OBJECTIVE To explore the in vivo pharmacokinetics and tissue distribution of zinc in different valence forms of arsenic in young rats after arsenic exposure in young rats, thus to provide scientific data to support the risk control and protection of arsenic exposure in young children. METHODS Twenty-four 3-week-old SD rats were randomly divided into three groups: a single-dose arsenic trioxide (ATO) group, a zinc pre-administration group, and an ATO+zinc co-treatment group. The rats were treated accordingly to their group assignments. High-performance liquid chromatography-inductively coupled plasma mass spectrometry (HPLC-ICP/MS) was used to measure the arsenic species and concentrations in plasma at different time points, as well as the arsenic accumulation in various organs. Pharmacokinetic parameters for different arsenic species were analyzed using DAS 2.0 software. RESULTS The pharmacokinetics of arsenic in plasma showed that 15 min after administration, the maximum concentration of trivalent arsenic (As) in plasma was highest in the ATO single-dose group, followed by the zinc pre-administration group, and then the ATO+zinc co-treatment group. The zinc pre-administration group reached lower levels 2 h after administration, earlier than the ATO group. Parameters such as ρmax, tmax, and t1/2 indicated that zinc supplementation accelerated the oxidation of As to pentavalent arsenic (As). The increase in the primary methylation index (SMI), the proportion of monomethylarsonic acid (MMA%), and the proportion of dimethylarsinic acid (DMA%), along with a significant decrease in the proportion of inorganic arsenic (iAs%), suggested enhanced methylation capacity. Tissue distribution measurements showed that zinc supplementation reduced the total arsenic (TAs) concentration in the brain, lungs, kidneys, testes, liver, and spleen. Notably, the brain, lungs, kidneys, and testes showed significantly lower TAs concentrations compared with the ATO single-dose group (P<0.05). CONCLUSION Zinc can effectively reduce the bioaccumulation of arsenic metabolites in the body by increasing the metabolic rates of As and As in plasma and organs, enhancing the methylation efficiency to achieve the protection against arsenic toxicity.

目的 探讨锌对幼龄大鼠砷暴露后不同价态砷的体内药动学及组织分布,为幼龄儿童砷暴露风险控制及防护提供科学数据的支撑。方法 24只3周龄SD大鼠随机分为三氧化二砷(arsenic trioxide, ATO)单次组、锌预给药组和ATO+锌共处理组等3组,按组干预,采用高效液相色谱-电感耦合等离子体质谱技术(HPLC-ICP/MS)测定各组不同时间点的血浆砷形态及浓度,测定各组脏器砷蓄积。采用DAS 2.0软件分析获取不同砷形态的药动学参数。结果 血浆砷代谢动力学显示,给药15 min,各组血浆中三价砷(As)最大浓度排序为ATO单次组>锌预给药组>ATO+锌共处理组,其中锌预给药组给药2 h后逐渐趋于低水平,早于ATO组的下降时间。ρmaxtmaxt1/2参数提示补充锌后加快了As向五价砷(As)氧化的速度;一级甲基化指数(primary methylation index,SMI)、单甲基砷比例(the proportion of monomethylarsonic acid,MMA%)、二甲基砷比例(the proportion of dimethylarsinic acid,DMA%)升高,无机砷比例(the proportion of inorganic arsenic,iAs%)显著降低则提示甲基化能力增强。组织分布测定结果表明,补充锌可降低全脑、肺脏、肾脏、睾丸、肝脏和脾脏组织的总砷(total arsenic,TAs)浓度,其中全脑、肺脏、肾脏、睾丸组织较ATO单次组显著降低(P<0.05)。结论 锌能通过提高As、As在血浆及脏器中代谢速度,增强甲基化效率,有效减少砷代谢物在体内的生物蓄积,以达到减轻砷毒性目的。

目的 探讨锌对幼龄大鼠砷暴露后不同价态砷的体内药动学及组织分布,为幼龄儿童砷暴露风险控制及防护提供科学数据的支撑。方法 24只3周龄SD大鼠随机分为三氧化二砷(arsenic trioxide, ATO)单次组、锌预给药组和ATO+锌共处理组等3组,按组干预,采用高效液相色谱-电感耦合等离子体质谱技术(HPLC-ICP/MS)测定各组不同时间点的血浆砷形态及浓度,测定各组脏器砷蓄积。采用DAS 2.0软件分析获取不同砷形态的药动学参数。结果 血浆砷代谢动力学显示,给药15 min,各组血浆中三价砷(As)最大浓度排序为ATO单次组>锌预给药组>ATO+锌共处理组,其中锌预给药组给药2 h后逐渐趋于低水平,早于ATO组的下降时间。ρmaxtmaxt1/2参数提示补充锌后加快了As向五价砷(As)氧化的速度;一级甲基化指数(primary methylation index,SMI)、单甲基砷比例(the proportion of monomethylarsonic acid,MMA%)、二甲基砷比例(the proportion of dimethylarsinic acid,DMA%)升高,无机砷比例(the proportion of inorganic arsenic,iAs%)显著降低则提示甲基化能力增强。组织分布测定结果表明,补充锌可降低全脑、肺脏、肾脏、睾丸、肝脏和脾脏组织的总砷(total arsenic,TAs)浓度,其中全脑、肺脏、肾脏、睾丸组织较ATO单次组显著降低(P<0.05)。结论 锌能通过提高As、As在血浆及脏器中代谢速度,增强甲基化效率,有效减少砷代谢物在体内的生物蓄积,以达到减轻砷毒性目的。

OBJECTIVE To explore the in vivo pharmacokinetics and tissue distribution of zinc in different valence forms of arsenic in young rats after arsenic exposure in young rats, thus to provide scientific data to support the risk control and protection of arsenic exposure in young children. METHODS Twenty-four 3-week-old SD rats were randomly divided into three groups: a single-dose arsenic trioxide (ATO) group, a zinc pre-administration group, and an ATO+zinc co-treatment group. The rats were treated accordingly to their group assignments. High-performance liquid chromatography-inductively coupled plasma mass spectrometry (HPLC-ICP/MS) was used to measure the arsenic species and concentrations in plasma at different time points, as well as the arsenic accumulation in various organs. Pharmacokinetic parameters for different arsenic species were analyzed using DAS 2.0 software. RESULTS The pharmacokinetics of arsenic in plasma showed that 15 min after administration, the maximum concentration of trivalent arsenic (As) in plasma was highest in the ATO single-dose group, followed by the zinc pre-administration group, and then the ATO+zinc co-treatment group. The zinc pre-administration group reached lower levels 2 h after administration, earlier than the ATO group. Parameters such as ρmax, tmax, and t1/2 indicated that zinc supplementation accelerated the oxidation of As to pentavalent arsenic (As). The increase in the primary methylation index (SMI), the proportion of monomethylarsonic acid (MMA%), and the proportion of dimethylarsinic acid (DMA%), along with a significant decrease in the proportion of inorganic arsenic (iAs%), suggested enhanced methylation capacity. Tissue distribution measurements showed that zinc supplementation reduced the total arsenic (TAs) concentration in the brain, lungs, kidneys, testes, liver, and spleen. Notably, the brain, lungs, kidneys, and testes showed significantly lower TAs concentrations compared with the ATO single-dose group (P<0.05). CONCLUSION Zinc can effectively reduce the bioaccumulation of arsenic metabolites in the body by increasing the metabolic rates of As and As in plasma and organs, enhancing the methylation efficiency to achieve the protection against arsenic toxicity.

砷是普遍存在环境中的一种金属元素,人类接触它的主要途径包括口服(水和食物)和吸入(粉尘)。砷暴露后广泛分布于人体多种组织和器官,例如血浆、尿液、心脏、肝脏和皮肤等,而器官特异性毒性通常与砷蓄积程度较高的组织有关。越来越多的证据表明,产前或儿童早期接触饮用水中的砷会对健康产生严重的长期影响[1-2],慢性砷暴露会引发儿童发育神经毒性、内皮功能障碍、致癌等不良反应[2-4]。此外急性早幼粒细胞白血病(acute promyelocytic leukemia,APL)在儿童群体发病率显著高于成年群体,三氧化二砷(arsenic trioxide, ATO)治疗儿童APL具有良好的临床疗效[5],因其本身具有毒性,而疗程较长又会加重砷的生物蓄积,更易引发毒性作用。因此无论是环境性还是药物性砷暴露,探索一种适宜于儿童砷暴露的减毒方案意义重大。
锌是一种必需的微量营养元素,由于其抗氧化特性[6],被认为具有保护作用,它也是幼龄儿童常用的一种膳食补充剂。现有的少量研究虽曾报道锌对砷的减毒作用,主要聚焦在成年大鼠,且仅关注了总砷(TAs)浓度的组织分布和生物积累[7]。关于幼龄动物补充锌后不同砷形态代谢物的药动学、组织分布及生物蓄积的研究还未见报道,考虑到砷形态在砷毒性的发生中扮演重要角色。基于此,本实验探讨了锌对幼龄大鼠砷暴露后不同价态砷的体内药动学及组织分布的影响,为幼龄儿童砷暴露风险控制及防护提供科学数据的支撑。
动物:SPF级雄性SD大鼠(前期数据表明,性别对砷代谢影响不显著),3周龄[斯贝福(北京)生物技术有限公司,动物许可证号:SYXK(赣)2022-0002]。动物实验程序和方案按照《护理和使用指南》进行实验动物,经江西中医药大学动物伦理委员会批准(批准文号JZLLSC20230354)。
药品与试剂:乙腈、甲醇(色谱级);枸橼酸(上海易恩化学技术有限公司,纯度:99%);己烷磺酸钠(上海展云化工有限公司,纯度:99%);氯化锌(上海易恩化学技术有限公司,纯度:99%);水为超纯水,背景检测无干扰。
仪器:高效液相色谱电感耦合等离子体质谱(HPLC-LCP-MS)(安捷伦科技公司)。低温高速离心机(贝克曼库尔特有限公司),超纯水处理系统(力康生物医疗科技控股有限公司)。
24只雄性SD大鼠适应性饲养1周后按体重随机分为3组:ATO单次组、锌预给药组、ATO+锌共处理组,每组8只。ATO单次组24 h内灌胃亚砷酸钠水溶液(2.5 mg·kg-1);锌预给药组连续灌胃氯化锌水溶液(1.5 mg·kg-1)7 d,第7天同时灌胃ATO;ATO+锌组共处理组连续灌胃ATO和氯化锌溶液7 d;实验结束后,禁食12 h取材。
称取动物体重,取出脑、心脏、肺脏、肝脏、脾脏、肾脏、睾丸称取重量,计算脏器指数{脏器指数(%)=[器官质量(g)/体质量(g)(死亡前)]×100%}。
在末次给药0.25、0.5、1、2、4、8、16、32、48 h,从动物眼眶采集0.2~0.5 mL的血样至肝素化的1.5 mL聚乙烯管中,在4 ℃下以12 000 r·min-1离心10 min,保存于-80 ℃。HPLC-ICP-MS分析时,每个时间点共测量8个重复(对应8只大鼠),使用每批样品运行的标准曲线中的方程评估各时间点血浆浓度,取平均浓度建立药动学时间曲线,并计算初级甲基化指数(公式1)、二级甲基化指数(公式2)、无机砷百分比(公式3)、单甲基砷百分比(公式4)、二甲基砷百分比(公式5)以评估砷代谢的甲基化效率。
初级甲基化指数(PMI)=单甲基砷(MMA)/无机砷(iAs)
二级甲基化指数(SMI)=二甲基砷(DMA)/MMA
无机砷百分比[iAs%)=(iAs/总砷(TAs)]×100%

大鼠眼眶取血结束后,腹腔注射戊巴比妥钠进行麻醉。取脑、心脏、肺脏、肝脏、脾脏、肾脏和睾丸,用预冷的0.9%生理盐水清洗组织残留血迹,滤纸干燥后称重,随后剪取0.5 g组织置于冻存管,保存于-80 ℃。使用每批样品运行的标准曲线中的方程评估各脏器内不同形态砷代谢物含量,并计算总砷(TAs=As+As+MMA+DMA)、无机砷(iAs=As+As)含量。
采用HPLC-ICP-MS,对可提取的砷代谢物进行测定。为测定所选组织及血浆样品中多种砷代谢物,将100 μL组织匀浆溶液(0.5 g组织-1 mL生理盐水)或血浆样品分别加入300 μL甲醛-乙腈溶液(7∶3),涡旋充分混合后在4 ℃下以12 000 r·min-1离心10 min沉淀蛋白质。取200 μL上清液在60 ℃环境中进行氮吹干燥,干燥后加入400 μL流动相(己烷磺酸钠+枸橼酸水溶液,pH 4.3)进行复溶,12 000 r·min-1离心5 min后取200 μL上清液,经过水系滤膜(0.22 μm)过滤后注入HPLC-LCP-MS系统。
根据实验动物数量,所有数据以均数±均数标准误差(SEM)表示。使用GraphPad Prism(8.0.2版)进行统计分析和图形展示。药物与统计(drug and statistics,DAS)软件(2.0版),采用非房室模型计算药动学参数。使用非配对t检验或单因素方差分析(ANOVA)评估差异的显著性,P值小于0.05被认为具有统计学意义。
血浆砷代谢动力学显示(图1),给药15 min,各组血浆中As最大浓度排序为ATO单次组>锌预给药组>ATO+锌共处理组,其中锌预给药组给药2 h后逐渐趋于低水平,早于ATO单次组的下降时间。另外,即使给药初期锌预给药组血浆As浓度最高,也会在给药6 h后低于ATO单次组。这种现象表明了锌预给药组血浆中As、As代谢速度较ATO单次组有所提高。各组MMA以较低水平存在,ATO+锌共处理组首次于0.25 h检测到。DMA作为砷主要代谢产物,它从血液中消除的速度低于iAs。共处理组血浆DMA浓度在给药1~48 h均处于高水平,并且始终高于ATO单次组,说明ATO+锌共处理组砷甲基化程度高于ATO单次组。值得注意的是,ATO+锌共处理组As、As血浆浓度降低时间与锌预给药组一致,但同一时刻浓度却更低,结合MMA、DMA血药浓度-时间曲线,表明ATO+锌共处理组提高As、As代谢速度、砷甲基化效率的能力优于锌预给药组。
血浆砷代谢物的药动学参数显示(表1),ATO单次组和锌预给药组给药后As最大血浆砷代谢物浓度(ρmax)、曲线下面积(AUC0-t)值和达到最大浓度的时间(tmax)分别为(21.06±4.18)vs(16.02±3.03)h、(210.67±29.26)vs(80.38±15.23)h、(2.00±1.13)vs(0.46±0.12)h,相较于ATO单次组,锌预给药组血浆中As的吸收似乎更快。此外还观察到该组As被迅速氧化为As,锌预给药组在(0.94±0.25)h到达ρmax(37.52±6.17)ng·mL-1,而ATO单次组则在(2.66±1.17)h时到达ρmax为(16.47±1.90)ng·mL-1。相较于As、As,各组中间代谢产物MMA的ρmax和AUC0-t较低,而主要代谢产物DMA的ρmax和AUC0-t最高。另外各处理组给药后血浆中DMA平均停留时间MRT0-t分别为(14.07±0.44) vs (15.15±1.07) vs (19.65±1.60)h,且ATO+锌共处理组在(0.69±0.12)h达ρmax(342.19±86.68)ng·mL-1,均高于ATO单次组及锌预给药组,以上结果表明该组对砷甲基化效率的提升有一定帮助。

基于以上数据可以看出,锌预给药组具备提高As、As代谢速度的能力,ATO+锌共处理组在提升As、As代谢速度、甲基化效率方面表现更优秀。表2的结果进一步支持了以上观点,与ATO单次组相比,锌预给药组和ATO+锌共处理组的SMI、MMA%、DMA%各有升高,iAs%显著降低。除此之外,ATO+锌共处理组PMI、MMA%、DMA%较锌预给药组提高,但并未出现组间差异,而iAs%明显下降。

每天监测大鼠的任何行为或临床毒性症状。在研究期间,ATO单次组、锌预给药组、ATO+锌共处理组给药后大鼠表现健康,没有观察到精神状态、行为或排便的变化。直至研究结束,实验过程中无大鼠死亡。
表3所示,锌预给药组及ATO+锌共处理组与ATO单次组相比,肺脏、肝脏、脾脏及睾丸的脏器指数略有下降。锌预给药组与ATO+锌共处理组相比,脏器指数未见明显差异。
ATO给药后大鼠脏器中TAs浓度排序如下:肺脏>肝脏>脾脏>肾脏>全脑>心脏>睾丸,锌预给药组TAs浓度排序为肝脏>肺脏>心脏>脾脏>肾脏>全脑>睾丸(图2A)。图2B显示,锌预给药组与ATO单次组相比,肝脏TAs比例由26.30%下降至23.75%,肺脏TAs比例由23.97%升高至43.23%,心脏由19.99%大幅下降为3.17%,说明锌预给药组TAs分布发生改变。同ATO+锌共处理组相比,预给药组心脏TAs比例由19.99%下降至12.68%,肝脏由26.3%上升至31.84%,这一结果可能与给药天数有关。另外,各处理组睾丸的TAs浓度及占比均位于末位。
了解脏器TAs生物蓄积情况,是证明锌对砷毒性保护作用的有力证据。从图2A可以看出,大鼠进行锌预给药处理后全脑、肺脏、肾脏、睾丸、肝脏和脾脏组织的TAs浓度降低,其中全脑、肺脏、肾脏、睾丸组织较ATO单次组显著降低(P<0.05)(图2C~2F)。除此之外,锌通过影响不同形态砷代谢物而展现其保护作用。从表4可以看出,大鼠全脑、睾丸组织可以检测到As、As、MMA、DMA 4种代谢物;而心脏、肺脏、肝脏、脾脏和肾脏组织中仅有As、As、DMA 3种。经过锌预给药处理后,心脏、肺脏、肝脏、脾脏和睾丸组织中iAs浓度较ATO单次组明显降低,说明锌预处理组脏器内As、As代谢速度增快。ATO单次组、锌预给药组中间代谢产物MMA均以较低水平存在。对DMA而言,ATO单次组全脑、肺脏、肾脏和睾丸浓度显著升高,这一结果与TAs生物蓄积情况相同。由此可见,锌预处理组脏器中DMA比ATO单次组更早、更快地排出动物体外。

基于砷的价态及其体内命运角度的减毒研究是对整体毒性评价层面的进一步深化。减毒取决于砷代谢水平的高低,而代谢水平又高度依赖代谢物的甲基化效率和价态。文献[8]研究表明,砷在体内外存在不同的价态,而不同的砷价态与砷的毒性密切相关。最近对野生型小鼠进行的一项研究发现,患糖尿病小鼠肝脏和胰腺砷代谢物主要为iAs,表明iAs的不良作用[9]。另一方面,iAs 进入机体后,通过甲基转移酶(AS3MT)将其酶促转化为MMA和DMA,因蛋白结合力降低及结合位点的减少,从而达到解毒的目的。研究表明,急性砷中毒期间儿童DMA%显著高于成人,而MMA%显著降低,相比之下成人症状进展更为严重[10]
有研究表明,长期砷暴露会引起儿童表观遗传修饰异常,导致DNA修复效率降低[11]。在细胞层面,暴露于砷对儿童先天免疫细胞、淋巴细胞等产生负面影响,从而损害身体对抗病原体的能力[12]。事实上,砷几乎会影响人体所有重要器官及组织,即使ATO在治疗儿童APL患者上存在显著效果,却仍存在诸如QT间期延长等不良反应。尽管这几项研究表明,无论环境性或是药物性砷暴露,均会对儿童健康产生严重影响,但现有研究对潜在的易感因素知之甚少,包括砷代谢的效率[13]。现有的研究结果表明幼儿尿砷代谢物似乎受同型半胱氨酸水平和性别等因素影响[14-15],这一点与成人不同;也有学者认砷甲基化效率与年龄相关,年龄较大会降低甲基化效率[16]。在本实验中,检测了单用ATO、锌预给药、ATO+锌处理后幼龄大鼠血浆及脏器组织中砷的不同形态代谢物,笔者认为锌可以通过影响砷不同形态代谢物的代谢水平从而展现其对砷毒性的保护作用,并评估各组iAs代谢水平、甲基化效率。血浆砷代谢物药动学显示,给药15 min,各组血浆中As最大浓度排序为ATO单次组>锌预给药组>ATO+锌共处理组,其中锌预给药组给药2 h后逐渐趋于低水平,早于ATO单次组的下降时间。随后,该组As在(0.94±0.25)h到达ρmax(37.52±6.17)ng·mL-1,表明As被迅速氧化为As,并在给药6 h后低于ATO单次组,这一现象证明了锌预给药处理能有效提高血浆中iAs的代谢速度。中间代谢产物MMA以低水平存在,而主要代谢产物DMA在给药0~48 h期间均以较高水平存在,特别是ATO+锌共处理组。值得注意的一点是,共处理组虽然与锌预给药组iAs浓度下降时间相近,但该组iAs%显著降低,PMI、MMA%、DMA%升高,表明ATO、锌共处理时不仅能提高血浆中As、As的代谢速度,还对甲基化效率的提升有积极影响。脏器分布及蓄积方面,锌预给药组通过降低脏器中iAs、DMA含量,以改变TAs在大鼠脏器中的分布,并有效减少全脑、肺脏、肾脏、睾丸TAs蓄积,这可能与锌是通过特定组织来影响砷代谢物水平的原因有关[7]。另外,各组仅在全脑及睾丸组织中检测到As、As、MMA、DMA 4种代谢物,这一结果与以往研究结果[17]不同,可能与动物年龄、ATO剂量、给药时间或样品分析方法的差异有关。
综上所述,本试验的结果表明,锌预给药处理能显著降低大多数脏器的总砷含量,改变组织分布,加速砷在血浆中的代谢速度。而ATO、锌共处理对提升血浆中iAs代谢速度、甲基化效率的能力优于锌预给药处理。研究的结果表明,锌能通过提高As、As在血浆及脏器中代谢速度,增强甲基化效率,有效减少砷代谢物在体内的生物蓄积,以达到对砷毒性保护的目的。这也为儿童口服ATO治疗APL的临床应用和长期安全性评价提供了初步依据。
