Objective

To explore the association of health literacy with overweight and obesity among middle school students in Chongqing, so as to provide evidence for obesity prevention and control strategies.

Methods

From November to December 2019, a stratified cluster sampling method was used to select 7 851 middle school students from 8 complete middle schools across 4 districts/counties in Chongqing. Participants completed the Adolescent Health Literacy Questionnaire and underwent nutritional status assessments. Chi-square tests and multivariate Logistic regression analyses were performed to evaluate associations.

Results

The prevalence of overweight and obesity among adolescents was 12.95%. The overall health literacy rate and rates for functional, interactive, and critical health literacy dimensions were 62.01%, 66.02%, 50.07%, and 72.42%, respectively. Significant differences in overweight and obesity prevalence were observed across districts/counties, urban/rural areas, sex, school stages, and only-child status (χ2=25.93, 3.86, 80.55, 8.80, 6.43, P<0.05). After adjusting for confounding factors, multivariate Logistic regression revealed that middle school students with overall health literacy (OR=0.86, 95%CI=0.75-0.99), interactive health literacy (OR=0.85, 95%CI=0.74-0.97), and critical health literacy (OR=0.84, 95%CI=0.73-0.98) had a lower risk of overweight and obesity (P<0.05).

Conclusions

Health literacy is inversely associated with overweight and obesity among middle school students. Strengthening health education and improving interactive and critical health literacy may create favorable conditions for adolescent overweight and obesity prevention and control.

目的

探讨重庆市中学生健康素养与超重肥胖的关联, 为中学生肥胖防控提供参考依据。

方法

于2019年11—12月, 采用分层整群抽样方法, 抽取重庆市4个区县8所完全中学7 851名中学生进行健康素养问卷调查和营养状况评定。采用χ2检验和多因素Logistic回归进行统计学分析。

结果

重庆市中学生超重肥胖检出率为12.95%, 总健康素养和功能性、互动性、批判性3个维度健康素养具备率分别为62.01%, 66.02%, 50.07%, 72.42%。不同区县、城乡、性别、学段、是否为独生子女的中学生超重肥胖检出率差异均有统计学意义(χ2值分别为25.93, 3.86, 80.55, 8.80, 6.43, P值均<0.05)。控制相关混杂因素后, 多因素Logistic回归分析结果显示, 具备总健康素养(OR=0.86, 95%CI=0.75~0.99)、互动性健康素养(OR=0.85, 95%CI=0.74~0.97)和批判性健康素养(OR=0.84, 95%CI=0.73~0.98)的中学生超重肥胖发生风险均较低(P值均<0.05)。

结论

中学生健康素养与超重肥胖的发生呈负相关。加强学生健康教育, 提升互动性、批判性健康素养水平可为控制和预防超重肥胖创造有利条件。

目的

探讨重庆市中学生健康素养与超重肥胖的关联, 为中学生肥胖防控提供参考依据。

方法

于2019年11—12月, 采用分层整群抽样方法, 抽取重庆市4个区县8所完全中学7 851名中学生进行健康素养问卷调查和营养状况评定。采用χ2检验和多因素Logistic回归进行统计学分析。

结果

重庆市中学生超重肥胖检出率为12.95%, 总健康素养和功能性、互动性、批判性3个维度健康素养具备率分别为62.01%, 66.02%, 50.07%, 72.42%。不同区县、城乡、性别、学段、是否为独生子女的中学生超重肥胖检出率差异均有统计学意义(χ2值分别为25.93, 3.86, 80.55, 8.80, 6.43, P值均<0.05)。控制相关混杂因素后, 多因素Logistic回归分析结果显示, 具备总健康素养(OR=0.86, 95%CI=0.75~0.99)、互动性健康素养(OR=0.85, 95%CI=0.74~0.97)和批判性健康素养(OR=0.84, 95%CI=0.73~0.98)的中学生超重肥胖发生风险均较低(P值均<0.05)。

结论

中学生健康素养与超重肥胖的发生呈负相关。加强学生健康教育, 提升互动性、批判性健康素养水平可为控制和预防超重肥胖创造有利条件。

Objective

To explore the association of health literacy with overweight and obesity among middle school students in Chongqing, so as to provide evidence for obesity prevention and control strategies.

Methods

From November to December 2019, a stratified cluster sampling method was used to select 7 851 middle school students from 8 complete middle schools across 4 districts/counties in Chongqing. Participants completed the Adolescent Health Literacy Questionnaire and underwent nutritional status assessments. Chi-square tests and multivariate Logistic regression analyses were performed to evaluate associations.

Results

The prevalence of overweight and obesity among adolescents was 12.95%. The overall health literacy rate and rates for functional, interactive, and critical health literacy dimensions were 62.01%, 66.02%, 50.07%, and 72.42%, respectively. Significant differences in overweight and obesity prevalence were observed across districts/counties, urban/rural areas, sex, school stages, and only-child status (χ2=25.93, 3.86, 80.55, 8.80, 6.43, P<0.05). After adjusting for confounding factors, multivariate Logistic regression revealed that middle school students with overall health literacy (OR=0.86, 95%CI=0.75-0.99), interactive health literacy (OR=0.85, 95%CI=0.74-0.97), and critical health literacy (OR=0.84, 95%CI=0.73-0.98) had a lower risk of overweight and obesity (P<0.05).

Conclusions

Health literacy is inversely associated with overweight and obesity among middle school students. Strengthening health education and improving interactive and critical health literacy may create favorable conditions for adolescent overweight and obesity prevention and control.

健康素养是指个人获取和理解基本健康信息和服务，并运用这些信息和服务做出正确决策，以维护和促进自身健康的能力[1-2]。健康素养概念复杂，Nutbeam[3]提出的健康素养三级模型：功能性、互动性、批判性健康素养，涵盖了健康素养各个方面，为系统评估健康素养提供了框架。良好的健康素养能帮助促进健康和预防疾病[4]，可以较好地预测国民健康；低健康素养与不良健康结局有关[5]。近年来，儿童青少年超重肥胖率骤升[6]，被视为威胁全球的公共卫生挑战。有证据表明，在成年人中，不良健康素养可能与肥胖有关[7]。健康素养可以增加儿童和成人对其身体健康的控制，提高克服肥胖问题的意识[8]。因此，为应对儿童青少年超重肥胖的严峻形势，应考虑儿童青少年健康信息的内化、理解和利用能力对其自身营养状况的影响。本研究通过对重庆市中学生健康素养与超重肥胖进行调查，深入探讨两者之间的关联，为改善儿童青少年超重肥胖现状提供依据。
1　对象与方法

1.1　对象
于2019年11—12月，采用分层整群抽样方法，参考重庆市功能区域划分和区县功能定位，从四大功能区(渝东北生态涵养发展区、城市发展新区、都市功能拓展区、都市功能核心区)中各抽取1个调查区(县)，分别命名为A、B、C、D区(县)，每个调查区(县)各抽取2所完全中学(城、乡各1所)，每所学校初一至高三年级各抽取4个班，对抽取班级调查当日全部在校学生进行问卷调查。最终对192个班级共9 338名中学生发放调查问卷，回收问卷9 338份，删除缺失、无效数据后，有效问卷7 851份，有效回收率为84.07%，其中男生3 726名，女生4 125名。本研究经重庆医科大学医学研究伦理委员会批准，参与调查学生均知情同意。
1.2　方法
1.2.1　自编一般情况问卷
学生基本信息：区县、城乡、性别、学段、是否为独生子女等；家庭背景信息：父母文化程度、父母教养方式、自评家庭经济情况等；学校相关信息：自评学习压力、睡眠时间、健康教育课频率等。其中家庭经济情况为采用Likert 5级评分自评，选项分别为1=差、2=较差、3=中等、4=较好、5=好，再进行三分类：差(1与2合并)，中等(3)，好(4与5合并)。学习压力为采用Likert 5级评分自评，选项分别为1=非常大、2=比较大、3=一般、4=比较小、5=非常小，再进行三分类：大(1与2合并)，一般(3)，小(4与5合并)。
1.2.2　青少年健康素养评价量表
在王翎懿等[9-10]编制的初中生健康素养评价量表基础上，引入Nutbeam[3]提出的健康素养三级模型，借鉴中国公民健康素养—基本知识与技能[11]，结合中学生身心发育特点，自行编制青少年健康素养评价量表，包括功能性健康素养(37个条目)、互动性健康素养(14个条目)、批判性健康素养(10个条目)3个维度，共61个条目。问卷各条目采用Likert 5级评分法赋分，从“非常不同意”(或“完全不符合”)到“非常同意”(或“完全符合”)分别赋值1~5分，总量表及3个维度的总分分别为305，185，70和50分。当研究对象总健康素养得分或每个维度得分分别大于等于总分或每个维度总分的80%，即为具备总健康素养或该维度健康素养[2]。以往研究中该量表总健康素养、互动性健康素养、功能性健康素养和批判性健康素养的重测信度为0.62~0.76，Cronbach α系数为0.88~0.92，分半信度为0.50，各维度之间的相关系数为0.40~0.56，各维度与总量表之间的相关系数为0.70~0.86，探索性因子分析(KMO=0.92, P<0.01)证实量表由功能性、互动性和批判性健康素养3个核心维度构成，具有良好信、效度[12]。本次调查中总量表及3个维度的Cronbach α系数为0.88~0.96。
1.2.3　中学生营养状况评估
采用体质量指数(body mass index，BMI)评估中学生营养状况，BMI=体重(kg)/[身高(m)]2。要求学生自填最近一学期学校组织体检时的身高、体重数据，均精确到小数点后1位。学生超重肥胖的判断参考卫生行业标准《学龄儿童青少年超重与肥胖筛查》(WS/T 586—2018)[13]。
1.3　质量控制
调查前对调查员进行统一培训，掌握调查流程和注意事项。现场调查时，在调查学校的配合下，以班级为单位展开调查。发放问卷时强调填写注意事项，承诺对数据保密且只用于人群研究，要求学生现场独立完成，调查员当场核查，对存在漏填项与填写错误项的问卷当场返还学生进行补充完善。调查后统一建立数据库，对问卷录入人员进行统一培训，删除缺失、无效问卷后再进行数据分析。
1.4　统计学分析
采用EpiData 3.1软件建立数据库，采用SPSS 26.0软件进行数据处理和统计分析。计量资料为非正态分布，用M(P25, P75)描述；计数资料使用频数和构成比进行描述，采用χ2检验进行不同特征中学生超重肥胖率的组间比较；采用多因素Logistic回归分析健康素养与超重肥胖的关联，检验水准α=0.05。

2.1　重庆市中学生超重肥胖和健康素养情况
重庆市中学生超重肥胖检出率为12.95%(1 017名)。中学生健康素养得分为251.0(235.0, 273.0)分，总健康素养具备率为62.01%(4 869名)；功能性健康素养得分为154.0(144.0, 169.0)分，具备率为66.02%(5 184名)；互动性健康素养得分为56.0(49.0，62.0)分，具备率为50.07%(3 931名)；批判性健康素养得分为42.0(39.0, 48.0)分，具备率为72.42%(5 686名)。
2.2　重庆市中学生超重肥胖单因素分析
不同性别、城乡、区县、学段、是否为独生子女中学生的超重肥胖检出率差异均有统计学意义(P值均<0.05)。见表1。
2.3　重庆市中学生健康素养与超重肥胖的关联
以是否超重肥胖(0=否，1=是)作为因变量，分别以总健康素养、功能性健康素养、互动性健康素养、批判性健康素养(赋值均为0=不具备，1=具备)为自变量，建立多因素Logistic回归模型。在控制区县、城乡、性别、学段、独生子女等因素后，结果显示，具备总健康素养、互动性健康素养、批判性健康素养的中学生超重肥胖发生风险均较低(P值均<0.05)。见表2。

研究结果表明，重庆市中学生超重肥胖检出率为12.95%，不仅低于“重庆市2019年度居民健康状况报告”中报道的中小学超重肥胖率[14]，同时还低于龚艳彬等[15]的调查结果，可能是学生在自我报告身高体重时存在回忆偏倚。男生超重肥胖检出率(16.53%)高于女生(9.72%)，与国内调研结果相吻合[16-18]，提示应更关注男生饮食习惯与体重管理问题；初中生(14.14%)超重肥胖检出率高于高中生(11.88%)，与多数研究结果一致[19-20]，可能是随着学生健康知识的积累和自律能力的提升[10]，再加上青春期身高快速增长的生理特点[21]，超重肥胖得以缓解。
研究结果显示，重庆市中学生具备总健康素养者超重肥胖发生风险降低，与以往研究结果一致[22]。一项针对12~16岁儿童青少年的研究显示，低健康素养水平与青少年许多方面的健康有关，包括超重肥胖[23]。另外一项研究显示，健康素养水平与青少年健康营养-运动行为和膳食模式呈正相关[24]，具备健康素养的青少年具有较强理解和应用健康信息的能力，能掌握基本的营养知识和运动技能，同时可能会更关注自己的饮食运动行为，注重平衡膳食和合理运动，保持健康体重。研究结果还显示，重庆市中学生具备互动性健康素养和批判性健康素养者超重肥胖发生风险均降低。功能性健康素养与超重肥胖的关联无统计学意义，原因可能是功能性健康素养着重强调基础阅读能力[4]，但是随着教育水平的持续提升，绝大多数中学生具备有效阅读各类健康材料能力。具备互动性健康素养的青少年能多途径获取健康信息并将其应用于不断变化的环境[25]，在一定程度上可增强预防和控制超重肥胖的动机和信心[4]。批判性健康素养越高，自我效能、自我管理能力、疾病应对能力越好[26]，具备批判性健康素养的青少年更清楚自身健康状况和需求，能更积极地参与健康管理，更有可能采取可行性的体重管理方案。综上所述，通过提升中学生的健康素养水平特别是互动性健康素养和批判性健康素养水平，可能有助于增强中学生预防超重肥胖的意识和能力，进而为控制和预防超重肥胖创造有利条件。
本研究也存在一定局限性：首先，研究为横断面研究，无法解释健康素养与超重肥胖的因果关系；其次，考虑到在校学生每年均进行常规健康体检，学生身高、体重数据便通过自我报告形式获取，可能存在回忆偏倚和数据缺失。尽管如此，本研究仍可作为健康素养与超重肥胖关系的探索性研究，在今后还应通过纵向研究方法来证实两者之间的关系。同时，在进行问卷调查时同步测量学生身高、体重以获取更可靠的数据，确保研究结果的准确性。
