To explore the effects of cardiac magnetic resonance imaging(CMRI) on the assessment of left ventricular function and myocardial changes in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS). Thirty-one patients with OSAHS who were admitted to Handan Central Hospital from November 2019 to June 2021 were selected as the observation group, and 30 healthy volunteers who received CMRI monitoring during the same period were selected as the control group.Two sets of parameters related to left ventricular function were compared, including left ventricular ejection fraction (LVEF), left ventricular end diastolic/end systolic volume (LVEDV/LVESV), left ventricular myocardial activity imaging indexes[T1pre value(measurement of T1 relaxation time before enhancement) before mean injection of contrast agent in left ventricular myocardium, 15 min T1post value(T1 value of myocardial tissue measured by T1 mapping technique after using contrast agent in CMRI) and T2 value(transverse relaxation time of myocardial tissue)]after injection,and histological parameter characteristics of different myocardial layers[mean extracellular matrix volume fraction (ECV) of left ventricular myocardium].The left ventricular function parameters (LVEF, LVED) in the observation group are significantly lower than those in the control group, while LVESV and T2 values are significantly higher than those in the control group (P<0.05). There is no significant difference in T1pre, T1post, ECV in basal segment, middle segment and apical segment between the observation group and the control group (P>0.05).CMRI has a significant clinical effect in patients with OSAHS and can effectively assess the left ventricular function, structure and early myocardial lesions.

探析心脏磁共振成像技术(cardiac magnetic resonance imaging,CMRI)应用于评估阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)患者左心功能及心肌的变化效果。选择邯郸市中心医院2019年11月—2021年6月就诊的31例OSAHS患者作为观察组,选择同期接受CMRI监测的30例健康志愿者作为对照组。对比两组左心功能相关参数,包括:左心室射血分数(LVEF)、左心室舒张末期/收缩末期容积(LVEDV/LVESV)、左室心肌活性成像指标[左心室心肌平均注射对比剂前T1pre值(在增强前T1弛豫时间的测量值)、注射后15 min T1post值(在CMRI中,使用对比剂后的T1 mapping技术所测量的心肌组织的T1值)、T2值(心肌组织的横向弛豫时间)]、不同心肌层面组织学参数特征[左室心肌平均细胞外间质容积分数(ECV)]。观察组左心功能参数(LVEF左心室射血分数LVED左心室舒张未期内径)均明显低于对照组,而LVESV、T2值明显高于对照组(P<0.05)。观察组T1pre、T1post值、基底段、中间段、心尖段ECV与对照组无统计学差异(P>0.05)。CMRI应用于OSAHS患者中临床效果显著,可有效评估患者的左心功能、结构及心肌早期病变。

探析心脏磁共振成像技术(cardiac magnetic resonance imaging,CMRI)应用于评估阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)患者左心功能及心肌的变化效果。选择邯郸市中心医院2019年11月—2021年6月就诊的31例OSAHS患者作为观察组,选择同期接受CMRI监测的30例健康志愿者作为对照组。对比两组左心功能相关参数,包括:左心室射血分数(LVEF)、左心室舒张末期/收缩末期容积(LVEDV/LVESV)、左室心肌活性成像指标[左心室心肌平均注射对比剂前T1pre值(在增强前T1弛豫时间的测量值)、注射后15 min T1post值(在CMRI中,使用对比剂后的T1 mapping技术所测量的心肌组织的T1值)、T2值(心肌组织的横向弛豫时间)]、不同心肌层面组织学参数特征[左室心肌平均细胞外间质容积分数(ECV)]。观察组左心功能参数(LVEF左心室射血分数LVED左心室舒张未期内径)均明显低于对照组,而LVESV、T2值明显高于对照组(P<0.05)。观察组T1pre、T1post值、基底段、中间段、心尖段ECV与对照组无统计学差异(P>0.05)。CMRI应用于OSAHS患者中临床效果显著,可有效评估患者的左心功能、结构及心肌早期病变。

To explore the effects of cardiac magnetic resonance imaging(CMRI) on the assessment of left ventricular function and myocardial changes in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS). Thirty-one patients with OSAHS who were admitted to Handan Central Hospital from November 2019 to June 2021 were selected as the observation group, and 30 healthy volunteers who received CMRI monitoring during the same period were selected as the control group.Two sets of parameters related to left ventricular function were compared, including left ventricular ejection fraction (LVEF), left ventricular end diastolic/end systolic volume (LVEDV/LVESV), left ventricular myocardial activity imaging indexes[T1pre value(measurement of T1 relaxation time before enhancement) before mean injection of contrast agent in left ventricular myocardium, 15 min T1post value(T1 value of myocardial tissue measured by T1 mapping technique after using contrast agent in CMRI) and T2 value(transverse relaxation time of myocardial tissue)]after injection,and histological parameter characteristics of different myocardial layers[mean extracellular matrix volume fraction (ECV) of left ventricular myocardium].The left ventricular function parameters (LVEF, LVED) in the observation group are significantly lower than those in the control group, while LVESV and T2 values are significantly higher than those in the control group (P<0.05). There is no significant difference in T1pre, T1post, ECV in basal segment, middle segment and apical segment between the observation group and the control group (P>0.05).CMRI has a significant clinical effect in patients with OSAHS and can effectively assess the left ventricular function, structure and early myocardial lesions.

阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)是指患者睡眠过程中由于上气道阻塞导致口鼻气流较基线水平降低,呼吸暂停,而中枢神经系统呼吸驱动功能仍能维持正常,进而导致慢性间歇性低氧和睡眠结构紊乱的失眠障碍性疾病[1-2]。OSAHS已经被临床广泛认知,目前主要临床症状为夜间睡眠频繁打鼾,出现短暂的呼吸暂停及间断觉醒表现,由于患者夜间睡眠质量不足导致白天嗜睡。长期的夜间缺氧以及睡眠紊乱,严重者会出现全身系统症状,譬如各种心脑血管及神经系统损伤等。若不及时干预控制OSAHS病情,将可能诱发缺血性心脏疾病、心律失常、高血压等心脏相关疾病,因此临床对于监测OSAHS患者病情及心脏功能越来越重视。据流行病学统计,中国的OSAHS发病率较高,约为4.73%,并且发病人群中成年男性比例高达15%,并且随着中国人口逐步迈入老龄化阶段,发病率呈逐年增高趋势。
近年来心脏磁共振成像技术(cardiac magnetic resonance imaging, CMRI)也逐渐应用于OSAHS心脏功能评估中。研究表明,CMRI评估OSAHS心脏功能效果显著[3]。 CMRI具有高时间及空间分辨率、最佳软组织对比度、视野广、成像参数多且信息量获取多、无辐射危害等优势,在临床鉴别心脑血管疾病、评估病情、疗效等方面有着突出效果[4-6]。但由于目前针对磁共振成像(magnetic resonance imaging,MRI)评估OSAHS认知脑功能研究报道较多,而对其心脏功能相关报道相对较少,还需要更多实践研究给予支持。基于此,将对 CMRI应用于评估OSAHS患者左心功能及心肌的变化效果进行分析,旨在为临床选择 CMRI评估OSAHS患者心脏功能提供数据支持。
选择2019年11月—2021年6月于邯郸市中心医院就诊并诊断为OSAHS(诊断标准:参照2011年发布的《阻塞性睡眠呼吸暂停低通气综合征诊治指南》[7]临床症状:患者每夜7 h睡眠过程中存在呼吸暂停或不通气症状,且反复发作次数≥30次,可能伴随着打鼾、白天嗜睡等相关症状,经多导睡眠图(PSG)监测证实患者31例,定义为观察组,并选择同期接受CMRI监测的30例健康志愿者作为对照组。观察组男性18例,女性13例,男女比例为18/13,年龄范围41~68岁,平均年龄(53.27±4.36)岁;对照组男性17例,女性13例,男女比例为17/13,年龄范围40~69岁,平均年龄(53.14±4.42)岁。
(1)纳入标准。①患者符合上述OSAHS临床症状及影像学诊断标准;②符合《心血管磁共振成像技术检查规范中国专家共识》[8]适应证;③研究对象签署同意知情书。
(2)排除标准。①排除合并上气道阻力综合征、发作性睡病等类似病症者;②排除合并心肺疾病、高血压、糖尿病等基础疾病者;③对CMR检测不耐受者:入体内含有金属植入物、无法根据医师指导配合呼吸运动等。
本文研究已通过邯郸市中心医院伦理委员会批准,并经所有患者知情同意。
两组均进行CMRI(仪器选择:西门子skyra3.0T)检测,具体操作如下:引导患者取仰卧位,采用心脏专用线圈、心电、屏气及呼吸导航门控技术进行操作,实时梯度回波多时相电影序列扫描,并获取心脏短轴电影图像。调整多序列扫描:重复时间(time of repetition,TR)设置为2.8 ms,回波时间(time of echo,TE)设置为1.4 ms,翻转角设置为60°,视场(field of view,FOV)设置为370 mm×370 mm,层厚/层距为8 mm/0 mm,并继续进行脉冲序列扫描,基于反转恢复脉冲序列(MOLLI)对比剂注射前后T1 mapping(高分辨率纵向弛豫时间定量成像)扫描获取对比剂注射前后T1 mapping图像,检测心肌组织定量参数成像,调整序列扫描:TR设置为2.5 ms,TE设置为1.2 ms,翻转角设置为60°,FOV设置为307 mm×361 mm,层厚/层距为8 mm/2 mm,采集矩阵设置为256×144。T2平衡稳态自由进动序列(SSFP)T2 mapping(T2 mapping是一种磁共振成像技术,主要用于测量组织的T2弛豫时间)扫描获取T2 mapping图像,检测心脏形态成像,调整序列扫描:TR设置为2.5 ms,TE设置为1.26 ms,翻转角设置为40°,FOV设置为289 mm×361 mm,层厚/层距8 mm/2 mm,采集矩阵设置为256×144。
序列均扫描结束后传至图像后处理工作站,心脏短轴电影图像选择心底至心尖层面,采用系统自动识别及人工未调整干预,计算左心功能相关参数,T1 mapping、T2 mapping图像中确定左室心肌基底段、中央及心尖段手动标记心内外膜范围,并进行测量,计算左心室心肌平均注射对比剂前后15 min T1pre(在增强前T1弛豫时间的测量值)、T2(心肌组织的横向弛豫时间),并计算心肌组织学参数。
(1)对比两组左心功能相关参数。包括左心室射血分数(LVEF)、舒张及收缩末期容积(LVEDV/LVESV)。
(2)对比两组左室心肌活性成像指标。包括左心室心肌平均注射对比剂前T1pre值、注射后15 min T1post值、T2值。
(3)对比两组心肌组织学参数特征。对比不同心肌层面(基底段、中间段、心尖段)的左室心肌平均细胞外间质容积分数(ECV),ECV计算为1与对比剂在血液、心肌细胞外间隙中浓度保持相对平衡状态时的血细胞比容差值与心肌、血液(1/T1pre-1/T1post)比值之比。
将研究对象CMRI数据纳入SPSS23.0软件中分析,以平均值±标准值($\stackrel{-}{x}$±s)表示左心功能相关参数、左室心肌活性成像指标、心肌组织学参数特征等计量资料,两组间比较采用独立样本t检验,P<0.05认为差异具有统计学意义。
表1所示,观察组与对照组(OSAHS患者vs.健康者)在性别、年龄上差异无统计学意义(P>0.05)。
观察组左心功能参数(LVEF、LVED)均明显低于对照组(P<0.05),而LVESV值明显高于对照组(P<0.05),如表2所示。
观察组T1pre、T1post与对照组无统计学差异(P>0.05);而T2明显高于对照组(P<0.05)。如表3所示。
观察组不同心肌层面(基底段、中间段、心尖段)ECV值均与对照组无统计学差异(P>0.05)。如表4所示。
近年来,OSAHS患者合并心脑血管疾病发生率逐渐提高,并被认为是导致心脑血管疾病的危险因素之一,包括心机梗死、肺动脉高压、心脏衰竭、中风甚至认知障碍等,因此临床对于OSAHS患者心脑血管病变的检测也逐渐给予重视[9-10]。心脏作为人体最重要的器官,耗氧量大、代谢率高和氧储备少,所以对于缺氧也很敏感,也是最易损伤的器官,严重缺氧或持续缺氧可使心脏功能及结构发生改变,甚至心肌细胞变性、坏死。由于OSAHS患者慢性间歇性缺氧的存在,因此越来越多的临床学者开始关注OSAHS患者的心脏收缩及舒张功能。
心脏多普勒超声心动图对于心脏的检测已在临床广泛应用,并且具有可重复性强、经济适用、方便快捷等优点,但是随着MRI成像技术的发展,其超高的软组织分辨率、多时相多空间成像的优势使临床能够得到更多的心脏功能信息。由于心脏本身处于节律性收缩及舒张运动,且呼吸状态下心脏会出现上下移动,以准确地对心脏结构和功能进行评估采用常规扫描无法克服心脏及呼吸运动干扰,发生运动伪影概率较高,获取图像质量不高,而CMRI中应用节段性K空间采集技术、心电及脉搏门控等技术,可以有效控制心脏及呼吸运动干扰,为OSAHS合并心脑血管疾病提供了影像学依据,且随着多种成像技术及后处理技术发展,也为患者心脏结构功能、心肌病变提供了影像学支持,因此在临床心脏功能评估中逐渐广泛应用[4,11-12]。此外临床针对MRI评估OSAHS认知脑功能研究报道较多,神经影像学为OSAHS患者合并脑部疾病、神经认知障碍提供了切实影像学依据;临床对OSAHS患者心脏功能影像学证据逐渐给予重视,但是有关心脏功能影像学相关报道相对较少,还有待进一步研究。
OSAHS患者出现心血管病风险远高于健康人群,其发生心功能及心肌改变原因可能为:呼吸受阻引发的心肌缺血缺氧,继而损伤到心脏心肌细胞;同时 OSAHS反复缺氧导致交感神经活性升高,增加肺动脉及主动脉压,继而加重心脏后负荷,引起左心室向心性肥厚,逐渐影响到左心室舒张及收缩,间歇性缺氧引起氧化应激及炎症反应,破坏机体内皮功能机制,加速心室结构破坏[12]。CMRI于20世纪80年代逐渐应用于临床检测,近年来随着临床对于MRI技术及设备的深入研究,CMRI具有诊断、鉴别、评估病情、风险、预后的一站式检查潜力,欢迎体验多模式、多参数、可重复性强、多序列,可以提供心室多方向的功能学、形态学信息优势,在临床测量心脏大血管结构、功能方面广泛应用。心脏电影属于常规序列,可以通过观察CMRI图像小网格线影子,评估心肌运动功能减弱区域[13]。T1 mapping定量技术属于新型检测组织弛豫时间技术,从生理上定量分析直观反应病变部位;T2 mapping技术用于评估心肌水肿、心肌过载效果显著。心脏MRI能可靠地获得心血管的形态、功能、灌注及心肌活性等综合信息,其对血流动力学、心肌代谢方面,以及对冠状动脉磁共振成像的研究也逐渐兴起[14]。当前的研究从解剖和功能逐步深入到细胞和分子水平,已越来越广泛地应用于心血管疾病的诊断、治疗决策和预后评价中,CMRI目前已成为无创性评估心脏结构和功能的“金标准”。结果显示,观察组左心功能参数(LVEF、LVED)均明显低于对照组,而LVESV值明显高于对照组,并且均具有统计学差异(P<0.05),OSAHS患者出现左心收缩功能性下降,其原因可能在于患者呼吸通气不足过程中需要采用体内储存氧气弥补代谢需求,继而导致机体发生一系列反射性作用机制收缩外周血管,增加收缩压,加重左心室后负荷及心肌代偿性肥厚,降低左心室顺应性,阻碍左心室舒张功能,与袁月等[3]研究结果一致。并且观察组T1pre、T1post值(在CMRI中,使用对比剂后的T1 mapping技术所测量的心肌组织的T1值)、基底段、中间段、心尖段ECV与对照组无统计学差异(P>0.05)。说明OSAHS患者发生了心肌水肿或炎性病变。其原因可能为OSAHS患者出现反复心肌缺氧缺血时,持续对细胞膜造成一定伤害,继而导致心肌细胞出现肿胀或坏死,而心肌细胞恢复血运后将会释放大量炎症介质,进一步加重心肌细胞损伤[15-16]
CMRI应用于OSAHS患者中临床效果显著,可有效评估患者的左心功能、结构及心肌早期病变。由于CMRI扫描技术及应用的局限性,首先CMRI多序列扫描时间长,不利于患者检查配合,同时该技术量化分析患者心脏结构及心肌特征还需要更多样本支持。并且未对受检者进行心肌活检的病理学证实,未来需进一步完善研究。
