Objective

To characterize longitudinal trajectories of testicular development in boys and breast development in girls, so as to provide reference data for understanding patterns of pubertal sexual maturation.

Methods

Based on the Shanghai Pudong New Area Cohort Study on Growth, Development and Health in Children and Adolescents, a baseline survey was conducted in 2020 using a mult-stage cluster random sampling method. A total of 2 184 children who completed all follow-ups during the primary school period from 13 elementary schools in Pudong New Area, Shanghai, with annual follow-ups during 2021-2025. Testicular volume and Tanner stage of breast development were assessed by professional physicians using standardized visual inspection and palpation. The age distribution of testicular volume and breast development was fitted by using cumulative link mixed models and Turnbull's nonparametric maximum likelihood estimation method.

Results

Median ages for testicular volumes of 2, 3, 4 and 5 mL in boys were 7.07, 9.24, 10.29, and 11.57 years old, respectively. Median ages for Tanner breast stages Ⅱ, Ⅲ, Ⅳ, and Ⅴ in girls were 8.55, 10.17, 11.18, and 13.78 years old, respectively. Based on overweight and obesity, stratified analysis showed that earlier pubertal onset among overweight/obesity children, and the key milestones for pubertal initiation were testicular volume reaching 4 mL in boys and breast Tanner Ⅱ in girls for 10.29, 10.83; 8.18, 9.00 years.

Conclusion

Overweight and obesity are associated with earlier pubertal initiation, but there are certain gender and developmental stage-specific patterns.

目的

拟合男生睾丸和女生乳房发育的纵向变化,为进一步理解青春期性征发育规律提供参考。

方法

基于“上海浦东新区儿童青少年生长发育与健康”队列于2020年进行基线调查,采用多阶段整群随机抽样方法,在上海市浦东新区选取13所小学2 184名完成小学阶段全部随访的儿童为研究对象,2021—2025年每年随访1次。睾丸容积和乳房发育Tanner分期数据由专业医生通过标准化视诊和触诊获得。采用累积链接混合模型和Turnbull非参数极大似然估计法拟合睾丸容积和乳房发育的年龄分布。

结果

男生睾丸容积达到2,3,4和5 mL的中位年龄分别为7.07,9.24,10.29和11.57岁;女生乳房发育进入Tanner Ⅱ、Ⅲ、Ⅳ和Ⅴ期的中位年龄分别为8.55,10.17,11.18和13.78岁。根据是、否超重肥胖的分层分析显示,超重肥胖儿童进入青春期普遍更早,青春期启动关键里程碑事件男生睾丸容积达到4 mL,女生乳房发育进入Tanner Ⅱ期的中位数年龄分别为10.29,10.83;8.18,9.00岁。

结论

儿童超重肥胖与青春期发育提前相关,但存在一定性别和发育阶段特异性。

目的

拟合男生睾丸和女生乳房发育的纵向变化,为进一步理解青春期性征发育规律提供参考。

方法

基于“上海浦东新区儿童青少年生长发育与健康”队列于2020年进行基线调查,采用多阶段整群随机抽样方法,在上海市浦东新区选取13所小学2 184名完成小学阶段全部随访的儿童为研究对象,2021—2025年每年随访1次。睾丸容积和乳房发育Tanner分期数据由专业医生通过标准化视诊和触诊获得。采用累积链接混合模型和Turnbull非参数极大似然估计法拟合睾丸容积和乳房发育的年龄分布。

结果

男生睾丸容积达到2,3,4和5 mL的中位年龄分别为7.07,9.24,10.29和11.57岁;女生乳房发育进入Tanner Ⅱ、Ⅲ、Ⅳ和Ⅴ期的中位年龄分别为8.55,10.17,11.18和13.78岁。根据是、否超重肥胖的分层分析显示,超重肥胖儿童进入青春期普遍更早,青春期启动关键里程碑事件男生睾丸容积达到4 mL,女生乳房发育进入Tanner Ⅱ期的中位数年龄分别为10.29,10.83;8.18,9.00岁。

结论

儿童超重肥胖与青春期发育提前相关,但存在一定性别和发育阶段特异性。

Objective

To characterize longitudinal trajectories of testicular development in boys and breast development in girls, so as to provide reference data for understanding patterns of pubertal sexual maturation.

Methods

Based on the Shanghai Pudong New Area Cohort Study on Growth, Development and Health in Children and Adolescents, a baseline survey was conducted in 2020 using a mult-stage cluster random sampling method. A total of 2 184 children who completed all follow-ups during the primary school period from 13 elementary schools in Pudong New Area, Shanghai, with annual follow-ups during 2021-2025. Testicular volume and Tanner stage of breast development were assessed by professional physicians using standardized visual inspection and palpation. The age distribution of testicular volume and breast development was fitted by using cumulative link mixed models and Turnbull's nonparametric maximum likelihood estimation method.

Results

Median ages for testicular volumes of 2, 3, 4 and 5 mL in boys were 7.07, 9.24, 10.29, and 11.57 years old, respectively. Median ages for Tanner breast stages Ⅱ, Ⅲ, Ⅳ, and Ⅴ in girls were 8.55, 10.17, 11.18, and 13.78 years old, respectively. Based on overweight and obesity, stratified analysis showed that earlier pubertal onset among overweight/obesity children, and the key milestones for pubertal initiation were testicular volume reaching 4 mL in boys and breast Tanner Ⅱ in girls for 10.29, 10.83; 8.18, 9.00 years.

Conclusion

Overweight and obesity are associated with earlier pubertal initiation, but there are certain gender and developmental stage-specific patterns.

青春期是儿童向成人过渡的关键阶段,主要表现为第二性征发育及生殖功能的成熟[1-2]。性发育是一个连续演进的过程,具有明确的时间进程规律[3]。20世纪60年代,Tanner等[4-5]根据第二性征发育表现,将青春期发育划分为5个阶段,即Tanner分期,该分期是目前评估青春期发育进程的“金标准”[6-7]
青春期发育启动时点与进程变化是揭示青春期发育规律的核心内容[8-9],但是基于人群队列的流行病学基础数据非常薄弱[10]。2012年,中国青春期发育研究协作组基于我国9个地区3万余名儿童青少年的横断面调查,建立了多项青春期发育里程碑事件的年龄参考值[11-12]。然而,该研究无法动态刻画青春期发育进程的演变轨迹。随着当前儿童青少年在营养、环境及生活方式等方面变化,开展基于纵向设计的队列研究,对探索我国儿童青春期发育进程规律具有重要意义。本研究通过累积链接混合模型和Turnbull非参数极大似然估计法[13-14]拟合男生睾丸和女生乳房的发育轨迹,旨在为我国儿童青春期发育时相评估提供基于纵向数据的年龄百分位参数。
依托“上海浦东新区儿童青少年生长发育与健康”队列开展前瞻性研究,该队列采用多阶段整群随机抽样方法在浦东新区不同经济水平社区抽取13所小学作为项目学校,其中城区6所,城郊5所,农村2所。队列于2020年开展基线调查,共招募2 244名一年级小学生,自2021年起每年随访1次。由于上海市小学为五年制学制,2025年研究对象升入初中后失访显著增加,故本研究纳入完成小学阶段全程随访的2 184名儿童,其中男生1 194名,女生990名。基线年龄范围6.00~7.87岁,平均年龄(6.63±0.33)岁。研究对象均为自愿参与并签署知情同意书,且长期定居于上海市。本研究已通过上海市浦东新区疾病预防控制中心医学伦理委员会审核批准(批号:PDCDC-2021-10)。
由儿科内分泌医师和经过培训的专业人员通过视诊及触诊体格检查进行评估。研究选取男生睾丸容积和女生乳房发育Tanner分期2个青春期发育关键指标纳入分析。
采用便携式身高测量仪和电子体重秤测量身高和体重,并计算体质量指数(body mass index,BMI)=体重(kg)/[身高(m)]2。依据《学龄儿童青少年超重与肥胖筛查》判定被试超重肥胖情况[15]。基线调查时收集儿童的出生日期、出生体重(< 2 500 g为低体重,2 500~3 999 g为正常,≥4 000 g为高体重)、是否早产、分娩方式、婴儿期喂养方式、家庭人均月收入、父母受教育程度等人口学特征,其中父母受教育程度中,初中及以下记1分,高中或中专记2分,大专及以上记3分;父母双方分数相加,2~3分定义为低教育水平,4~5分为中等水平,6分为高水平。
队列建立规范化调查方案与操作流程,所有调查人员经统一培训与考核。现场调查实行全过程质控;测量设备在调查前统一校准,调查期间每日检查并记录。问卷与信息采集当场核对完整性和逻辑一致性,数据录入采用双人独立录入与一致性校验,缺失或异常信息及时追溯原始记录并更正。
采用R 4.2.2软件进行数据分析,定量资料以(x±s)表示,组间比较采用t检验;定性资料以频数及构成比表示,组间比较采用χ2检验。为充分利用纵向随访信息并降低青春期发育指标缺失值对分析的影响,结合基本人口学特征,采用基于Logit链接函数的累积链接混合模型拟合青春期发育阶段的纵向变化,并引入个体水平随机截距处理重复测量相关性[16]。根据模型估计的阈值参数和回归系数进行计算,取最大概率对应的阶段作为估计值;对个体施加纵向逻辑约束,以保证青春期发育阶段不可随时间逆向变化。以首次达到对应发育阶段为事件,根据相邻年度青春期发育指标的变化识别并定义左、右和区间删失。采用Turnbull非参数极大似然估计法拟合生存函数[13-14],获得第15,25,50,75和85百分位年龄,并利用Bootstrap算法(重复抽样5 000次)计算95%CI[17]。双侧检验水准α=0.05。
2 184名儿童中,超重肥胖儿童共786名(36.0%)。男女生在是否超重肥胖、出生体重、婴儿喂养方式、家庭人均月收入、父母受教育水平方面的分布差异均有统计学意义(P值均 < 0.05)。见表 1
男生8岁时绝大多数睾丸容积为2 mL,至9岁时以3 mL为主,10岁时4 mL比例明显上升,≥11岁者以4 mL占比最高,并有部分进入5 mL。女生8岁时超过半数乳房发育处于Tanner Ⅱ期,但已有约1/5进入Ⅲ期;9~10岁时Ⅲ期比例进一步增加,≥11岁时超过1/3达到Ⅳ期,甚至进入Ⅴ期。见表 2
男生睾丸容积达到2,3,4和5 mL的中位年龄分别为7.07,9.24,10.29和11.57岁。女生乳房发育进入Tanner Ⅱ、Ⅲ、Ⅳ和Ⅴ期的中位年龄分别为8.55,10.17,11.18和13.78岁。见表 3。由于随访时间限制,部分高百分位年龄受右删失影响,无法稳定估计。基于Turnbull估计的男生睾丸容积达到4 mL和女生乳房发育进入Tanner Ⅱ期的累计发生率见图 1
超重肥胖儿童的青春期启动呈现提前趋势。男生睾丸容积达到4 mL的中位年龄在超重肥胖组为10.29岁,比非超重肥胖组(10.83岁)提前0.54岁。女生乳房发育进入Tanner Ⅱ期的中位年龄在超重肥胖组为8.18岁,比非超重肥胖组(9.00岁)提前0.82岁。见表 4。根据BMI分层的男生睾丸容积达到4 mL和女生乳房发育进入Tanner Ⅱ期的累计发生率见图 1
在全球范围内儿童青春期发育年龄呈长期趋势性提前的背景下,建立基于纵向队列数据的青春期发育参数具有紧迫的公共卫生意义[7, 18]。本研究结果显示,男生睾丸容积达到2,3,4和5 mL的中位年龄分别为7.07,9.24,10.29和11.57岁;女生乳房发育进入Tanner Ⅱ、Ⅲ、Ⅳ.55,10.17,11.18和13.78岁。此外,本研究还提供了男生睾丸容积达到4 mL和女生乳房发育进入Tanner Ⅱ期的多个百分位年龄分布情况,为界定发育“正常”与“偏早/偏晚”提供了量化参考。
与2010—2011年全国九地区数据(男生睾丸容积≥4 mL:11.02岁;女生乳房Tanner Ⅱ期:9.18岁)[11-12]及近年基于北京市通州区女生的数据(9.0岁)[19]相比,浦东新区儿童青春期启动更早,提示我国儿童青春期启动存在地区差异及持续提前趋势。系统综述显示,1977—2013年间全球女童青春期启动年龄平均每10年提前0.24岁[7]。本研究中女生启动中位年龄较2010—2011年九地区女生提前0.63岁[11-12]。国内多项研究亦支持该趋势[20-23],全国学生体质调研数据显示,相比于2010年,2014和2019年女生月经初潮中位年龄分别提前了3.60和5.04个月[20]。男生首次遗精中位年龄在2019年较2010年提前了2.00个月[23]
本队列女生乳房发育进入Tanner Ⅱ期的P25~P75间距为1.12年,男生睾丸容积达到4 mL的P25~P75跨度为1.78年,全国对应指标则分别为2.33和2.61年。与全国九地区儿童青春期发育年龄百分位数分布相比,本队列各发育阶段年龄分布变异较小[11-12]。这一差异可能主要源于以下几方面:(1)样本代表性。本队列集中于单一城市区域,人群同质性高;全国数据覆盖城乡及不同地理经济条件人群,异质性大。(2)调查时间。全国数据基于2010—2011年横断面调查,而本队列为2020年启动的前瞻性随访,在青春期发育长期提前的趋势下,数据整体前移且分布更集中。(3)调查方法。本研究采用纵向重复测量数据拟合发育轨迹,全国数据为横断面单时点评估,可能引入更大的测量变异。
分层分析显示,超重肥胖儿童的青春期发育进程普遍提前,该趋势在女生中表现更为一致且贯穿多个分期,与既往研究相一致[24-25]。在男生BMI对青春期发育的影响中并非贯穿所有阶段,而是在某些睾丸容积阶段观察到提前,展现出阶段特异性差异,既往研究也有类似发现,值得进一步关注[26]
本研究还存在一定局限性:首先,随访时间尚不足以覆盖青春期全程;其次,每年1次的随访密度会产生较宽区间的删失数据,难以精确定位各发育阶段的起始时间。此外,所有样本均来自上海市浦东新区,结果不足以外推至全国,尤其是农村地区。
综上所述,本研究通过前瞻性纵向队列设计,描绘了男生睾丸容积与女生乳房Tanner分期的发育轨迹,提供了关键发育阶段的年龄百分位数,为青春期发育时相及进程的评估提供了本地化、精细化的量化数据。研究还发现超重肥胖与青春期发育提前相关,但存在一定性别和发育阶段特异性,进一步验证并丰富了体重状态与青春期发育时序相关的科学证据。
