Red yeast rice is a kind of traditional Chinese medicine(TCM) commonly used in China and all over the world in medicine, food additives, fermented food and other fields, but the research in this TCM is far from enough. Recently, the safety of red yeast rice health care product has aroused great concern around the world. This article reviews studies on the applied history, fermentation processing technology, material basis and quality control status of red yeast rice, aiming to clarify the factors that may lead to the risk of quality and safety of red yeast rice, and provide references for the safety quality control and risk prevention and control of red yeast and its products in China.

发酵类中药红曲食药用历史悠久,在国内外被广泛应用于医药、食品添加剂、发酵食品等领域,但目前对其研究尚不深入。近来,红曲作为保健食品的安全性问题引起国际上高度关注。本文综述了红曲的食药用历史、生产工艺、发酵产物和物质基础研究及质量安全控制现状,旨在探究可能导致红曲质量安全风险的因素,为我国红曲及红曲产品的安全性质量控制及风险防控提供参考。

Red yeast rice is a kind of traditional Chinese medicine(TCM) commonly used in China and all over the world in medicine, food additives, fermented food and other fields, but the research in this TCM is far from enough. Recently, the safety of red yeast rice health care product has aroused great concern around the world. This article reviews studies on the applied history, fermentation processing technology, material basis and quality control status of red yeast rice, aiming to clarify the factors that may lead to the risk of quality and safety of red yeast rice, and provide references for the safety quality control and risk prevention and control of red yeast and its products in China.

近期,日本出现多人在服用小林制药公司生产的含红曲成分保健品后因肾脏疾病住院甚至死亡的情况。据人民网报道[1],截至2024年6月26日,服用小林制药含红曲成分问题保健品的消费者中,已有289人住院治疗。小林制药公司总部调查问题原因时发现其2023年4月~10月生产的红曲原料样品中检测出“软毛青霉酸”。该事件引发日本国内外高度关注,再次敲响发酵类中药及保健品安全警钟。
红曲是将红曲菌(Monascus spp.)接种于大米经过发酵后的米面曲类产品,在我国已有千年应用历史,是一种传统的食疗兼备中药,具有健脾消食、活血化瘀等功效。发酵是将待炮制品加规定的辅料拌匀后,制成一定形状,置适宜的湿度和温度下,使微生物生长至其中酶含量达到规定程度后,晒干或低温干燥而得[2]。目前,红曲是应用面最广的曲类产品,既在临床使用,也作为原料广泛应用于中成药和保健食品的生产。各地红曲菌种及发酵工艺不尽相同,导致质量参差不齐。红曲发酵过程中产生莫纳可林类、红曲色素等具有一定药理活性的次级代谢产物,同时可能产生具有毒性的桔青霉素等真菌毒素[3],给公众健康及生命安全带来隐患。本研究对红曲的食药用历史、生产工艺、发酵产物和物质基础研究及质量安全控制现状进行综述,探究可能导致红曲质量安全风险的因素,以期为我国红曲及红曲产品的监管与质量控制提供参考。
红曲起源于我国,其食用、药用历史至少有一千年,广泛用于酿酒、食品着色、调味剂、肉类保存和中医药等方面。东汉末年《七释》曰:“西旅游梁,御宿素粲,瓜州红麹,参糅相半。软滑膏润,入口流散。”这可能是关于红曲的最早记载[4]。唐宋以后,红曲“健脾消食、活血化瘀”的药用功能渐渐被挖掘,元代《本草衍义补遗》[5]云:“活血消食,健脾暖胃,治赤白痢,下水谷,陈久者良。”
南北朝中药炮制学专著《雷公炮炙论》[6]中记载了“曲”的制作工艺:“麹,凡使,捣作末后,掘地坑,深二尺,用物裹,内坑中至一宿,明出,焙干用。”其中,麹是“麯”(简化字作“曲”)的异体字,本义指酿酒或制酱时引起发酵的东西,即酒母。古代红曲的炮制方法可以简单概括为浸米、蒸饭、拌种培养、清洗和干燥。《天工开物》[7]中记载:“凡造此物,曲工盥手与洗净盘簟,皆令极洁,一毫滓秽,则败乃事也”,强调用料及环境的清洁,避免杂菌污染。
传统红曲炮制采用“种类不明的混合微生物”的复合菌种进行自然发酵[8],其中能使稻米长出紫红色霉衣的关键菌种红曲菌可能来自于霉变的谷物或是由稻米经红土掩埋发酵所形成。先人筛选红曲的标准主要还是根据其红色的外观,因此,传统红曲生产用的曲种中势必混有多种外观相似但种类不同的菌株。此外,受限于当时的科技水平,无法实现对“浸、蒸、发酵、洗、晒”过程中水分、温度及洁净程度的准确控制,难以避免杂菌污染,所生产的红曲质量稳定性较差。
自然菌种发酵过程中易受杂菌污染,可能导致红曲质量不稳定,存在一定的安全风险问题。近年来随着微生物学、酶工程、现代发酵工程等现代生物技术的快速发展,越来越多研究者对曲类中药的现代发酵工艺进行探索,通过优选控制发酵菌种,改进发酵工艺参数等方法,以期提高红曲的质量、安全性和稳定性[9]
曲类中药的现代发酵炮制借助微生物的自身代谢及其产生的胞外胞内酶,对生产原料中化学成分进行分解、加工、结构修饰,从而产生新的活性成分及多种次级代谢产物[10]。红曲发酵是多因素共同作用的综合性发酵,其产品质量受多种因素影响,如菌种、发酵培养基及其营养组成、发酵过程工艺中的参数(包括发酵pH值、固体发酵基质的初始含水量等)。
现代红曲生产过程采用单菌种发酵工艺,通过接入优良的纯种红曲菌进行发酵。根据1983年Hawksworth和Pit提出的分类系统,大部分红曲菌株来源于丛毛红曲菌(Monascus pilosus Sato ex Hawksworth & Pitt)、紫色红曲菌(Monascus purpureus Went)和红色红曲菌(Monascus ruber van Thieghem)[11]等3种,红曲菌在发酵产生功能性成分红曲色素、莫纳可林K(即洛伐他汀)的同时,也产生有肝肾毒性的桔青霉素成分。
我国规定用于中药和保健品的红曲菌菌种为紫色红曲菌[12],日本采用的菌种为丛毛红曲菌[13]。这可能是由于紫色红曲菌菌种的色素产量高,应用面广;丛毛红曲菌产生洛伐他汀的能力明显高于紫色红曲菌[14]。目前,研究报道[15]大多使用紫外诱变、化学诱变、基因工程菌株构建等方法对发酵菌株进行优化以达到不同的预期目标。
我国红曲发酵的生产原料多选择禾本科植物稻(Oryza sativa L.)的种仁,稻又分为粳稻(Oryza sativa L. subsp. japonica Kato)和籼稻(Oryza sativa L. subsp. indica Kato)两个亚种。日本红曲的发酵通常采用以大米或者大豆为主要原料。在现代工业生产中,为了提高产品的质量和安全性,通常会对其进行灭菌处理。我国大部分红曲标准规定以稻的种仁为红曲的生产原料,河南、福建地区中药饮片炮制规范中特别规定以粳米作为生产原料。不同的生产原料会影响几种发酵产物的含量,粳米发酵后桔青霉素的生成量较低,安全性更高且更利于红、黄色素的产生[16];籼米发酵后洛伐他汀的产量明显优于粳米[12]。还有研究表明[17],不同种类稻米的直链淀粉与支链淀粉含量有所不同,因此蒸煮后的品质也有所差异,其中直链淀粉含量高的稻米,淀粉颗粒排列较疏松,有利于氧气和菌丝进入,较适合红曲霉菌生长。
我国现代红曲的发酵工艺主要分为固态发酵、液态发酵、双向发酵3种(图1~3)。
红曲固态发酵工艺复杂,经过筛选、浸泡、分装、灭菌、接种、培养、干燥等多个步骤加工制得红曲成品,后续对其进行色价、细度、水分等理化指标及感官检测等。固体发酵基质构成了一个碳源浓度梯度微环境[18],可减少代谢阻遏,利于菌株生长及产物合成。
红曲液态发酵工艺相对简单,将大米磨碎、配料、灭菌后,再进行接种和培养。发酵基质混合更加均匀、发酵周期短、污染率小、易控制,但功能代谢产物产量普遍低且后处理成本高。
红曲双向发酵工艺起源于固态发酵工艺,区别在于将固态发酵中农副产品等营养基质以具有一定活性成分的药材或药渣替代,并接种优选菌种进行发酵的制备手段[19],双向发酵技术实现了中药的二次利用,增加了中药有效成分的利用率,提高药效。
红曲菌能产生莫纳可林K(洛伐他汀)、红曲色素、桔青霉素等多种次级代谢产物。
1979年, 日本学者远藤章从红色红曲菌菌株1005的代谢产物中分离出莫纳可林K[13]。1987年, 美国食品药品监督管理局(Food and Drug Administration, FDA)批准默克公司研发的莫纳可林K产品成为第一个临床治疗高脂血症的药物, 商品名为洛伐他汀(Lovastatin)[13]。洛伐他汀能有效抑制胆固醇合成过程中的关键酶3-羟基-3-甲基戊二酸单酰辅酶A(HMG-CoA) 还原酶,帮助降低胆固醇水平[20]。洛伐他汀在常态下以无色针状结晶的形式存在,难溶于水,不溶于醚和正己烷,易溶于甲醇、乙醇、丙酮、三氯甲烷和乙酸乙酯等[21]。红曲中洛伐他汀以开环和闭环两种形态存在,开环洛伐他汀含量较高且降胆固醇能力活性大约是闭环洛伐他汀的2倍,闭环洛伐他汀在人体内需要羟基酯酶参与并水解成酸式才能发挥降血脂功能[22]。洛伐他汀是红曲“健脾消食、活血化瘀”的物质基础,也是市面上降胆固醇、降血脂红曲产品的主要活性成分。
红曲色素是由红曲菌发酵形成的由多种物质组成的聚酮类混合物,根据最大吸光值的不同,可分为红、橙、黄3种色素[3],具有着色、保健、防腐等功效。红曲色素的应用价值主要体现于6种醇溶性色素上,分别为红斑胺、红曲红胺、红曲素、红曲黄素、红斑素和红曲红素。红曲色素一直是我国允许使用的天然食用色素,与合成色素相比,红曲色素具有更好的稳定性,在食品工业中被广泛应用。红色色素占红曲色素含量的大部分,因此红曲在视觉上呈现深红色。
桔青霉素是一种真菌毒素,是由多种青霉属、曲霉属和红曲霉属真菌产生的次级代谢产物,于1931年首次从桔青霉培养物的过滤液中分离发现,1995年Blanc从红曲霉代谢产物中分离得到桔青霉素[23]。桔青霉素常温下呈柠檬黄色针状棱形结晶,微溶于水,可溶于稀碱液和三氯甲烷、乙醇和乙酸乙酯等有机溶剂[24]。桔青霉素具有明显的肾毒性,能引起动物肾小管扩张和上皮细胞变性坏死等[25],还具有生殖毒性和胚胎毒性,给红曲产品带来一定的安全性风险,红曲及部分红曲产品已对其开展限量控制。
麦角甾醇主要存在于酵母菌、霉菌等真菌和某些植物中,是一种重要的植物甾醇,呈白色或无色光亮的小叶晶或白色结晶粉末。麦角甾醇是维生素D2的前体物质之一[26],经紫外线照射发生结构转化生成维生素D2,参与人体内钙和磷的代谢,并影响细胞增殖分化;麦角甾醇可作为重要的医药化工原料,用于生产“可的松”“激素黄体酮”等甾醇类药物。麦角甾醇作为真菌细胞膜的重要组成成分,结构稳定,专一性强,可用于测定真菌的生物量[27]
豆甾醇是一种植物甾醇,广泛存在于多种植物种子中。2012年,Ge等[28]首次从红曲中分离得到了豆甾醇。豆甾醇可以竞争性抑制人体对胆固醇的吸收,有效降低血清胆固醇水平,是调节血脂平衡、预防心脑血管疾病的重要活性成分。
γ-氨基丁酸是一种天然的非蛋白质氨基酸,在自然界中广泛存在,是哺乳动物体内重要的中枢神经系统抑制性神经递质,拥有良好的水溶性与热稳定性,具有改善睡眠和降血压等生理作用[29]。1987 年,Kohama等[30]研究发现红曲的降血压功能来自于其发酵产生的γ-氨基丁酸。
红曲作为药食两用原料已有一千多年的应用历史,随着对其化学成分和药理作用的深入研究,开发了以红曲为原料的血脂康、脂必妥等34个中成药产品,扩大了红曲的应用范围。此外,红曲还作为保健品原料发挥调节血脂的功能,开发的保健食品包括纳豆红曲胶囊、红曲灵芝丹参胶囊、红曲胶囊、沙棘红曲胶囊等品种。
目前洛伐他汀、桔青霉素作为红曲及红曲产品质量控制的有效性和安全性指标(表1)。
洛伐他汀是红曲及红曲产品的主要降脂成分。2020年版《中国药典》未单独收载红曲,仅血脂康胶囊和血脂康胶囊片制剂后附了红曲原料的标准,该红曲原料标准收载了洛伐他汀的含量测定以及指纹图谱。全国共有14家地方炮制规范收载了红曲的质量标准,其中河南、四川、山东、江苏、浙江、海南等6家地方标准中规定了红曲中洛伐他汀的含量测定。
红曲在我国作为功能性食品,执行标准为QB/T 2847-2007《功能性红曲米(粉)》,该标准收载了洛伐他汀的含量测定,相关保健食品使用的红曲原料应符合该行业标准要求。
红曲中含有的洛伐他汀为降血脂一线药物,此类药物在临床应用中表现出少数较为严重的不良反应,常见毒副作用包括肝肾毒性、肌毒性及神经毒性。红曲含有他汀类成分,与降脂药物联用时需要注意用量。2010年,原国家食品药品监督管理局对以红曲为原料的保健食品食用量、不适宜人群等作出了明确规定:红曲推荐量暂定不超过2 g·d-1,总洛伐他汀推荐量暂定不超过10 mg·d-1。产品说明书中不适宜人群增加“少年儿童、孕妇、哺乳期妇女”,注意事项增加“本品不宜与他汀类药物同时使用”。
红曲中的桔霉素具有较强的毒性,体外实验结果表明,桔青霉素对人类胚胎肾脏细胞的半数致死量(median lethal dose,LD50)为1.8~4.7 μg·kg-1[31]。桔青霉素的毒性作用中肾毒性最为明显,病理变化主要表现为肾脏肿大、尿量增多、管状上皮细胞变性坏死、肾小球萎缩和尿素氮升高等[24]。研究表明,50 μmol·L-1的桔青霉素即可引起PK15(猪肾细胞)细胞的钙稳态失衡,导致细胞死亡[32]。因此,桔青霉素的限量检查成为红曲及红曲产品的关注重点。
日本对红曲产品中桔青霉素的含量要求是,酶活力为500 U·g-1的红曲产品中桔青霉素含量不得高于0.2 mg·kg-1,这可能是由于受限于当时的仪器检测限[26]。韩国规定,红曲产品中的桔青霉素含量不得高于0.05 mg·kg-1,欧盟规定红曲相关产品中桔青霉素含量不得高于2.0 mg·kg-1,澳大利亚及新西兰规定红曲相关产品中桔青霉素含量不得高于0.2 mg·kg-1[33]
我国四川、河南省中药饮片炮制规范及轻工行业标准——功能性红曲米(粉)对红曲中桔青霉素做出严格的限量要求,规定不得超过50 μg·kg-1。近年来,各标准中对于食品中桔青霉素的测定方法也在不断完善。高效液相色谱法是目前比较常用的红曲中桔青霉素的检测方法。检测器方面,由于荧光检测器的灵敏度明显优于紫外检测器,GB 5009.222-2016《食品安全国家标准食品中桔青霉素的测定》、QB/T 2847-2007《功能性红曲米(粉)》以及四川等省中药饮片炮制规范均采用高效液相色谱-荧光检测器法测定桔青霉素限量。前处理方面,固相萃取和免疫亲和柱等净化前处理技术适用于成分复杂的红曲基质,使得方法的检出限得以不断降低。其中,免疫亲和柱依赖的抗原-抗体反应具有高灵敏、高特异性的特点,大大提高了样品的净化效果和检测灵敏度,现行国标关于红曲中桔青霉素的测定方法就是采用的免疫亲和柱净化前处理方法。GB 5009.222-2008中规定,桔青霉素在固态的红曲产品中不能够高于1 mg·kg-1;此后GB 5009.222-2016中对红曲相关产品中桔青霉素限量制定了更严格的规定,规定定量限为80 μg·kg-1,检出限为25 μg·kg-1[23]。可见,随着检测技术的发展,新的标准相比旧的标准更加灵敏、严格。
红曲现行安全性质量控制中未建立软毛青霉酸的限量方法。据报道[34],小林制药对问题产品进行召回检测发现,问题产品涉及33批次红曲原料,其中10个批次检测出了软毛青霉酸。软毛青霉酸(puberulic acid,C8H6O6)最早由Birkinshaw等[35]于1932年发现,由青霉菌属的真菌分泌,属于环庚三烯酚酮类天然产物。青霉菌产生软毛青霉酸被证明是通过非还原聚酮合成酶(nr-PKS)途径,以聚酮为起源生成苯环结构,通过氧化环膨胀形成七元环酚酮核心骨架,再经过一系列剪切酶或基于内在反应性自发转化,生成软毛青霉酸[36-38]。Ishiyama等[39]从青霉菌(Penicillium sp.) FKI-4410的培养液中分离出几种环庚三烯酮相关化合物,并对其进行了抗疟疾活性筛选,发现软毛青霉酸具有良好的抗疟活性。目前对软毛青霉酸的毒性研究报道较少,不能排除其具有肾脏毒性的可能。这提示需要对红曲发酵生产过程中受到青霉杂菌污染的风险进行评估并建立科学的安全性控制指标。
红曲菌是较安全的菌种,但红曲的发酵过程复杂,其中涉及的设施设备、菌种以及生产原料均有可能受到杂菌污染。
生产设备或环境清洁度不足时,可能会将残留的微生物带入发酵体系,形成污染源。红曲发酵过程需要保证生产、加工环境和发酵设施的洁净程度,包括高效的空气过滤和净化系统、用于控制和优化发酵条件的设备、精确的温湿度监控设备、无菌操作设备以及专用的发酵容器等。
大米作为红曲生产原料,大米中存在的微生物可能会与发酵菌竞争营养,或者产生目标外的代谢产物(如真菌毒素),影响红曲质量和安全性。我国现代红曲发酵工艺要求对大米进行发酵前灭菌处理(图1),灭菌的主要目的是去除大米中可能存在的微生物,包括细菌、真菌和其他微生物。大米发酵期间清洁度和无菌状态对整个发酵过程至关重要,固态发酵过程中原料在发酵前受到微生物(如霉菌)的污染后未开展灭菌或灭菌效果不好,都可能会影响红曲的质量安全[40]
为防止杂菌的引入,存放及发酵过程中应定期进行发酵菌种纯度检测,以确保所使用的菌株没有受到外源菌污染且菌种的特性未发生改变。杂菌或其他非目标微生物可能对发酵过程产生不利影响,影响产品质量甚至带来安全性风险,对菌种进行纯度检测可以及时识别以上风险。
此次出现问题的含红曲保健食品在日本属于“功能性标示食品”,日本对该类产品准入宽松,采用事前备案管理制度,即企业只需根据食品的安全性和功能性等必要事项,在上市前向日本消费者厅提供有关表示其功能性的依据,相关部门不会对其具体内容进行审查[41],产品质量完全依靠企业自律,当企业的自律意识达不到要求时,就可能引发公共安全事件。
我国对红曲保健食品采用严格的注册制与备案制相结合的制度,既抓面,又抓点,监管更严格。生产企业需事先进行产品注册,并经过审评机构统一组织现场核查和复核检验[42]。以红曲为原料的保健食品申请注册时,除了提交常规的注册资料外,还需提供红曲菌种及原料中桔青霉素检测报告和产品洛伐他汀的检测图谱。同时,需要明确红曲的推荐日摄入量及洛伐他汀的含量。此外,产品说明书中还注明了不适宜人群以及注意事项中需增加的相关声明。相比日本的保健食品审批制度,我国的制度更加严格和规范。
根据发酵基质可将发酵类中药分为米面曲类、胆汁发酵类和其他类3种[43]。以米面等营养物质为基质进行发酵的常见的米面曲类有红曲、六神曲、建曲、半夏曲、沉香曲等。以动物胆汁为基质、中药为底物进行双向发酵的胆汁发酵类中药包括:胆南星、体外培育牛黄、胆黄连等。直接使用药材不外加基质进行发酵的中药包括淡豆豉、百药煎等。
发酵类中药的发酵过程决定了其质量和疗效需要对其进行严格控制,现行质量控制中对发酵类中药的发酵工艺缺乏规范性约束,往往采用“发酵”二字高度概括,未对其发酵条件和发酵终点进行具体要求。除了六神曲、建曲等制法中对发酵条件和终点进行了规定,其余发酵类中药的规定太过宽泛,导致存在潜在的安全性质量风险,因此,对于发酵类中药需要从以下几方面入手。
发酵类中药标准较多,但国家标准所占比例较小。1953年至2020年颁布的11版《中国药典》中总共仅收载了3种发酵类中药,即淡豆豉、胆南星、体外培育牛黄,《中华人民共和国卫生部药品标准》中收录了六神曲、建曲、半夏曲等11种曲剂的质量标准。各地标准在处方、发酵方式、工艺参数等方面不统一,对于处方组成、发酵用辅料(如胆汁、黄酒、面粉等)尚无统一的来源及规格规定。工艺条件、发酵终点判断标准,发酵条件控制都是保证曲类中药质量稳定性的关键因素。 建议对以上要素展开针对性研究,明确发酵工艺关键参数以保证发酵类中药安全性和有效性,并为建立统一的国家标准提供依据。
严格控制发酵工艺以避免杂菌污染,在发酵过程中,保持无菌状态是非常重要的。需要对生产原料、设备、工具、培养基及操作人员进行严格的消毒和无菌处理。操作人员对整个工艺流程能够实时在线控制监测,保证无菌操作。发酵设施的设计和布局需要综合考虑多种因素,发酵间需要良好的通风系统,以保证空气流通,避免微生物污染。在设计和建造发酵设施时,应严格遵守相关标准和规范,对于陈旧的设施设备,应定期对其开展安全性和效率的检查,对于不适宜的设施设备,应及时进行更新替换。对发酵过程进行数学建模,得到输入(如菌种、培养基)与输出(如发酵产物)之间的关系,通过模型对发酵工艺中关键参数如温度、pH值、溶氧量、搅拌转速等进行优化,明确最佳发酵条件并建立相应的操作规范。
运用形态学、生物化学等多种方法,筛选优势菌种,针对发酵菌建立简便有效的标准化纯度检测手段,在菌种保存过程中定期进行纯度检测,及时发现菌种的特性变化;并对安全风险高的环节开展在线菌种纯度分析,监控发酵过程中有无杂菌引入。目前,对发酵过程进行在线监测所采用的较为先进的技术包括实时近红外光谱[44]、传感技术[45]等,可以实现对微生物存在和活动状态的实时分析。
发酵类中药在大生产中仍多采用传统自然发酵及固体发酵的方式,而涉及优势菌种、提高有效成分转化效率和生产效率的液体发酵和双向发酵技术仅停留在实验室成果阶段,并未规模化转化应用[46]。因此,后续应加强对优势菌种和发酵工艺的开发应用。
微生物发酵过程中可能受到杂菌污染而产生生物毒素,给发酵类中药的应用带来安全隐患,特别应加强对生产过程控制不规范导致的菌种变化及有毒代谢产物(如真菌毒素等)的安全性控制研究。
发酵类中药应用历史悠久,临床价值高,但其安全性质量控制一直处于初级阶段,亟待开展深入研究。本研究通过对红曲的生产工艺、发酵产物、物质基础研究以及质量安全控制现状探究,详细分析了红曲质量安全风险的种种诱因,并针对问题风险点提出完善现行标准,筛选优势菌种,开发高效发酵技术等改进措施,旨在为我国发酵类中药及相关产品的监管与企业生产过程风险防控提供参考。
