目的

童年创伤与大学生认知灵活性降低密切相关，为探索对该群体更安全有效的认知干预路径，本研究旨在评估闪动技术（flash technique，FT）对提升童年创伤大学生主观认知灵活性及客观认知灵活性的作用效果。

方法

结果

最终完成54例（干预组与等待组各27例）：①2组在人口统计信息、CTQ、CFI、转换时间代价以及错误率转换代价等基线数据上差异不显著（P>0.05）；②重复测量方差分析结果显示，在CFI（F=24.654，P<0.001，偏η²=0.322）以及转换时间代价上（F=6.603，P=0.013，偏η²=0.113）的组别与测量时间的交互作用显著，在错误率转换代价上，组别与测量时间的交互作用不显著（F=1.397，P=0.243，偏η²=0.026）；③组别与时间简单效应分析结果显示，干预结束后干预组CFI显著高于等待组（t=3.054，P=0.004，95%CI：3.022~14.608），转换时间代价显著低于等待组（t=-3.117，P=0.003，95%CI：-93.438~-20.251），T2时，干预组的CFI显著高于T1（t=7.424，P<0.001，95%CI：9.999~17.408），转换时间代价显著低于T1（t=-2.875，P=0.006，95%CI：-60.582~-10.780），等待组在T2时的CFI、转换时间代价较T1均无显著差异（P>0.05）。

结论

闪动技术能有效提升童年创伤大学生的主观认知灵活性与客观认知转换效率，可作为该群体心理干预有效辅助手段。

Objective

Childhood trauma is closely associated with decreased cognitive flexibility in college students. To explore a safer and more effective cognitive intervention approach for this population, this study aims to determine the effects of flash technique (FT) on improving subjective and objective cognitive flexibility among college students with childhood trauma.

Methods

A randomized controlled study design was adopted. From April to October 2025, Childhood Trauma Questionnaire (CTQ) and semi-structured interviews were used to assess the levels of childhood trauma among college students in some universities in Changqing and Kunming. A total of 54 eligible participants were randomly assigned into an intervention group (n=27) and a waitlist group (n=27). The intervention group received 70-minute FT intervention, once per week for 6 weeks, while the waitlist group received no intervention during the same period. At baseline (T1) and post-intervention (T2), subjective cognitive flexibility was assessed using the Cognitive Flexibility Inventory (CFI). In the letter-number switching task, objective switching efficiency was measured by switching time cost, and switching accuracy was determined by switching error rate cost.

Results

All 54 participants completed the study (27 in each group). ① No significant differences were observed between the 2 groups in demographic characteristics, CTQ, CFI, switching time cost, or switching error rate cost at baseline (P>0.05). ② Repeated measures analysis of variance revealed significant group-by-time interactions for CFI (F=24.654, P<0.001, partial η²=0.322) and switching time cost (F=6.603, P=0.013, partial η²=0.113), whereas the group-by-time interaction for switching error rate cost was not significant (F=1.397, P=0.243, Partial η²=0.02).③ Simple effect analysis demonstrated that post-intervention CFI was significantly higher in the intervention group than in the waitlist group (t=3.054, P=0.004, 95%CI: 3.022 to 14.608), with switching time cost significantly lower (t=-3.117, P=0.003, 95%CI: -93.438 to -20.251). At T2, the intervention group showed significantly higher CFI (t=7.424, P<0.001, 95%CI: 9.999 to 17.408) and significantly lower switching time cost (t=-2.875, P=0.006, 95%CI: -60.582 to -10.780) compared with T1. No significant differences were observed CFI or switching time cost between T1 and T2 in the waitlist group (P>0.05).

Conclusion

FT can effectively improve subjective cognitive flexibility and objective cognitive switching efficiency in college students with childhood trauma, serving as an effective auxiliary psychological intervention for this population.

童年创伤是一种常见的心理创伤，通常是指在18岁以前发生且超出儿童应对能力的负面事件，包括身体虐待、情感虐待以及忽视等^［1］。作为一种长期、慢性的应激形式，童年创伤在人群中具有较高的发生率。研究显示，在美国成年人中有63.7%报告至少经历过一种童年创伤^［2］；我国成年人的童年创伤经历比例为28.76%^［3］，提示该问题在人群中的普遍性。

大学阶段是青少年向成年过渡的关键时期，有童年创伤经历的大学生由于缺乏应对资源，更容易出现各类心理困扰，持续影响心理健康与学业发展^［4］。已有研究证实，童年创伤可通过多种路径对个体心理状态产生负面影响，既会通过神经质、压力感知、述情障碍等中介因素增加焦虑、抑郁等情绪问题的风险^［5-6］，也会对认知功能造成持续性损害，且这种认知损害不受当前心理健康状况的影响^［7］，在成年后仍可能持续存在。

认知灵活性是一种基于前额叶控制的适应动态环境变化的核心认知功能^［8］，在个体环境适应、生存发展方面发挥着重要作用。认知灵活性的受损会进一步产生认知固着、行为模式习惯化、决策僵化等问题，降低应对环境变化的能力^［9］。多项研究表明，童年创伤与认知灵活性的降低存在直接关联^［10-11］，童年创伤不仅能直接预测认知灵活性的下降^［12］，还能通过损害认知灵活性间接增加焦虑、抑郁等心理障碍的风险^［13］，进而影响大学生的学业表现、情绪调节及社会适应等多个方面的能力。荟萃分析亦证实，童年创伤对执行功能、注意分配、情绪调节等认知过程具有广泛而持久的负面影响^［14］，这凸显对童年创伤群体开展认知功能干预的现实必要性。因此，探究能够有效改善童年创伤大学生认知灵活性的干预方法具有重要的理论与实践意义。

当前，对于童年创伤的心理干预方法主要是认知行为疗法（cognitive behavioral therapy，CBT）、眼动脱敏与再加工技术（eye movement desensitization and reprocessing，EMDR）、暴露疗法及正念疗法等，尽管部分研究已尝试通过相关干预提升认知灵活性^［15］，但现有相关干预研究的结局评估多局限于主观问卷层面，缺乏客观指标对干预效果的验证。此外，传统干预普遍存在治疗周期长、创伤暴露、操作复杂等不足，在大学生群体中的推广应用受到一定限制。

闪动技术（flash technique，FT）是MANFIELD等^［16］基于EMDR开发的创伤处理技术，具有隐私性高、创伤暴露低、简便快速等优势。已有研究证实该项技术能够在短时间内降低创伤记忆的情感性和生动性，改善创伤相关应激症状^［17］。认知灵活性作为童年创伤个体适应不良的关键靶点，直接关系到个体情绪调节与认知功能的提升。尽管目前闪动技术在降低创伤痛苦和改善负性情绪方面已得到初步证实^［18］，但该技术能否作用于认知功能层面，帮助改善认知灵活性，目前仍缺乏实证研究。证实闪动技术可同时改善创伤情绪与认知灵活性，可为童年创伤大学生提供适用于非临床群体的安全高效干预方案，也可为理解创伤干预的认知机制补充新证据。

结合现有研究背景与现实需求，本研究采用主观问卷与客观行为学指标，综合评估闪动技术对认知灵活性的干预效果，为闪动技术在非临床创伤群体认知干预中的应用提供实证依据。

1　对象与方法

1.1　研究对象

2025年4—10月，通过问卷星平台（https://www.wjx.cn/）在重庆市、昆明市部分高校发布研究招募问卷，共386人自愿填写问卷报名，采用方便取样法选取符合入组标准的在校大学生为研究对象。纳入条件：①符合童年创伤评分标准^［19］（当分量表中情感虐待>13，躯体虐待>10，性虐待>8，情感忽略≥15，躯体忽略≥10，只要有一个分量表得分满足以上条件的为中重度的童年创伤者，视为伴童年创伤者）；②自愿参与。排除条件：①近3个月有过精神心理问题诊断及服用相关药物经历；②同时参与其他研究项目的。采用Gpower v3.1样本量计算软件，选择重复测量分析方法，具体参数为：Effect size=0.25，α=0.05，Power=0.90，Number of groups=2，Number of measurements=2，得出总共需要46名研究对象，考虑到可能存在的15%流失率，本研究至少需要研究对象54名，经过专业心理人员半结构化入组访谈，明确符合童年创伤的报名者，最终招募到54名研究对象。所有被试入组前签署了书面知情同意，本研究经陆军军医大学医学伦理委员会审批（2025第6-03号）。

1.2　研究工具

1.2.1　童年创伤量表（childhood trauma questionnaire，CTQ）

该量表由BERNSTEIN等^［20］编制，中文版由赵幸福等^［21］修订，量表共28个条目构成，分为情感虐待、躯体虐待、性虐待、情感忽视、躯体忽视5个分量表。采用1~5分的5级评分（1=从不，2=偶尔，3=有时，4=常常，5=总是），总分为28~140分，分数越高，说明童年创伤水平越高^［22］。傅文青等^［23］对该量表进行了信效度研究，结果表明该量表信效度尚好，适用于国内高校大学生群体。

1.2.2　认知灵活性量表（cognitive flexibility inventory，CFI）

该量表由DENNIS等^［24］编制，用于评估个体认知灵活性水平，由20个条目组成，每个条目采用1~5级评分（1=从不，2=很少，3=有时，4=经常，5=总是），总分为20~100分，得分越高表示个体认知灵活性越好。王阳等^［25］对认知灵活性量表在大学生群体中的信效度进行了测评，结果显示该量表在大学生样本中具有良好的测量效果。

1.2.3　数字字母转换范式

该任务要求被试在电脑程序（E-Prime）中的两种任务中进行转换：在数字任务中，判断数字的奇偶性（3、5、7、9以及2、4、6、8），在字母任务中，判断字母是元音（a、e、i、o、u）还是辅音（p、n、s、k），总共180个试次，可分为重复以及转换两种转换模式：重复试次的前后任务类型不变，如数字-数字或字母-字母，转换试次指转换了任务类型，如数字-字母或者字母-数字。为评估认知灵活性，将转换时间代价以及错误率转换代价作为核心指标进行计算^［26］。转换时间代价指转换试次与重复试次之间的反应时间差异，错误率转换代价指转换试次与重复试次之间的错误率差异。转换代价越高，意味着认知灵活性越低。在开始正式试次前，被试均需要通过练习试次。

1.3　研究方法

采用随机对照研究设计，对入组的54名被试采用随机数字表法随机分为干预组（27人）与等待组（27人），入组后，所有被试于干预开始前（T1）完成了问卷数据采集以及数字字母转换任务测试。前测结束后，干预组接受闪动技术干预^［18］（表1），约70 min/次、1次/周，共6次；等待组暂不接受干预，在本研究结束后可以参加闪动技术干预。干预结束后（T2），2组均完成认知灵活性量表以及数字字母转换任务后测。整个干预过程由受过规范化闪动技术培训的精神心理科执业医师实施。

1.4　统计学分析

采用SPSS 29.0统计软件进行数据分析，计数资料以频数（%）表示，计量资料以

x ¯

±s表示，采用重复测量方差分析考察干预的效果，对交互作用显著的指标进行时间简单效应以及分组简单效应分析，以P<0.05表示差异具有统计学意义。

2　结果

2.1　被试基本情况与基线特征

共完成54例研究，其中干预组与等待组各27例，比较干预组与等待组的一般人口统计学信息、CTQ以及在T1时的认知灵活性量表与行为学指标，结果表明2组在各人口统计学特征、CTQ以及T1时的CFI、转换时间代价、错误率转换代价的差异均无统计学意义（表2）。

2.2　干预组CFI显著提升

对CFI重复测量方差分析结果表明（表3）：组别的主效应不显著（F=0.730，P=0.397，偏η²=0.014），测量时间的主效应显著（F=30.611，P<0.001，偏η²=0.371），测量时间与组别的交互效应显著（F=24.654，P<0.001，偏η²=0.322）。组别简单效应检验结果显示（图1A）：T1时，干预组与等待组CFI差异不显著（F=1.721，P=0.195，偏η²=0.032，t=-1.312，95%CI：-10.494~2.197），在T2时，干预组CFI显著高于等待组（F=9.324，P=0.004，偏η²=0.152，t=3.054，95%CI：3.022~14.608）；时间简单效应检验结果显示：干预组在T1到T2的CFI变化显著（F=55.103，P<0.001，偏η²=0.514，t=7.424，95%CI：9.999~17.408），而等待组在T1到T2的CFI变化不显著（F=0.161，P=0.690，偏η²=0.003，t=0.401，95%CI：-2.964~4.445）。

2.3　干预组转换时间代价显著降低

对数字字母转换时间代价重复测量方差分析结果显示：组别的主效应显著（F=4.902，P=0.031，偏η²=0.086）；测量时间的主效应显著（F=2.240，P=0.141，偏η²=0.041）；测量时间与组别的交互效应显著（F=6.603，P=0.013，偏η²=0.113）。组别简单效应检验结果显示（图1B）：在T1时，2组转换时间代价差异不显著（F=0.458，P=0.502，偏η²=0.009，t=-0.677，95%CI：-46.582~23.087），在T2时，干预组字母转换时间代价得分显著低于等待组（F=9.716，P=0.003，偏η²=0.157，t=-3.117，95%CI：-93.438~-20.251）；时间简单效应检验结果显示：干预组在T1到T2间的转换时间代价变化显著（F=8.268，P=0.006，偏η²=0.137，t=-2.875，95%CI：-60.582~-10.780），而等待组在T1到T2间的转换时间代价变化不显著（F=0.576，P=0.451，偏η²=0.011，t=0.759，95%CI：-15.486~34.317）。对2组错误率转换代价在2个时间点上进行的重复测量方差分析结果显示，组别与时间的交互作用不显著（F=1.397，P=0.243，偏η²=0.026）。

3　讨论

3.1　闪动技术可提升童年创伤大学生的认知灵活性

本研究发现闪动技术可有效提升童年创伤大学生的主观认知灵活性，这与YAN等^［27］基于EMDR技术的研究结论具有一致性。本研究结果的潜在机制可以结合现有理论进行初步推测：其一，基于SHAPIRO等^［28］提出的适应性信息加工模型，童年创伤经历由于强烈的情绪唤醒中断了其被大脑加工为适应性记忆的过程，创伤记忆以原始形式（包含图像、情绪、身体感觉等）存储在神经系统中，无法有效整合。这些具有侵入性特征的创伤记忆会触发当下的痛苦反应（如闪回、焦虑等），同时占用认知资源，持续引发情绪困扰与认知功能损害。记忆消退再巩固理论^［29］指出，记忆在被重新提取时会进入可塑性增强、易被修改的不稳定阶段，在该时间段内通过特定干预可以实现记忆的更新或者强化，呈现与原有记忆冲突的信息或情境，可形成新的记忆联系，削弱原有记忆及其情感强度。如LI等^［30］采用暴露疗法与药物相结合阻断记忆再巩固过程，有效地削弱了不良记忆，降低了复发风险。闪动技术通过短暂、边缘性的接触创伤记忆，触发记忆再巩固过程，在避免强烈情绪唤醒的条件下为记忆更新创造了契机。这一过程让创伤记忆从原始储存形式转化为可整合的适应性信息，同时既往创伤记忆引发的主观痛苦及其对认知资源的占用可得到一定程度缓解。由于本研究未测量记忆再巩固的直接指标（如创伤记忆的情绪强度变化），这一机制仍需后续研究验证。其二，闪动技术中对于自身积极资源的提取也发挥了至关重要的作用，FREDRICKSON等^［31］的拓展建构理论认为积极情绪能拓展个体瞬间知行能力，建构持久性资源。个体对积极记忆的提取有利于建立稳定的积极情绪偏向，缓冲负面情绪的消极影响^［32］。这种积极情绪与正性资源的积累，一方面强化了主动切换认知策略的自我效能感，另一方面减少了负性情绪对认知资源的占用，为认知转换提供了关键资源支持。

3.2　双侧刺激可能提升童年创伤大学生客观认知转换效率

行为学数据分析结果显示，干预组数字字母转换任务的转换时间代价有所缩短，表明闪动技术可有效提升童年创伤大学生的认知转换效率，尽管直接探讨闪动技术对神经认知功能影响的研究尚少，但SUSANTY等^［33］的研究证实EMDR干预可改善神经认知功能，为本研究结果提供了间接佐证。STINGL等^［34］基于脑电以及外周生理测量的研究结果发现，视觉与触觉双侧刺激均可显著提升额叶脑电总功率，降低自主神经唤醒水平，促进创伤记忆的整合和处理。前额叶皮质是支持认知灵活性的核心脑区，其激活水平提升可直接加快转换任务中的信息加工速度；其次，ROUSSEAU等^［35］的脑成像研究表明，双侧交替刺激还可增强颞上回、额中回等执行控制相关脑区的结构连接性，同时抑制杏仁核的恐惧相关活动，强化神经网络的协同工作效率^［36］。这一效应既提升了双侧脑区协同效率，又减少了情绪唤醒对认知转换过程的干扰，使更多认知资源可集中于核心任务的加工与处理，一定程度上印证了适应性信息加工模型中“创伤记忆整合释放认知资源”的核心逻辑。

3.3　转换错误率无显著改善的潜在原因分析

本研究的转换任务中，被试错误率转换代价干预前后差异并不显著，闪动技术干预只提高了任务转换效率，没有提高转换的准确性。可能的原因包括以下两点：大学生群体认知能力处于较高水平，转换错误率或已接近较优结果；认知任务中，速度与准确率之间存在权衡效应^［37］，反应速度越快，错误率相对越高。因此，闪动干预提高反应的速度，改善作用尚未体现在对错误率的控制上。

闪动技术通过边缘性接触创伤记忆，强烈的情绪反应可得到有效避免，同时帮助打开安全的记忆消退的窗口；提取积极自传体记忆的过程，能为存留创伤记忆，背负负性情感的个体补充正性资源，替换原有负面心理内容，辅助强化认知转换过程中的主观自我效能感，一定程度上帮助降低了负面情绪感受与认知负荷；以蝴蝶拍为核心的双侧刺激，可能增强脑区连接性、抑制杏仁核活动、提高双侧大脑协同效率、加快认知转换速度，最终实现认知灵活性的提高。对上述作用路径的讨论，是结合本研究结果与相关理论做出的初步推测，具体作用机制，仍需结合神经生理方面的直接测量，纳入更大样本量进行验证性研究，才可得出明确结论。

本研究同时运用主观问卷与数字字母转换任务客观指标，完成双重检验，可缩小单一测量工具带来的偏差；过往针对创伤进行干预，研究方向多聚焦于情绪调节方面，本研究把闪动技术的应用延伸至认知功能领域，面向非临床创伤群体进行认知干预，积累了实证支撑。本研究存在多方面不足：①未设置干预后的追踪期，只通过即时后测验证干预效果，无法明确认知灵活性改善的远期效果；②纳入研究的样本量相对有限，错误率转换代价统计检验的效能可能降低；③影响干预结局的因素较多，焦虑抑郁睡眠质量都可能参与其中，结果因果推论的力度受到限制；④研究未设置阳性对照，研究结果准确性仍有提高空间；⑤本研究未纳入神经生理方面的测量，未来研究可拓展样本范围，设置阳性对照，加入追踪测试环节，控制可能影响认知功能的混杂因素，结合神经生理指标，验证闪动技术干预的实际效果。

Desensitization	First， ask the client to imagine a book or a box and place the trauma memory inside. Then guide the client to alternate tapping on the shoulders， while focusing on a positive engaging focus （PEF） through discussion of positive memories or imagery. Throughout the process， the therapist will say the word “blink” at irregular intervals， requiring the client to blink rapidly 3 to 5 times upon hearing the word while maintaining focus on the PEF. After 6 sets of “blink” operations， briefly recontact the trauma memory and assess the current SUD level. Repeat the same procedure until the SUD decreases and physical and psychological discomfort weakens	30－45
