OBJECTIVE To study the effect of different particle size of nifedipine on the in vitro and in vivo equivalence of its sustained-release tablets (Ⅰ), and confirm the appropriate range. METHODS The effect of API with different particle size on the dissolution of nifedipine sustained-release tablets (Ⅰ) was evaluated by experimental design and dissolution similarity factors (f2), and the in vitro dissolution and in vivo bioequivalence of the commercial products with the reference preparations were compared. RESULTS The dissolution of the industrial products prepared by micronizing the API to Dv90 ≤20 μm is similar to the reference preparations in four dissolution media in vitro, and both are bioequivalent in vivo. CONCLUSION The particle size of API is the key factor affecting the release and equivalence of nifedipine sustained-release tablets (Ⅰ).

目的 研究硝苯地平不同粒度对其缓释片(Ⅰ)体内外等效性的影响,并确定合适的粒度范围。方法 采用溶出f2相似因子法,通过试验设计评价不同粒度分布的原料对硝苯地平缓释片(Ⅰ)溶出行为的影响,并比较放大的自研制剂与参比制剂的体外溶出行为和体内生物等效性。结果 将原料微粉化至Dv90≤20 μm所制样品与参比制剂在体外4种溶出介质中溶出行为均相似,在体内生物等效。结论 原料药粒度是影响硝苯地平缓释片(I)体内外等效性的关键因素。

OBJECTIVE To study the effect of different particle size of nifedipine on the in vitro and in vivo equivalence of its sustained-release tablets (Ⅰ), and confirm the appropriate range. METHODS The effect of API with different particle size on the dissolution of nifedipine sustained-release tablets (Ⅰ) was evaluated by experimental design and dissolution similarity factors (f2), and the in vitro dissolution and in vivo bioequivalence of the commercial products with the reference preparations were compared. RESULTS The dissolution of the industrial products prepared by micronizing the API to Dv90 ≤20 μm is similar to the reference preparations in four dissolution media in vitro, and both are bioequivalent in vivo. CONCLUSION The particle size of API is the key factor affecting the release and equivalence of nifedipine sustained-release tablets (Ⅰ).

硝苯地平最早由德国拜耳公司开发,1975年于德国上市胶囊剂,规格为10 mg,主要用于治疗原发性高血压、肾性高血压及心绞痛。由于硝苯地平溶解性差,生物利用度低,半衰期短,患者多次用药导致血药浓度波动大,临床应用时易引发头痛、面部潮红及心律失常等不良反应[1-4]。而硝苯地平缓控释制剂能提高生物利用度,降低不良反应,提高患者依从性[5-6]。相较于硝苯地平控释片的复杂制备工艺,高生产成本和低产量,硝苯地平缓释片生产工艺更简单,患者治疗成本更低,是当前该药物研发领域的更好选择。
国内外硝苯地平缓释片(Ⅰ)的仿制药多采用亲水凝胶材料如羟丙甲纤维素、羟丙纤维素,制备骨架型缓释片[7-10],查询参比制剂的日本(Interview Form,IF)文件及相关专利,硝苯地平缓释片的缓释机理是基于硝苯地平的难溶性特点,对硝苯地平原料进行微粉化处理,使其粒径呈一定粒度分布,在溶出介质中缓慢、逐步溶解,从而达到缓释的效果[11-12]。因此对该品种而言,硝苯地平原料药的粒度分布对其缓释制剂的溶出有显著影响,在药物制剂生产过程中进行粒度控制至关重要。一般情况下,随着粒径的减少,比表面积增大,孔隙率增加,吸附性增强,溶解性增强,从而影响体内外释放和吸收情况。硝苯地平的粒度如何影响其制剂体内外等效性,本文通过对原料粒度进行探究以确定合适的粒度控制范围,并根据放大的自研制剂与参比制剂的体外溶出及体内吸收结果进行确证。
硝苯地平缓释片(商品名:Adalat®-L10,规格:10 mg,批号:JPS2206,日本バイエル薬品株式会社);硝苯地平对照品(批号:RS19081201-201912,中国食品药品检定研究院);硝苯地平(批号:051907003,河北广祥制药有限公司);乳糖(批号:L101853619,德国美剂乐沃斯堡牛奶房两合公司);微晶纤维素(批号:P119832920,DuPont Nutrition USA,Inc);淀粉(批号:190207,曲阜市天利药用辅料有限公司);聚山梨酯80(批号:20190643,南京威尔药业集团股份有限公司);羟丙甲纤维素(批号:7038095,信越化学工业株式会社);硬脂酸镁(批号:190501,浙江中维药业股份有限公司);薄膜包衣预混剂(批号:SH613831,上海卡乐康包衣技术有限公司)。甲醇、乙腈为HPLC级,聚山梨酯80等其他试剂均为分析纯。
YQ100-01型气流粉碎机(上海赛山粉体机械制造有限公司);HLSH4-10A湿法混合制粒机(北京航空制造工程研究所);Y-160摇摆式颗粒机(泰州第三制药厂);FLZB-3型流化床(创志科技股份有限公司);C&C800旋转式压片机(北京创博佳维科技有限公司);YPD-300C片剂硬度测定仪(上海黄海药检仪器有限公司);VFC-LDCS高效薄膜包衣机(德国VECTOR公司);Mastersizer 3000激光粒度仪(英国马尔文公司);AT7 Smart Sotax溶出仪(瑞士Sotax公司);Agilent 1260高效液相色谱仪(美国安捷伦公司)。
查阅参比制剂说明书,本品辅料组成为:乳糖、微晶纤维素、淀粉、聚山梨酯80、羟丙甲纤维素、聚乙二醇4000、氧化钛及三氧化二铁[13]。其中每片含主药硝苯地平10mg,乳糖、微晶纤维素、淀粉作为填充剂,聚山梨酯80作为增溶剂,羟丙甲纤维素作为黏合剂,硬脂酸镁作为润滑剂,薄膜包衣粉(羟丙甲纤维素、聚乙二醇4000、氧化钛及三氧化二铁)。硝苯地平缓释片(Ⅰ)的制备工艺采用湿法制粒,避光条件下[14],按处方量称取原辅料,混合均匀,将黏合剂倒入制粒锅中湿法制粒,湿颗粒经整粒后置流化床中干燥得干颗粒,整粒后加入外加辅料总混,压片并包衣。
硝苯地平粒度和粒度分布测定方法参照2020年版《中国药典》四部通则中的粒度和粒度分布测定法中的第三法[15],搅拌速度为1 800 r·min-1,遮光度为8%~20%,预分散介质为0.3%聚山梨酯80,样品外超声时间为1 min[16],该方法稳定、结果准确、重现性良好,适用硝苯地平原料药的粒度分布测定。
参照化学药物口服缓释制剂药学研究技术指导原则,本研究按照2020年版《中国药典》四部通则0931溶出度与释放度测定法的第二法的规定进行试验[17]。测定方法如下:取本品,避光操作。溶出介质为0.3%聚山梨酯80水溶液、含0.3%聚山梨酯80的pH 1.0盐酸溶液、含0.3%聚山梨酯80的pH 4.0醋酸盐缓冲液、含0.3%聚山梨酯80的pH 6.8磷酸盐缓冲液,体积900 mL,转速75 r·min-1,温度(37±0.5)℃,分别于15、30 min、1、2、3、10、12 h时取样。以外标法计算硝苯地平的累积溶出度。采用相似因子f2值法评价自研制剂与参比制剂的溶出行为相似性。
硝苯地平原料药通过控制气流粉碎机不同参数,制得不同粒度范围的原料药粒度(20~30 μm、10~20 μm和小于10 μm),以探究原料药的粒度对其缓释片溶出情况的影响。并在确认的粒度范围内,以相同的处方工艺连续制备3批样品,确认原料粒度对制剂溶出行为的影响。
按照确定的处方工艺进行放大,批量放大至商业化生产规模(百万级),按照“2.3”项下方法检测样品,确认硝苯地平原料粒度范围的合理性。
不同粒度范围见表1。按照“2.1”项下制备3批硝苯地平缓释片(Ⅰ)样品进行溶出曲线测定。粒度在Dv90>20 μm时所制样品的溶出行为明显快于参比制剂,结果见表2图1,粒度Dv90≤20 μm时,样品的溶出行为与参比制剂更为相似,故原料药粒度Dv90初步定在≤20 μm。
对原料药粒度范围进一步确认。对原料药进行气流粉碎,原料药粒度控制在粒度Dv90≤20 μm,优先0~10 μm粒度,结果见表3,并对3批样品在体积分数0.3%聚山梨酯80水溶液中进行溶出曲线检测,结果见表4,图2。结果表明,3批确认批样品与参比制剂溶出行为相似,且批间差异小,表明该范围粒度控制制得的样品重现性良好。
采用确定粒度范围的原料,按照既定处方工艺将批量逐步放大至商业化生产规模(百万级),粉碎过程中的前、中、后在出料口取样检测粒度(表5),经确认粉碎生产过程中粒度控制较为稳健。
自制样品放大至生产规模后在pH 1.0盐酸+0.3%聚山梨酯80溶液、pH 4.0醋酸+0.3%聚山梨酯80溶液、pH 6.8磷酸+0.3%聚山梨酯80溶液及水+0.3%聚山梨酯80溶液4种介质中的溶出曲线与参比制剂比较f2值分别为77、79、74、74,均大于50,体外溶出行为相似(图3)。
本研究经武汉大学中南医院伦理委员会批准(登记号:CTR20192546),在健康成年受试者中进行的随机、开放、单剂量、双周期、自身交叉空腹及餐后状态下生物等效性试验,纳入生物等效性试验的84例受试者空腹/餐后给予硝苯地平缓释片(Ⅰ)(自制制剂,10 mg)与Adalat®-L10(参比制剂,10 mg)。受试制剂T和参比制剂在空腹及餐后状态下ρmax、AUC0-t、AUC0-∞的90%CI均在80%~125%内,2种制剂具有生物等效性,见表6图4
硝苯地平原料药属于生物药剂学分类系统(BCS)Ⅱ类(低溶解高渗透)化合物,溶出速率是影响吸收的关键因素[18],而原料的粒度分布会直接影响片剂的溶出。水溶性差的原料药的粒度分布会显著影响药物的溶出速率,通常粒度较小的颗粒具有较大的表面积和较小的扩散层厚度,可增大溶解速率。但经研究硝苯地平缓释片(Ⅰ)的粒度经不同粒度范围发现,硝苯地平原料药粒度越小,自研制剂溶出行为越接近参比制剂,与相关文献[19]报告结论吻合,即当粉末过细后则会形成静电堆积,颗粒周围形成气泡囊,阻碍水分进入颗粒,从而延缓药物的溶出,达到缓释效果。最终按照确定的原料粒度范围所制的硝苯地平缓释片(Ⅰ)具有理想的释药特性,与参比制剂Adalat®-L10在体外不同溶出介质中溶出行为均相似(f2>50),在体内生物等效,质量与疗效一致。
笔者研究的硝苯地平缓释片(Ⅰ)制备方法采用传统的湿法制粒工艺,其处方工艺简单可控,可大大降低制剂难度和生产成本,更易于产业化生产。不可否认,除原料药的粒度分布外,处方工艺中如辅料的型号、制粒时间等其他因素也有可能会影响产品的药学质量,同时处方工艺的放大过程中需同步关注。
