Aim

To investigate the mechanism of Piezo1 activation participating in rat coronary artery smooth muscle cells (CASMCs) calcification under high calcium-phosphate stimulation.

Methods

CASMCs were treated with β -glycerophosphate and CaCl2 for 10 days to induce calcification, and cells were divided into the general group(GM)and the calcification group (CM). Then, CASMCs were intervened with inhibitor GsMTx4, siRNA-Piezo1 and agonist Yoda1, respectively. The calcification of CASMCs was examined via alizarin red staining, and the expression of proteins associated with phenotype transition and mitochondrial dynamics was examined via Western blot.

Results

Compared with the GM, the CM had more obvious calcification(P < 0.01). The expression of Piezo1, Runx2, BMP2 and OPN increased(P < 0.01), and the expression of SM22α and SM-MHC decreased(P < 0.01)in the CM. After intervention with GsMTx4 and siRNA-Piezo1, calcification was significantly alleviated(P < 0.01), while intervention with Yoda1 enhanced calcification(P < 0.01). In addition, when stimulated with high calciumphosphorus, there was an increase in the expression of DRP1Ser616 and FIS1(P < 0.01), and a decrease in the expression of MFN1 and MFN2(P < 0.01). Intervention with Yoda1 could aggravate mitochondrial fission (P < 0.01), and intervention with Mdivi-1 could alleviate CASMCs calcification (P < 0.01).

Conclusions

Piezo1 activation can mediate the calcification of CASMCs under high calcium and phosphorus stimulation, and its mechanism may be related to the induction of mitochondrial dynamics imbalance.

目的

探究Piezo1通道在高钙磷刺激的大鼠冠脉平滑肌细胞(coronary arterial smooth muscle cells,CASMCs)钙化中的作用机制。

方法

使用β-甘油磷酸盐和CaCl2干预CASMCs 10 d以诱导钙化,分为对照组(GM)和钙化组(CM)。期间分别采用Piezo1抑制剂GsMTx4、siRNA-Piezo1及激动剂Yoda1进行干预,用茜素红染色确定细胞钙化程度,Western blot检测表型转化及线粒体动力学相关蛋白的表达情况。

结果

相比GM组,CM组钙化更明显(P < 0.01),Piezo1及成骨表型相关蛋白Runx2、BMP2、OPN表达均呈上调趋势(P < 0.01),而收缩表型相关蛋白SM22α、SM-MHC表达下降(P < 0.01);给予GsMTx4和siRNA-Piezo1干预后,钙化相比DMSO组明显缓解(P < 0.01),使用Yoda1干预后钙化进一步加剧(P < 0.01)。此外,高钙磷刺激下线粒体动力学相关蛋白DRP1Ser616、FIS1表达升高(P < 0.01),MFN1、MFN2表达下降(P < 0.01),使用Yoda1处理可加剧线粒体分裂(P < 0.01),且给予线粒体分裂抑制剂Mdivi-1干预可改善CASMCs钙化(P < 0.01)。

结论

Piezo1激活可介导高钙磷刺激的CASMCs钙化,其机制可能与诱导线粒体动力学失衡相关。

探究Piezo1通道在高钙磷刺激的大鼠冠脉平滑肌细胞(coronary arterial smooth muscle cells,CASMCs)钙化中的作用机制。

方法

使用β-甘油磷酸盐和CaCl2干预CASMCs 10 d以诱导钙化,分为对照组(GM)和钙化组(CM)。期间分别采用Piezo1抑制剂GsMTx4、siRNA-Piezo1及激动剂Yoda1进行干预,用茜素红染色确定细胞钙化程度,Western blot检测表型转化及线粒体动力学相关蛋白的表达情况。

结果

相比GM组,CM组钙化更明显(P < 0.01),Piezo1及成骨表型相关蛋白Runx2、BMP2、OPN表达均呈上调趋势(P < 0.01),而收缩表型相关蛋白SM22α、SM-MHC表达下降(P < 0.01);给予GsMTx4和siRNA-Piezo1干预后,钙化相比DMSO组明显缓解(P < 0.01),使用Yoda1干预后钙化进一步加剧(P < 0.01)。此外,高钙磷刺激下线粒体动力学相关蛋白DRP1Ser616、FIS1表达升高(P < 0.01),MFN1、MFN2表达下降(P < 0.01),使用Yoda1处理可加剧线粒体分裂(P < 0.01),且给予线粒体分裂抑制剂Mdivi-1干预可改善CASMCs钙化(P < 0.01)。

结论

Piezo1激活可介导高钙磷刺激的CASMCs钙化,其机制可能与诱导线粒体动力学失衡相关。

Aim

To investigate the mechanism of Piezo1 activation participating in rat coronary artery smooth muscle cells (CASMCs) calcification under high calcium-phosphate stimulation.

Methods

CASMCs were treated with β -glycerophosphate and CaCl2 for 10 days to induce calcification, and cells were divided into the general group(GM)and the calcification group (CM). Then, CASMCs were intervened with inhibitor GsMTx4, siRNA-Piezo1 and agonist Yoda1, respectively. The calcification of CASMCs was examined via alizarin red staining, and the expression of proteins associated with phenotype transition and mitochondrial dynamics was examined via Western blot.

Results

Compared with the GM, the CM had more obvious calcification(P < 0.01). The expression of Piezo1, Runx2, BMP2 and OPN increased(P < 0.01), and the expression of SM22α and SM-MHC decreased(P < 0.01)in the CM. After intervention with GsMTx4 and siRNA-Piezo1, calcification was significantly alleviated(P < 0.01), while intervention with Yoda1 enhanced calcification(P < 0.01). In addition, when stimulated with high calciumphosphorus, there was an increase in the expression of DRP1Ser616 and FIS1(P < 0.01), and a decrease in the expression of MFN1 and MFN2(P < 0.01). Intervention with Yoda1 could aggravate mitochondrial fission (P < 0.01), and intervention with Mdivi-1 could alleviate CASMCs calcification (P < 0.01).

Conclusions

Piezo1 activation can mediate the calcification of CASMCs under high calcium and phosphorus stimulation, and its mechanism may be related to the induction of mitochondrial dynamics imbalance.

血管钙化是钙磷在血管内部发生异常沉积的过程,其主要特征体现为血管僵硬度上升及顺应性降低。其中,血管中膜钙化在慢性肾病(chronic kidney disease,CKD)、糖尿病、衰老及高血压等病症中较为常见。临床研究数据显示,心血管并发症是CKD患者首要的死亡原因,其死亡率可高达普通人群的10~20倍[1],而冠脉血管钙化是导致其心血管事件高发的关键[2]。当存在病理刺激时,血管平滑肌细胞(vascular smooth muscle cells,VSMCs)可由收缩型转变为成骨样表型,推动中膜钙化的发展进程[3]。对于CKD患者而言,肾功能减退普遍会引发高磷血症和钙磷代谢紊乱,高磷是驱动VSMCs向成骨表型转化最核心、最直接的病理刺激[4],其机制可能与细胞内钙调控异常有关,但其具体机制仍不清楚。
近年来,机械敏感性离子通道Piezo1的功能已超越单纯的力学感知范畴,被证实在化学传感和代谢调控中发挥关键作用。最新研究表明,病理性高刚度微环境可有效激活Piezo1介导Ca2+内流[5],且在主动脉瓣钙化中,Piezo1表达明显上调,调节YAP相关的细胞代谢过程以加速成骨分化,抑制Piezo1功能可明显缓解血管病变[6]。此外,既往研究已发现,Piezo1的激活与线粒体稳态调控密切相关[7],而线粒体功能障碍已被公认为血管钙化的核心机制,线粒体分裂过度会破坏其网络结构,导致活性氧大量生成,进而加剧VSMCs钙化[8]。然而,Piezo1是否能够响应高钙磷这一病理性微环境刺激,进而促进冠脉平滑肌细胞(coronary arterial smooth muscle cells,CASMCs)钙化,以及其调控机制是否与诱导线粒体动力学失衡相关,尚不明确。综上,本研究以CASMCs作为研究对象,通过构建高钙磷诱导的细胞钙化模型,初步探讨Piezo1、线粒体和CASMCs之间的联系。
SPF级♂ Wistar大鼠,体质量250~300 g,共35只,由广州锐格生物科技有限公司提供,实验动物生产许可证号:SCXK(粤)2023-0059。大鼠饲养于广州锐格生物科技有限公司的SPF级屏障环境内。本研究涉及的所有动物实验均已得到广东省人民医院(广东省医学科学院)伦理委员会的正式批准(伦理号:KY-Z-2021-581-01)。

Nikon ECLIPSET2倒置显微镜、Nikon TI-S倒置显微镜(南京尼康江南光学仪器有限公司);XTL250型体视显微镜(上海光密仪器有限公司);蛋白电泳仪、转膜仪、ChemiDoc显影仪(美国Bio-Rad Laboratories公司);MultiskanGO全波长酶标仪(赛默飞世尔科技公司)。
冠脉分离的具体方法参考课题组之前的文献[9]。大鼠用异氟烷实施麻醉后,通过颈椎脱臼法处死。将大鼠皮肤浸入75% 乙醇中去除体表细菌,随后用无菌器械开胸,取心脏,放入无菌PBS溶液。在生物安全柜内,借助体视显微镜,使用无菌显微工具分离大鼠心脏冠脉,清除血管周边的脂肪与结缔组织。在小皿中加入100 μL含20% FBS和1% 青霉素-链霉素的基础培养基,把分离好的冠脉放入其中剪切,再用无菌巴氏管吸取血管悬液,均匀铺在25 cm2培养瓶中。将培养瓶倒置,加入5 mL基础培养基,置于培养箱中静置1.5 h后,将培养瓶正面朝上放置继续培养。后续实验选用P2~P4代细胞。
将P2~P4代CASMCs置于基础培养基(含10% FBS和1% 青霉素-链霉素)中培养,培养条件为37 ℃、5% CO2。钙化造模实验分为对照组(general medium,GM)和钙化组(calcification medium,CM),待细胞在6孔板中融合密度达80%时,开始给药。GM组使用基础培养基,CM组使用补充有10 mmol·L-1 β-甘油磷酸盐和3 mmol·L-1 CaCl2的成骨培养基。细胞造模使用6孔板,第10天收样,期间每隔3天换1次液。
为探究Piezo1在CASMCs钙化中的关键作用,使用Piezo1抑制剂GsMTx4和激动剂Yoda1进行干预。①GsMTx4干预:在钙磷造模的基础上,分别给予0.1、0.3、0.6 μmol·L-1的GsMTx4干预细胞钙化,分组为:GM组、DMSO组、GsMTx4组;②Yoda1干预:在钙磷造模的基础上,分别给予1、2.5、5 μmol·L-1的Yoda1干预细胞钙化,分组为:GM组、DMSO组、Yoda1组。
细胞在钙磷造模前,使用Li-pofectamine 3000和Opti-MEM试剂进行转染,分组为:si-NC+GM组、si-NC+CM组、si-Piezo1+GM组、si-Piezo1+CM组。制备Lipofectamine 3000混合液时,取125 µL Opti-MEM培养液与5 µL Lipofectamine 3000溶液充分混匀;制备siRNA混合液时,将125 μL Opti-MEM培养液与5 μL浓度为20 μmol·L-1的siRNA溶液混合均匀。随后按体积比1∶1将上述两种混合液混合,加入细胞中转染48 h。
Mdivi-1是一种有效的线粒体分裂抑制剂,主要靶向抑制DRP1蛋白活性,为探究线粒体分裂在CASMCs钙化中的关键作用,分别给予10、15、20 μmol·L-1的Mdivi-1干预细胞钙化,分组为:GM组、DMSO组、Mdivi-1组。
诱导完毕后,弃除培养基,用1×PBS洗涤2次。每孔中添加1 mL 4% 多聚甲醛,在室温下固定15 min。弃去固定液,使用无钙磷的去离子水洗涤2次,确保固定液无任何残留。接着在孔内加1 mL染液,于室温条件下染色15 min。弃除染色液,用去离子水洗涤2次,每孔留存1 mL去离子水,借助倒置显微镜查看染色效果。染色后,用去离子水充分清洗细胞,直至洗脱液不再有颜色。将培养板在室温下完全晾干,向孔中加入800 μL 10%(W/V)氯化十六烷基吡啶溶液,把培养板放在摇床上,室温孵育30 min,确保染料被彻底溶解萃取,将每孔中的萃取液转移至96孔板内,使用酶标仪,在540 nm处检测各孔的吸光度值。
收样时每孔加100 μL强裂解液,4 ℃裂解30 min后,收集裂解液,4 ℃、12 000 r·min-1离心10 min,取上清,按BCA试剂盒测蛋白浓度。采用12% 预制胶进行SDS-PAGE电泳,80 V恒压20 min后,调至120 V恒压65 min,至染料达胶底。湿法转膜,条件为200 mA恒流120 min(每2块胶一组),室温BSA封闭20 min,4 ℃一抗孵育过夜,次日室温二抗孵育1 h,滴加显影液后,用凝胶成像系统捕集信号。以GAPDH作为内参,分析条带灰度值,并计算蛋白相对表达量。
统计学分析通过GraphPad Prism 8软件完成。计量资料以x±s方式表示;两组间差异比较采用t检验,多组间差异比较使用单因素方差分析(One-way ANOVA)。检验水准α=0.05。
课题组前期研究证实,Piezo1在大鼠CASMCs中高度表达[10]。与GM组相比,CM组茜素红染色明显加深,钙结节增多(P < 0.01);关键分子Piezo1表达增加,成骨表型相关蛋白Runx2、BMP2和OPN表达上调,收缩表型相关蛋白SM22α、SM-MHC表达下降(P < 0.01);线粒体动力学相关蛋白DRP1ser616、Fis1表达上调(P < 0.01),MFN1、MFN2表达下调(P < 0.01),见 Fig 1
Fig 2结果显示,相比于DMSO组,CASMCs钙化程度随抑制剂浓度增加而改善(P < 0.01);且成骨表型相关蛋白Runx2、BMP2表达随GsMTx4浓度升高而下降(P < 0.01),收缩表型相关蛋白SM22α表达随抑制剂浓度升高而上调(P < 0.01)。
Fig 3所示,相比于si-NC+GM组,si-Piezo1+GM组Piezo1蛋白被明显敲低(P < 0.01);相比于si-NC+CM组,si-Piezo1+CM组的钙化程度明显减轻(P < 0.01),成骨表型相关蛋白Runx2(P < 0.01)、BMP2(P < 0.05)表达水平下降,收缩表型相关蛋白SM22α表达水平上调(P < 0.01)。
相比于DMSO组,CASMCs钙化程度随Yoda1浓度增加而加剧(P < 0.01);且在激动剂干预后,成骨表型相关蛋白Runx2、BMP2表达水平随Yoda1的浓度升高而升高(P < 0.01),收缩表型相关蛋白SM22α表达水平随激动剂的浓度升高而下降(P < 0.01),见 Fig 4
Fig 5结果显示,给予Yoda1激动后,分裂相关蛋白DRP1ser616、FIS1表达相比DMSO组进一步上调(P < 0.01),融合相关指标MFN1、MFN2下调(P < 0.01)。
Fig 6所示,与DMSO组相比,CASMCs钙化程度随Mdivi-1浓度增加而改善(P < 0.01),且成骨表型相关蛋白Runx2、BMP2表达水平随Mdivi-1的浓度升高而下降(P < 0.01),收缩表型相关蛋白SM22α表达水平随Mdivi-1的浓度升高而升高(P < 0.01)。
本研究结果显示,长期给予CASMCs高钙磷刺激可上调Piezo1及成骨表型相关蛋白Runx2、BMP2,并下调收缩表型相关蛋白SM22α、SM-MHC,促进CASMCs钙化。为进一步探究Piezo1与高钙磷刺激下CASMCs钙化之间的关系,分别给予Piezo1抑制剂GsMTx4和siRNA-Piezo1干预后,成骨表型相关蛋白表达下降,高钙磷所致的CASMCs钙化有所缓解,且给予Piezo1激动剂Yoda1干预后,细胞钙化进一步加剧。此外,高钙磷诱导的CASMCs钙化伴有线粒体分裂增加,Piezo1激动剂Yoda1干预可加剧钙磷诱导的线粒体分裂,且给予线粒体分裂抑制剂Mdivi-1可改善钙磷相关的CASMCs钙化。以上提示,Piezo1激活可介导高钙磷刺激下的CASMCs钙化,其机制可能与细胞线粒体动力学失衡有关。
临床冠脉钙化与CKD患者发生心肌梗死、心力衰竭的风险呈独立且明显相关,对于冠脉钙化评分较高的CKD患者而言,因冠脉疾病导致的全因死亡率会明显增加[2]。血磷水平升高是诱发血管钙化的主要成因及危险因子,现有研究表明,高磷通过激活Wnt/β-catenin通路,可实现在转录水平上调节Pit-1,促进主动脉钙化的发生发展[4],且暴露于高水平钙和磷的大鼠主动脉钙化程度明显高于仅暴露于高水平磷的大鼠[11]。临床上,当血液透析患者同时发生高钙血症和高磷血症时,冠脉钙化的发生也会加速[12],显示高钙和高磷水平对血管钙化具有协同作用。CASMCs位于冠脉血管中层,其功能状态对维持冠脉的舒缩能力与血流稳定性具有重要作用。因此,我们通过给予CASMCs长时间的高钙磷刺激以模拟疾病状态下血液中的高钙磷环境。结果显示,成骨表型相关蛋白Runx2等表达升高,而收缩表型相关蛋白SM22α等表达下降,细胞发生明显钙化,这与临床样本所呈现的趋势相一致,但高钙磷通过何种机制诱导CASMCs钙化还有待进一步探究。
作为一种非选择性阳离子通道,Piezo1可通过感知细胞周围的机械力刺激,来维持血管的结构稳定状态与舒缩功能。最新研究表明,高钙磷刺激可增加小鼠主动脉VSMCs中Piezo1和Runx2的表达水平,而抑制Piezo1可阻断钙磷诱导的NLRP3炎性小体激活和细胞焦亡,从而减轻钙化[13]。肝硬化疾病模型中,Piezo1可被病理性高刚度微环境有效激活[5],并改变YAP相关的谷氨酰胺代谢以调节主动脉瓣间质细胞的成骨分化[6]。本研究中,钙磷诱导的CASMCs钙化过程中伴有Piezo1表达上调,提示Piezo1在血管钙化的发生发展过程中具有重要作用。为进一步证实Piezo1与钙磷诱导的CASMCs钙化的相关性,我们分别采用Piezo1抑制剂GsMTx4和siRNA-Piezo1进行干预,结果发现,钙磷诱导的CASMCs钙化在抑制或敲低Piezo1后有所缓解,成骨表型相关蛋白表达减少。已有研究表明,Yoda1特异性激活Piezo1可促进主动脉瓣间质细胞的成骨样分化[6],且这种促进作用可在结合一定机械刺激后增强[14]。本研究在钙磷刺激的基础上给予Piezo1激动剂Yoda1干预,结果显示钙化进一步加剧,成骨表型相关蛋白表达升高。以上提示高钙磷刺激可通过激活Piezo1通道,进而促进CASMCs钙化。
Piezo1介导的Ca2+内流可激活细胞内由Ca2+介导的信号通路,对细胞的代谢过程与功能产生影响,且Ca2+已被确定为激活多种线粒体酶的标志性刺激信号,由坚硬基质激活的Piezo1相关Ca2+内流可加速线粒体裂变与细胞凋亡[15]。此外,线粒体功能障碍在过往研究中也被公认为是血管钙沉积的主要原因,线粒体结构和呼吸功能受损最终会加速VSMCs的成骨细胞表型转变和钙沉积[8]。本研究也发现,钙磷刺激下的CASMCs钙化伴有DRP1Ser616等线粒体分裂相关蛋白表达增加,这种现象在给予Piezo1激动剂Yoda1干预后进一步加剧,提示Piezo1对CASMCs钙化的促进作用可能与线粒体动力学失衡有关。此外,借助线粒体分裂抑制剂Mdivi-1靶向抑制DRP1相关的线粒体分裂,结果显示钙磷诱导的细胞钙化明显减轻,成骨表型相关蛋白表达下降,这表明维持线粒体稳态的疗法可能对改善CKD模型中的血管钙化具有积极作用。
综上,本研究初步证实了Piezo1激活可介导高钙磷刺激下的CASMCs钙化,其机制可能与线粒体动力学失衡有关,但仍存在不足。一方面,研究缺乏在组织和动物水平上的进一步验证,单一的细胞模型具有一定局限性;另一方面,本研究未深入监测钙磷诱导下Piezo1激活相关的细胞内游离Ca2+浓度变化,Piezo1与线粒体动力学之间的具体调控机制尚不明。后续将继续在动物和组织水平上验证Piezo1可能调控的信号通路,并探究Piezo1激活的下游靶点,为防治CKD相关的心血管疾病提供新的理论依据。
