Hypertension is the most common adverse reaction associated with anlotinib during anti-tumor therapy, with an incidence rate as high as 76. 9%. More importantly, the occurrence and progression of hypertension and its complications significantly impair the quality of life for cancer patients, potentially leading to treatment discontinuation and increasing the risk of disease progression. Currently, research on anlotinibrelated hypertension remains limited. Given this, this paper primarily explores the specific mechanisms underlying anlotinibrelated hypertension and reviews its treatment strategies, aiming to provide a theoretical basis for individualized blood pressure management in clinical practice.

高血压是安罗替尼在抗肿瘤治疗过程中最常见的不良反应,发生率最高可达76. 9%。高血压及其并发症的发生与发展会严重影响肿瘤患者生活质量,甚至导致治疗中断,增加患者病情进展的风险。目前,关于安罗替尼相关高血压的研究报道相对有限。鉴于此,该文主要对安罗替尼相关高血压的具体发生机制进行探讨,并对其治疗策略进行综述,以期为临床个体化血压管理提供理论依据。

Hypertension is the most common adverse reaction associated with anlotinib during anti-tumor therapy, with an incidence rate as high as 76. 9%. More importantly, the occurrence and progression of hypertension and its complications significantly impair the quality of life for cancer patients, potentially leading to treatment discontinuation and increasing the risk of disease progression. Currently, research on anlotinibrelated hypertension remains limited. Given this, this paper primarily explores the specific mechanisms underlying anlotinibrelated hypertension and reviews its treatment strategies, aiming to provide a theoretical basis for individualized blood pressure management in clinical practice.

安罗替尼(anlotinib, AL3818)是我国自主研发的口服小分子多靶点酪氨酸激酶抑制剂(tyrosine kinase inhibitors, TKI), 能够抑制血管内皮生长因子受体(vascular endothelial growth factor receptor, VEGFR)、血小板衍生生长因子受体(platelet-derived growth factor receptor, PDGFR)、成纤维细胞生长因子受体(fibroblast growth factor receptor, FGFR)和干细胞因子受体(stem cell factor receptor, c-Kit)等多种激酶, 具有抗肿瘤血管生成和抑制肿瘤生长的双重作用[1-2]。目前, 该药已被批准用于晚期非小细胞肺癌、小细胞肺癌、转移性软组织肉瘤等恶性肿瘤的治疗[2-3]。然而, 作为多靶点TKI, 安罗替尼在抑制肿瘤的同时, 也会干扰正常细胞功能, 引发心血管毒性, 其中高血压尤为突出。多项研究表明, 接受安罗替尼治疗的患者中, 超过半数出现高血压, 发生率最高达到76. 9%[4-5]。尽管安罗替尼为肿瘤患者提供了新的治疗选择, 但高血压导致的药物减量或停药, 严重影响了疗效和患者生活质量, 并可能增加病情进展风险。因此, 深入理解其发生机制并实施有效的治疗策略至关重要。本文围绕安罗替尼相关高血压的发生机制及治疗进展进行综述, 以期为未来研究和临床治疗提供参考。
目前, 在哺乳动物中已鉴定出3种VEGFR亚型, 分别为VEGFR-1、VEGFR-2和VEGFR-3, 其中VEGFR-1和VEGFR-2主要在内皮细胞表面表达; 血管内皮生长因子(vascular endothelial growth factor, VEGF)家族包括VEGF-A、-B、-C、-D和胎盘生长因子(placental growth factor, PLGF)5种配体, 其中VEGF-A是主要活性成分[6]。在VEGF抑制剂诱导高血压机制的研究中, 一氧化氮(nitric oxide, NO)与内皮素-1(endothelin-1, ET-1)间的失衡一直备受关注。
NO是由内皮细胞通过内皮一氧化氮合酶(endothelial nitric oxide synthase, eNOS)产生的一种强效内源性血管舒张因子, 通过激活可溶性鸟苷酸环化酶、提高环鸟苷酸水平而发挥作用。在生理状态下, VEGF与内皮细胞表面的VEGFR-1或VEGFR-2结合, 触发受体二聚化及胞内酪氨酸激酶结构域的自磷酸化, 进而激活下游信号通路。其中, 磷酸肌醇3-激酶/蛋白激酶B(phosphoinositide 3-kinase-protein kinase B, PI3K/AKT)通路活化可增强eNOS磷酸化, 明显增加NO生成, 从而促进内皮细胞增殖与血管生成[6]。然而, 当VEGF信号通路(vascular endothelial growth factor signaling pathway, VSP)被抑制时, 该过程发生逆转: NO通路受到明显抑制, 而ET-1通路则被激活。这种失衡最终导致血管收缩增强及高血压发生[7]。目前普遍认为, NO水平降低是VEGFR酪氨酸激酶抑制剂(vascular endothelial growth factor receptor-tyrosine kinase inhibitors, VEGFR-TKI)诱发高血压的关键机制[8]。这一观点得到了多项研究的支持: Daher等[9]发现, 接受抗VEGFR-2治疗的小鼠肾脏中eNOS表达下调, NO合成减少, 诱发高血压; 仑伐替尼的临床研究也报道了高血压的发生与内皮功能受损及血清NO浓度降低明显相关[10]。临床前研究显示, 安罗替尼作为强效VEGFR-2抑制剂(其对VEGFR-2的半数抑制浓度IC50为0. 2±0. 1 nmol·L-1)[1], 其诱发高血压的机制同样主要归因于VEGFR-2抑制所致的NO生成减少。因此, 鉴于NO减少在VSP抑制剂所致高血压中具有关键作用, 补充NO供体可能成为改善此类药物相关高血压的有效策略。
除NO减少外, ET-1分泌增加可能是VSP抑制剂诱发高血压的另一关键机制。ET-1是由内皮细胞释放的强效血管收缩肽, 主要通过激活血管平滑肌细胞上的ETA受体或ETB受体发挥其缩血管效应, 值得注意的是, ETB受体同时也作为ET-1的清除受体[11]。Lankhorst等[12]研究发现, 舒尼替尼以剂量依赖方式增加大鼠ET-1水平; 另一项研究显示, 舒尼替尼对VSP的抑制与血压升高明显相关, 伴随着ET-1血浆浓度明显升高, 而使用内皮素拮抗剂可有效逆转高血压[13]。尽管如此, VSP抑制剂导致内皮素信号通路激活的具体分子机制尚未完全阐明。现有研究提出几种潜在机制: 其一可能与内皮细胞上ETB受体表达下调有关, 导致ET-1清除减少、蓄积增加[14]; 此外, 有研究报道, Ras同源家族成员A(Ras homolog family member A, RhoA)/ Rho相关蛋白激酶(rho-associated protein kinase, ROCK)信号通路的激活可能是阿帕替尼诱导高血压、激活内皮素信号通路的潜在机制[15]。上述结果提示, ET受体拮抗剂可能治疗VSP抑制剂诱导的高血压。
氧化应激是指细胞内信号分子活性氧(reactive oxygen species, ROS)生成与清除失衡, 导致ROS过度蓄积, 进而引发分子、细胞及组织损伤的病理过程。细胞内ROS主要来源于还原型烟酰胺腺嘌呤二核苷酸磷酸氧化酶(nicotinamide adenine dinucleotide phosphate oxidase, NOX)和线粒体。ROS在调节正常生理活动中非常关键; 在病理条件下, 氧化还原稳态失衡可导致血管氧化损伤, 在高血压发生发展中扮演重要角色。主要表现在: 1)在多种高血压模型中, 血管NOX4的表达和ROS的产生都增加[16]; 2)氧化应激在VEGFR-TKI诱导的高血压机制中也起着重要作用。VSP抑制剂可激活NOX亚型(NOX1/NOX4)触发氧化应激, 导致血管功能障碍与高血压[17]。舒尼替尼的大鼠实验显示, 血压升高的同时伴随胸主动脉NOX1、NOX2和NOX4表达明显上调, 内皮依赖性血管舒张受损[18], 提示氧化应激所致内皮功能障碍是其重要机制, 靶向抑制NOX活化可能是潜在干预策略。此外, p47phox作为NOX的调节亚基之一, 其介导的RhoA/ROCK/p47phox信号轴激活可促进ROS的生成, 抑制AKT/eNOS/NO信号通路, 导致eNOS解偶联及血管功能障碍[17]。阿帕替尼动物实验也证实, 抑制RhoA/ROCK通路可减轻血管平滑肌氧化应激[19]。这表明RhoA/ROCK信号通路的激活可能是触发氧化应激的潜在机制。然而, Han等[20]研究发现, 安罗替尼可通过抑制RhoA/ROCK信号通路调节细胞外基质(extracellular matrix, ECM)表达和物理性质, 增加抗肿瘤药物在肿瘤中的分布和滞留。由此可见, 安罗替尼可能并非通过RhoA/ROCK通路触发氧化应激及ET-1信号激活。目前, 安罗替尼诱导的高血压是否存在RhoA/ROCK通路参与, 尚需进一步研究阐明。除直接激活NOX外, 氧化应激与ET-1信号存在明显交互。ET-1轴的经典激活可增强NOX, 进一步增加ROS的生成, 后者可能通过加剧NO生物利用度下降和血管收缩促进高血压[21]
总体而言, 氧化应激是VSP抑制剂诱导高血压的重要机制, 其与NO减少和ET-1激活通路存在复杂互作, 但其确切作用机制仍需更深入研究阐明。
微血管稀疏以微血管空间密度降低为主要特征。生理状态下, VSP对维持毛细血管网络的完整性至关重要。然而, VSP被抑制会干扰血管内皮细胞的正常增殖和分化, 并促进其凋亡, 最终导致毛细血管数量减少和血管稀疏[22]。这一结构改变已被临床研究所证实: Milling等[23]利用侧流暗场成像技术观察到VEGFR-TKI治疗后黏膜毛细血管明显减少; Mäki-Petäjä 等[24]也报道, 帕唑帕尼治疗仅两周即可引起明显的血管结构重塑, 表现为微血管密度和直径同步减小。值得注意的是, 微血管的结构性稀疏会导致全身血管阻力增加, 进而成为血压升高的直接机制之一。然而, 微血管稀疏在VEGFR-TKI相关高血压发展中的确切作用仍存在争议。部分学者认为, 微血管稀疏可能是VEGFR-TKI相关高血压持续状态下的继发性结果, 而非初始诱因, 因为高血压本身即可诱导血管重塑和血流量减少[25]。支持此观点的证据包括: 1)时间因素: 微血管从功能性改变进展至结构性稀疏是一个长期过程, 而Kappers等[26]观察到舒尼替尼诱导的高血压常在短期内迅速发生, 难以完全归因于全身性微血管稀疏; 2)可逆性证据: VEGFR-TKI相关高血压在停药后通常迅速缓解, 这与结构性微血管稀疏作为病因所预期的持续性不符。由于缺乏安罗替尼是否引起人体微血管密度下降的直接证据, 基于现有研究可推断, 微血管稀疏与安罗替尼诱导的高血压可能存在相关性, 但在疾病早期阶段的作用可能相对有限。综上所述, VEGFR-TKI诱导的高血压与微血管密度下降之间存在复杂关联, 其因果关系及相对作用仍需更深入地研究。
在肾脏中, VEGF及VEGFR在足细胞和肾小球内皮细胞中高表达。足细胞分泌的VEGF与肾小球内皮细胞表面的VEGFR结合, 对肾小球滤过屏障建立和维持至关重要[27]。因此, 抑制VSP会影响肾小球结构与功能, 导致肾小球滤过率下降。不仅如此, VSP激活对肾小球发育不可或缺, VEGF可诱导肾小球内皮细胞产生NO, 而NO在调节肾脏灌注和钠离子(Na+)重吸收中发挥关键作用。VSP抑制剂对肾脏的损害是多方面的: 不仅影响肾小球内皮完整性, 还可阻断包括eNOS、RAF、MAPK/ERK和mTOR在内的多条下游信号通路。这种多重抑制最终损害肾功能和肾血管稳态, 进而引发高血压、蛋白尿及血栓性微血管病等不良反应[28]。此外, 有学者提出肾性高血压的另一潜在机制: 肾小球毛细血管损伤可能导致流经肾小管的Na+浓度改变, 进而激活肾素分泌, 增加下游血管紧张素Ⅱ生成, 最终引发全身性血管收缩[29]
总之, 安罗替尼诱导的高血压可能与NO减少、ET-1增加、氧化应激、微血管稀疏和肾损伤有关。鉴于目前针对安罗替尼的机制研究仍相对有限, 其诱发高血压的具体通路及相对贡献, 有待通过更深入的基础研究和大样本临床观察进一步阐明。
安罗替尼诱导高血压的发病率与剂量成正相关。因此, 选择合适的给药方案十分重要。一项I期临床试验显示, 安罗替尼具有较长的消除半衰期(96±17)h, 可能导致药物在体内逐渐蓄积[30]。该研究最终确定口服安罗替尼的推荐给药方案为12 mg·d-1(服用2周/停1周), 并获得了可接受的毒性和良好的依从性用于后续研究。一般而言, 针对3/4级高血压的管理应遵循阶梯式调整原则: 首次发生时暂停安罗替尼, 并联合应用≥ 2种降压药物, 待血压恢复至≤ 2级(收缩压 < 160 mmHg且舒张压 < 100 mmHg)后重启治疗; 若复发, 则逐步减量(首次复发降至10 mg·d-1, 复发降至8 mg·d-1), 剂量调整后仍无法控制或出现顽固性高血压者需永久停药[31]。需警惕的是, 即使规范给药, 仍可能出现高血压危象及严重并发症(如视网膜病变、主动脉夹层)[32], 提示未来需探索更优化的给药策略。
根据2020年《盐酸安罗替尼治疗晚期肺癌中国专家共识》, 安罗替尼相关高血压需进行分级干预: 1级高血压主要进行生活方式干预, 如戒烟、限酒、低钠饮食及适度运动。2级高血压推荐单药降压治疗; 3/4级高血压需给予抗高血压药物联合治疗, 情况严重需停用安罗替尼, 直至血压恢复2级以下。然而, 共识未具体列出针对不同级别高血压应选择何种药物(Tab 1)。NCT02388919临床试验[33]证实, 使用二氢吡啶钙通道阻滞剂、血管紧张素转换酶抑制剂、血管紧张素受体阻滞剂、利尿剂和β受体阻滞剂可使89. 3% 的2/3级高血压患者血压得到控制, 40例3/4级高血压患者经药物治疗后, 22例血压降至1/2级, 17例仍维持在较高水平。该研究仅列出药物类别, 未详细说明具体给药方案、目标人群或疗效差异。有病例报道显示, 患者在使用安罗替尼过程中除发生高级别高血压外, 还出现了视网膜病变和主动脉夹层[32]。提示安罗替尼诱导的高血压可能在严重并发症中起关键作用, 临床医生在治疗期间应高度重视血压监测与管理。此外, 还应根据患者个体差异进行药物选择。例如, 存在电解质紊乱或腹泻风险的患者不应服用噻嗪类利尿剂。安罗替尼应注意避免与维拉帕米、地尔硫䓬等非二氢吡啶类钙通道阻滞剂联合使用, 因为后者通过细胞色素P450(cytochrome P450, CYP)3A4代谢, 而安罗替尼主要由CYP3A4/5和CYP1A2代谢, 联用可能导致安罗替尼体内血药浓度升高, 增加心血管不良反应风险。对于老年或合并慢性肾病的肿瘤患者, 肾功能通常逐渐下降, 应用抗血管生成药物易导致蛋白尿, 因此《中国高血压防治指南2018修订版》推荐此类患者首选血管紧张素转换酶抑制剂或血管紧张素受体阻滞剂; 合并心血管疾病的肿瘤患者可选用钙通道阻滞剂, 有助于防止或逆转靶器官损害(心肌肥厚、动脉粥样斑块等)。综上, 及时调整剂量、暂停给药及启动降压治疗对管理安罗替尼诱导的高血压至关重要。
临床实践中, 血管紧张素转换酶抑制剂、血管紧张素受体阻滞剂、β受体阻滞剂和二氢吡啶类钙通道阻滞剂是降压首选药物。基于NO减少在VSP抑制剂相关高血压发病机制中的关键作用推测, 增强NO信号转导的药物可能更具治疗潜力, 如兼具β1受体阻滞与NO激活作用的奈必洛尔[16]。但目前奈必洛尔的应用尚缺乏临床证据支撑, 且无安罗替尼联合奈必洛尔的随机对照试验或病例系列报道, 其对VSP抑制剂诱导高血压的疗效有待验证。临床前研究揭示了更多潜在治疗策略: Mirabito等[34]证实选择性ETA或ETA/ETB双重拮抗剂可有效预防舒尼替尼诱导的大鼠高血压; Dabiré 等[35]研究表明, 磷酸二酯酶抑制剂西地那非通过靶向NO途径可减轻索拉非尼引起的大鼠高血压。除药物治疗外, 需同步推行生活方式干预, 包括均衡饮食(尤其限制钠盐)、避免应激、减少咖啡因及酒精摄入, 并增加规律运动。由于VSP抑制剂诱导高血压的高盐敏感性, 限制饮食中Na+的摄入在患者日常管理中具有重要意义。因此, 安罗替尼相关高血压的治疗需采取综合策略。肿瘤心脏病学多学科协作模式的建立, 有助于为患者制定兼顾抗肿瘤疗效与心血管风险最小化的个体化方案。总体而言, 安罗替尼相关高血压治疗的核心目标在于通过给药剂量调整与降压药物的应用, 降低心血管并发症及肿瘤相关死亡风险。
安罗替尼作为多靶点TKI, 已广泛用于各种实体肿瘤治疗。尽管患者生存获益得到明显提升, 但其诱发的高血压已成为最常见的不良反应, 可能导致严重并发症并影响治疗进程。Fig 1对现有研究报道该不良反应的发生机制和治疗策略进行了总结, 主要机制涉及VEGFR-2抑制介导的血管舒缩失衡(NO生成减少/ET-1水平升高)、氧化应激、微血管稀疏及肾损伤等多通路相互作用。因此, 亟须通过动物实验与临床研究深入阐明其分子机制, 并基于大规模临床试验优化给药方案。同时, 结合基因组学与代谢组学技术开发个性化防治策略具有重要价值。此外, 应强调对治疗相关高血压的个体化管理, 适宜的治疗措施和生活方式干预对有效控制安罗替尼诱导的高血压至关重要, 最终有望改善患者的长期预后。
