目的

评价结构化症状干预方案在慢性肾脏病围透析期患者中的应用效果。

方法

采用非同期对照研究设计，通过便利抽样法选取2024年4-9月在陆军军医大学第二附属医院肾脏内科门诊随访的151例CKD 5期非透析患者，根据患者就诊的时间阶段进行分组：2024年4-6月就诊的患者作为对照组（n=75），2024年7-9月就诊的患者作为试验组（n=76）。对照组实施常规症状干预（电话症状报告+健康教育）；试验组在常规症状干预基础上，应用涵盖症状识别、评估、干预、结局4个循证模块的结构化症状干预方案实施干预。干预前及干预3个月后评价效果：采用透析症状指数量表评估症状困扰程度及症状数量；采用健康调查简表（the MOS 36 Short Form Health Survey，SF-36）评估生活质量；比较2组患者干预后临床指标及终点事件差异。

结果

试验组症状困扰程度总分下降幅度显著大于对照组（P=0.021）；2组症状数量均较干预前减少（P<0.001），但组间差异无统计学意义。试验组SF-36心理健康维度评分显著提升（P=0.004），且提升幅度大于对照组（P=0.033）。试验组血清前白蛋白水平高于对照组（P=0.019）；血清白蛋白浓度保持稳定，而对照组显著下降（P=0.035）。试验组终点事件发生率低于对照组（P=0.028）。

结论

结构化症状干预方案通过闭环式的症状干预模块实施干预，可有效缓解围透析期患者的症状困扰，改善心理健康状况，并降低短期内终点事件发生风险。

Objective

To evaluate the application efficacy of structured symptom intervention program in patients with chronic kidney disease (CKD) during the peri-dialysis period.

Methods

A non-simultaneous control study was conducted on 151 peri-dialysis outpatients having not yet initiated dialysis and being followed up who were subjected with convenience sampling from Department of Nephrology of Second Affiliated Hospital of Army Medical University from April to September 2024. According to the time period of their visits, the patients who visited from April to June 2024 were assigned into a control group (n=75), and those from July to September 2024 into an experimental group (n=76). The control group received conventional symptom intervention (telephone symptom reporting + health education), while the experimental group received the intervention as the control group and a structured symptom intervention program covering 4 evidence-based modules: symptom identification, assessment, intervention, and outcome. The efficacy of above treatments was evaluated before and at 3 months after intervention. Dialysis symptom index was used to assess the degree of symptom distress and the number of symptoms. MOS 36-Item Short Form Health Survey (SF-36) was employed to evaluate the quality of life. The differences in clinical indicators and endpoint events were compared between the 2 groups after intervention.

Results

The experimental group obtained more significant reduction in the total score of symptom distress than the control group (P=0.021). After intervention, the number of symptoms was decreased in both groups (P<0.001), but no statistical difference was observed between the groups. The score of mental health dimension in SF-36 was obviously improved in the experimental group (P=0.004), which was notably better than that in the control group (P=0.033). The experimental group exhibited significantly higher prealbumin level than the control group (P=0.019), and stable albumin level, which was significantly decreased in the control group (P=0.035). The incidence of endpoint events was remarkably lower in the experimental group than the control group (P=0.028).

Conclusion

The structured symptom intervention program implements intervention through a closed-loop symptom module, which can effectively alleviate the symptom distress of patients during the peri-dialysis period, improve mental health and reduce the short-term risk for endpoint events.

慢性肾脏病（chronic kidney disease，CKD）是一种不可逆转且进行性恶化的疾病，约2%的患者进展至终末期肾脏病（end-stage kidney disease，ESKD）^［1］。围透析期是指CKD患者的估算肾小球滤过率（estimated glomerular filtration rate，eGFR）降低至小于15 mL/（min·1.73 m²）起，至开始透析后3个月内这一时间段。围透析期标志着ESKD患者从保守治疗阶段向稳定透析治疗阶段过渡，具有并发症发生率高、病死率高、治疗费用高和eGFR下降快的特点^［2］。此阶段患者因肾脏排泄和内分泌功能严重受损，面临尿素氮、肌酐等代谢废物蓄积、酸碱失衡^［3-5］；同时，由于高并发症发生率、药物治疗副作用、饮食控制不当等因素，患者易出现疲乏、恶心、瘙痒等不适症状，不仅损害生理功能^{［4，6-8］}，还会引起焦虑、抑郁等心理问题，影响治疗信心和依从性。生理和心理的双重打击，加速推进肾脏疾病进展速度，降低生活质量^［9-11］。因此，如何减轻症状和改善生活质量是CKD围透析患者需要重点关注的内容。

围透析期症状干预涉及症状评估、干预措施、症状结局等多个维度且复杂的程序，其目标是尽早预防、识别和干预由疾病本身、治疗引起的症状以及与疾病或其治疗有关的心理、社会和精神问题^［12］。既往研究通过改良评估工具、评估风险因素、制订基于循证的症状干预策略等来改善ESKD患者的症状和生活质量，但未形成系统的症状干预模式^［13-15］。本研究团队前期以包含症状体验、管理策略和管理效果的症状管理理论为框架^［16］，并基于循证证据总结，构建了涵盖症状识别、评估、干预、结局4个循证模块的结构化症状干预方案。本研究将结构化症状干预方案应用于CKD围透析期患者实施症状干预并验证方案的有效性，以期为CKD症状干预提供临床经验和实践依据。

1　资料与方法

1.1　研究设计

本研究为单中心、类实验研究，为避免组间干预污染、保障干预同质性、符合伦理与资源分配原则和提高患者依从性，故选择非同期对照设计进行分组。本方案经陆军军医大学第二附属医院伦理委员会批准（2024-研第118-02），并在中国临床研究注册中心注册（ChiCTR2400093081）。

1.2　研究对象

采用便利抽样法，选取2024年4-9月于陆军军医大学第二附属医院肾脏内科门诊随访的CKD围透析期患者作为研究对象。

1.2.1　纳入及排除标准

纳入标准：①年龄18~65岁；②符合围透析期诊断标准，即eGFR＜15 mL/（min·1.73 m²）尚未启动透析，或透析启动≤3个月^［2］；③在院内CKD管理中心建立长期随访档案；④有基本的沟通能力、理解能力及读写能力；⑤采用《透析症状指数量表》^［17］对患者实施评估，患者至少报告一种症状；⑥愿意参加本项目并签署知情同意书。排除标准：既往接受肾移植手术者。退出标准：从患者意愿方面考虑，如患者在研究过程中提出不愿意继续接受干预，则终止本研究。

1.2.2　样本量

本研究的主要结局指标为透析症状指数量表的症状困扰程度总分。样本量计算基于以下步骤。

1.2.2.1　初始估算依据

由于围透析期人群症状干预研究的数据有限，参考李九红^［18］报道的针对维持性透析患者的研究数据，试验组症状困扰程度总分为（58.45±20.12）分，对照组症状困扰程度总分为（75.08±34.09）分，试验组和对照组样本量比例为1∶1，设定假设检验的Ⅰ类错误α=0.05，Ⅱ类错误β=0.1，根据样本量计算公式：

。其中Z_1-α/2=1.96，Z_1-β=1.28，得到观察组的样本量为60，对照组的样本量为60。假定失访率为20%，则观察组和对照组各需纳入75例。

1.2.2.2　预实验验证与调整

考虑到文献^［18］中的维持性透析患者与本研究的围透析期人群存在差异，本研究开展预实验（n=36），结果显示试验组症状困扰程度总分为（41.44±9.56）分，对照组的症状困扰程度总分为（48.39±10.56）分，据此重新计算，每组各需57例，但因预实验样本量较小，为保障统计效力，仍保留初始估算结果为每组75例。

1.2.3　分组方法

根据纳入及排除标准，采用便利抽样法在连续时间段内顺序纳入符合标准的患者，直至达到样本量要求。基于本研究设计的前后对照性质，干预措施于2024年7月开始实施，将2024年4-6月入组的患者设为对照组，2024年7-9月入组的患者设为试验组。

1.3　干预方法

2组患者均每月定期随访，内容包括：症状、临床指标和生活方式指导等，第3个月末随访时评估症状、生活质量、临床指标及终点事件。

1.3.1　对照组

采用常规症状干预方法：①CKD管理中心护士向患者发放症状相关健康教育资料，内容不涉及症状干预手册的内容；②手机端推送“慢性肾脏病自我报告结局量表”指导患者症状自我报告方法；③患者通过CKD管理中心电话进行居家自我症状报告，由研究团队成员记录患者症状报告日期、方式、名称及处理方法；④所有患者按照要求每月门诊随访。

1.3.2　试验组

在常规干预方法的基础上，应用结构化CKD围透析期患者症状干预方案实施干预，干预载体为《慢性肾脏病围透析期患者症状干预手册》。研究护士在入组当天发放手册，现场示范阅读与填写方法，确保研究对象完全掌握和使用，并向患者提供答疑电话。患者居家独立完成手册，并在干预后第3个月末携带手册返院随访。手册由本团队基于循证证据及CKD管理经验编制，包括以下4个结构化模块。

1.3.2.1　症状识别

本模块基于团队在本研究前期开展的围透析期患者症状横断面调查，构建心理、神经-电解质、胃肠道、性功能障碍、皮肤、睡眠6个症状群体系（表1）。每个症状群包含2~6个症状，共涵盖20个症状，形成“症状群-症状”二级认知框架。每个症状名称后给出相应症状的定义，便于患者理解。

1.3.2.2　症状评估

患者对比手册给出的症状定义，结合自身有无不适症状体验判断该症状“有”或“无”，如果判定为“有症状”，则标注该症状，并记录症状困扰程度，注明症状发生的日期。

1.3.2.3　症状干预

该部分提供特定症状的指导建议，每个症状对应一组指导建议。指导建议由营养指导、运动指导、水电解质平衡指导、皮肤健康指导、心理支持、睡眠优化指导、音乐疗法、穴位应用、芳香疗法及药物指导10个核心内容版块组成。所有的症状指导建议通过排列组合的方式展示，形成症状干预路径，实现对各个症状群内的不同症状进行针对性干预（表1）。

1.3.2.4　症状结局

该模块对应1.3.2.2症状评估患者报告的症状，要求患者对比自身症状，判断各个症状“有”或“无”，困扰程度按 “完全没有、少许、有一些、较多、很多”勾选，并记录填写日期。

1.4　效果评价

1.4.1　评价工具

透析症状指数（dialysis symptom index，DSI）^［17］，用于描述患者具体的精神、躯体症状，共30个条目，患者被要求通过回答“无”或“有”来确定在过去7 d内任意时间出现症状，并使用Likert 5级评分（1=完全没有，2=少许，3=有一些，4=较多，5=很多）来判定症状困扰程度，得分越高说明症状困扰越严重。

健康调查简表（the MOS 36 Short Form Health Survey，SF-36）^［19］，用于评价干预前后试验组与对照组的生活质量。SF-36包括生理健康和心理健康两大类，包含8个维度共计36个条目，各维度Cronbach’s α系数均>0.7，总表Cronbach’s α系数为0.830。

1.4.2　资料收集方法

基本资料收集：研究者在登记入组时录入患者人口学资料、人体测量、治疗状态、临床指标等信息，由研究者双人核对并录入Excel表。

量表数据收集：入组时及干预后随访时，由研究者向患者发放DSI和SF-36量表，通过面对面或电话随访的方式收集，再由研究者双人核对录入。

1.4.3　结局指标

主要评价指标：症状困扰程度总分。采用干预前后DSI量表困扰程度总分评价症状管理方案实施效果，入组时及干预3个月末由研究小组成员向患者发放DSI量表并指导患者填写，根据量表评分及变化情况进行评价。

次要评价指标：①症状数量，采用DSI量表进行调查，患者被要求通过回答“无”或“有”来确定在过去7 d内任意时间各个症状是否出现，回答“有”判断为有症状发生，记为“1”，“无”则记为“0”。②生活质量评分，采用SF-36量表评价干预前后生活质量评分。③终点事件发生率，干预3个月后再次评估患者的治疗状态，如果患者的治疗状态为血液透析、腹膜透析或死亡，则判定为发生终点事件，终点事件发生率=（发生终点事件例数/总例数）×100%。

其他指标：干预前后临床指标，包括血清肌酐、eGFR、尿酸、血钾、血钠、血钙、血磷、血红蛋白、血清前白蛋白、血清白蛋白浓度。

1.5　研究质量控制

研究开始前，对研究人员统一培训。严格按照研究设计的纳入与排除标准筛选患者，确保研究对象在入组时的基线特征一致；所有基线资料收集、主要结局指标和次要结局指标的测量，采用完全相同的标准化工具和评估方法，最大限度减少测量偏倚；对照组和试验组患者均在同一肾脏内科门诊环境下接受研究相关的护理和评估，由同一研究团队提供服务，确保了除干预措施外，2组患者经历的研究环境和常规干预具有可比性。干预过程中，严格“一患一诊室”，确保干预过程独立。

1.6　统计学分析

Excel 2017录入原始数据，数据分析遵循意向性分析原则，采用末次观测值结转方法处理缺失数据。所有资料采用SPSSAU进行在线数据导入与分析。对2组的基本资料、症状量表评分、症状数量、生活质量评分、临床指标进行描述和比较。计量资料若满足正态分布，采用

表示；若不满足正态分布，采用M（P₂₅，P₇₅）表示；计数资料采用例（%）表示。组间计量资料若满足正态分布采用独立样本t检验；若不满足正态分布则采用非参数检验；组间计数资料采用卡方检验。P＜0.05表示差异有统计学意义。

2　结果

2.1　纳入研究人群基线特征的组间比较

本研究共纳入围透析期患者151人，均为CKD5期非透析患者，对照组75人，试验组76人，年龄（49.80±10.93）岁。对照组和试验组在基线时各人口学及临床特征均衡可比：性别、年龄、BMI、学历、婚姻状态、工作状态、肾病原发病、饮酒史、吸烟史、高血压病史及糖尿病病史的差异均无统计学意义（表2）。

2.2　干预前后症状及生活质量比较

干预后，试验组在症状及生活质量方面的改善优于对照组（表3）。2组症状困扰程度总分干预后均有显著下降（对照组P=0.002，试验组P<0.001），且试验组下降幅度显著大于对照组（P=0.021）。2组症状数量干预后均显著减少（对照组P<0.001，试验组P<0.001），但组间比较差异无统计学意义。试验组生活质量总分干预后显著改善（P=0.046），而对照组无显著变化，组间比较差异无统计学意义；生理健康评分在2组干预前后均无显著变化，组间比较差异也无统计学意义；心理健康评分仅在试验组干预后显著提升（P=0.004），且干预后试验组心理健康评分显著高于对照组（P=0.033）。

2.3　干预前后两组患者临床指标比较

干预3个月后，2组肾功能、尿酸、电解质及营养指标均发生一定变化，但变化幅度和显著性不同（表4）。2组血清肌酐浓度均升高，仅试验组干预前后的差异显著（P=0.009）；2组eGFR水平均下降，仅试验组干预前后差异显著（P=0.003）；血清肌酐和eGFR组间比较差异均无统计学意义。尿酸在对照组干预后明显下降（P=0.014），试验组无显著变化，组间差异不显著；血钾在对照组下降（P=0.016），在试验组无显著变化，干预后试验组血钾高于对照组（P=0.004），但平均血钾浓度维持在正常值范围内；血钠、钙、磷浓度2组干预前后均无明显变化。干预后试验组血清前白蛋白显著高于对照组（P=0.019）；干预后对照组血清白蛋白显著下降（P=0.035），试验组无显著变化，组间比较差异无统计学意义；两组血红蛋白在干预前后组内及组间比较均无显著变化。

2.4　干预后2组患者发生终点事件比较

干预后2组患者发生终点事件比较结果显示，干预后试验组相较于对照组终点事件发生率更低（P=0.028，表5）。

3　讨论

3.1　结构化症状干预方案的双重获益：症状控制与生活质量提升

3.1.1　结构化症状干预方案提升症状识别能力，有效缓解患者症状困扰

本研究证实，结构化症状干预方案可显著降低围透析期患者的症状困扰，组间差异具有统计学意义（P=0.021）。其核心机制可能在于通过系统性干预提升患者症状识别与报告的及时性，同时促进医护人员的早期干预。症状识别作为围透析期患者症状管理的关键环节，需通过连续、动态、常规化的监测实现。本研究基于结构化症状干预方案构建的手册，将20个离散的症状科学归类为6大症状群，通过可操作的定义帮助围透析患者精准识别症状，有效突破了ESKD患者普遍存在的症状认知盲区^［20-21］。同时，方案建立了“识别-评估-干预-结局”的结构化路径，使患者症状报告及时率提升，干预后症状数量由12.5个下降至6.5个，显著缩短症状识别到干预的时间窗。结构化症状干预方案实现从依赖主观描述到客观量化评估的转变，为医护协同干预提供精准靶点。

3.1.2　症状干预显著改善患者心理健康，提升整体生活质量

既往研究表明，患者在进入围透析期阶段后，均有较突出的心理症状^［22］。这是因为准备透析或开始透析治疗均会打乱他们原有的饮食、生活节奏，使他们的自信心受到打击，对透析治疗存在担忧^［23-24］。本研究中，结构化症状干预对患者心理健康的改善作用尤为显著：试验组干预后心理健康评分显著提升（P=0.004），且组间差异具有统计学意义（P=0.033），而对照组无变化。具体机制包括：症状干预路径中明确将“心理症状群”作为独立模块，含焦虑、忧虑、紧张、易怒和伤心，让患者直面心理问题；同时，在心理症状群干预路径中匹配了心理支持、音乐疗法、芳香疗法等多样化非药物干预，为患者提供可操作的心理症状调节工具；症状困扰的减轻间接缓解心理症状，与缓解患者症状困扰形成协同效应，提升了整体生活质量。本研究中生理健康评分干预前后无明显变化，提示症状干预虽能减轻症状困扰和提升心理健康，但无法逆转围透析期患者的生理功能衰退，此结果与CKD进展的病理生理特征一致。

3.2　临床指标变化的病理生理学内涵

3.2.1　肾功能监测敏感性与疾病进展的关系

干预3个月后，两组患者血清肌酐浓度均升高、eGFR均下降，提示肾功能呈缓慢进展趋势。其中，试验组血清肌酐浓度及eGFR变化达到统计学显著水平（P<0.05），而对照组未达显著水平。这可能反映了试验组患者因使用了症状干预手册，对自身症状和身体状况的监测与报告更为主动和细致，使医护人员能更早、更全面地捕捉到疾病进展的信号^［25］。对照组相对“不显著”的变化，可能源于患者报告频率或深度的差异，而并非实际上肾功能进展速度的不同，组间比较无统计学差异也支持了这种解释，即结构化症状干预方案短期内未改变肾功能自然进展轨迹，但提高了疾病监测的敏感性。

3.2.2　维持电解质稳态的临床意义

围透析期是肾功能快速恶化的关键窗口期，肾小球滤过功能显著下降导致电解质调节障碍，患者常常并发高钾、高钠、低钙、高磷血症，易诱发心律失常、循环负荷过重等严重并发症，显著增加死亡风险^［2］。本研究结果显示，试验组干预后血清钾、钠、钙、磷浓度维持稳定，提示症状干预路径中的“水电解质平衡指导”模块可以帮助患者避免过度限制钾、钠、磷等营养素摄入，通过动态监测及时调整干预策略，最终实现电解质稳态，助力围透析期患者安全平稳过渡至维持性透析阶段。

3.2.3　维持营养状态对患者的保护作用

围透析期患者常因代谢性酸中毒加速体内蛋白质的分解代谢，导致负氮平衡^［26］；同时，尿毒症毒素（如尿酸、肌酐）直接刺激胃肠道黏膜，引起食欲不振、恶心、呕吐等胃肠道症状，进一步加剧营养不良^［27］，增加患者感染的风险，可能阻碍部分患者顺利过渡至维持性透析阶段。本研究中，尽管两组肾功能指标均逐步恶化，但试验组营养状态维持稳定，试验组血清前白蛋白显著高于对照组（P=0.019），且血清白蛋白保持稳定，而对照组显著下降（P=0.035）。这可能与症状干预路径中营养指导的精准落实有关，同时通过症状缓解提升患者饮食依从性，延缓了营养不良进程^［28］。

3.3　结构化症状干预方案显著降低终点事件发生率

干预3个月后，试验组终点事件发生率显著低于对照组（P=0.028），是本研究的重要临床获益结局指标。本研究中结构化的症状干预路径可以提高患者自我应对症状的能力，通过症状与心理干预减轻了患者症状困扰程度，提升治疗和饮食依从性，使患者血清前白蛋白上升、血清白蛋白稳定、电解质稳定，可进一步有效预防感染、心律失常等并发症，缓解不适症状，延迟患者进入透析的时间^{［25，29］}。这与指南建议的ESKD肾脏替代治疗启动时机应基于患者症状和体征（如容量负荷过重、难治性高血压）而非单纯依赖血清肌酐水平的理念一致^［30］，证实结构化症状干预方案对围透析期患者管理的临床价值。

3.4　研究优势与局限

本研究以症状管理理论为框架、基于循证证据总结构建结构化症状干预方案，并以症状干预手册为干预载体，采用症状、生活质量、临床指标、终点事件作为结局指标，全面评价了症状干预方案的有效性，具有临床推广意义。但由于本研究为单中心研究，且样本量有限，随访时间仅3个月，不足以评估长期肾脏结局；未对围透析期治疗各阶段亚组分析，不能明确不同阶段的症状干预效果；同时，研究过程未使用盲法，可能存在报告偏倚；手册使用依赖患者读写能力，对低文化水平人群需进一步优化图文及语音版本。未来将进一步优化研究方案、开展更长时间的随访研究，为围透析症状干预提供更具价值的依据。

综上所述，本研究初步验证了在常规治疗基础上联合结构化症状干预方案，可更有效减轻CKD围透析期患者的症状困扰，提升患者心理健康等维度的生活质量，并在稳定营养状态、维持电解质平衡、降低终点事件发生率等方面展现出潜在益处，为优化围透析期症状干预提供了循证依据与实践思路。
