目的

探究正念注意觉知对抑郁合并失眠患者抑郁情绪和失眠严重程度的中介作用。

方法

采用横断面研究，选取2024年3—5月于陆军军医大学第二附属医院医学心理科门诊就诊的267例抑郁合并失眠患者为研究对象。采用一般资料调查表收集患者基本人口学及临床资料，患者健康问卷抑郁量表（patient health questionnaire，PHQ-9）测量抑郁情绪水平，失眠严重指数量表（insomnia severity index scale，ISI）测量失眠症状严重程度，知觉压力量表-10（perceived stress scale-10，PSS-10）测量知觉压力水平，正念注意觉知量表（mindful attention awareness scale，MAAS）测量正念注意觉知水平。采用Pearson相关法分析患者抑郁、失眠严重程度与知觉压力、正念注意觉知的相关性；采用Process 4.1进行中介效应分析。

结果

267例抑郁合并失眠患者PHQ-9得分为13.80±5.98，ISI得分为17.10±5.56。Pearson 相关分析结果显示：抑郁、失眠严重程度均与知觉压力呈正相关（r=0.531、0.351，P<0.001），与正念注意觉知呈负相关（r=-0.373、-0.350，P<0.001）。Process 4.1中介效应分析提示：正念注意觉知和失眠在患者知觉压力水平与抑郁情绪水平之间的中介效应值为0.157，95%CI为0.102~0.217，总中介效应显著（P<0.001）。

结论

知觉压力既直接正向影响抑郁，又通过失眠中介间接加剧抑郁；正念注意觉知可削弱知觉压力对失眠的促进作用。

Objective

To explore the mediating role of mindful attention and awareness in depressive symptoms and insomnia severity among patients with comorbid depression and insomnia.

Methods

A cross-sectional study was conducted, enrolling 267 patients with comorbid depression and insomnia who were treated in the outpatient Department of Medical Psychology of Second Affiliated Hospital of Army Medical University, from March to May 2024. Basic demographic and clinical data were collected using a general information questionnaire. Depressive symptom severity was measured via the Patient Health Questionnaire-9 (PHQ-9), insomnia severity via the Insomnia Severity Index (ISI), perceived stress via the Perceived Stress Scale-10 (PSS-10), and mindful attention and awareness via the Mindful Attention Awareness Scale (MAAS). Pearson correlation analysis was used to examine the correlations between depressive severity, insomnia severity, perceived stress, and mindful attention and awareness. Mediation analysis was performed using Process 4.1.

Results

The PHQ-9 score was (13.80±5.98) and the ISI score was (17.10±5.56) in the 267 patients. Pearson correlation analysis showed that depressive severity and insomnia severity were positively correlated with perceived stress (r=0.531, 0.351, P<0.001) and negatively correlated with mindful attention and awareness (r=-0.373, -0.350, P<0.001). Mediation analysis using Process 4.1 indicated that the combined mediating effect of mindful attention and awareness and insomnia between perceived stress level and depressive level was 0.157, with a 95% confidence interval (CI) of 0.102~0.217, and the total mediating effect was significant (P<0.001).

Conclusion

Perceived stress directly positively affects depression and indirectly exacerbates depression through insomnia as a mediator, and mindful attention and awareness can weaken the promoting effect of perceived stress on insomnia.

抑郁与失眠共病是全球重大公共卫生挑战，流行病学数据显示，抑郁患者中失眠发生率达72%~80%，慢性失眠患者患抑郁风险为普通人群的3.2~5.3 倍^［1-3］，共病群体呈现出症状叠加、病程迁延、预后差的特点^［4-5］。传统单一干预，如抗抑郁药、失眠认知行为疗法（cognitive behavioral therapy for insomnia，CBT-I）对共病患者有效应答率有限，且复发风险更高^［6］，提示“单症状干预”局限，亟待解析共病传递机制。知觉压力是个体对内外压力源的主观解读与心理反应，其对睡眠质量的负面影响已被压力系统模型^［7］及既往研究^［8］证实；正念即对当下体验持开放、非批判的接纳态度^［9］，正念干预作为压力管理方法，可有效降低失眠患者知觉压力、改善睡眠质量^［10］，二者均为影响共病患者症状的重要因素。现有研究虽揭示正念、知觉压力的独立作用，但缺乏对“压力-认知-睡眠-情绪”链式中介效应的探索。鉴于此，本研究聚焦临床抑郁合并失眠患者，拟验证“知觉压力-正念注意觉知-失眠-抑郁”的链式中介效应，系统探索知觉压力影响抑郁情绪的具体路径，同时考察失眠的中介效应与正念注意觉知的调节效应。本研究突破现有研究仅关注单一因素独立作用的局限，系统解析上述四变量间的多环节交互路径，不仅可填补抑郁与失眠共病领域多因素交互机制的研究空白，还能为临床制定“多环节阻断”型精准干预方案，以期为共病的机制解析与精准干预提供新视角。

1　对象与方法

1.1　研究对象

本研究为横断面研究，采用便利抽样法，于2024年3—5月选取陆军军医大学第二附属医院医学心理科门诊的抑郁合并失眠患者作为研究对象。拟采用回归分析方法了解抑郁合并失眠的相关影响因素，初步确定拟纳入回归因素个数为7个（见表1），包括分组合计15层，根据每变量事件数（events per variable，EPV）≥10法则，所需纳入分析的最小样本量为150例^［11］。

1.2　纳入和排除标准

纳入标准：①年龄≥18岁；②符合失眠障碍的标准^［1］：a.难以入睡/维持睡眠（睡眠潜伏期≥30 min、睡眠时间≤6.5 h）；b.每周发作≥3次且持续≥3个月；c.伴日间功能损害（如疲劳、注意力/记忆力减退、情绪波动、工作/社交能力下降等）；③符合抑郁障碍的诊断标准^［2］：a.至少存在1项核心症状（显著情绪低落、兴趣/愉悦感丧失），可伴精力下降、睡眠/食欲异常、注意力减退、无价值感、自杀相关想法等；b.症状持续≥2周（几乎每天大部分时间存在）；c.伴工作/学习效率下降、社交回避等功能损害（躯体症状无法用躯体疾病解释）；④具备中文读写能力；⑤失眠严重指数量表（insomnia severity index scale，ISI）≥8分；⑥患者健康问卷抑郁量表（patient health questionnaire，PHQ-9）≥5分。

排除标准：①由于严重的躯体疾病、疾病的急性阶段或明显的症状（如疼痛）导致的继发性失眠；②有听力或交流障碍、没有能力完成问卷者。

1.3　研究工具

1.3.1　一般资料调查表

该问卷为自编问卷，主要包括年龄、性别、婚姻状况、学历、职业。

1.3.2　PHQ-9量表

本研究采用PHQ-9评估被试者抑郁情绪水平。该量表由SPITZER等^［12］根据DSM-4诊断标准编制，该量表将DSM-4中抑郁发作诊断标准中的9个条目，分为1~4级评分，从“完全没有”到“几乎每天”（0分：完全不会，1分：好几天，2分：超过1周，3分：几乎每天）。以条目总分表示抑郁程度，得分越高表明抑郁越严重。总分≥5分即可视为有临床意义的抑郁。PHQ-9将抑郁程度分类如下：无抑郁（0~4分），轻度抑郁（5~9分），中度抑郁（10~14分），中重度抑郁（15~19分），重度抑郁（20~27分）。量表灵敏度为88%，特异度为89%，在不同人群中有很高的信效度。本研究中，该量表的内部一致性Cronbach’s α系数为0.875。

1.3.3　ISI量表

由 Charles 教授等进行编制，是目前临床上使用最多的失眠评估量表之一，被推荐用作失眠的筛查和评估工具^［13］。该量表共包含7个条目，分别为入睡困难、睡眠维持困难、早醒、对生活质量的影响、对睡眠的担心程度（0分：无，1分：轻度，2分：中度，3分：重度，4分：极重度）、对睡眠的满意程度（0分：很满意，1分：满意，2分：一般，3分：不满意，4分：很不满意）、对日常功能的影响（0分：没有干扰，1分：轻微，2分：有些，3分：较多，4分：很多干扰）。每个条目为0~4分，总分为0~28分，得分越高表明失眠程度越严重。总分>7分即可视为有临床意义的失眠。ISI将失眠严重程度分类如下：无失眠（0~7分）、轻度失眠（8~14分）、中度失眠（15~21分）和重度失眠（≥22分）。ISI量表内部一致性信度为0.804，重测信度为0.887，推荐作为中国临床患者的失眠评估。本研究中，该量表的内部一致性 Cronbach’s α系数为0.905。

1.3.4　知觉压力量表-10（perceived stress scale-10，PSS-10）

由COHEN等^［14］编制、王振等^［15］汉化，共10个条目，各条目采用Likert 5级评分法，从“没有”到“大部分时候”依次计0~4分（0分：没有，1分：很少，2分：有时候，3分：时常，4分：大部分时候），总分0~40分，其中4、5、7、8题反向计分，得分越高表示个体知觉压力越大。PSS-10的Cronbach’s α系数为0.91，重测信度为0.69，信效度良好，现已得到广泛应用。本研究中，该量表的内部一致性 Cronbach’s α系数为0.782。

1.3.5　正念注意觉知量表（mindful attention awareness scale，MAAS）

采用 BROWN等编制的正念注意觉知量表^［16］，该量表由陈思佚等^［17］汉化。包含15道题目，指导语明确要求受试者在每个条目下，依据最近一周（含当天）的实际状况，选择最贴合自身情况的描述等级；其中等级“1”至“6”按程度梯度变化，分别对应从“几乎总是”到“几乎从不”的情况（1表示几乎总是、2表示非常频繁、3表示有些频繁、4表示有些不频繁、5表示非常不频繁、6表示几乎从不）。量表中每个条目为正念概念的反向表述：评定等级越高，意味着该条目所描述的情形出现的频率越低。量表得分越高，说明个体在日常生活中对当下的觉知与关注度越高。MAAS的项目区分度良好，各项目与总量表的相关系数介于0.422~0.711；其内部一致性系数为 0.890，重测信度为0.870，表明该量表信度良好。在本研究中，该量表的内部一致性 Cronbach’s α系数为0.902。

1.4　资料收集

本研究通过陆军军医大学第二附属医院伦理委员会审核（2024－临研第042-01），收集284份问卷，剔除ISI和PHQ-9得分不合格者问卷17份，获得有效问卷267份，有效率 94.01%。

1.5　质量控制与实施

组建研究小组，本研究由1名硕士生导师牵头负责，小组成员包括 1名副教授、1名主管护师、2名心理治疗师、1名统计学专家。本次研究的问卷制定根据文献查阅以及小组会议讨论，并结合临床实际应用以及专家咨询以确定，问卷内容通过医院科学性审查以及伦理审查。研究者在患者签署《知情同意书》后，通过统一指导语向其说明填写方法和注意事项，并发放问卷星问卷，当场检查问卷填写情况。提取问卷结果后，采取双人核对录入并核查校正；数据分析由专门的统计学人员进行。

1.6　统计学分析

采用SPSS 27.0统计软件分析数据，计数资料采用频数、构成比表示；计量资料：正态分布数据以

±s表示，非正态分布数据采用M（P₂₅，P₇₅）表示。用单因素方差分析探究一般资料对抑郁及失眠严重程度的影响；用Pearson相关法分析PHQ-9、ISI评分与PSS-10、MAAS评分的相关性；用Process 4.1构建中介效应模型，通过Bootstrap法（重复抽样5 000次）检验效应显著性。以P<0.05为差异有统计学意义。

2　结果

2.1　患者抑郁、失眠严重程度、知觉压力和正念注意觉知水平

采用PHQ-9、ISI、PSS-10及MAAS，分别测量患者抑郁情绪水平、失眠严重程度、知觉压力水平及正念注意觉知水平。结果显示：抑郁得分为5.00~27.00（13.80±5.98），失眠严重程度得分为8.00~28.00（17.10±5.56），其中轻度抑郁、中度抑郁、中重度抑郁、重度抑郁分别占比35.2%、20.3%、25.4%、18.0%；轻度失眠、中度失眠、重度失眠分别占33.3%、43.4%、23.2%；知觉压力得分为3.00~40.00（20.02±6.61），正念注意觉知得分为24.00~90.00（60.74±14.7）。提示本研究纳入的抑郁合并失眠患者以中重度症状为主，同时存在知觉压力较高及正念注意觉知水平偏低。

2.2　患者抑郁、失眠的单因素分析

采用描述性统计分析研究对象的人口学特征，采用单因素分析筛选患者抑郁及失眠严重程度的影响因素。研究对象的人口学特征显示，年龄18~86岁，女性占比70%。提示在抑郁合并失眠患者中，年龄、知觉压力及正念注意觉知可能共同作用于抑郁症状，而知觉压力与正念注意觉知可能是调控失眠症状的关键关联因素（P<0.05，表1）。

2.3　抑郁、失眠严重指数、知觉压力、正念注意觉知相关性分析

采用 Pearson 相关法，分析四者间的相关性。结果显示：抑郁与知觉压力呈正相关（r=0.531，P<0.001），与正念注意觉知呈负相关（r=-0.373，P<0.001）；失眠严重指数与知觉压力呈正相关（r=0.351，P<0.001），与正念注意觉知呈负相关（r=-0.350，P<0.001）；失眠严重指数与抑郁呈正相关（r=0.507，P<0.001）；知觉压力与正念注意觉知呈负相关（r=-0.381，P<0.001）。提示在抑郁合并失眠患者中，知觉压力水平越高、正念注意觉知能力越低，其抑郁与失眠症状可能越严重，且抑郁与失眠症状间亦存在明显的正向关联，为后续中介效应分析提供了关联基础。见表2。

2.4　正念注意觉知在患者知觉压力与抑郁之间的中介作用分析

在控制年龄的情况下，采用Process 4.1模型6验证正念注意觉知在抑郁合并失眠患者知觉压力和抑郁之间的中介作用。结果表明：知觉压力对正念注意觉知有反向预测作用（β=-0.837，P<0.001），当知觉压力和正念注意觉知同时作用于抑郁时，知觉压力对抑郁有正向预测作用（β=0.309，P<0.001），正念注意觉知对抑郁有反向预测作用（β=-0.047，P<0.05），见表3。使用偏差校正的非参数百分位Bootstrap法对中介效应进行检验，结果表明：知觉压力-正念注意觉知-抑郁的效应值为0.040 0，知觉压力-失眠-抑郁的效应值为0.085 4，知觉压力-正念注意觉知-失眠-抑郁的效应值为0.031 8（表4）。提示在抑郁合并失眠患者中，正念注意觉知在知觉压力与抑郁的关联中发挥中介作用，且“知觉压力-正念注意觉知-失眠-抑郁”的链式中介路径成立，表明知觉压力可通过降低患者正念注意觉知水平、间接影响失眠症状，最终加剧抑郁情绪。链式中介模型见图1。

3　讨论

3.1　患者临床特征与心理状态的临床意义

本研究经量表分析发现，抑郁合并失眠患者以“中重度症状为主、心理调节失衡”为核心临床表型，可为共病机制研究与干预靶点筛选提供基线依据。ISI分度显示，中度（43.4%）与重度失眠^［18］（23.2%）累计占比66.6%，均值17.10分（高于临床阈值15^［19］）；PHQ-9分度中，中重度（25.4%）与重度抑郁^［20］（18.0%）累计占比43.5%，均值13.80分。中重度共病的高占比，可能提示传统单症状干预（如单独使用抗抑郁药或镇静催眠药）存在局限性。正念注意觉知量表均值60.74分，提示共病患者正念水平中等偏低^［21］，其不足或削弱睡眠与情绪调控能力^［22］；知觉压力均值20.02分（≥16分提示高压力^［23-24］），反映患者可能长期处于慢性压力暴露中。结合本研究发现的“知觉压力-正念注意觉知-失眠-抑郁”链式中介机制，提示临床干预可聚焦“强化正念韧性，打破压力-睡眠-情绪恶性循环”。

3.2　人口学特征的影响价值

人口学数据结果显示：性别对失眠、抑郁影响极微弱，临床干预无需特别关注性别差异，更应聚焦压力源、睡眠习惯等核心影响因素。男性失眠（ISI均值17.46）和抑郁程度（PHQ-9均值13.88）略低于女性，可能与男性能更好调节负性情绪有关^［25］；年龄越大，抑郁情绪水平越低（P=0.016），可能因老年人情绪调节更成熟，对负性事件存在“钝化效应”^［26］。

3.3　相关性结果的机制启示

Pearson 相关显示：知觉压力与抑郁、失眠均呈正相关（r=0.531、0.351，P<0.001），正念觉知水平与二者均呈负相关（r=-0.375、-0.350，P<0.001），且失眠与抑郁正相关（r=0.507，P<0.001）。这一结果印证“压力-正念-症状”的关联逻辑：高压力可能通过挤占正念认知资源，同时加剧失眠与抑郁，而失眠又进一步恶化抑郁，形成闭环。此关联为后续中介效应分析提供了前提，也支持“多因素交互影响共病”的假设。

3.4　链式中介机制的核心价值

中介模型回归分析表明：知觉压力与共病患者失眠、抑郁关系密切，可正向预测二者，提示知觉压力越高，失眠、抑郁程度越高，与徐晓晓等^［27-28］研究一致。正念觉知在知觉压力与失眠、抑郁间起中介作用，提示知觉压力可直接通过正念觉知间接影响二者，与李艳娟等^［29-30］研究一致。正念觉知还起链式中介作用，初步揭示“知觉压力通过正念觉知链式中介影响失眠、抑郁”的机制，即较高正念觉知水平可能帮助患者接纳压力，降低其对失眠、抑郁的影响。因此，临床或可通过正念减压提升患者正念觉知水平，改善抑郁情绪与失眠症状。

需特别说明的是，本研究的变量设定并非否定失眠与抑郁“互为因果”的特性，我们充分认可二者存在“失眠加重抑郁、抑郁诱发失眠”的双向循环关系，仅因篇幅聚焦需求，优先剖析“知觉压力-正念注意觉知-失眠-抑郁”这一以抑郁为终点的中介链条，以精准揭示知觉压力影响抑郁的核心心理路径。

本研究存在局限性：横断面研究设计难以确定变量因果时序，后续可通过纵向研究与随机对照试验验证机制；单中心样本缺乏客观指标，后续可开展多中心研究并纳入肠道菌群等生物学指标。

综上所述，本研究提示抑郁合并失眠患者中重度症状占比高，存在正念觉知水平偏低知觉压力较高的心理失衡；知觉压力通过“知觉压力-正念-失眠-抑郁” 链式中介加剧共病，正念觉知为关键保护因素。治疗中可通过提高正念觉知水平、降低知觉压力改善症状，如采用 CBT-I 联合正念疗法（CBT-I 改善睡眠结构，正念缓冲压力对情绪的影响）。
