OBJECTIVE To establish the quality risk management mode based on failure mode and effect analysis (FMEA),and explore the application effect in the collinear production of external solution preparations. METHODS FMEA was performed on each link during the whole process of collinear production of external solution preparations. The high and medium risk failure modes were identified according to risk priority number (RPN), and failure causes were analyzed for formulating and implementing improvement measures. The risk level and the continuous verification effect of cleaning verification were evaluated before and after the implementation of control measures, the incidence of equipment failure, the qualified rate of first clearance, the rate of quality control compliance and the frequency of deviation treatment before and after the application of quality risk management mode were compared, and the improvement effect was examined. RESULTS A total of 21 failure modes were identified in the whole process of collinear production, of which 4 were high-risk, 11 were medium-risk and 6 were low-risk. The 15 items of medium and high risk failure modes were downgraded to low risk, and all cleaning verification indicators are controlled within the limits by continuous improvement of pollution and cross-contamination control measures, continuous supervision and research of collinear production risk control measures, continuous confirmation of equipment cleaning effect and other measures. Moreover, after the implementation of quality risk management mode, the incidence of equipment failure and the frequency of deviation treatment were reduced markedly, and the qualified rate of the first clearance and the quality control compliance rate were significantly increased, and the differences were statistically significant (P<0.05). CONCLUSION Applying the quality risk management model in the whole process of collinear preparation production can effectively reduce the risk of confusion, contamination and cross contamination, ensure the quality of collinear preparation products, and improve the operation level of the quality management system.

目的 建立基于失效模式与效应分析(failure mode and effect analysis, FMEA)的质量风险管理模式,探讨其在外用溶液剂共线生产中的实施效果。方法 对外用溶液剂共线生产的各个环节进行FMEA,根据优先风险数值(risk priority number, RPN)识别出中、高风险的失效模式,整理分析其失效原因,制定并实施改进措施。评估实施控制措施前后的风险等级、清洁验证持续确认效果、比较应用质量风险管理模式前后的设备故障发生率、首次清场合格率、质控达标率及偏差处理频率,评价改进效果。结果 外用溶液剂共线生产各环节共识别出21项失效模式,其中高风险4项,中风险11项,低风险6项。通过持续改进污染和交叉污染控制措施,持续开展共线生产风险控制措施的监督和研究、持续进行设备清洁效果确认等措施,中、高风险失效模式均降级为低风险。各项清洁验证指标均控制在限度范围之内。并且,实施共线生产质量风险管理模式后,设备故障发生率及偏差处理频率明显降低,首次清场合格率和质控达标率显著上升,且差异均具有统计学意义(P<0.05)。结论 将质量风险管理模式应用于外用溶液剂共线生产的全过程,能显著降低混淆、污染与交叉污染发生的风险,确保外用溶液剂的质量,提高质量管理体系的运行水平。

目的 建立基于失效模式与效应分析(failure mode and effect analysis, FMEA)的质量风险管理模式,探讨其在外用溶液剂共线生产中的实施效果。方法 对外用溶液剂共线生产的各个环节进行FMEA,根据优先风险数值(risk priority number, RPN)识别出中、高风险的失效模式,整理分析其失效原因,制定并实施改进措施。评估实施控制措施前后的风险等级、清洁验证持续确认效果、比较应用质量风险管理模式前后的设备故障发生率、首次清场合格率、质控达标率及偏差处理频率,评价改进效果。结果 外用溶液剂共线生产各环节共识别出21项失效模式,其中高风险4项,中风险11项,低风险6项。通过持续改进污染和交叉污染控制措施,持续开展共线生产风险控制措施的监督和研究、持续进行设备清洁效果确认等措施,中、高风险失效模式均降级为低风险。各项清洁验证指标均控制在限度范围之内。并且,实施共线生产质量风险管理模式后,设备故障发生率及偏差处理频率明显降低,首次清场合格率和质控达标率显著上升,且差异均具有统计学意义(P<0.05)。结论 将质量风险管理模式应用于外用溶液剂共线生产的全过程,能显著降低混淆、污染与交叉污染发生的风险,确保外用溶液剂的质量,提高质量管理体系的运行水平。

OBJECTIVE To establish the quality risk management mode based on failure mode and effect analysis (FMEA),and explore the application effect in the collinear production of external solution preparations. METHODS FMEA was performed on each link during the whole process of collinear production of external solution preparations. The high and medium risk failure modes were identified according to risk priority number (RPN), and failure causes were analyzed for formulating and implementing improvement measures. The risk level and the continuous verification effect of cleaning verification were evaluated before and after the implementation of control measures, the incidence of equipment failure, the qualified rate of first clearance, the rate of quality control compliance and the frequency of deviation treatment before and after the application of quality risk management mode were compared, and the improvement effect was examined. RESULTS A total of 21 failure modes were identified in the whole process of collinear production, of which 4 were high-risk, 11 were medium-risk and 6 were low-risk. The 15 items of medium and high risk failure modes were downgraded to low risk, and all cleaning verification indicators are controlled within the limits by continuous improvement of pollution and cross-contamination control measures, continuous supervision and research of collinear production risk control measures, continuous confirmation of equipment cleaning effect and other measures. Moreover, after the implementation of quality risk management mode, the incidence of equipment failure and the frequency of deviation treatment were reduced markedly, and the qualified rate of the first clearance and the quality control compliance rate were significantly increased, and the differences were statistically significant (P<0.05). CONCLUSION Applying the quality risk management model in the whole process of collinear preparation production can effectively reduce the risk of confusion, contamination and cross contamination, ensure the quality of collinear preparation products, and improve the operation level of the quality management system.

企业常通过优化管理及合理利用厂房、生产设施和设备,将多种产品共线生产以节约成本。药品生产质量管理规范(good manufacturing practice,GMP)(2010 年修订)第46条指出,应当综合考虑药品的特性、工艺和预定用途等因素,确定厂房、生产设施和设备多产品共用的可行性,并有评估报告[1]。因此,不同药品共线生产符合GMP的规定,但是这种生产模式可能会带来较大的质量风险,容易造成混淆和交叉污染。与药品生产企业相比,医疗机构制剂普遍存在规模小、批量小、效期短等问题[2],常需共线生产多个制剂品种,并且更换频繁。因此,对医疗机构制剂共线生产提出了更高的控制要求,必须做好全面地风险评估和制定有效的控制措施。
质量风险管理是一个全面地对风险进行识别、评估和控制的过程。现阶段《医疗机构制剂配制管理规范(试行)》中还没有关于质量风险管理的具体要求,依靠传统的经验管理不能全面地发现共线生产的质量风险[3-4]。失效模式与效应分析(failure mode and effect analysis, FMEA)是目前药品生产企业开展风险管理工作最常用的风险评估工具之一,具有前瞻性,通过确定失效模式相对应的风险等级,对不能接受的风险制定必要的控制措施,可以将风险降低到可接受范围内[1,5]。因此,借鉴药品生产企业的质量风险管理经验[6-8],结合我院外用溶液制剂的生产现况,运用FMEA工具建立可持续改进的质量风险管理模式,对控制外用溶液制剂共线生产带来的质量风险,进而系统、全面地把控外用溶液剂的质量具有一定的科学性和可行性。
风险评估小组由研发、生产、检验、质量、设备、软件、维保等技术人员组成,依据历史生产数据、偏差回顾、现场考察、生产经验等对共线生产过程中潜在的失效模式进行分析。对直接接触制剂的设备内表面开展清洁效果确认,检测20批制剂生产后的清洁效果。回顾性地收集2021年7月—2022年6月(模式应用前)和2022年7月—2023年6月(模式应用后)的外用溶液制剂的批生产记录、质控检查表、维修清单。并统计在上述时间段内,所有批次制剂生产期间设备故障的发生次数、首次清场即检查合格的次数、质控达标次数以及偏差处理的次数,并计算发生率。
我院生产的外用溶液剂包括硫酸镁溶液、硼酸溶液以及醋酸氯己定溶液,均为500 mL玻璃瓶装的外用液体制剂,用于清洗无破损的皮肤或腔道。主要成分(硫酸镁、硼酸、醋酸氯己定)无毒性、致敏性,不同品种之间不存在相互作用及配伍禁忌。3种制剂生产采用相同的工艺流程(图1),共用厂房、生产设施和设备。生产完成后对共用生产区、生产设施、设备及工器具彻底清洁,再进行下一品种的生产。该生产线投入使用前,已对3种制剂共线生产的可行性,生产区、设施及设备共线生产的适用性进行了评估和确认。本研究主要基于建立的质量风险管理模式分析共线生产产生混淆、污染和交叉污染的途径,科学确定残留的可接受限度,采取降低风险的控制措施,持续监控混淆、污染和交叉污染水平,确保风险得到有效控制,保障制剂质量和患者用药安全。
目前《医疗机构制剂配制管理规范(试行)》中还没有质量风险管理的具体要求,系统化、标准化的质量风险管理模式在医疗机构制剂领域应用相对较少[9-11]。但是,医疗机构制剂也属于药品的范畴,应该以先进的质量风险管理理念指导制剂生产过程。本研究根据质量风险管理在制药领域应用的基本工作方法[12],以及人用药品注册技术要求国际协调会(International Conference for Harmonization,ICH)中关于质量风险管理的相关规定(quality risk management-Q9,Q9)的相关内容[8],结合工作实际,建立基于FMEA的制剂共线生产质量风险管理模型(图2)[1,5],基本流程见图3
采用FMEA 工具从严重性(severity,S)、可能性(occurrence,O)、可检测性(detection,D)3个维度对每一个潜在失效模式的风险程度进行量化。对3个维度进行分级,分别给予“10分”“8分”“5分”“3分”“1分”的权重[6]。风险程度的大小用优先风险数值(risk priority number,RPN)来表示(公式1)。
RPN=S×O×D
RPN值越大表示该项失效模式的风险越高。根据RPN值的大小,可判断是否需要采取控制措施以及采取控制措施的紧迫程度,从而以较低成本减少事后损失。根据已有数据及生产经验,规定“RPN<27”为“低风险”,表示风险在可接受的范围内,当前的措施能有效控制风险;“27≤RPN<125”为“中风险”,表示必须采取措施,在合理的前提下将风险降低到尽可能低的范围;“125≤RPN≤1 000或S=10”为“高风险”,表示必须立即采取措施。
①外用溶液剂共线生产风险评估与再评价:共线生产的主要风险因素包括混淆、污染和交叉污染。混淆主要是指包装材料、原辅料、半成品及制剂成品的混淆。污染和交叉污染主要是指微生物或化学成分残留影响下一批次制剂产品的质量,产生的原因包括残留、机械转移和空气传播[12-13]。依据共线生产风险评估与控制模型(图2)及质量风险管理基本流程(图3),组织风险评估小组成员对外用溶液剂共线生产的各个环节进行分析,识别出每一道工序可能发生混淆、污染和交叉污染的失效模式。针对风险等级为中、高的失效模式,根据严重性、发生概率及可检测性采取与风险等级相适应的控制措施,主要降低风险发生的可能性或提高风险的可检测性,并在措施实施后重新进行风险评估,评价残余风险是否降低到可接受范围内;②质量风险管理模式的应用效果:风险控制措施实施后,评估高、中、低风险点的变化趋势,计算高、中风险点平均RPN值的变化幅度。分析质量风险管理模式应用前后,在预设时间段内,设备发生故障需要第三方维修的次数。每批制剂生产后由专人负责清场检查,记录首次清场即检查合格的次数。每周开展质控检查,统计质控检查的达标次数。查阅批生产记录,统计制剂生产全过程中开展的偏差处理次数。计算各个观察指标的发生率;③外用溶液剂共线生产持续清洁效果确认:通过清洁验证可以确保开发和设计的清洁工艺可重复产生预期的结果,通过持续监控清洁过程关键参数和清洁效果,可以确保清洁工艺处于受控状态。清洁验证主要验证化学残留和微生物残留,一般通过评估选择最差条件进行验证,确保残留不会影响到下一批产品质量或引入新的残留风险[14-16]。根据3种制剂成分(硫酸镁、硼酸、醋酸氯己定)的活性、毒性、致敏性、溶解度及性状等数据[17],选择醋酸氯己定为活性指标成分进行外用溶液剂生产线的清洁验证和持续清洁确认。醋酸氯己定的含量测定采用紫外分光光度法,线性范围为4.47~15.66 μg·mL-1。若检测数据低于定量限或未检出,则以低于线性范围最低值4.47 μg·mL-1出具报告。微生物检查采用薄膜过滤法,30~35 ℃培养5 d点计菌落数。擦拭样以浓度限度十万分之一(10×10-6)为化学残留限度,即上一批制剂残留在设备中的活性物质全部溶解到下一批制剂中的质量浓度不得高于10 μg·mL-1。根据下一批制剂最小批量、与制剂直接接触的设备内表面积、取样回收率等计算得到擦拭样的化学残留限度为7.53 μg·mL-1。冲淋水样化学残留限度以浓度限度十万分之一(10×10-6)为化学残留限度,即最终冲淋水化学残留限度不得过10 μg·mL-1
每次生产完成清洁后进行取样检测,按照冲淋水、目视、擦拭样的顺序进行取样。取样时应先取空白样品,再取冲淋水样品,避免污染。其中搅拌桨、配液灌内表面、板框过滤器、计量泵及灌装针头与物料直接接触,取擦拭样。出料口及灌装针头为生产线的取样及出料部位,取冲洗水可代表整个设备及管道的清洁效果。取样结束后立即将所取样品送至药检室或至冰箱2~8 ℃保存,样品取样结束时间至检验开始不得过24 h。汇总20批制剂生产完成后的微生物及化学残留检测结果,评价外用液体制剂生产线的持续清洁确认效果。
采用SPSS 30.0统计软件进行统计分析。计量资料采用Shapiro-WilK检验进行正态性检验,正态分布资料以$\bar{x}±s$表示,两组间比较采用两独立样本t检验。计数资料采用频数或百分率描述,采用卡方检验。P<0.05为差异具有统计学意义。
外用溶液剂共线生产各环节共识别出21项失效模式,在现有的控制条件下,对列出的失效模式进行预分析,筛选出高风险4项,中风险11项,低风险6项,具体见表1。通过持续改进污染和交叉污染控制措施,持续开展共线生产风险控制措施的监督和研究、持续进行清洁效果确认(表2),15项高、中风险失效模式均降低为低风险,平均RPN值由128.00±48.28降低为15.47±8.26,与采取措施前相比,差异具有统计学意义(P<0.05)(表3)。虽然以上措施不能改变风险发生引起危害的严重性程度,但采取措施后的RPN 水平由高、中变为低,这些风险在采取控制措施后是可以接受的,并且在实施过程中未引入新的风险。
表3所示,与质量风险管理模式应用前相比,设备故障发生率、偏差处理频率显著下降,首次清场合格率、质控达标率得到明显提高,差异具有统计学意义(P<0.05)。以上结果说明运用质量风险管理模式,能够预判共线生产各个环节的潜在风险,并且采取的控制措施切实有效,同时有效地提高了制剂生产管理和质量管理层次。
经评估选择冲淋水取样位点2个,擦拭样取样位点5个。汇总20批制剂生产后的设备清洁检测数据,结果见表4,清洁后设备内表面目测无明显可见残留物。冲淋水及擦拭样的化学残留均低于线性范围的下线4.47 μg·mL-1。冲淋水样品澄清无色、无杂质,电导率达到纯化水标准[≤5.10 μS·cm-1(25 ℃)]。冲淋水及擦拭样的微生物残留均低于限度标准。表明制定的清洁程序有效且处于持续受控状态。
质量风险管理不仅可以控制潜在的质量风险,还可在出现问题时改善决策过程。基于FMEA 建立的制剂共线生产质量风险管理模式是一种前瞻性的、简便易用的、可量化的风险管理工具,将其应用于外用溶液剂共线生产的全过程,在潜在问题发生前,便可采取控制措施将风险降至可接受范围,实现了质量风险管理的最终目的,使成本-效益最大化。本研究建立了风险评估与控制模型,设计了风险评估控制表,制定了风险评估控制方案,对共线生产质量风险管理模式进行了案例实现,提高了制剂生产管理和质量管理层次,形成了良性的生产质量持续改进循环。本文所提出的质量风险管理模式,虽然是针对外用溶液剂共线生产的,但其基本做法和框架结构可用于其他生产过程和管理方向。
需要注意的是,该模式实施过程中,风险评估小组应当由不同专业背景和经验的人员组成,依靠与各个领域的专业人员沟通并尽可能地获取充分的数据和信息,通过平衡利益与投入关系,制定科学的关于评估对象的风险可接受标准,客观地评判评估对象的潜在的失效模式。并且,在控制措施实施后需要重新进行风险评估,确保残余风险降低到可接受范围内。如果风险仍不能接受,则需采取后备措施直至风险可被接受,否则需要中止或暂停项目。同时,在采取降低风险的行动时,可能会给原来的体系带来额外的潜在质量风险点。在采取降低风险的行动后,需要对该行动进行评估,确认其风险等级。另外,风险评估过程需要定期进行回顾,避免因评估时的一些评价标准改变而引起评估结果的变化,同时也防止评估对象因管理方式或工艺控制发生改变,影响原评估结果。
本研究基于浓度限度的十万分之一来设定指标成分的残留标准,对于大多数化合物来说,这种传统限度值相当于每日允许暴露量(permitted daily exposure,PDE)值是相对严格的[12]。但是对于某些抗生素类、细胞毒类抗肿瘤药物或高活性产品,如果采用传统方法设定的限度值,对患者长期暴露可能起不到充分保护,这种情况下,使用药品的毒理学关注阈值、药品职业接触限值、或基于健康的暴露限度的可接受标准(如PDE值)等方法在评估清洁残留数据时更具有科学性和优势。
