolium (MTT) was used to detect the survival rate of GES-1 cells. Flow cytometry and TUNEL assay were used to detect apoptosis of GES-1 cells. Dichlorofluorescein diacetate (DCFH-DA) fluorescent probe was used to detect the levels of reactive oxygen species (ROS) of GES-1 cells. Corresponding kits were used to detect the levels of superoxide dismutase (SOD), catalase (CAT) and malondialdehyde (MDA) of GES-1 cells. Western blot was used to detect the expression levels of B-lymphocytoma-2 (Bcl-2), B-lymphocytoma-2-associated X protein (Bax), nuclear factor E2 related factor 2 (Nrf2) and heme monooxygenase 1 (HO-1) of GES-1 cells. RESULTS Compared with the control group, the apoptosis rate, apoptosis index, ROS, MDA levels, nuclear Nrf2/ cytoplasmic Nrf2 ratio, Bax, and HO-1 protein expression levels of GES-1 cells in 19.27 mmol·L-1 aspirin group were significantly increased, while the cell survival rate, Bcl-2 protein expression level, SOD and CAT levels were significantly decreased (P<0.05). Compared with 19.27 mmol·L-1 aspirin group, the apoptosis rate, apoptosis index, ROS, MDA levels and Bax protein expression levels in aspirin 19.27 mmol·L-1+TP (50, 100, 200 μmol·L-1)groups were significantly decreased, while the cell survival rate, nuclear Nrf2/cytoplasmic Nrf2 ratio, Bcl-2, Nrf2, HO-1 protein expression levels, SOD and CAT levels were significantly increased (P<0.05).CONCLUSION Tea polyphenols can alleviate aspirin-induced GES-1 cell damage, and the mechanism may be related to the activation of Nrf2/HO-1 antioxidant pathway and the inhibition of mitochondrial pathway mediated apoptosis.

目的 探讨茶多酚(tea polyphenols,TP)对阿司匹林诱导的人胃黏膜上皮细胞GES-1损伤的影响及分子机制。方法 采用阿司匹林(19.27 mmol·L-1)和不同浓度的茶多酚(50、100、200 μmol·L-1)处理GES-1细胞24 h,将GES-1细胞随机分为对照组、阿司匹林组(19.27 mmol·L-1)、阿司匹林19.27 mmol·L-1+茶多酚50 μmol·L-1组、阿司匹林19.27 mmol·L-1+茶多酚100 μmol·L-1组、阿司匹林19.27 mmol·L-1+茶多酚200 μmol·L-1组。采用四甲基偶氮唑盐(MTT)检测GES-1细胞增殖水平,采用流式细胞术和TUNEL实验检测GES-1细胞凋亡水平。采用二氯荧光素双醋酸盐(DCFH-DA)荧光探针检测GES-1细胞活性氧(ROS)水平。采用试剂盒检测GES-1细胞超氧化物歧化酶(SOD)、过氧化氢酶(CAT)、丙二醛(MDA)水平。采用蛋白免疫印迹(Western blot)检测GES-1细胞B淋巴细胞瘤-2(Bcl-2)、B淋巴细胞瘤-2相关X蛋白(Bax)、核因子E2相关因子2(Nrf2)、血红素单加氧酶1(HO-1)蛋白表达水平。结果 与对照组比较,阿司匹林组GES-1细胞凋亡率、凋亡指数、ROS、MDA水平、细胞核Nrf2/细胞质Nrf2比值、Bax、HO-1蛋白表达水平显著增加,细胞存活率、Bcl-2蛋白表达水平、SOD、 CAT水平显著降低(P<0.05);与阿司匹林组比较,阿司匹林19.27 mmol·L-1+茶多酚(50、100、200 μmol·L-1)组GES-1细胞亡率、凋亡指数、ROS、MDA水平、Bax蛋白表达水平显著降低,细胞存活率、细胞核Nrf2/细胞质Nrf2比值、Bcl-2、HO-1蛋白表达水平、SOD、 CAT水平显著增加(P<0.05)。结论 茶多酚可减轻阿司匹林诱导的GES-1细胞损伤,其机制可能与茶多酚激活Nrf2/HO-1抗氧化通路、抑制线粒体通路介导的细胞凋亡有关。

目的 探讨茶多酚(tea polyphenols,TP)对阿司匹林诱导的人胃黏膜上皮细胞GES-1损伤的影响及分子机制。方法 采用阿司匹林(19.27 mmol·L-1)和不同浓度的茶多酚(50、100、200 μmol·L-1)处理GES-1细胞24 h,将GES-1细胞随机分为对照组、阿司匹林组(19.27 mmol·L-1)、阿司匹林19.27 mmol·L-1+茶多酚50 μmol·L-1组、阿司匹林19.27 mmol·L-1+茶多酚100 μmol·L-1组、阿司匹林19.27 mmol·L-1+茶多酚200 μmol·L-1组。采用四甲基偶氮唑盐(MTT)检测GES-1细胞增殖水平,采用流式细胞术和TUNEL实验检测GES-1细胞凋亡水平。采用二氯荧光素双醋酸盐(DCFH-DA)荧光探针检测GES-1细胞活性氧(ROS)水平。采用试剂盒检测GES-1细胞超氧化物歧化酶(SOD)、过氧化氢酶(CAT)、丙二醛(MDA)水平。采用蛋白免疫印迹(Western blot)检测GES-1细胞B淋巴细胞瘤-2(Bcl-2)、B淋巴细胞瘤-2相关X蛋白(Bax)、核因子E2相关因子2(Nrf2)、血红素单加氧酶1(HO-1)蛋白表达水平。结果 与对照组比较,阿司匹林组GES-1细胞凋亡率、凋亡指数、ROS、MDA水平、细胞核Nrf2/细胞质Nrf2比值、Bax、HO-1蛋白表达水平显著增加,细胞存活率、Bcl-2蛋白表达水平、SOD、 CAT水平显著降低(P<0.05);与阿司匹林组比较,阿司匹林19.27 mmol·L-1+茶多酚(50、100、200 μmol·L-1)组GES-1细胞亡率、凋亡指数、ROS、MDA水平、Bax蛋白表达水平显著降低,细胞存活率、细胞核Nrf2/细胞质Nrf2比值、Bcl-2、HO-1蛋白表达水平、SOD、 CAT水平显著增加(P<0.05)。结论 茶多酚可减轻阿司匹林诱导的GES-1细胞损伤,其机制可能与茶多酚激活Nrf2/HO-1抗氧化通路、抑制线粒体通路介导的细胞凋亡有关。

OBJECTIVE To investigate the effect of tea polyphenols (TP) on aspirin-induced injury of human gastric epithelial cells GES-1 and its molecular mechanism. METHODS GES-1 cells were treated with aspirin (19.27 mmol·L-1) and different concentrations of tea polyphenols (50, 100, 200 μmol·L-1) for 24 h. GES-1 cells were randomly divided into control group, aspirin group(19.27 mmol·L-1), aspirin 19.27 mmol·L-1+TP 50 μmol·L-1 group, aspirin 19.27 mmol·L-1+TP 100 μmol·L-1 group, aspirin 19.27 mmol·L-1+TP 200 μmol·L-1 group. Methyl thiazolyl tetrazolium (MTT) was used to detect the survival rate of GES-1 cells. Flow cytometry and TUNEL assay were used to detect apoptosis of GES-1 cells. Dichlorofluorescein diacetate (DCFH-DA) fluorescent probe was used to detect the levels of reactive oxygen species (ROS) of GES-1 cells. Corresponding kits were used to detect the levels of superoxide dismutase (SOD), catalase (CAT) and malondialdehyde (MDA) of GES-1 cells. Western blot was used to detect the expression levels of B-lymphocytoma-2 (Bcl-2), B-lymphocytoma-2-associated X protein (Bax), nuclear factor E2 related factor 2 (Nrf2) and heme monooxygenase 1 (HO-1) of GES-1 cells. RESULTS Compared with the control group, the apoptosis rate, apoptosis index, ROS, MDA levels, nuclear Nrf2/ cytoplasmic Nrf2 ratio, Bax, and HO-1 protein expression levels of GES-1 cells in 19.27 mmol·L-1 aspirin group were significantly increased, while the cell survival rate, Bcl-2 protein expression level, SOD and CAT levels were significantly decreased (P<0.05). Compared with 19.27 mmol·L-1 aspirin group, the apoptosis rate, apoptosis index, ROS, MDA levels and Bax protein expression levels in aspirin 19.27 mmol·L-1+TP (50, 100, 200 μmol·L-1)groups were significantly decreased, while the cell survival rate, nuclear Nrf2/cytoplasmic Nrf2 ratio, Bcl-2, Nrf2, HO-1 protein expression levels, SOD and CAT levels were significantly increased (P<0.05).CONCLUSION Tea polyphenols can alleviate aspirin-induced GES-1 cell damage, and the mechanism may be related to the activation of Nrf2/HO-1 antioxidant pathway and the inhibition of mitochondrial pathway mediated apoptosis.

阿司匹林是常用的止痛药和消炎药,其在心血管疾病中应用广泛,然而,阿司匹林会使胃肠道产生严重的不良反应,严重限制了它的应用[1]。据报道[2]包括阿司匹林在内的非甾体抗炎药引发胃损伤的机制与其诱导胃黏膜上皮细胞脂质过氧化,增加氧化应激水平有关。许多中药都具有抗氧化功能,中药活性成分是目前胃部疾病研究的热点之一。茶多酚是一种从茶叶中提取的类黄酮化合物,它是一种天然的抗氧化剂[3],茶多酚可能通过抗炎、抗氧化、增强胃黏膜保护因子等方式减轻酒精性胃损伤[4]。茶多酚对阿司匹林所致胃部疾病的影响目前尚未有文献报道。Nrf2/HO-1信号通路是重要的抗氧化应激通路,是机体内源性抗氧化系统的核心调控元件[5],Nrf2/HO-1信号通路的激活可通过降低氧化应激水平减轻阿司匹林引起的胃黏膜上皮细胞GES-1的氧化应激损伤[6]
本研究探究茶多酚对阿司匹林诱导的GES-1细胞损伤的影响及Nrf2/HO-1抗氧化通路在其中的作用,为茶多酚在阿司匹林所致胃损伤中的开发利用提供理论基础。
人胃黏膜上皮细胞GES-1(美国ATCC细胞库);DMEM培养基、胎牛血清(美国Gibco公司);茶多酚(纯度≥98%,中国上海源叶生物科技有限公司);阿司匹林(纯度99%)、DCFH-DA荧光探针(美国Sigma公司);噻唑蓝(MTT)试剂、TUNEL染色液、DAPI染色液(上海碧云天生物技术有限公司);二喹啉甲酸(bicinchoninicacid,BCA)蛋白检测试剂盒(上海生工生物工程有限公司);兔抗人Bcl-2、Bax、Nrf2、HO-1多克隆抗体(美国Abcam公司);超氧化物歧化酶(SOD)、过氧化氢酶(CAT)、丙二醛(MDA)试剂盒(南京建成生物工程研究所)。BD FACSCanto Ⅱ型流式细胞仪(美国BD公司);ECLIPSE TS100型荧光显微镜(日本Nikon公司)。
GES-1细胞用含10%胎牛血清、100 U·mL-1青霉素和100 μg·mL-1链霉素的DMEM培养基,于37 ℃、5% CO2 培养箱中培养。取对数期的GES-1细胞,按是否用阿司匹林和茶多酚处理随机分为对照组(不用阿司匹林和茶多酚处理)、阿司匹林组(19.27 mmol·L-1)、阿司匹林19.27 mmol·L-1+茶多酚50 μmol·L-1组、阿司匹林19.27 mmol·L-1+茶多酚100 μmol·L-1组、阿司匹林19.27 mmol·L-1+茶多酚200 μmol·L-1组。各组细胞于37 ℃、5% CO2 条件下培养24 h。
收集培养24 h后的各组GES-1细胞,加入100 μL的MTT溶液,孵育4 h,弃上清后用二甲基亚砜重悬,酶标仪测定490 nm波长处的吸光度值,细胞增殖水平用细胞存活率(公式1)表示。

收集培养24 h后的各组GES-1细胞,用0.25%的胰蛋白酶消化,4 ℃、1 000 r·min-1离心10 min,PBS洗涤,离心收集细胞,加入500 μL结合缓冲液,然后加入5 μL 膜联蛋白V(Annexin V)-异硫氰酸荧光素(FITC)与5 μL PI,充分混匀后在1 h内置于流式细胞仪中,应用Cellauest软件检测各组细胞凋亡率。
收集培养24 h后的各组GES-1细胞,添加4%多聚甲醛固定20 min,磷酸盐缓冲液(PBS)洗涤,用含有3%双氧水的甲醇封闭10 min,PBS洗涤,加入0.1% TritonX-100冰上通透10 min,PBS洗涤,加入50 μL TUNEL染色液37 ℃避光孵育60 min,PBS洗涤,加入DAPI染色液避光孵育15 min,PBS漂洗,200倍荧光显微镜下观察拍照,凋亡细胞核呈绿色,随机选择6个视野,统计每个视野中凋亡细胞核数与总细胞核数,计算凋亡指数(公式2)。

收集培养24 h后的各组GES-1细胞,弃培养液,加入RIPA裂解液提取细胞总蛋白。用核蛋白抽提试剂盒获取细胞质与细胞核蛋白。采用BCA法检测蛋白质的浓度,SDS-PAGE分离蛋白,分离的蛋白凝胶电转至PVDF膜上,用5%脱脂牛奶室温封闭,加一抗稀释液(Bcl-2、Bax、Nrf2、HO-1的稀释度均为1∶2 000)在4 ℃孵育过夜,PBS洗涤后室温下孵育二抗稀释液(1∶5 000)1 h,PBS洗涤,滴加电化学发光(ECL)工作液,暗室X线胶片曝光,显影,定影,以GAPDH为内参,使用Image J图像分析软件测定各条带灰度值。
收集培养24 h后的各组GES-1细胞,弃上清,PBS洗涤,加入500 μL DCFH-DA工作液,孵育 30 min,PBS洗涤3次, 用荧光显微镜观察拍照,Image J软件进行荧光处理计算,用荧光强度值表示细胞内ROS水平。
收集培养24 h后的各组GES-1细胞,超声破碎,离心,收集上清液,按试剂盒说明书测定SOD、 CAT活性和MDA含量。SOD采用黄嘌呤氧化酶法,CAT采用紫外吸收法,MDA采用硫代巴比妥酸法。
采用SPSS 26.0软件分析实验数据。符合正态分布的计量资料以均数±标准差($ \bar{x} \pm s$)表示,多组间比较采用单因素方差分析,多组间两两比较采用SNK-q 检验,多个剂量组与1个对照组的比较采用Dunnett检验,以P<0.05为差异具有统计学意义。
用不同浓度的阿司匹林或茶多酚处理GES-1细胞24 h,MTT检测显示,随着阿司匹林浓度的增加,GES-1细胞存活率逐渐降低,且呈剂量依赖性(P<0.05),阿司匹林对GES-1细胞的半数抑制浓度(IC50)为(19.27±1.86) mmol·L-1。浓度在400 μmol·L-1以下的茶多酚对GES-1细胞存活率没有显著影响(P>0.05),为便于研究茶多酚对阿司匹林诱导的GES-1细胞损伤的影响,后续实验中阿司匹林浓度选取IC50的浓度19.27 mmol·L-1,茶多酚浓度选取对GES-1细胞没有细胞毒作用的50、100、200 μmol·L-1,见图1
MTT实验检测显示,与对照组比较,阿司匹林19.27 mmol·L-1组GES-1细胞存活率显著降低(P<0.05),与阿司匹林19.27 mmol·L-1组比较,阿司匹林19.27 mmol·L-1+茶多酚50 μmol·L-1组、阿司匹林19.27 mmol·L-1+茶多酚100 μmol·L-1组、阿司匹林19.27 mmol·L-1+茶多酚200 μmol·L-1组GES-1细胞存活率显著增加(P<0.05),见图2

DCFH-DA荧光探针检测显示,与对照组比较,阿司匹林19.27 mmol·L-1组GES-1细胞ROS、MDA水平显著增加,SOD、CAT水平显著降低(P<0.05),与阿司匹林19.27 mmol·L-1组比较,阿司匹林19.27 mmol·L-1+茶多酚50 μmol·L-1组、阿司匹林19.27 mmol·L-1+茶多酚100 μmol·L-1组、阿司匹林19.27 mmol·L-1+茶多酚200 μmol·L-1组GES-1细胞ROS、MDA水平显著降低,SOD、CAT水平显著增加(P<0.05),见图5

茶多酚在多种胃损伤疾病中发挥保护作用,如有研究表明茶多酚可通过抗炎、抗氧化及抑制胃酸分泌等方式保护胃黏膜,改善小鼠酒精性胃损伤[4,7]。茶多酚可通过调节胃酸环境、改善炎症减轻幽门螺杆菌感染所致胃部损伤[8]。但目前缺少茶多酚对阿司匹林所致胃损伤的影响方面的研究。本研究发现,茶多酚可显著促进阿司匹林诱导的GES-1细胞增殖并抑制细胞凋亡,表明茶多酚可减轻阿司匹林引起的胃黏膜上皮细胞损伤。Bax、Bcl-2是调控细胞凋亡的线粒体通路的重要成员[9],本研究中茶多酚可显著降低阿司匹林诱导的GES-1细胞Bax蛋白表达水平,增加Bcl-2蛋白表达水平,表明线粒体通路参与了茶多酚对阿司匹林诱导的GES-1细胞凋亡的调控。
氧化应激是指机体自由基生成增加或清除能力降低引起自由基在体内过度积累导致氧化损伤的过程,ROS是诱发氧化应激的高反应活性物质[10-11]。SOD、CAT是机体重要的抗氧化酶,MDA是脂质过氧化终产物,SOD、CAT、MDA还参与胃损伤的病理过程[12]。本研究发现,茶多酚可显著降低阿司匹林诱导的GES-1细胞MDA与ROS水平,并提高SOD、 CAT水平,表明茶多酚可降低阿司匹林诱导的GES-1细胞氧化应激水平。氧化应激是引起细胞凋亡、炎症反应的重要因素,也是胃损伤的重要机制之一,并参与药物对阿司匹林所致胃损伤的治疗过程,如卡维地洛减轻阿司匹林致大鼠胃损伤的机制与其抗氧化作用有关[13]。螺旋藻通过减轻氧化应激和炎症反应改善阿司匹林诱导的胃溃疡[14]。因而推测茶多酚减轻阿司匹林所致胃损伤的机制可能与其降低氧化应激水平有关。
Nrf2/HO-1信号通路是机体抵御氧化应激的重要通路,在正常细胞中Nrf2主要存在于细胞质中处于非活性状态,受到外来刺激后Nrf2转运至细胞核中与抗氧化反应元件(antioxidant response element,ARE)结合,促进下游多种抗氧化酶和Ⅱ相解毒酶基因的表达,进而增强细胞的抗氧化能力,HO-1是最易被Nrf2激活的抗氧化酶[15-16]。本研究显示,阿司匹林显著增加GES-1细胞核Nrf2/细胞质Nrf2比值和HO-1蛋白表达,表明Nrf2/HO-1抗氧化通路被阿司匹林激活。文献研究发现阿司匹林对胃黏膜组织或细胞Nrf2/HO-1通路的影响具有争议,如有文献[1,6]表明阿司匹林会增加胃黏膜组织或GES-1细胞Nrf2的表达,与本研究结果一致,但也有文献[17]表明短时间阿司匹林处理会使胃黏膜上皮细胞CRL-1739 中Nrf2的表达增加,长时间处理则会使Nrf2的表达降低,还有文献[18]表明阿司匹林会降低小鼠胃黏膜组织Nrf2的表达。这可能与细胞株、剂量、体内与体外实验等因素的差异有关。本研究中GES-1细胞Nrf2/HO-1抗氧化通路的激活可能是细胞为降低阿司匹林所致氧化应激的自我保护机制。本研究还发现,茶多酚可进一步激活阿司匹林诱导的GES-1细胞Nrf2/HO-1通路,推测茶多酚在本研究中的抗氧化作用与此有关。
综上所述,茶多酚可减轻阿司匹林诱导的GES-1细胞损伤,其机制可能与茶多酚激活Nrf2/HO-1抗氧化通路、抑制线粒体通路介导的细胞凋亡有关。本研究在细胞层面对茶多酚在阿司匹林所致胃损伤方面的研究进行了初步探索,下一步将进行动物实验,为茶多酚在阿司匹林所致胃损伤中的开发利用提供参考。
