目的

后勤装备保障人员作为作战体系中的关键支撑，其健康素养水平直接关系战斗力生成与维持，但目前缺乏该群体健康素养的专项研究。通过探讨后勤装备保障人员健康素养现状及其影响因素，为构建健康管理体系提供实证依据。

方法

采用横断面研究设计，于2025年4月15—30日，以整群抽样法选取驻西部某市722名后勤装备保障人员为研究对象。采用《军队人员健康素养调查问卷》进行现场测评，该问卷涵盖基本健康理念、基本健康知识、基本健康行为、基本健康技能4个方面，以及军人健康理念、军事训练伤防护、军人心理健康、战伤自救互救、部队常见病防治、健康管理6类问题；问卷共包含40个条目，总分为50分，以得分达总分80%（得分≥40分）判定为具备健康素养，其水平计算公式为：健康素养水平=（具备健康素养人数/有效调查总人数）×100%。采用Spearman分析健康素养的相关因素，通过多因素Logistic回归模型分析健康素养的影响因素。

结果

后勤装备保障人员健康素养具备率为38.14%。4个方面素养中，基本健康理念素养水平最高（57.57%），而基本健康知识素养水平最低（22.57%）；6类问题素养中，军人健康理念素养水平高达99.57%，而部队常见病防治素养水平仅16.00%。健康素养的总体知晓率为77.99%，7个条目知晓率低于60%，其中四肢骨折固定技术知晓率最低（31.86%）。多因素Logistic回归分析显示，军官、义务兵等身份类别和吸烟状况是健康素养的独立影响因素。相较于义务兵，军官具备健康素养的优势更为显著（OR=4.960，95%CI：1.785~13.783）；吸烟者的健康素养水平低于不吸烟者（OR=0.662，95%CI：0.471~0.930）。

结论

后勤装备保障人员健康素养呈现健康理念强、战救技能弱、常见病防治差的特征，建议后期以吸烟人群及义务兵为重点，常规开展部队常见病防治、军人心理健康及战伤自救互救技能培训干预。

Objective

As a critical support component of the operational system, military logistics and equipment support personnel play a pivotal role in the generation and maintenance of combat force, with their health literacy levels directly influencing these outcomes. However, research focusing on the health literacy of this specific population remains lacking. This study aimed to investigate the current status of health literacy and its influencing factors among logistics and equipment support personnel, thereby providing empirical evidence for constructing a health management system.

Methods

A cross-sectional study design was adopted. From April 15 to 30, 2025, 722 logistics and equipment support personnel stationed in a city in western China were selected by cluster sampling. Military Personnel Health Literacy Questionnaire was employed for on-site assessments, encompassing 4 dimensions (basic health concepts, basic health knowledge, basic health skills, and basic health behaviors) and 6 specific categories (military personnel health concepts, prevention of military training injuries, military personnel mental health, combat self-aid and buddy-aid, prevention and control of common diseases in military units, and health management). The questionnaire consists of 40 items with a total score of 50 points. Health literacy was defined as achieving ≥80% of the total score (score ≥40 points). The health literacy rate was calculated using the formula: Health literacy rate = (Number of individuals with adequate health literacy/Total number of valid respondents) × 100%. Spearman analysis was used to examine health literacy-related factors, and multivariable logistic regression was applied to analyze influencing factors for health literacy.

Results

The overall rate of adequate health literacy among the surveyed military logistics and equipment support personnel was 38.14%. Among the 4 dimensions of health literacy, the rate of basic health concept literacy demonstrated the highest level (57.57%), while the rate of basic health knowledge literacy showed the lowest level (22.57%). Among the 6 specific categories of health literacy, the rate of military health concept literacy was as high as 99.57%, whereas the rate of literacy in the prevention and control of common diseases in military units was only 16.00%. The overall correct response rate for health literacy was 77.99%, with 7 items having a correct response rate of less than 60%; among these, the correct response rate for limb fracture fixation techniques was the lowest (31.86%). Multivariate logistic regression analysis revealed that identity category (officer, conscript, etc.) and smoking status were independent influencing factors for health literacy. Compared with conscripts, officers had a significantly higher likelihood of having adequate health literacy (OR=4.960, 95%CI: 1.785 to 13.783). In addition, smokers had a lower level of health literacy than non-smokers (OR=0.662, 95%CI: 0.471 to 0.930).

Conclusion

Health literacy among logistics and equipment support personnel is characterized by strong health concepts, weak combat casualty care skills, and poor competence in the prevention and control of common diseases. It is recommended that future health intervention measures focus on smokers and conscripts, with routine training programs focusing on the prevention and control of military common diseases, military personnel mental health, and combat self-aid and buddy-aid skills.

健康素养是个人获取、理解并运用基本健康信息和服务^［1］，以做出有利于自身及他人健康的决策^［1］，能够增强个人参与健康促进行动的能力^［2］，与健康相关行为^［3］、健康结局^［4］及生活质量等密切相关^［5］。健康素养不单单是提升个体健康水平的必要因素^［6］，还在改善人群健康状况等方面具有不可替代的作用；是影响军队整体健康素质的关键因素^［7-8］，更是推动军队卫勤保障从被动医疗向主动健康转型的关键抓手。然而，该群体正面临着前所未有的健康挑战，包括高强度的脑力负荷、非稳态的作业节律、复杂的职业暴露环境以及突出的工训矛盾。这些挑战使该人群成为超重肥胖、代谢综合征、肌肉骨骼疾患及心理倦怠等生活方式疾病的高危人群，呈现出显著的健康脆弱群体特征^［9-11］。然而，当前针对该群体健康素养的系统性研究较为缺乏，其健康素养现状、关键薄弱环节及作用机制尚不清晰，故难以为精准干预提供科学依据。

为此，本研究聚焦于后勤装备保障人员这一特殊军事群体，旨在揭示后勤装备保障群体健康素养水平现状、特征与关键影响因素，厘清健康素养与健康行为、健康状况之间的内在关联。这一科学问题的解决，不仅有助于填补军队特殊作业人群健康素养研究的空白，深化对该群体健康风险形成机制的理解，还能基于实证数据识别关键干预靶点，从而为制定差异化、精准化的军人健康促进策略提供循证依据。

1　对象与方法

1.1　研究对象

本研究采用横断面研究设计。研究样本量依据公式

n = Z 1 - α / 2 δ 2 × P × 1 - P

计算。其中n为最小样本量，Z为对应置信水平的标准正态分布分位数，δ为允许误差，P为预期健康素养水平。参考2023年胡翰林研究中军校学员的总体健康素养水平为41.37%^［12］，设定δ取5%，检验水准α=0.05，Z_1-_α_/2=1.96，计算所需样本量≥373，即基础样本量不少于373例。预留10%~20%的无效问卷剔除率并考虑到目标人群分布集中且便于管理等情况，采用整群抽样法，于2025年4月15—30日选取驻西部某市722名后勤装备保障人员完成现场调查。纳入标准：①调查期间在岗；②年龄>18岁；③无影响体育锻炼的疾病。排除标准为：问卷填写不完整或存在明显逻辑错误的样本。共发放问卷722份，剔除22份因关键信息缺失或明显逻辑错误的无效问卷，最终共回收700份有效问卷，有效率为96.95%。所有参与者知情同意，并自愿参与本次调查。本研究通过西部战区陆军医学伦理委员会审批【西陆伦审〔2025〕002号】。

1.2　调查方法及工具

本研究通过纸质问卷进行调查，问卷包括基本信息及健康素养2部分内容。基本信息部分根据后勤装备保障人员特点自行设计人口学特征指标。健康素养部分采用《军队人员健康素养调查问卷》^［12］，该问卷由全军健康教育中心编制，包括4个方面素养（基本健康理念、基本健康知识、基本健康行为、基本健康技能）及6类问题素养（军人健康理念、军事训练伤防护、军人心理健康、战伤自救互救、部队常见病防治、健康管理）；该问卷共包含40个条目，题型涵盖判断题、单选题及多选题。评分标准为：判断题与单选题答对计1分，多选题全部答对方可获得2分（错选或漏选均不得分），问卷总分为50分。将得分达到总分80%及以上者判定为具备健康素养^［12］，其水平计算公式为：健康素养水平=（具备健康素养人数/总调查人数）×100%。4个方面及6类问题的健康素养具备情况判定标准与总体健康素养水平类似。各条目知晓率=（正确回答该条目的人数/调查总人数）×100%。

1.3　统计学分析

用EpiData 3.02软件对原始数据进行双人独立录入建库，使用SPSS 26.0进行统计分析。正态分布的计量资料以

x ¯

±s表示，计数资料以频数（%）进行描述；不同人口学特征的健康素养比较采用χ²检验；健康素养的影响因素分析采用多元Logistic回归模型。所有统计检验为双侧，检验水准设定为α=0.05。

2　结果

2.1　健康素养水平在文化程度、身份类别、吸烟状况、自评健康状况上存在差异

本次共调查后勤装备保障人员700人，其中男性660人（94.29%），女性40人（5.71%），平均年龄为（31.30±7.04）岁；汉族663人（94.71%）；未婚、已婚分别为273人（39.00%）、409人（58.43%）；文化程度多为大专以上，587人（83.86%）；超重肥胖人群284人（40.57%）；自评健康状况较好及以上的有543人（77.57%），具体见表1。

差异分析结果显示，调查群体总体健康素养水平在文化程度、身份类别、吸烟状况及自评健康状况上差异有统计学意义（P<0.05）。4个方面健康素养水平，在文化程度和身份类别上均存在显著差异（P<0.05）；在婚姻状况上仅基本健康行为方面健康素养差异有统计学意义（P<0.05）；在吸烟状况上仅基本健康技能方面健康素养未见显著差异，其他维度差异有统计学意义（P<0.05）；在自评健康状况上基本健康理念和基本健康行为方面健康素养差异有统计学意义（P<0.05），具体见表1。6类问题健康素养水平，在性别上军人健康理念和健康管理方面健康素养存在显著差异（P<0.05）；在民族上6类问题素养水平各维度均无显著差异；在婚姻状况上仅部队常见病防治素养水平差异有统计学意义（P<0.01）；在文化程度上军事训练伤防护、军人心理健康、部队常见病防治3个维度的素养水平差异具有统计学意义（P<0.05）；在身份类别和自评健康状况上军人健康理念及战伤自救互救素养水平差异无统计学意义，其他维度差异显著（P<0.05）；在6类问题素养方面，其分布趋势与4个方面基本一致：绝大多数指标在文化程度、身份类别、吸烟状况及自评健康状况上存在不同程度的显著差异，具体见表2。

2.2　后勤装备保障人员健康素养水平呈健康理念强、健康知识弱的特征

本次参与调查的700名后勤装备保障人员中共267人（38.14%）具备健康素养，健康素养平均得分36.90±6.20。在4个方面素养中，基本健康理念素养（57.57%）最高，平均得分4.04±1.07；基本健康行为素养次之（52.86%），平均得分11.56±2.54；基本健康技能素养水平为51.86%，平均得分15.30±2.67；而基本健康知识素养水平最低，仅为22.57%，平均得分6.00±1.89；在6类问题素养中，军人健康理念素养水平（99.57%）最高，平均得分2.89±0.33；军人心理健康素养水平（31.57%）和部队常见病防治素养水平（16.00%）较低，平均得分分别为3.18±1.42、3.94±1.68；健康管理、军事训练伤防护、战伤自救互救素养水平分别为64.14%、54.71%、49.43%，平均得分分别为12.92±2.40、8.40±2.34、5.57±1.37。

2.3　后勤装备保障人员基本健康知识及特定健康技能知晓率偏低

后勤装备保障人员总体健康素养的知晓率为77.99%。其中4个方面知晓率由高到低分别为：基本健康理念86.71%、基本健康行为81.10%、基本健康技能79.69%和基本健康知识65.77%。6类问题知晓率由高到低分别为：军人健康理念96.43%、健康管理82.15%、军事训练伤防护80.24%、战伤自救互救75.02%、军人心理健康67.19%、部队常见病防治62.74%。健康素养条目中知晓率低于60%的有7条，其中四肢骨折固定技术知晓率最低（31.86%），其次是战斗应激的描述（33.57%），其他详见表3。

2.4　身份类别与吸烟状况是后勤装备保障人员健康素养的影响因素

将后勤装备保障人员是否具备健康素养设为因变量，并将表1中健康素养水平差异有统计学意义的4个指标（文化程度、身份类别、吸烟状况、自评健康状况）纳入模型作为自变量，进行多因素Logistic回归分析。分析显示，身份类别与吸烟状况是后勤装备保障人员健康素养的影响因素。具体而言，军官的健康素养水平最高，其优势显著高于义务兵（OR=4.960，95%CI：1.785~13.783）；文职人员素养水平相对义务兵更高（OR=2.466，95%CI：0.998~6.093），军士素养水平与义务兵素养水平差异无统计学意义，在编职工素养水平相对最低；吸烟者及戒烟者健康素养水平低于不吸烟者（OR=0.662，95%CI：0.471~0.930；OR=0.460，95%CI：0.200~1.056），见表4。

3　讨论

3.1　后勤装备保障人员健康素养总体水平较高但存在群体与维度差异

调查结果显示，后勤装备保障人员总体健康素养水平为38.14%，这一水平高于2024年全国城市居民健康素养平均水平（34.74%）^［13］，也高于高原某部官兵（20.03%）^［14］和海军官兵（30.02%）^［15］的报告数据，这可能源于后勤装备保障部队作为技术密集型单位，其人员遴选标准相对较高，且长期处于相对稳定的环境，享有较完善的医疗保障资源和健康支持系统。另一方面，可能与所使用的调查问卷不同有关。本次调查采用全军健康教育中心统一编制的《军队人员健康素养调查问卷》，但调查结果与接受系统化医学或健康课程教育的军校学员（41.37%）相比，本研究对象总体健康素养得分仍显不足^［12］。这提示，虽然后勤装备人员的总体健康素养基线水平优于部分特殊环境部队，但其健康素养的提升瓶颈受限于非医学专业背景及碎片化的学习模式，尚未形成系统化的健康知识体系。

3.2　后勤装备保障人员健康理念与知识素养发展失衡制约行为的有效转化

在健康素养的4个方面，呈现理念强、知识弱的特征。基本健康理念素养水平（57.57%）最高，反映出国家及军队积极推进健康中国建设的背景下，后勤装备保障人员已普遍树立科学的基本健康理念，这是开展健康促进工作的宝贵基础。然而，基本健康知识素养水平仅为22.57%，成为明显的薄弱环节，且远低于其他方面。这种知行分离的结构性矛盾，深刻揭示了当前部队健康教育的痛点：教育形式多停留在口号式、动员式的理念宣贯，而缺乏针对后勤装备保障人员岗位特点，如油料接触、噪声暴露、久坐作业等方面的专业化及理论化知识传授。基本健康行为与技能素养处于中等水平，进一步印证了由于缺乏扎实的理论支撑，许多健康行为仅依赖于经验习惯而非科学认知，导致在面对慢性病自我管理、职业病早期识别等复杂健康问题时，该群体往往有心无力，难以将科学的健康理念转化为精准的健康行动。此外，部队常见病防治素养水平（16.00%）极低，这与后勤人员长期处于保障他人的角色定位有关，往往忽视自身作为高风险职业人群的疾病防治需求，提示未来的干预必须从通用宣教向职业健康精准赋能转变。

3.3　后勤装备保障人员战伤自救互救与心理健康素养发展的不均衡特征

本研究重要的创新点在于揭示了后勤装备保障人员在军事特色健康素养上与其他群体（如军校学员）截然不同的分布模式。数据显示，该群体战伤自救互救素养（49.43%）高于军人心理健康素养（31.57%）。这与学员的调查结果形成差异，学员的战伤自救互救素养（34.07%）低于心理健康素养（44.99%）^［12］。这种现象归因于岗位特征：对于后勤装备保障单位而言，战伤自救互救训练明确纳入训练大纲，组织实施相对严密，为战备训练的重要内容；而心理健康常识普及工作尚未全面展开，加之后勤装备保障人员长期面临装备抢修压力大、野外伴随保障孤独感强等隐性心理压力，导致心理素养未能引起足够的关注。知晓率进一步分析印证了这一薄弱点，总体知晓率为77.99%，但四肢骨折固定技术（31.86%）、战斗应激的描述（33.57%）、犬咬伤正确做法（40.00%）等涉及战场急救与特殊伤病处置的条目知晓率均低于60%，暴露出在实用化、军事化健康技能训练上存在的缺口。这表明，当前的战救训练虽覆盖面广，但在实战化、精细化层面仍有欠缺，可能存在为了考核而训练的形式主义倾向，未能真正内化为应对复杂战场环境的生存本能。这一发现表明，后勤装备保障人员的健康素养提升不能仅看平均分，更要关注那些决定战场生存率的关键少数技能点，精准聚焦于填补关键知识盲区，特别是加强部队常见病、训练伤防护、战救技能和心理应激应对等实用性强、与军事任务关联度高的知识与技能培训。

3.4　身份类别导致的健康资源可及性差异是健康素养分层的主因

多因素分析结果显示，军官、义务兵、文职人员等不同身份类别是影响后勤装备保障人员健康素养水平的显著相关因素。具体而言，军官健康素养水平显著高于义务兵（OR=4.960，95%CI：1.785~13.783，P=0.002），文职人员也呈现出优于义务兵的趋势（OR=2.466，95%CI：0.998~6.093，P=0.051），这一分布特征证明了受教育程度与健康素养呈正相关的经典理论^［16-17］。军官与文职人员普遍具有更高的学历背景，具备更强的自主信息获取意愿与能力^［18］，能够更容易理解、准确识别和正确利用健康信息，掌握健康技能，进而形成较高健康素养水平。而义务兵因服役初期核心学习和训练内容侧重点在于军事技能、纪律养成、体能训练等方面，健康信息获取渠道相对有限、健康教育接触机会较少；加之该群体年龄较小、社会生活经验尚浅，这些因素共同制约了其健康素养水平的提升。此外，本研究中文化程度在单因素分析时有一定影响，但在后续多因素分析中却不再显著。这提示身份类别可能是一个涵盖教育背景、职业环境及资源获取权限的复合变量。这表明，在军队特定情境下，身份类别所赋予的资源可及性差异（如培训机会、信息权限、时间自主权等）比单纯的文化程度更能解释健康素养的分层现象。

3.5　吸烟行为与健康素养的双向关联及干预启示

吸烟状况分析显示，吸烟者健康素养水平显著低于不吸烟者（OR=0.662），戒烟者也呈现较低趋势，与宋允雪等^［19-20］的结果一致。这种关联揭示了健康素养与吸烟行为之间存在双向、动态的互构机制：一方面，高健康素养个体更能认识到吸烟的危害，具备更强的自控能力，从而主动避免吸烟或成功戒烟；另一方面，不吸烟者往往拥有更健康的生活方式，这种正向的生活习惯会进一步强化其个人健康管理的效能，促使其主动获取健康信息，形成高素养-健康行为的良性循环。这提示烟草控制不仅是健康促进行动的重要内容，也可能与整体健康素养的提升密切相关，二者需在干预中协同推进。反之，吸烟行为作为一种不良生活方式的表征，可能反映了个体在健康信息利用和行为改变上的困境，进而导致素养水平停滞不前。基于此，烟草控制不仅是健康促进行动的重要内容，更是提升整体健康素养的关键切入点。未来的干预工作不应将二者割裂，而应采取协同推进策略：在开展控烟行动的同时，嵌入健康信息获取与利用技能的培训；在提升健康素养的教育中，强化对吸烟危害的认知与行为干预，通过以控烟促素养，以素养助控烟的双向驱动，实现人群健康水平的整体跃升。

本次调查发现，后勤装备保障人员总体健康素养水平高于部分军兵种及全国城市居民，但低于军校学员。具有以下特点：健康理念与健康知识失衡，战救素养发展不均衡；不同身份类别群体在健康资源获取方面存在差异；吸烟行为与健康素养水平呈双向关联。在基本健康知识、部队常见病防治、军人心理健康及部分战伤救治技能方面存在明显短板。

基于上述发现，建议今后健康干预工作聚焦以下重点：①重点人群强化普及；针对义务兵和吸烟群体，强化健康知识理论普及，内容聚焦部队常见病防治、军人心理健康知识及战场实用急救技能。②素养与行为协同干预；将健康素养提升与行为干预相结合，以控烟宣教为重要切入点，将戒烟成效作为检验健康素养提升的硬指标。③开发专项课程与强化示范引领。根据后勤装备保障人员岗位特点，开发职业病防治专项健康教育课程，并发挥军官与骨干的示范引领作用，构建健康胜任力模型，系统提升该群体健康素养水平。
