OBJECTIVE To construct a competency evaluation index system for clinical pharmacists. METHODS Based on previous research, the method of analytic hierarchy process was used to establish the evaluation hierarchy for competency of clinical pharmacists. A total of 20 experts were included, with an average age of (44.9±5.2) years and an average working seniority of (16.2±8.8) years. RESULTS The weight coefficients of 6 first class indictors, 18 secondary class indictors and 54 third class indictors were confirmed. The weight of each indictor was all consistent with the test. CONCLUSION This study preliminarily constructs a competency evaluation index system for clinical pharmacists, providing reference for the selection, evaluation, and training of clinical pharmacists, and further improvements are still needed.

目的 构建医院临床药师胜任力评价指标体系。方法 在前期研究基础上,利用层次分析法确定胜任力评价指标权重,构建医院临床药师胜任力评价指标体系。共纳入咨询专家20位,年龄(44.9±5.2)岁,工作年限(16.2±8.8)年。结果 最终确立了6个一级指标,18个二级指标和54个三级指标的权重系数,并满足一致性检验。结论 初步构建了临床药师胜任力评价指标体系,为临床药师人才选拔、评价培养等提供参考。

OBJECTIVE To construct a competency evaluation index system for clinical pharmacists. METHODS Based on previous research, the method of analytic hierarchy process was used to establish the evaluation hierarchy for competency of clinical pharmacists. A total of 20 experts were included, with an average age of (44.9±5.2) years and an average working seniority of (16.2±8.8) years. RESULTS The weight coefficients of 6 first class indictors, 18 secondary class indictors and 54 third class indictors were confirmed. The weight of each indictor was all consistent with the test. CONCLUSION This study preliminarily constructs a competency evaluation index system for clinical pharmacists, providing reference for the selection, evaluation, and training of clinical pharmacists, and further improvements are still needed.

随着经济社会发展、健康中国战略实施和医药卫生体制改革深化,人民健康需求日益增加,对药师队伍建设也提出了更高的要求。2015年国家卫生健康委、人力资源社会保障部等印发的《关于加强公立医疗卫生机构绩效评价的指导意见》[1],2017年国家卫生健康委、国家中医药管理局印发的《关于加强药事管理转变药学服务模式的通知》[2],均突出强调了药师等医务人员的绩效考核和薪酬管理,要求各地要重视药师队伍建设,保障并逐步提高药师待遇,吸引优秀药学人才,稳定药师队伍。2020年,经国务院批准,国家卫生健康委联合教育部等六部门共同印发的《关于印发加强医疗机构药事管理促进合理用药的意见的通知》[3]再次强调要加强药师队伍建设,优化药学部门人才结构,支持药学服务发展,激励药学人员在促进合理用药、减少资源浪费等方面发挥积极作用。目前,我国对于临床药师能力的考核也并无统一标准,缺乏对个人能力持续发展的关注,不利于药师自身的发展,更不利于药学人才队伍的建设。本研究基于前期制定的胜任力评价指标,采用层次分析法确定各级指标的权重,初步构建符合我国国情的医疗机构临床药师胜任力评价指标体系,为药学人才队伍建设提供参考。
咨询专家随机选自于三甲医院的专家共计20名。专家纳入标准:临床科室、药学部、职能部门等了解临床药师日常工作的人员;高级职称;硕士及以上学历;有5年以上工作经历。排除条件:因各种原因无法参与调研的人员。
前期的研究中,采用内容分析法制定临床药师胜任力评价指标。该指标包含提取职业素质、职业能力、职业发展、药学服务、公共健康、教研6个一级类目、18个二级类目及54个三级类目。该指标为运用层次分析法设置权重提供基础。依据层次分析法的原理,设计《临床药师胜任力评价指标权重调查问卷》,对临床药师胜任力指标相对重要程度进行两两比较。
本研究在前期研究所确定的临床药师胜任力评价指标的基础上,应用层次分析法确定模型各层次指标体系的权重,主要步骤:确定层次结构、确定两两比较矩阵、各层次因素的权重和组合权重的计算、比较矩阵的一致性检验。
使用Excel表进行数据录入,统计分析专家年龄、性别、职称、工作年限等。
构建指标体系层次结构,根据相对重要性计算Saaty标度,并构建判断矩阵,通过 Yaahp10.5软件获得评价指标体系中各指标的权重与组合权重。采用一致性指标(consistency index,CI)和一致性比例(consistency ratio,CR)检视评估矩阵的一致性。通常情况下CR值越小,则说明判断矩阵一致性越好,当CR<0.1时,说明该判断矩阵的重要度赋值符合数学逻辑,即各因素的权重分配是合理的,满足一致性检验;当CR>0.1时,矩阵应该作适当修正,直到一致性检验通过为止[4]
咨询专家随机选自于三甲医院的专家共计20名,其中男性6名,女性14名;学历分布:13名博士研究生、7名硕士研究生;职称:主任医师4名,副主任医师4名,主任药师5名,副主任药师5名,副研究员2名。 年龄(44.9±5.2)岁;工作年限(16.2±8.8)年。工作科室覆盖外科、内科、药学部和行政管理等部门。
共发放调查问卷20份,回收问卷20份,问卷有效回收率为100%。
根据前期研究结果,形成了医院临床药师胜任力三级指标[5]。目标层为研究的预定目标,本研究目标层为构建医院临床药师胜任力评价指标体系。准则层指为达到预期效果考虑的准则,本研究准则层为职业素质、职业能力、职业发展、药学服务、公共健康、教研6个一级指标,基本情况、职业素养、专业知识、专业能力等18个二级指标。方案层为达到预期目标采取的措施,本研究方案层为学历学位、专业技术职称、工作年限、医德医风等54个三级指标。
根据20位专家的问卷咨询结果,经过几何平均计算,得到各级别指标的两两比较矩阵。一级指标两两比较矩阵见表1,其余指标均参考一级指标方法构建两两矩阵,考虑文章篇幅有限,暂未列出。
根据上述三级指标的两两比较矩阵,利用前面介绍的方根法,可以得到 20 位专家对每层指标的权重,进一步对20名专家的权重结果进行合成,确定二级指标的组合权重,再通过乘积法计算整体评价指标体系的最终组合,见表2
本研究构建的判断矩阵经检验具有良好的一致性,CR值均小于0.1,通过矩阵的一致性检验,见表3
随着我国经济社会发展、疾病谱结构的改变等,我国对于合理用药和药学服务的需求日益增长,然而我国药学服务体系建设尚未满足人民群众的药学服务需求。2021年10月,国家卫生健康委印发了医疗机构药学门诊服务规范等5项规范[6],对推进药学服务规范化建设,促进患者合理用药,提升药学服务水平具有重要作用,但当前医院药学的发展还面临很多问题,如药师岗位能力未能完全匹配开展医院药学工作的需求、科学化精细化管理不够完善等[7]。胜任力是完成一项工作应具备的知识、技能、态度、特质以及动机等总和[8]。临床药师胜任力的构建可用于临床药师的培养、能力评价、绩效考核等多方面,对于临床药师开展相关工作有积极促进作用。本研究在前期研究的基础上,通过层次分析法,将专家主观判断进行两两比较[9],构建了临床药师胜任力指标体系。现将本研究构建的指标体系分析如下。
①职业素质。权重排序排在一级指标排在第6位。该指标主要是指临床药师工作中应具备一定的基本条件和职业操守,是从事临床药学工作的基本要求。其中医德医风和职业态度占比最重,在药学实践中,临床药师应与其他医务工作人员一样,爱岗敬业、诚实守信、严谨负责,坚持以患者为中心[10]。同时,从事临床药学服务应具备相关的专业技术资格[11]。②职业能力。权重排序在一级指标第2位。职业能力主要为开展药学服务实践所应具备的基本知识和能力,是开展药学服务的前提和基础。在专业知识方面,临床药学专业知识权重最高;在专业能力方面持续发展能力权重最高,国外医院药师胜任力标准中,均提及该项指标,药师应持续学习,更新专业知识、提高专业技能,不断为患者提供更优质的药学服务[12-15]。③职业发展。权重排序在一级指标第3位,为促进学科发展,临床药师应积极参加学术交流活动,介绍相关工作经验、工作模式、专业动态,与同行或医务人员分享、交流。鼓励临床药师申报各类奖项和学术兼职。④药学服务。权重排序在一级指标第1位。药学服务是临床药师向患者提供的药学专业技术服务,包括但不限于处方和用药医嘱审核、合理用药指导与教育、药物治疗管理、药学信息服务等,是临床药师的工作职责,也是临床药师价值的体现。《推进药学服务体系建设和医疗保障协同发展专家共识》提出,加强药学服务,提升药学服务能力必将成为控制不合理用药的重要环节,实现全面建成小康社会和社会主义现代化强国的重要保障[16]。2024年6月,为进一步加强医院临床药师工作,国家卫生健康委印发《关于开展驻科药师工作模式试点的通知》,强调驻科药师工作职责为审核用药医嘱、参加联合查房与会诊、开展药学监护和药物重整、为患者提供用药咨询和教育等[17]。本部分同时增加病区药品管理和满意度相关指标,旨在完善和进一步提升临床药师的药学服务质量。⑤公共卫生健康。权重排序在一级指标第5位。随着国家对居民健康素养的重视,大力推进健康知识普及及行动,鼓励医疗卫生人员参与健康科普作品创作[18]。临床药师作为医务人员应具备为公众提供药学科普的能力,也可根据实际情况参与各类科普活动举办。近年,国家高度重视公共卫生问题,并要求提升突发公共卫生事件应急处置能力[19],临床药师作为多学科治疗团队的一员,也应该利用专业知识,参与救治工作中。⑥教研。权重排序在一级指标第4位,是指临床药师能够传播临床药学知识、工作模式以及解决工作实践中发现问题的能力,是药学服务的拓展和升华。本部分中,科研相关指标权重较高。科研能力已成为衡量医院实力的重要指标,临床药师作为深入临床一线工作的药学人员,是药学部门科研力量的主力军,加强科研能力和成果的评估有利于药学学科建设与发展[20]
本研究初步构建了医院临床药师胜任力评价指标体系,对以往的胜任力评价模型进行了扩充与更新。各级胜任力指标矩阵模型均通过一致性检验,运用层次分析法得出的各级指标权重具有一定的客观性和科学性可为临床药学人才的选拔、培养、评价提供参考。
