目的

基于全球疾病负担研究（Global Burden of Disease Study，GBD）2023数据，分析中国与全球良性前列腺增生（benign prostatic hyperplasia，BPH）疾病负担的差异，并预测中国未来发病率趋势，为制定防控策略提供依据。

方法

提取GBD 2023中1990—2023年中国及全球204个地区的BPH发病、患病、伤残损失寿命年（years lived with disability，YLDs）数据，采用预计年度百分比变化（estimated annual percentage change，EAPC）评估疾病负担趋势，通过年龄-时期-队列模型分解时空变异特征，并利用贝叶斯APC模型预测2024—2048年中国发病率。

结果

2023年发病人数、患病人数、YLDs值较1990年分别增加138.02%、155.87%、154.05%，但标化发病率、患病率、YLDs率呈下降趋势。中国BPH发病率在75~79岁达峰，患病率及YLDs率在70~95岁以上人群持续较高；与全球相比，中国标化率降幅高于高SDI地区，但低SDI地区病例发现较晚。年龄效应显示95岁以上人群发病率风险最高，时期效应自2001年后发病风险下降，队列效应显示出生年份越晚风险越低。2024—2048年中国BPH标化发病率呈持续下降趋势，2048年全人群标化发病率预计降至649.75/10万，75~79岁仍为高负担年龄组。

结论

中国BPH疾病负担呈绝对数增长、标化率下降的双重特征，防控措施初见成效，但老年人群仍是重点干预对象。未来发病率虽在提示医疗干预和预防措施的积极干预下，呈持续下降趋势，仍需加强预防教育、早期筛查，防治危险因素，共同管理慢性病以应对BPH未来带来的挑战。对策建议重点加强老年人群早期筛查与共病管理，优化基层尤其是农村医疗资源配置，面向中青年开展危险因素防控与健康教育，并构建基于预测的中长期防控体系，推动实现良性前列腺增生“主动预防、精准干预”的防治模式。

Objective

To analyze the differences in the disease burden of benign prostatic hyperplasia (BPH) between China and the global population based on data from the Global Burden of Disease Study (GBD) 2023, predict future incidence trends in China, and provide a basis for formulating prevention and control strategies.

Methods

Data on BPH incidence, prevalence, and years lived with disability (YLDs) from 1990 to 2023 in China and 204 global regions were extracted from GBD 2023. The estimated annual percentage change (EAPC) was used to evaluate the trends of disease burden. Spatiotemporal variation characteristics were decomposed using an age-period-cohort model, and a Bayesian APC model was employed to predict BPH incidence in China from 2024 to 2048.

Results

In 2023, the number of incident cases, prevalent cases, and YLDs were increased by 138.02%, 155.87%, and 154.05%, respectively, when compared with the values in 1990. However, the age-standardized incidence rate, prevalence rate, and YLDs rate showed decreasing trends. In China, BPH incidence reached a peak in the aged 75~79 group, while the prevalence and YLDs remained quite high in the 70~95⁺ age group. Compared with the global average, the decline in age-standardized rates in China was greater than that in high socio-demographic index (SDI) regions, but the cases in low SDI regions were detected later. The age effect indicated the highest incidence risk in the population aged ≥95 years; the period effect showed a decrease in incidence risk after 2001; The cohort effect revealed that the later the birth year, the lower the risk. From 2024 to 2048, the age-standardized incidence rate of BPH in China will continue to decline, with the projected age-standardized incidence rate in the total population reaching 649.75 per 100 000 by 2048, and the aged 75~79 group remaining the high-burden age group.

Conclusion

The disease burden of BPH in China exhibits dual characteristics: an increase in absolute numbers and a decrease in age-standardized rates. Prevention and control measures have shown initial effectiveness, but the elderly population remains a key target for intervention. Although the incidence will continue to decline in the future, efforts to strengthen health education and resource allocation are needed to address the challenges posed by population aging. Countermeasure: It is advised to strengthen early screening and comorbidity management in the elderly population, optimize medical resource allocation especially in rural primary care settings, implement risk factor prevention and health education for young and middle-aged groups, and establish a long-term prevention and control framework based on predictive data, so as to promote a shift toward “active prevention and precise intervention” in the management of benign prostatic hyperplasia.

良性前列腺增生（benign prostatic hyperplasia，BPH）是老年男性的常见病，以前列腺间质与上皮细胞异常增殖导致腺体肥大、膀胱出口梗阻及下尿路功能障碍为核心特征^［1-2］。BPH患者的主要表现包括储尿期与排尿期症状，部分患者还可出现并发症表现：储尿期症状以尿频、尿急为突出特征，排尿期症状则表现为排尿迟缓，断续，排尿时间延长，部分患者还会出现尿后滴沥，甚至血尿的症状^［3-5］。

随着全球人口老龄化进程加速，BPH的疾病负担已成为公共卫生领域的重要挑战，其临床与社会影响亟待深入关注^［6］。BPH对患病人群造成了很大的经济负担。在英国的一项研究中发现，2007—2008年使用α受体阻滞剂和 5α-还原酶抑制剂的平均年度医疗费用估计约为6 900万英镑，而住院治疗良性前列腺增生的费用估计约为1.01亿英镑^［7］。

既往研究已基于全球疾病负担研究（Global Burden of Disease Study，GBD）数据库开展了相关分析。基于GBD 2019及GBD 2021数据库，有研究团队对BPH的发病率、患病率等指标，进行了初步的分析^［8-9］。然而，这些研究主要依赖于过往的数据，并且缺乏对未来疾病负担变化的科学预测，这使得公共卫生政策的制定缺乏前瞻性依据。为此，本研究基于GBD 2023最新数据，旨在实现两大目标：其一，通过更全面、更新的数据，系统分析中国与全球各地区BPH疾病负担的差异，揭示其时空演变规律；其二，首次利用贝叶斯年龄-时期-队列模型，对中国未来25年的发病率进行科学预测。进而填补BPH疾病负担预测领域的空白；并为我国制定更具前瞻性和针对性的BPH防控策略、优化医疗资源配置提供关键的数据支持，以积极应对人口老龄化带来的严峻挑战。

1　资料与方法

1.1　数据来源

GBD 2023估计了1990年至2023年204个国家和地区按年龄组和性别划分的375种疾病和88种危险因素的疾病负担。GBD 2023 根据《国际疾病分类》第十版进行疾病分类编码BPH的分类编码为N40-N40.9，600-600.91。在进行筛选时疾病选择“良性前列腺增生”，年龄选择“40~44、45~49、50~54、55~59、60~64、65~69、70~74、75~79、80~84、85~89、90~94、≥95岁年龄组、年龄标准化”，年份选择“1990年至2023年”。

1.2　疾病负担评价指标

本研究提取了GBD 2023中BPH的发病数量、患病数量、伤残损失生命年（years lived with disability，YLDs）及其标化率进行分析。由于BPH的死亡负担为0，因此不对包含死亡相关指标进行分析。社会人口学指数（socio-demographic Index，SDI）是GBD 2023评价社会人口学发展水平的指标^［10］。

1.3　统计学方法

预计年度百分比变化（estimated annual percentage change，EAPC）用于评估BPH的发病率、患病率、YLDs率的趋势。EAPC的计算采用线性回归法。假设各指标的标化率的自然对数时间呈线性关系，满足y=α+βx+ω，其中x表示年份，y为速率的自然对数，α为截距，β为斜率，ω为随机误差。具体EAPC值及95%CI并采用“EAPC=100×［exp（β）-1］”进行计算。在全球视域下，采用Spearman相关分析比较中国与不同SDI地区归因于BPH的疾病负担差异。以5年为划分间隔（1994—1998年至2019—2023年共6个时期）和出生队列，以评估年龄、时期和队列效应。采用贝叶斯年龄-时期-队列（Bayesian age-period-cohort，BAPC）模型在整合嵌套拉普拉斯近似（integrated nested laplace approximation，INLA）框架下进行贝叶斯推断。该模型整合了时间序列平稳性和未来变异不确定性，提供了一个95%的不确定区间（Uncertainty Interval，UI）。全面捕捉BPH发病率随年龄、队列、时期变化的动态，并对中国未来25年的标化发病率、及各年龄组的发病数量进行预测。

2.1　1990—2023年中国及全球BPH负担状况及趋势

2.1.1　发病人数及标化发病率

2023年全球BPH的发病人数为14 704 527人。与1990年相比增加了129.18 %。标化发病率从1990年至2023年呈相对稳定趋势，其EAPC为0.05（95%CI：0.01~0.10）。2023年中国BPH的发病人数为3 493 467人。与1990年相比增加了138.02%。标化发病率从1990年至2023年呈下降趋势，其EAPC为-0.19 （95% CI：-0.32~-0.05）。

全球各地区的发病人数在1990年至2023年间呈上升趋势。但1990至2023年标化发病率变化趋势存在一定差异。仅中SDI地区标化发病率呈下降趋势，其他地区均为上升趋势（表1、图1A）。

2.1.2　患病人数及标化患病率

2023年全球BPH的患病人数为120 684 005人。与1990年相比增加了137.71%。标化患病率从1990年至2023年呈相对稳定趋势，其EAPC为0.02 （95%CI：-0.03~0.06）。2023年中国BPH的患病人数为25 649 289人。与1990年相比增加了155.87%。标化患病率从1990年至2023年呈下降趋势，其EAPC为-0.22 （95%CI：-0.37~-0.07）。

全球各地区的患病人数在1990年至2023年间呈上升趋势。仅中SDI地区标化患病率呈下降趋势（表1、图1B）。

2.1.3　YLDs数值及标化YLDs率

2023年全球BPH的YLDs数值为2 371 576。与1990年相比增加了137.27%。标化YLDs率从1990年至2023年呈相对稳定趋势，其EAPC为0.03 （95%CI：-0.02~0.07）。2023年中国BPH的YLDs数值为502 485。与1990年相比增加了154.05%。标化YLDs率从1990年至2023年呈下降趋势，其EAPC为-0.22（95%CI：-0.37~-0.07）。

全球各地区的YLDs数在1990年至2023年间呈上升趋势。仅中SDI地区标化YLDs率呈下降趋势（表1、图1C）。

2.2　1990—2023年中国及全球年龄组别BPH疾病负担状况

从不同年龄组别的角度分析，BPH的疾病负担存在一定差异。从发病率方面，全球40~44岁年龄组为最低值（2008年53.50/10万）。65~69岁年龄组为最高水平（2022年2 624.52/10万）。在中国40~44岁年龄组为最低值（2001年16.61/10万）。75~79岁年龄组为最高水平（1990年3 324.77/10万），见图2A。

从患病率方面，全球40~44岁年龄组为最低值（2009年86.90/10万）。85~89岁年龄组为最高水平（2021年21 548.52/10万）。在中国40~44岁年龄组为最低值（2001年28.35/10万）。80~84岁年龄组为最高水平（1990年36 531.53/10万），见图2B。

从YLDs率方面，全球40~44岁年龄组为最低值（1990年1.81/10万）。75~79岁年龄组为最高水平（1990年517.78/10万）。在中国40~44岁年龄组为最低值（2000年0.61/10万）。80~84岁年龄组为最高水平（1990年692.58/10万），见图2C。

2.3　1990—2023年BPH疾病负担与SDI相关性分析

全球疾病负担的变化趋势在不同SDI地区呈现出复杂的关联模式。在21个地区中呈“M”型分布，标化发病率、标化患病率、标化YLDs率在SDI数值0.5~0.7之间平稳，BPH疾病负担峰值大约在0.70。且在中高SDI地区呈波动上升趋势。

在204个国家地区分析中标化发病率（r=0.01，P=0.83）、标化患病率（r=0.01 ，P=0.925）、标化YLDs率（r=0.00，P=0.99）与SDI相关性差异不明显（图3）。

2.4　中国BPH的发病率的年龄-时期-队列分析

在全局层面，BPH发病率的净偏移值为-4.09%（95%CI：-4.34%~-3.83%）（图4A）。从年龄效应来看，发病率随年龄增长总体呈上升趋势，相对危险度（relative risk，RR）在40~44岁年龄组最低（0.03，95%CI：0.03~0.05），在95~99岁年龄组达到最高（1 070.96，95%CI：983.61~1 166.07）（图4B）。时期效应分析显示，以2001年左右为参考时期（RR=1），2001年之后的发病风险相对更低，RR值整体呈持续下降趋势（图4C）。队列效应方面，发病率随出生年份推移而逐步降低：RR值在1899年出生队列中最高（4.930 7，95%CI：4.25~5.72），在1969年出生队列中最低（0.10，95%CI：0.08~0.13）；以1914年出生队列为参考（RR=1），1914年之前出生者的发病风险较高，而之后出生者的发病风险较低（图4D）。

2.5　中国标化发病率及各年龄组人群发病数量未来25年预测

基于BAPC模型，预测了2024到2048年未来25年的中国全人群BPH疾病负担的标化发病率及其各年龄段的发病人数。预测结果表明，2024到2048年总体呈下降趋势。到2048年全人群的标化发病率预计达到649.75（95%UI：377.51~921.98）。到2048年，70~74岁年龄组疾病负担最重，发病人数为769 147.51人（95% UI：381 694.46~1 156 600.57），见图5。

BPH正在成为一项重大的公共卫生挑战，其进展受到多种因素的影响，并影响着越来越多的人^［11］。本研究提供了1990年至2023年中国BPH发病率、患病率和YLDs率的最新数据，并与全球趋势进行了比较。这些发现可能有助于指导公共卫生资源的优化配置以及有效干预策略的制定和实施。

3　讨论

3.1　中国总体BPH负担状况

本研究结果显示，与1990年数据相比，中国BPH的疾病负担指标呈双重特征：一方面发病人数、患病人数、YLDs值上升幅度较大，特别是患病人数及YLDs值涨幅均在150%左右。另一方面，经过年龄标准化后的发病率、患病率、YLDs率呈小幅下降趋势。这与既往的研究结论保持一致^［8-9］。绝对数量的飙升主要归因于人口基数扩大和老龄化的结构性压力。年龄标准化率的下降，是在此不利人口背景下，公共卫生与临床医学共同努力所取得的实质性成效。随着早期筛查覆盖率提升、微创手术的推广和药物治疗的规范化、健康宣教普及等措施，在一定程度上缓解了BPH的实际流行强度与危害程度^［12］。

先前的研究表明BPH的发病率、患病率等疾病负担指标与患者的发病年龄有着密切的关系^［13］。本研究通过年龄亚组分析发现，与全球其他地区相比，中国BPH患者的发病年龄相对较晚，但其相对高发年龄区间则更为宽泛；其中，65~84岁人群的发病率持续处于相对较高水平。而患病率及YLDs率则表现几乎一致，在70到95岁以上人群，均处于较高水平。有研究表明亚洲人群BPH具有更高的发病率^［14］。而亚洲人群中具有独特的基因，则会更容易的导致BPH的发生，如ACE、ALDH2、BID等^［15］。70~95岁人群的高患病率和YLDs率则是年龄累积效应、预期寿命延长及高龄患者合并症的综合体现。

3.2　SDI相关性分析

通过比较SDI与各疾病负担指标的关系发现，总体上SDI与发病率、患病率、YLDs率之间呈负相关关系。而我国处于东亚地区也随经济水平的上升，各指标缓慢下降。这应当归因于随着经济建设水平的提高，人们生活方式的改变，医疗卫生保障的改善等经济因素^［16］。但低SDI地区受限于诊疗条件，病例可能更晚被发现，也提示SDI较低地区及我国经济水平落后地区应完善BPH诊疗条件。并且一项评估我国的BPH患病率的荟萃分析也表明，各省份间的患病率差异显著，而农村地区与城市地区也存在一定差异^［17］。中国整体SDI从1990年的0.46持续攀升至2023年的0.73，但由于中国幅员辽阔区域发展严重不均衡，GBD公开数据中又无法细致到具体某市、县级地区，因此可以参考全球其他地区，来分析我国部分地区情况，尤其是经济欠发达地区的情况。

3.3　发病率的年龄-时期-队列分析

年龄效应分析显示BPH的发病率在95岁以上人群达到了峰值，这也印证了先前的研究结论。从生理机制看，随年龄增长，男性体内雄激素和雌激素水平长期刺激前列腺组织，导致上皮细胞与间质细胞增殖-凋亡失衡，腺体逐渐增生^［18］。

综合时期效应和队列效应分析表明，自2001年后BPH的发病率逐年稳步下降，出生越晚的队列BPH发病风险越低。这一变化深刻反映了近30年来医学与公共卫生领域的进步。高血压、糖尿病、肥胖等代谢性疾病是BPH的危险因素^［19-20］。近年来，公共卫生教育的普及，以及对合并症的规范管理，降低了高风险人群的发病概率。同时，患者对疾病的认知提升也降低了症状恶化风险。此外，一项基于NHANES研究表明，高钙饮食可以作为老年人群预防策略之一^［21］。

3.4　未来25年发病率趋势

BAPC模型的预测结果显示，中国2024年至2048年的BPH标化发病率呈下降趋势。其主要原因除了医学的进步还包括了队列效应的延续。《中国老龄化报告2024》指出“未来30多年处于老龄化快速深化阶段”。尽管人口老龄化可能增加BPH患病人数，但标化发病率的下降表明，在排除年龄因素后，个体的发病风险实际有所降低。这提示医疗干预和预防措施正在发挥积极作用，部分抵消了老龄化带来的负面影响。

此外，本团队先前研究表明未来我国因代谢性因素导致的疾病负担也会有所缓解^［22］。并且随着SDI提升，饮食结构优化、高血压、糖尿病等代谢性疾病管理加强，人群保健意识的提高可能减少BPH危险因素暴露。

本研究发现以60岁为分界，60岁以下人群，到2048年人数将比现在减少，而60岁以上人群将多于目前情况，特别是80岁以上的人群。高龄人群在未来仍是BPH疾病负担的主力军。研究表明老年BPH人群普遍存在合并心血管、内分泌系统的共病现象^［23］。甚至BPH可以增加因精神障碍而自杀的风险^［24］。因此尤其对于高龄人群，BPH已不仅是单一的泌尿外科疾病，更是严重影响其生活自理能力、加剧衰弱状态、并与其他慢性病管理产生复杂交互的健康问题。

3.5　局限性

首先，我们的分析依赖于建模的GBD 2023估计值，该估计值将国家报告的统计数据与多个数据系统相结合。在许多中低收入国家，发病登记不完整或缺失，这可能会使水平和趋势产生偏差。其次疾病负担基于现有情况进行预测，若未来发生疫病或其他未知情况，我们无法估计。再次，不同国家的诊断标准有所不同，这将会导致数据产生不确定性。最后，由于BAPC预测模型固有的不确定性，应谨慎看待这些预测。

3.6　卫生政策建议

应重点加强老年人群特别是高龄组的早期筛查与多病共管；持续优化基层尤其是农村地区的诊疗资源配置，着力缩小服务可及性差距；面向中青年群体开展针对性健康教育，推进危险因素综合防控；并依据预测数据科学布局中长期防控策略，推动良性前列腺增生防治向“主动预防、精准干预”模式转变，系统化应对人口老龄化带来的健康挑战。综上所述，与1990年相比中国BPH的疾病负担呈下降趋势，但由于人口老龄化加重，BPH的发病人数、患病人数、YLDs数值还将持续攀升。因此未来需进一步强化年龄分层管理：对中青年男性侧重预防教育、早期筛查，对老年群体进一步优化治疗手段，防治危险因素，共同管理慢性病。并且同时探索基因检测对高风险个体的精准识别，最终实现BPH从被动治疗向主动防控的转变。从而推动健康中国2030的建设。
