Objective

To investigate the levels of knowledge and skills in infectious diseases among school doctors and health teachers in Beijing's primary and secondary schools in 2023, and analyze the influencing factors, so as to provide a reference basis for enhancing the professional competencies of school doctors and health teachers.

Methods

From October to November 2023, a census method was used to conduct a questionnaire survey among all school doctors and health teachers in 16 districts of Beijing. Chi-square tests and multivariate Logistic regression analysis was used perform statistical analysis.

Results

The awareness rate of infectious disease-related knowledge among school doctors and health teachers in primary and secondary schools in Beijing in 2023 ranged from 34.44% to 98.57%, while the behavior formation rate ranged from 65.90% to 98.64%. The proportions of those with literacy in infectious disease knowledge and behavior among school doctors and health teachers were 82.76% and 85.70%, respectively. Multivariate Logistics regression analysis showed that being a full-time employee, having a bachelor's degree or above, and holding a senior professional title were positively correlated with having literacy in infectious disease knowledge (OR=1.76, 2.57, 1.42, P < 0.01). Compared to medical professionals, those in education and other professions were negatively correlated with having literacy in infectious disease knowledge (OR=0.37, 0.55, P < 0.01). Being a full-time employee, being female, and age were positively correlated with having literacy in infectious disease behavior (OR=1.66, 2.18, 1.02, P < 0.01).

Conclusions

The level of health literacy for infectious diseases among school doctors in Beijing primary and secondary schools is relatively high. Targeted training on key professional knowledge and skills should be prioritized for individuals with deficiencies in infectious disease prevention and control.

目的

了解北京市中小学校2023年校医和保健教师传染病知识和行为素养水平及其影响因素，为提高校医和保健教师的专业技术水平提供参考依据。

方法

2023年10—11月，采用自编问卷对北京市16个区所有中小学校校医和保健教师进行普查，共收集2 581份问卷。采用χ2检验和多因素Logistic回归进行统计学分析。

结果

2023年北京市中小学校校医和保健教师传染病相关知识知晓率为34.44%~98.57%，行为具备率为65.90%~98.64%。校医和保健教师传染病相关知识素养、行为素养的具备率分别为82.76%和85.70%，多因素Logistic回归分析结果显示，专职、本科及以上、高级职称与校医和保健教师具备传染病知识素养均呈正相关(OR值分别为1.76，2.57，1.42)；相对于医学专业，教育类和其他专业与校医和保健教师具备传染病知识素养均呈负相关(OR值分别为0.37，0.55)；专职、女性、年龄与校医和保健教师具备传染病行为素养均呈正相关(OR值分别为1.66，2.18，1.02)(P值均 < 0.01)。

结论

北京市中小学校医和保健教师传染病健康素养水平较高。应针对传染病防控知识和行为的薄弱部分，对重点人群进行重点专业知识和技能培训。

目的

了解北京市中小学校2023年校医和保健教师传染病知识和行为素养水平及其影响因素，为提高校医和保健教师的专业技术水平提供参考依据。

方法

2023年10—11月，采用自编问卷对北京市16个区所有中小学校校医和保健教师进行普查，共收集2 581份问卷。采用χ2检验和多因素Logistic回归进行统计学分析。

结果

2023年北京市中小学校校医和保健教师传染病相关知识知晓率为34.44%~98.57%，行为具备率为65.90%~98.64%。校医和保健教师传染病相关知识素养、行为素养的具备率分别为82.76%和85.70%，多因素Logistic回归分析结果显示，专职、本科及以上、高级职称与校医和保健教师具备传染病知识素养均呈正相关(OR值分别为1.76，2.57，1.42)；相对于医学专业，教育类和其他专业与校医和保健教师具备传染病知识素养均呈负相关(OR值分别为0.37，0.55)；专职、女性、年龄与校医和保健教师具备传染病行为素养均呈正相关(OR值分别为1.66，2.18，1.02)(P值均 < 0.01)。

结论

北京市中小学校医和保健教师传染病健康素养水平较高。应针对传染病防控知识和行为的薄弱部分，对重点人群进行重点专业知识和技能培训。

Objective

To investigate the levels of knowledge and skills in infectious diseases among school doctors and health teachers in Beijing's primary and secondary schools in 2023, and analyze the influencing factors, so as to provide a reference basis for enhancing the professional competencies of school doctors and health teachers.

Methods

From October to November 2023, a census method was used to conduct a questionnaire survey among all school doctors and health teachers in 16 districts of Beijing. Chi-square tests and multivariate Logistic regression analysis was used perform statistical analysis.

Results

The awareness rate of infectious disease-related knowledge among school doctors and health teachers in primary and secondary schools in Beijing in 2023 ranged from 34.44% to 98.57%, while the behavior formation rate ranged from 65.90% to 98.64%. The proportions of those with literacy in infectious disease knowledge and behavior among school doctors and health teachers were 82.76% and 85.70%, respectively. Multivariate Logistics regression analysis showed that being a full-time employee, having a bachelor's degree or above, and holding a senior professional title were positively correlated with having literacy in infectious disease knowledge (OR=1.76, 2.57, 1.42, P < 0.01). Compared to medical professionals, those in education and other professions were negatively correlated with having literacy in infectious disease knowledge (OR=0.37, 0.55, P < 0.01). Being a full-time employee, being female, and age were positively correlated with having literacy in infectious disease behavior (OR=1.66, 2.18, 1.02, P < 0.01).

Conclusions

The level of health literacy for infectious diseases among school doctors in Beijing primary and secondary schools is relatively high. Targeted training on key professional knowledge and skills should be prioritized for individuals with deficiencies in infectious disease prevention and control.

传染病健康素养是指个人获取、理解和处理有关传染病的信息，并运用这些信息维护和促进个人及他人健康的能力[1]。新型冠状病毒感染疫情暴发以来，公众深刻认识到提高全民健康素养对于正确理解和应对突发公共卫生事件，以及维护个人健康的重要性[2]。2023年，我国有各级各类学校49.83万所，各级各类学历教育在校生2.91亿人[3]。学校卫生是国家公共卫生服务体系建设的重点，中小学校医与保健教师是学校的医疗服务者和学校卫生监督者，在学校传染病管理中不仅能够运用专业知识指导传染病防治，还在疫情上报和处理中发挥关键作用[4]。校医和保健教师的传染病健康素养是学校公共卫生工作的重要环节，对于提高学校传染病管理、应对突发公共卫生事件的能力具有重要意义。本研究于2023年10—11月在北京市开展调查，旨在了解北京市中小学校医和保健教师传染病健康素养水平，以采取有针对性的健康教育措施，提高校医和保健教师的传染病健康素养水平。
１　对象与方法

1.1　对象
2023年10—11月，采用普查法对北京市16个区教委在册各类中小学校(含职高)共1 685所的校医和保健教师开展匿名调查。共计回收问卷2 766份，剔除重要变量缺失和漏答率超过20%的问卷，最终合格问卷2 581份，有效率93.31%。其中男性190名(7.36%)，女性2 391名(92.64%)；校医1 689名(65.44%)，保健教师892名(34.56%)；平均年龄(42.13±8.01)岁。调查前已通过北京市疾病预防控制中心伦理审查同意，并获得调查对象知情同意。
1.2　方法
通过问卷网进行线上调查，由校医和保健教师通过网页或手机填写。问卷根据国家及北京市学校传染病管理工作的相关法律法规、标准、文件等自行编制，该问卷经过多轮专家咨询，并在正式调查前开展了预调查，调查问卷中的问题均为客观题，具有较高的内容效度，该调查表Cronbach α系数为0.74。问卷共20道题，包括10道知识题和10道行为题，内容包括人口学特征、传染病知识和行为。知识知晓率指正确知晓某一特定知识的人数占总调查人数的百分比，行为形成率指实际形成某种特定行为的人数占总调查人数的百分比。具备传染病知识素养指知识题答对8道及以上，具备传染病行为素养指行为题答对8道及以上。
1.3　质量控制
利用问卷网平台开展问卷调查，每个条目均设为必答项目，控制作答设备、IP、作答者只能填写一次，设置合理的作答时间，并设置甄别条件题。调查前统一开展培训，向校医和保健教师详细解释调查目的、内容及填写方法，承诺对数据保密。数据清洗、逻辑核查、汇总由专业人员统一管理和分析。
1.4　统计学分析
采用SPSS 20.0统计软件包对数据进行分析。计数资料以n(%)表示，计量资料以(x±s)描述；不同组别校医和保健教师的知识知晓率、行为形成率比较均采用χ2检验，采用二分类Logistic回归模型分析校医和保健教师传染病知识、行为素养的相关因素。双侧检验水准α=0.05。

2.1　校医和保健教师传染病相关知识知晓率、行为形成率
见表1。
校医和保健教师知识知晓率为34.44%~98.57%，其中对“艾滋病的传播途径”“诺如病毒用哪种消毒液消毒”“手足口病的主要感染人群”“儿童预防水痘最有效的措施”“肺结核的可疑症状”的知晓率较高，均达到95%以上；对“学校流感的日常防控措施”知晓率略为不足，为75.47%；对“学校发生疫情时需要上报的情况”知晓率严重不足，为34.44%。校医和保健教师行为形成率为65.90%~98.64%，其中对手卫生相关行为、“打喷嚏时遮掩口鼻”“呼吸道传染病流行期间采取哪些预防措施”“使用砧板和刀具加工食物时生熟分开”的形成率较高，均达到95%以上；对“因为餐馆卫生不好不去就餐”“外出旅游时关注目的地传染病流行情况”形成率较低，分别为65.90%和65.98%。除“外出旅游时关注目的地传染病流行情况”题目外，其他题目校医的知识知晓率和行为形成率均大于保健教师，城区校医和保健教师的知识知晓率和行为形成率均大于郊区人员。
2.2　校医和保健教师具备传染病知识和行为素养的单因素分析
不同人员类别、城郊区、职业类别、性别、年龄、学历、专业、职称、工作年限、是否有执业资格证书、近1年是否接受过传染病培训校医和保健教师传染病知识素养的具备率差异均有统计学意义；不同人员类别、城郊区、职业类别、性别、专业、工作年限、是否有执业资格证书校医和保健教师传染病行为素养的具备率差异均有统计学意义(P值均 < 0.01)。见表2。
2.3　校医和保健教师具备传染病知识和行为素养的多因素Logistic回归分析
分别以校医和保健教师是否具备传染病健康知识、行为素养为因变量(不具备=0，具备=1)，以单因素分析有统计学意义的因素为自变量、多分类变量以哑变量的形式纳入，用向前LR法逐步回归进行多因素Logistic回归分析。结果显示，专职、本科及以上、高级职称与校医和保健教师具备传染病知识素养均呈正相关(OR值分别为1.76，2.57，1.42)；相对于医学专业，教育类和其他专业与校医和保健教师具备传染病知识素养均呈负相关(OR值分别为0.37，0.55)(P值均 < 0.01)。专职、女性、年龄与校医和保健教师具备传染病行为素养均呈正相关(OR值分别为1.66，2.18，1.02)(P值均 < 0.01)。见表3。
3　讨论

《“健康中国2030”规划纲要》[5]和《健康中国行动(2019—2030年)》[6]对居民健康素养和学生健康行动提出了明确要求，为各地区开展传染病防控工作提供了依据，同时也是评估传染病健康教育和促进工作效果的重要工具。健康素养是能反映个体或群体传染病相关健康理念、认知、行为和技能水平的综合性指标[1]。研究表明，健康素养水平与疾病发生结局之间存在相关性，低健康素养可导致疾病发病率上升、疾病指标异常以及健康服务利用不合理[7]。作为健康素养的重要组成部分，传染病健康素养对校医和保健教师的传染病早期识别能力、学校传染病及突发公共卫生事件的应急能力、以及学生和家长的传染病素养具有重要影响[8]。因此，提升校医和保健教师的传染病素养对于保障学生的健康、维护学校卫生安全以及促进社会公共卫生事业的发展具有重要意义。
本次调查结果显示，校医和保健教师的传染病健康素养水平较高，但仍有薄弱环节。校医和保健教师在学校流感的日常防控措施、学校发生疫情时需要上报的情况知晓方面存在明显不足，这些薄弱环节是学校流感等传染病暴发的主要因素之一[9]。此外，校医和保健教师在传染病行为形成方面，对选择餐馆卫生、旅游时关注目的地传染病疫情状况等关注不够。
多因素Logistic回归分析结果表明，专职人员、本科及以上学历者、医学类专业以及高级职称者的传染病知识素养较高，与以往研究结果一致[10-13]。原因可能是高学历和高职称者通常具有较强的知识学习能力；相对于教育类和其他类专业，医学类人员传染病知识素养较高，与以往研究结果一致[14]。但不同专业人员传染病行为素养方面差异无统计学意义，可能与新型冠状病毒感染疫情期间，公众对传染病相关行为的重视以及卫生习惯的普遍提升有关[15-16]。在单因素分析中，校医的传染病素养水平高于保健教师，原因也与校医通常要求具备医学学历和专业资质、而保健教师是医学或教育学相关的专业有关。
与专职人员相比，兼职人员可能同时从事教学或行政等工作，因此对传染病工作的关注度和投入度较低，传染病知识和行为素养均较低。因此，应通过规范化、梯度化的培训重点关注兼职人员的业务能力。此外，传染病行为素养水平还与性别和年龄有关，女性的传染病行为素养高于男性，与以往研究一致[11, 16]，可能与女性较为细致、多扮演照顾者的角色等有关。本研究中，传染病行为素养随着年龄增长而升高；可能随着年龄增长，相关健康风险增加，人们会更加关注健康问题。但在上海宝山区的研究中发现，年龄段的提升与居民传染病防治素养具备率呈负相关[16]；在山东省的研究中，中间年龄组的居民传染病防治素养水平更高[12]，具体原因今后还需进一步探讨。
