目的

本研究旨在基于FDA不良事件报告系统（FDA adverse event reporting system，FAERS）数据库，分析曲贝替定的真实世界不良事件（adverse drug event，ADE）特征，为临床用药安全性管理提供参考。

方法

提取2007年第1季度至2024年第4季度FAERS数据库中曲贝替定相关报告（1 349例），采用MedDRA编码分类系统器官类别（system organ class，SOC）摘要及首选术语（preferred term，PT），通过报告比值比（reporting odds ratio，ROR）、比例报告比（proportional reporting ratio，PRR）等四种比例失衡方法检测信号，并进行性别、年龄亚组及时序分析。

结果

血液及淋巴系统疾病、肝胆系统疾病为主要累及SOC；中性粒细胞减少症（123例）、贫血（117例）等为高频PT；发现6项说明书未收录的潜在ADE；ADE中位发病时间21 d，呈早期失败型，且存在性别（女性更易发生血液学毒性）及年龄（老年人更易发热性中性粒细胞减少）差异。

结论

曲贝替定需重点关注血液学毒性、肝毒性及新发现的多系统潜在风险，临床应根据时间窗口及人群特征加强监测，优化用药方案。对策　建议加强抗肿瘤药物全周期监测、规范不良反应上报、建立多部门协作研究平台。

Objective

To analyze the characteristics of real-world adverse drug events (ADEs) of trabectedin based on the FDA Adverse Event Reporting System (FAERS) database in order to provide references for clinical drug safety management.

Methods

A total of 1 349 trabectedin-related reports were extracted from the FAERS database from Q1 2007 to Q4 2024. Using the MedDRA coding classification system for system organ class (SOC) and preferred term (PT), signal detection was performed through 4 proportional imbalance methods, including reporting odds ratio (ROR) and proportional reporting ratio (PRR). Subgroup analyses by gender, age, and temporal trends were also conducted.

Results

Hematological and lymphatic system disorders and hepatobiliary system disorders were the primary SOCs involved. High-frequency PTs included neutropenia (123 cases) and anemia (117 cases). Eight potential ADEs that have not been listed in the drug product instruction were identified. The median onset time of ADEs was 21 d, showing an early failure pattern, with differences observed by gender (females more prone to hematological toxicity) and age (elderly more susceptible to febrile neutropenia).

Conclusion

Trabectedin requires close attention to hematological toxicity, hepatotoxicity, and newly identified multi-system potential risks. Clinically, monitoring should be strengthened based on time windows and population characteristics to optimize drug regimens. Countermeasure　It is recommended to strengthen the full cycle monitoring of anti-tumor drugs, standardize the reporting of adverse reactions, and establish a multi-departmental collaborative research platform.

“蓝色药库”指海洋中孕育的药用资源^［1］。这一形象的称谓源于海洋生态系统蕴藏的独特药用价值。曲贝替定是海洋药学领域的突破性成果，是从加勒比海特有的红树海鞘体内分离纯化得到的天然活性物质^［2］。曲贝替定于2007年获得欧盟批准用于治疗晚期软组织肉瘤，成为首个源自海洋生态系统的抗肿瘤靶向药物；2015年获得美国食品及药物管理局（Food and Drug Administration，FDA）批准上市。但截止到2025年7月，在我国尚未上市。目前，曲贝替定单药治疗方案和与其他药物联合治疗方案已应用于软组织肉瘤、卵巢癌^［3］。

然而既往研究均集中于其药理机制及临床有效性^［4-5］，而其安全性问题，仅有少量文献研究^［6］。随着曲贝替定在多癌种治疗中的普及，其潜在不良反应的“多样性”与“复杂性”逐渐显现。但相关研究的匮乏使得临床在合并用药、毒副反应管理环节缺乏科学依据。因此，本团队通过真实世界研究，基于FDA不良事件报告系统（FDA adverse event reporting system，FAERS）数据库对曲贝替定相关药物不良事件（adverse drug event，ADE）进行挖掘分析。为临床上的合并用药，减轻毒副反应，提供参考。

1　材料与方法

1.1　数据来源及处理

本研究数据来源于FAERS数据库，采用R 4.3.3软件提取FAERS数据库2007年第一季度至2024年第四季度的曲贝替定不良事件数据。限定检索词为目标药物的通用名“TRABECTEDIN”及商品名“YONDELIS”。根据FDA官方指南，数据处理先合并各季度人口学信息，再进行重复数据清洗。取人口统计学信息表中PRIMARYID、CASEID、FDA_DT字段，按CASEID、FDA_DT、PRIMARYID排序。若存在重复报告，优先留存FDA_DT最新的记录。当FDA_DT与CASEID完全一致时，则选取PRIMARYID值更大的记录予以保留。

本研究采用《监管活动医学词典》（medical dictionary for regulatory activities，MedDRA）中的首选术语对不良事件首选术语（preferred term，PT）进行编码，然后映射到系统器官类别（system organ class，SOC）进行全面分类及分析。为降低混杂因素干扰，仅纳入药物信息中曲贝替定被明确记录为主要疑似药物（preferred suspect，PS）的报告。具体关键步骤见图1。

1.2　分析方法

本研究采用了4种不同的比例失衡方法，报告比值比（reporting odds ratio，ROR）、比例报告比（proportional reporting ratio，PRR）、贝叶斯可信传播神经网络法（Bayesian confidence propagation neural network，BCPNN）和多项伽马泊松压缩法（multi- item gamma poisson shrinker，MGPS）来检测ADE信号。这些方法依据比例失衡法（补充表1），比较与曲贝替定相关的特定ADE的报告频率与其他药物在FAERS数据库中报告的频率。具体的计算方式见（补充表2）。为挖掘不同特征的人群的ADE信号，采用ROR法将按照年龄（<18岁、18~65岁、>65岁），性别进行分组分析。通过比对FDA官网中曲贝替定产品说明书识别潜在ADE信号。对ADE的发生时间计算了中位数及四分位间距（interquartile range，IQR），采用韦伯分析检验ADE的变化比例。并结合文献及FAERS数据库检索，剔除常见合并抗肿瘤药物，进行敏感性分析。

2.1　曲贝替定相关 ADE 报告的基本信息

截至2024年第四季度，曲贝替定在FAERS 数据库中总计1 349例报告。其中，男792例（58.7%），女392例（29.1%），缺失165例（12.2%）；上报数据主要源于美国380例（28.2%）；主要发生在18~65岁年龄段556例（41.2%）和65~85岁年龄段379例（28.1%）；主要上报人群为医疗专业人员，共上报1 232例（91.3%），见表1。自2015年以后FAES数据库的报告例数急剧上升，2016年达到219例，为历年最高，见补充图1。

2.2　曲贝替定相关ADE 报告累及的SOC分布

截至2024年第四季度，共挖掘涉及26种SOC。从SOC的报告例数来看，以各类检查所包含的信号最多，为719例，其次为血液及淋巴系统疾病，全身性疾病及给药部位各种反应，胃肠系统疾病。

从SOC的信号强度来看，信号最强的是血液及淋巴系统疾病；其次则是肝胆系统疾病。而全身性疾病及给药部位各种反应，虽然ADE报告数量较多，但其信号均提示为阴性结果，表明曲贝替定与该ADE信号关联程度较低。其余见表2。

2.3　曲贝替定相关ADE报告累及的前50PT分布

截至2024年第四季度，其中使用ROR方法发现了144个PT，使用PRR方法发现了138个PT，用EBGM方法发现了272个PT，并使用BCPNN方法发现了364个PT。最终共同满足4种方法共104个PT，如图2。

本团队基于PT报告例数截取了信号排名前50的PT。从PT的报告例数来看，以中性粒细胞减少症所包含的信号最多。其次分别是贫血、发热性中性粒细胞减少症、血小板减少症、恶心。

从PT信号强度角度分析，以外渗为最强的信号，其次则分别为血毒性、横纹肌溶解、全血细胞减少症、天门冬氨酸氨基转移酶升高等。而恶心虽然报告例数排在第五位，但仅有IC=0.92提示该ADE信号为阳性，说明曲贝替定与恶心之间的相关性较弱。其余见表3。

2.4　性别亚组分析

剔除未知性别报告后，针对男、女性报告例数最多的前50PT进行了分析。男性以横纹肌溶解所包含的信号最多，其次则分别为急性肾损伤、丙氨酸氨基转移酶升高、贫血、天门冬氨酸氨基转移酶升高等。其余见补充表3。女性以中性粒细胞减少症所包含的信号最多，其次则分别为发热性中性粒细胞减少症、贫血、恶心、全血细胞减少症等。其余见补充表4。

男、女之间ADE信号存在明显性别差异性，女性相较于男性更容易出现中性粒细胞减少症、发热性中性粒细胞减少症、呕吐、虚弱、疲劳等；而男性相较于女性则更容易出现横纹肌溶解、急性肾损伤、肝功能异常等。其余见图3。

2.5　年龄亚组分析

剔除缺失的数据后，本团队针对未成年人（<18岁）、成年人（18~65岁）、老年人（>65岁）的常见PT进行了分析。未成年人ADE报告较少，结果存在不确定性，见补充表5。成年人以全血细胞减少症所包含的信号最多；其次则分别为中性粒细胞减少症、横纹肌溶解、贫血等。其余见补充表6。老年人以发热性中性粒细胞减少症所包含的信号最多；其次则分别为贫血、恶心、横纹肌溶解等。其余见补充表7。

本研究分析了成年人群及老年人群之间的差异，总体上差异并不明显，仅有发热性中性粒细胞减少在老年人群中更易出现，肾功能衰竭在成年人群中更易出现。其余见图4。

2.6　曲贝替定的潜在ADE信号

本团队将排名前50的ADE信号，并与曲贝替定药物说明书对比，其中6项不良反应尚未被当前药品说明书收录，见表4。

2.7　曲贝替定相关不良事件发生的时序分析

剔除缺失的数据后，本团队分析了曲贝替定的461份报告的不良时间诱发时间，中位发病时间为21 d，IQR为（7~54） d，见图5。韦伯分布检验表明，形状参数β的95%CI的上限<1（0.75）为早期失败型，表明ADE的概率随着时间的推移逐渐降低，表5。曲贝替定诱发绝大部分PT均在60 d时接近100%的发病率，而贫血、血小板减少、白细胞减少等PT，相对发病时间较缓慢，见图6。

2.8　敏感性分析

结合文献及检索FAERS数据库发现在应用曲贝替定的过程中，往往会联合阿霉素类药物化疗，因此本研究剔除阿霉素类药物，进行敏感性分析。从PT的报告例数来看排名前五位的分别是横纹肌溶解、中性粒细胞减少症、恶心、全血细胞减少症、急性肾损伤。从PT的信号强度来看排名前五位的是毛细血管渗漏综合征、外渗、血毒性、横纹肌溶解、输液部位渗出。这和之前的分析及既往临床试验结论具有一致性。见补充表8。

3　讨论

3.1　基线分析

目前曲贝替定已经成为软组织肉瘤的研究热点，WANG等^［7］通过文献计量学研究发现曲贝替定已成为近年来的高频关键词。本研究通过FAERS数据库挖掘以探究其在临床的安全性。本研究对基线信息的分析显示，FAERS数据库中有关曲贝替定ADE的人口统计学数据存在显著差异。大多数ADE报告为男性792例（58.7%）。其中18~65岁年龄段556例（41.2%）占大多数。体质量数据显示，43.2%的患者体质量为50~100 kg，但49.3%的病例缺失体质量记录。从2015年欧洲上市来，广泛收到大家关注，最高年上报数量达219例，而近几年趋于平稳一直在年上报数量均在百例左右。曲贝替定是晚期软组织肉瘤的二线用药^［8］。一项爱尔兰关于软组织肉瘤的流行病学调查发现，女性的发病率是男性的1.28倍^［9］。而基于全球疾病负担数据库2021的分析，则发现软组织和骨外肉瘤的发病率及死亡率男性略高于女性^［10］。因此本团队认为导致男性的ADE较多的原因与流行病学关系并不密切。

3.2　SOC及PT水平分析

中性粒细胞减少症是曲贝替定治疗中常见的血液学毒性反应，其发生与药物对骨髓造血功能的抑制密切相关。FAERS数据库分析显示，中性粒细胞减少症在曲贝替定相关不良事件中报告例数最高（n=123），信号强度显著（ROR=13.65，PRR=13.27，EBGM=13.25，IC=3.73），表明其与药物的强关联性，这和既往的报道一致。在阿霉素的单独治疗中，中性粒细胞减少症的发生率为13%，而在阿霉素与曲贝替定联合化疗中，这一比例可上升至80%。性别也是关键风险因素，女性比男性更易出现中性粒细胞减少及发热性中性粒细胞减少的不良反应。研究表明，绝经组与未绝经组的白细胞水平存在明显差异，其雌激素的水平会影响中性粒细胞水平，但具体机制仍需进一步明确血小板减少症的报告例数为102例，信号强度显著，表明其与曲贝替定存在明确关联。该ADE信号在既往临床研究中发生率也印证了这一结论^［12-14］。

曲贝替定治疗过程中需重点关注肝脏毒性风险，其治疗相关毒性反应主要表现为肝功能指标异常。基于FAERS数据，该药物可引起多种肝功能指标的改变，包括天冬氨酸氨基转移酶、丙氨酸氨基转移酶、碱性磷酸酶及γ-谷氨酰转移酶等水平异常增高，同时可能伴随胆红素代谢障碍。基础研究表明，高剂量的地塞米松与曲贝替定联用可以有效保护大鼠免受肝脏损伤^［15-16］。一项意大利的探索性临床试验发现N-乙酰半胱氨酸在接受曲贝替定的患者中具有保肝活性，从而允许肝肾功能受损或发生治疗诱导的肝毒性的患者保持足够的剂量强度和持续给药^［17］。因此肝病或有肝病风险的患者应谨慎使用此类药物，并根据预先存在的肝功能损害的潜在严重程度进行评估用药指征。

横纹肌溶解是曲贝替定治疗中较为常见但严重的肌肉骨骼系统不良事件。一项美国与英国开展的临床试验表明，接受曲贝替定治疗的378名患者中有3名因发生横纹肌溶解导致死亡，122名出现肌酸激酶升高^［18］。FAERS数据库分析显示，横纹肌溶解的报告例数为99例，信号强度显著（ROR=37.89，PRR=37.0，EBGM=36.9，IC=5.21），表明其与曲贝替定的关联性较强。目前可能引起横纹肌溶解的机制目前尚不清楚。但一些研究表明在伴有中性粒细胞减少症和血小板减少症的患者中，化疗第2周期及以后发生横纹肌溶解的可能性更高。并且出现横纹肌溶解后，更容易导致肝衰竭^［19］。因此更应密切监测相关肝功能指标。

曲贝替定存在6种药品说明书未载入的潜在药物不良反应，包括感染性肺炎、感染性休克、尿路感染、胸腔积液、肾功能损害及房颤。其发生机制尚未完全阐明，但临床用药时需提高警惕。

3.3　ADE诱发时序分析

不同器官系统对药物毒性机制的敏感性差异，可能通过ADE发生的时序特征得以体现。在药物安全性评价中，分析用药与不良反应发生的时间关联是至关重要的。通过比较两者间的时间差，明确治疗过程中的高风险时段，并为及时干预或识别副作用提供依据^［20］。本研究分析发现ADE总体呈早发趋势，但在不同的时间节点，主要发生的ADE信号不同。以丙氨酸氨基转移酶升高、天门冬氨酸氨基转移酶为代表的肝功能指标发病时间最早，其中位发病时间仅为3天；以中性粒细胞减少症、发热性中性粒细胞减少症为代表的血液及淋巴系统疾病中位发病时间均为14天。这与曲贝替定对骨髓造血功能的直接抑制作用一致^［21］。而横纹肌溶解中位发病时间则为29天。这提示在临床应用中需根据治疗时间窗口针对性关注不同类型的ADE信号。

3.4　我国曲贝替定研究现况

目前虽然曲贝替定还未在我国上市，但近年来已有不少国内学者开展了相关基础研究，特别是在卵巢癌与小细胞肺癌中。DU等^［22］开发了安全高效的pH响应性晶核胶束，利用紫杉醇和曲贝替定平行递送方式治疗卵巢癌与传统化疗相比具有较高的安全性。FANG等^［23］发现利用新型定向褐藻糖胶和曲贝替定复合物负载的聚乳酸-羟基乙酸共聚物纳米颗粒可以更好的杀死非小细胞肺癌细胞。FANG Q L等研究表名曲贝替定通过调节非小细胞肺癌细胞中的 HIF-1α/IRP1/TFR1和Keap1/Nrf2/GPX4 轴诱导铁死亡进而杀伤癌细胞。GUO等^［24］研究表明曲贝替定PD-1联用可以限制性髓系白血病的进展和复发。

3.5　对曲贝替定药物警戒的建议

基于本研究对曲贝替定真实世界安全性特征的深入分析，建议在以下几个方面加强药物安全监管与临床用药管理：①完善药品上市前审评与上市后监测衔接机制。对于尚未在国内上市但已有显著临床需求的抗肿瘤药物，应提前建立药物警戒计划，明确重点监测的不良反应类别，并将其纳入药品说明书更新和风险管理体系中。②加强医疗机构不良反应上报与数据质量管理。通过培训提升医务人员对抗肿瘤药物毒性认知，鼓励规范、完整填报AE报告，尤其关注联合用药情境下的复杂毒性反应。③建立多部门协作的海洋药物安全性研究平台。联合药监、临床、科研机构开展真实世界数据整合与信号验证，为同类海洋药物的安全使用提供循证依据。

本研究存在以下局限性需重点说明：①报告偏倚：本研究依赖的ADE报告由医生、患者、制药公司等多类主体上报，数据存在漏报、错报及信息缺失等问题，可能引入报告偏倚，进而影响分析结果^［25］。②关键变量缺失：尽管开展了亚组分析，但数据中仍存在年龄、性别等关键变量的缺失，可能影响结果准确性；同时，低报与多报问题也可能引入额外偏倚。③信号评估局限性：本研究仅通过信号强度评估ADE关联性，无法量化具体风险或明确药物与ADE的因果关系，因此需通过前瞻性研究进一步验证新发现的ADE^［27］。

海洋药学作为生物医药领域的新兴交叉学科，正迎来技术突破与产业升级的关键期。从全球视角看，海洋生物资源的丰富性与独特性为新药研发提供了广阔空间，其在抗肿瘤、抗病毒、抗炎等领域的应用潜力被持续挖掘^［28-30］。

本研究通过多层面的系统性分析，为曲贝替定的临床安全性管理提供了全面的真实世界证据支持。基于FAERS数据库，研究采用ROR、PRR、BCPNN和MGPS四种方法，对2007年至2024年的1 349例曲贝替定相关ADE信号进行深度挖掘与验证。研究结果不仅复现了说明书中已明确ADE信号，更识别出6项未被当前说明书收录的潜在风险，需进一步监管关注与机制验证。研究同时揭示ADE的时序特征及人群差异，为个体化用药方案优化提供了关键依据。未来，随着海洋药学研究的深入，更多结构新颖、疗效独特的海洋药物有望问世，而曲贝替定的安全性研究也将为同类药物的安全性探索提供重要参考。
