OBJECTIVE To investigate the characteristics of severe cutaneous adverse reactions (SCARs) induced by proton pump inhibitors (PPIs) and provide references for clinical safe use of drugs. METHODS Case reports of SCARs caused by PPIs were retrieved from Pubmed, CNKI, Wanfang and VIP databases up to June 2024. Information such as demographic characteristics, drug use, occurrence time, types, clinical manifestations, treatment measures and outcomes of patients was collected, and descriptive statistical analysis was conducted. RESULTS A total of 22 patients were included. There were 8 males (36.4%) and 14 females (63.6%), aged from 23 to 89 years, with an average age of years. The PPIs involved included omeprazole in 10 cases, esomeprazole in 6 cases, pantoprazole in 3 cases, and lansoprazole in 3 cases. SCARs occurred from 1 to 45 days, with an average time of days after treatment, and 18 cases (94.7%) within 3 weeks. Ten cases were diagnosed as Stevens-Johnson syndrome (SJS)/toxic epidermal necrolysis (TEN), 7 cases were diagnosed as drug reaction with eosinophilia and systemic symptom (DRESS), and 5 cases were diagnosed as acute generalized exanthematous pustulosis (AGEP). The clinical symptoms and laboratory indicators of 18 patients (81.8%) were improved after discontinuation and treatment with glucocorticoid and/or globulin, but three patients died of multiple organ failure eventually, and one patient was discharged automatically. CONCLUSION SCARs can be caused by a variety of PPIs, with omeprazole and esomeprazole being relatively more common, and mainly manifest as SJS/TEN and DRESS. Patients should be closely monitored for any allergic symptoms such as fever and rash within 6 weeks of using PPIs. Once SCARs are suspected, the drug should be stopped in time, and symptomatic supportive treatment such as glucocorticoids should be provided if necessary.

目的 探讨质子泵抑制剂(proton pump inhibitors,PPIs)致严重皮肤不良反应(severe cutaneous adverse reactions,SCARs)的发生特点,为临床安全用药提供参考。方法 检索截至2024年6月收录在Pubmed、中国知网、万方和维普期刊数据库中PPIs致SCARs的病例报道,采集患者的人口学特征、药物使用情况、SCARs的发生时间、类型、临床表现、处置措施及转归等信息,并进行描述性统计分析。结果 共纳入22例患者,其中男性8例(36.4%),女性14例(63.6%),年龄23~89岁。涉及的PPIs包括奥美拉唑10例、埃索美拉唑6例、泮托拉唑3例和兰索拉唑3例。SCARs的发生时间为用药后1~45 d,其中18例(94.7%)发生于用药后3周内。10例诊断为史蒂文斯-约翰逊综合征(Stevens-Johnson syndrome,SJS)/中毒性表皮坏死松解症(toxic epidermal necrolysis,TEN),7例诊断为伴嗜酸性粒细胞增多和系统症状的药物反应(drug reaction with eosinophilia and systemic symptoms,DRESS),5例为急性泛发性发疹性脓疱病(acute generalized exanthematous pustulosis,AGEP)。18例(81.8%)患者经停药、糖皮质激素和/或球蛋白等治疗后临床症状和实验室检查指标均好转,但有3例患者最终死于多器官功能衰竭,1例患者自动出院。结论 多种PPIs可导致SCARs,以奥美拉唑和埃索美拉唑相对较多,且主要表现为SJS/TEN和DRESS。在使用PPIs的6周内应密切关注患者有无发热、皮疹等过敏症状,一旦怀疑可能为SCARs应及时停药,必要时予以糖皮质激素等药物对症支持治疗。

目的 探讨质子泵抑制剂(proton pump inhibitors,PPIs)致严重皮肤不良反应(severe cutaneous adverse reactions,SCARs)的发生特点,为临床安全用药提供参考。方法 检索截至2024年6月收录在Pubmed、中国知网、万方和维普期刊数据库中PPIs致SCARs的病例报道,采集患者的人口学特征、药物使用情况、SCARs的发生时间、类型、临床表现、处置措施及转归等信息,并进行描述性统计分析。结果 共纳入22例患者,其中男性8例(36.4%),女性14例(63.6%),年龄23~89岁。涉及的PPIs包括奥美拉唑10例、埃索美拉唑6例、泮托拉唑3例和兰索拉唑3例。SCARs的发生时间为用药后1~45 d,其中18例(94.7%)发生于用药后3周内。10例诊断为史蒂文斯-约翰逊综合征(Stevens-Johnson syndrome,SJS)/中毒性表皮坏死松解症(toxic epidermal necrolysis,TEN),7例诊断为伴嗜酸性粒细胞增多和系统症状的药物反应(drug reaction with eosinophilia and systemic symptoms,DRESS),5例为急性泛发性发疹性脓疱病(acute generalized exanthematous pustulosis,AGEP)。18例(81.8%)患者经停药、糖皮质激素和/或球蛋白等治疗后临床症状和实验室检查指标均好转,但有3例患者最终死于多器官功能衰竭,1例患者自动出院。结论 多种PPIs可导致SCARs,以奥美拉唑和埃索美拉唑相对较多,且主要表现为SJS/TEN和DRESS。在使用PPIs的6周内应密切关注患者有无发热、皮疹等过敏症状,一旦怀疑可能为SCARs应及时停药,必要时予以糖皮质激素等药物对症支持治疗。

OBJECTIVE To investigate the characteristics of severe cutaneous adverse reactions (SCARs) induced by proton pump inhibitors (PPIs) and provide references for clinical safe use of drugs. METHODS Case reports of SCARs caused by PPIs were retrieved from Pubmed, CNKI, Wanfang and VIP databases up to June 2024. Information such as demographic characteristics, drug use, occurrence time, types, clinical manifestations, treatment measures and outcomes of patients was collected, and descriptive statistical analysis was conducted. RESULTS A total of 22 patients were included. There were 8 males (36.4%) and 14 females (63.6%), aged from 23 to 89 years, with an average age of years. The PPIs involved included omeprazole in 10 cases, esomeprazole in 6 cases, pantoprazole in 3 cases, and lansoprazole in 3 cases. SCARs occurred from 1 to 45 days, with an average time of days after treatment, and 18 cases (94.7%) within 3 weeks. Ten cases were diagnosed as Stevens-Johnson syndrome (SJS)/toxic epidermal necrolysis (TEN), 7 cases were diagnosed as drug reaction with eosinophilia and systemic symptom (DRESS), and 5 cases were diagnosed as acute generalized exanthematous pustulosis (AGEP). The clinical symptoms and laboratory indicators of 18 patients (81.8%) were improved after discontinuation and treatment with glucocorticoid and/or globulin, but three patients died of multiple organ failure eventually, and one patient was discharged automatically. CONCLUSION SCARs can be caused by a variety of PPIs, with omeprazole and esomeprazole being relatively more common, and mainly manifest as SJS/TEN and DRESS. Patients should be closely monitored for any allergic symptoms such as fever and rash within 6 weeks of using PPIs. Once SCARs are suspected, the drug should be stopped in time, and symptomatic supportive treatment such as glucocorticoids should be provided if necessary.

质子泵抑制剂(proton pump inhibitors,PPIs)是胃壁细胞H+/K+- ATP酶的不可逆抑制剂,鉴于其起效快、抑酸作用强大而持久,且耐受性良好,是目前临床治疗和预防酸相关性疾病的首选药物[1]。早期研究显示,长期使用PPIs不良反应的发生风险仅为1%~3%[2],但随着临床应用日趋广泛,其导致的骨折风险及对钙吸收的影响、感染的风险和胃泌素升高及相关改变等已成为该类药物主要的安全性问题[3]。近年来,PPIs引起的皮肤不良反应也逐渐受到关注,发现其可从局部的皮肤不适症状到全身性的严重皮肤不良反应(severe cutaneous adverse reactions,SCARs)[4]。考虑到SCARs可能会危及生命并导致严重的、潜在的慢性后遗症[5],且属于PPIs罕见的不良反应,本研究拟系统检索国内外已发表的PPIs致SCARs病例报道,分析和探讨其发生特点,以期为临床进一步安全使用该类药物提供参考。
以“omeprazole”“esomeprazole”“pantoprazole”“lansoprazole”“rabeprazole”“ilaprazole”“proton pump inhibitors”“PPIs”“severe cutaneous adverse reactions”“Stevens-Johnson syndrome”“toxic epidermal necrolysis”“drug reaction with eosinophilia and systemic symptoms”“drug-induced hypersensitivity syndrome”“acute generalized exanthematous pustulosis” “SCARs”“SJS”“TEN”“DRESS”“DIHS”“AGEP”为检索词检索Pubmed数据库;以“奥美拉唑”“埃索美拉唑”“泮托拉唑”“兰索拉唑”“雷贝拉唑”“艾普拉唑”“质子泵抑制剂”“严重皮肤不良反应”“史蒂文斯-约翰逊综合征”“中毒性表皮坏死松解症”“伴嗜酸性粒细胞增多和系统症状的药物反应”“药物超敏反应综合征”“急性泛发性发疹性脓疱病”为检索词检索中国知网、万方以及维普数据库。检索时限自各数据库建库起至2024年6月。同时对纳入文献的参考文献进行手工补充检索。
①PPIs致SCARs的病例报道,包括个案或群案,SCARs包括史蒂文斯-约翰逊综合征(Stevens-Johnson syndrome,SJS)、中毒性表皮坏死松解症(toxic epidermal necrolysis,TEN)、伴嗜酸性粒细胞增多和系统症状的药物反应(drug reaction with eosinophilia and systemic symptoms,DRESS)以及急性泛发性发疹性脓疱病(acute generalised exanthematous pustulosis,AGEP)[6];②病例信息描述相对完整,至少应包含患者的性别、年龄、PPIs的品种、SCARs的类型及其临床表现等;③关联性评估结果为“可能”及以上。
①重复报道、会议摘要或论文集;②内容不相关或无法获取原文。
由两名研究者按预先制定的纳入与排除标准独立筛选文献并提取最终纳入文献的以下信息:作者及发表年限,患者性别、年龄、基础疾病和过敏史,PPIs的品种及合并用药,SCARs的类型、发生时间、临床表现、处置措施和转归等。同时,采用表皮坏死松懈症的药物因果关系算法(algorithm of drug causality for epidermal necrolysis,ALDEN)评分量表[7]评价PPIs与SJS/TEN的关联性(<0分为“极不可能”,0~1分为“不可能”,2~3分为“可能”,4~5分为“很可能”,≥6分为“非常可能”),采用诺氏评分量表[8]评价PPIs与DRESS和AGEP的关联性(≤0分为“可疑的”,1~4分为“可能”,5~8分为“很可能”,≥9分为“肯定”)。在此过程中如有分歧通过讨论协商解决。
检索共获得文献1 358篇,经剔除重复文献、阅读标题、摘要和全文后最终筛选出20篇病例报道[9-28],其中外文18篇[9-15,18 -28],中文2篇[16-17]。文献筛选流程见图1
共纳入22例患者,其中男性8例(36.4%),女性14例(63.6%),年龄23~89岁[13,27]。基础疾病以肿瘤、消化系统疾病和感染居多,分别有7例、7例和5例。10例患者提及有无过敏史,其中2例对PPIs过敏[10,22],2例对食物过敏[16,23],1例对磺胺类药物过敏[27],见表1
涉及的PPIs包括奥美拉唑10例(45.5%)、埃索美拉唑6例(27.3%)、泮托拉唑3例(13.6%)和兰索拉唑3例(13.6%),其中口服给药8例(36.4%),静脉给药4例(18.2%),另有10例未描述给药途径。16例患者有合并用药,以抗菌药物和非甾体类抗炎药居多,2例无合并用药,4例未提及。20例报道了SCARs的发生时间,其中1例[15]患者较为特殊,发生于用药49 d后。该患者开始服用奥美拉唑前就已经在使用甲基强的松龙64 mg·d-1治疗慢性炎症性脱髓鞘性多神经病变,直到甲基强的松龙减量至12 mg·d-1才开始出现SJS的皮损表现, Van tendeloo[15]认为持续的糖皮质激素治疗可能会延迟SJS的临床发作。排除该病例后,剩余的19例患者SCARs的发生时间为用药后1~45 d[17,20,27],其中18例(94.7%)发生于用药后3周内,见表1
10例(45.5%)患者诊断为SJS/TEN[9-18],均表现为大面积皮疹、红斑和表皮剥脱等典型皮损症状,其中5例伴发热,4例伴心动过速;所有患者均提及黏膜受损情况,其中累及2处及以上黏膜、1处黏膜和未累及的分别有8例、1例和1例;8例提及有器官受累,排名前三位的是口腔、眼和生殖器,分别有7例、6例和5例。7例(31.8%)患者诊断为DRESS[19-25],均有弥漫性丘疹这一皮损症状,其中6例伴发热,2例伴流感样症状,仅1例累及黏膜;所有患者均合并嗜酸性粒细胞增多,其中5例伴肝功能异常,4例伴肾功能异常,1例伴肌钙蛋白异常;7例提及有器官受累,排名前两位的是肝脏和肾脏,各有5例。5例(22.7%)诊断为AGEP[26-28],均有大面积红斑、针尖样脓疱等典型皮损表现,其中3例伴发热,1例伴白细胞增多,1例伴中性粒细胞增多。此外,有13例患者进行了皮肤活检,结果均支持SCARs的诊断[9-11,14-15,18,20,25-28],见表2
21例患者考虑可能为药物引起的SCARs后均立即停用了包括PPIs在内的可疑药物,其中1例因病情需要重启奥美拉唑后致DRESS加重[23],1例因3个月后误服埃索美拉唑致皮损再发[14],上述2例均再次停药。另有1例患者因同时使用了多种可能致SCARs的药物,医疗团队最初未将PPIs视为首要怀疑药物,但在后续治疗中及时发现并予以停用[19]。除停药外,多数DRESS(6/7,85.7%)和SJS/TEN(6/10,60.0%)患者使用了糖皮质激素,部分SJS/TEN患者还使用了人免疫球蛋白/丙种球蛋白(4/10,40.0%),仅2例AGEP(2/5,40.0%)患者分别使用了糖皮质激素和抗组胺药物。经上述处理后16例患者的临床症状和实验室检查指标均好转,4例转归不良,其中SJS、TEN和DRESS各有1例死于多器官功能衰竭[10,12,21],另有1例TEN因病情严重自动出院[16]。转归不良的4例患者中有2例既往存在药物或食物过敏史[10,16]。此外,所有患者中只有3例描述了发生SCARs后抑酸治疗方案的调整,均选择了H2受体阻滞剂[23-24,27]
根据“1.3”项下相应评分量表评估后,2例判定为“肯定/非常可能”,8例判定为“很可能”,12例判定为“可能”,见表1
PPIs自1980年推出以来其使用量急剧增加,如今已成为全球最常用的处方药之一[29]。期间,美国、欧盟和我国药品监督管理部门均先后发布了有关PPIs类药品的安全性信息[30]。此外,近年来国内外相关调查均显示PPIs的临床应用存在超适应证、超剂量、超疗程用药以及药物联合使用不当等情况,可能进一步增加患者用药的风险[31-32],尤其是我国将PPIs纳入辅助用药药品目录后,其安全性更是受到广泛关注[33]。虽然PPIs引起的速发型和迟发型超敏反应较为罕见,但由于PPIs的处方率和服用率极高,故其发生风险不容忽视,且如何正确识别并选择安全的替代药物更是至关重要[34]。本研究经系统检索后共纳入了22例PPIs致SCARs的病例报道,涵盖4种PPIs,涉及SJS/TEN、DRESS和AGEP 各种SCARs,现对其发生特点进行如下分析、讨论和总结。
SCARs作为一种迟发型超敏反应(delayed hypersensitivity reactions,DHRs)可分为4个亚型:①由1型辅助性T细胞介导的Ⅳa型;②由2型辅助性T细胞、白介素和嗜酸性粒细胞趋化因子共同介导的Ⅳb型(如DRESS);③由细胞毒性T细胞介导的Ⅳc型(如SJS/TEN);④由T细胞、中性粒细胞趋化因子8和粒细胞-巨噬细胞集落刺激因子共同介导的Ⅳd型(如AGEP)[35-36]。PPIs最常见的超敏反应是由免疫球蛋白E(immunoglobulin E,IgE)介导的速发型超敏反应,而DHRs较少见,其机制也尚未明确[34]。有研究显示,胃内低pH值(1.0~3.5)是激活胃蛋白酶所必需的,只有酸性乳糜才能刺激十二指肠分泌物和胰酶的释放,当抗酸药物抑制胃酸分泌时,与食物有关的过敏原可能保持完整并被吸收,从而促进过敏原致敏[37]。Lin等[38]则通过对PPIs致DHRs患者进行体外T细胞反应性研究发现其粒溶素释放倍数的增长显著高于健康对照组,提示粒溶素可能是PPIs致DHRs的关键因子之一。
根据苯并咪唑环和吡啶环上取代基的不同,PPIs可分为具有相似吡啶环上取代基的奥美拉唑相似组(包括奥美拉唑、埃索美拉唑和泮托拉唑)和苯并咪唑环上无取代基的兰索拉唑相似组(包括兰索拉唑和雷贝拉唑)[39]。有研究显示,对奥美拉唑过敏者更易对埃索美拉唑和泮托拉唑产生交叉过敏反应,但更能耐受结构不同的兰索拉唑;反之,对兰索拉唑过敏者也更易对雷贝拉唑产生交叉过敏[40]。本研究纳入的22例患者中,具有相似吡啶环取代的奥美拉唑、埃索美拉唑和泮托拉唑共有19例,占比明显高于苯并咪唑环无取代的兰索拉唑,而雷贝拉唑未找到相关病例报道,提示与奥美拉唑结构相似的PPIs致SCARs发生风险可能更高。此外,有针对PPIs相关性DHRs的研究发现成年女性的发生比例更高[34],而本研究纳入的病例中女性占比亦明显高于男性,提示女性可能是PPIs致SCARs的易发人群。
SCARs的治疗以对症支持为主。整体而言,AGEP和DRESS的症状相对较轻,一般采取退热和支持性护理[6]。一项回顾性研究显示停药足以控制AGEP的进展,局部糖皮质激素对患者有一定的获益,通常情况下不需要全身糖皮质激素治疗[41]。本研究纳入的5例AGEP患者中4例未采用糖皮质激素,且均预后良好,与文献报道一致。对于DRESS,全身应用糖皮质激素已被公认为是迅速改善急性期症状的标准治疗[42]。本文纳入的7例DRESS患者中6例使用了全身糖皮质激素,仅1例预后不良[21],另有1例虽经治疗后好转,但因长疗程(2个月)全身使用糖皮质激素治疗最终死于侵袭性肺曲霉菌病[22],提示临床亦应注意糖皮质激素的使用疗程和相关治疗风险。相比之下,SJS/TEN则是SCARs中最为严重的一种类型,需要皮肤和黏膜护理、肠内营养、补液、镇痛和抗焦虑等综合性支持治疗措施,但不建议预防使用抗生素,应尽量简化治疗方案[6]。对于SJS/TEN的系统性治疗效果,目前尚缺乏明确的高级别循证医学证据。基于SJS/TEN的免疫学机制,临床上的常用药物包括糖皮质激素、静脉注射免疫球蛋白、环孢素及肿瘤坏死因子α受体拮抗剂[43]。本研究纳入的10例SJS/TEN患者主要选择了全身糖皮质激素和/或人免疫球蛋白/丙种球蛋白治疗,最终3例转归不良,提示临床对于PPIs引起的该类型的SCARs应引起足够重视,早期准确诊断、及时停药并采取对症措施是挽救患者生命的关键。此外,鉴于本研究纳入的病例中有3例描述了后期将PPIs调整为H2受体阻滞剂[23-24,27],故笔者按“1.1”项下方法系统检索了H2受体阻滞剂(如西咪替丁、雷尼替丁、法莫替丁、尼扎替丁、拉呋替丁和罗沙替丁)的相关文献,结果发现多种H2受体阻滞剂可导致SCARs[44-46],提示临床如调整为该类药物同样应关注SCARs的发生风险。对于既往使用PPIs后发生过DHRs但必须再次使用PPIs的患者,2023年欧洲过敏及临床免疫学会发布的关于PPIs致过敏反应的意见书指出,可基于淋巴细胞活化试验和/或斑贴试验结果选择其他类型的PPIs[34]
①本研究为文献病例分析,仅纳入了公开发表的PPIs致SCARs的文献报道,病例数较少,故只能对其发生情况进行描述性分析,后续可通过检索美国食品药品监督管理局不良事件报告系统获取更多PPIs致SCARs的病例资料,如能进一步获得我国不良反应监测中心的相关病例将更有参考价值;②多数病例报道的原始文献中作者并未对其不良反应的关联性进行评估,虽然笔者根据相应的评估量表进行评价,但因部分病例的信息不够完整,评估结果可能与实际情况存在偏倚;③本研究英文文献只检索了Pubmed数据库,并未检索Embase和Cochrane Library数据库,故可能存在一定的发表偏倚。
综上所述,多种PPIs可导致SCARs,以奥美拉唑和埃索美拉唑的病例报道相对较多,且主要表现为SJS/TEN和DRESS。多数患者于用药后3周内发病,如出现发热、皮疹等过敏反应需引起警惕,一旦怀疑可能为SCARs应及时停药,必要时予以糖皮质激素等药物对症支持治疗。
