In order to study the task-based functional connectivity of the whole brain in schizophrenia patients and healthy controls during task switching, the NSP(nested-spectral partition) method was used to quantify the degree of integration and segregation of the whole brain network and explain differential remodeling patterns of brain networks in schizophrenia patients and healthy controls. Results show that changes in whole-brain functional connectivity patterns in schizophrenia are driven by lower network integration and higher segregation, suggesting global and network-wide local information transfer during task switching in schizophrenia patients efficiency was lower than that of healthy controls. Furthermore, the association between task switching costs (reaction time and switching costs) and task state modularity was analyzed using simple linear correlation. Lower whole-brain modularity and lower salience network modularity in schizophrenia were found to be associated with larger switch costs but not with mean reaction times across the task. In contrast, higher cortical network modularity in healthy control patients was associated with faster task reaction times but not with shorter switch costs. Taken together, the results suggest that quantifying functional network configurations across different task states can provide information about cognitive control performance.

In order to study the task-based functional connectivity of the whole brain in schizophrenia patients and healthy controls during task switching, the NSP(nested-spectral partition) method was used to quantify the degree of integration and segregation of the whole brain network and explain differential remodeling patterns of brain networks in schizophrenia patients and healthy controls. Results show that changes in whole-brain functional connectivity patterns in schizophrenia are driven by lower network integration and higher segregation, suggesting global and network-wide local information transfer during task switching in schizophrenia patients efficiency was lower than that of healthy controls. Furthermore, the association between task switching costs (reaction time and switching costs) and task state modularity was analyzed using simple linear correlation. Lower whole-brain modularity and lower salience network modularity in schizophrenia were found to be associated with larger switch costs but not with mean reaction times across the task. In contrast, higher cortical network modularity in healthy control patients was associated with faster task reaction times but not with shorter switch costs. Taken together, the results suggest that quantifying functional network configurations across different task states can provide information about cognitive control performance.

精神分裂症是一种严重的、使人衰弱的精神疾病,以复杂的认知、情感和行为症状为特征,导致个人生活质量显著下降。在全球范围内,精神分裂症是导致残疾的 20 大原因之一,在所有精神疾病中,精神分裂症的社会经济成本最高[1]。精神分裂症以妄想、幻觉和精神错乱为特征,据估计影响着全球 2%~3% 的人口[2]。精神分裂症患者的预期寿命比普通人短约 15 年,主要原因是自杀率高和合并躯体疾病[3]。精神分裂症是一种严重的慢性重度精神障碍,其核心症状之一是认知功能损害。这种损害通常在精神分裂症的典型症状出现之前就已存在,并且很难缓解。认知功能受损常常导致患者在康复和回归社会功能方面表现不佳,严重时甚至会导致精神残疾[4]。精神分裂症以认知灵活性缺陷为主要特征[5]。具体来说,与健康对照组相比,精神分裂症患者在任务转换过程中需要更长的时间才能从上一个任务集中脱离出来,并转向下一个任务集[6]
越来越多的研究将精神分裂症视为一种由大脑不同区域之间的异常互动引起的疾病,最新的人类连接组研究也支持这一观点[7]。例如,许多静息态功能磁共振成像(functional magnetic resonance imaging, fMRI)研究发现,精神分裂症患者的连接模式普遍失调,导致与该疾病相关的大脑网络组织出现明显异常。这些异常包括局部聚类和效率降低,模块化(将大脑网络划分为更小的功能组)的变化[8],以及即使删除特定节点,全局整合性和网络稳健性的增强[9-10]
任务转换范式被认为是迄今为止最精确的认知僵化测量方法。该范式的一个显著优点是,它涵盖了刚性的各个方面,包括实际转换的成本、保持转换准备状态的成本、脱离前一组任务的程度、脱离的速度等[11]。此外,任务切换范式中的表现是多因素的,这意味着不同因素之间可以明确区分。在任务切换范式中,分类规则会显示在屏幕上,即使有也只需要极少的记忆参与,而且无需记录过去的选择。这就确保了刚性测量不受工作记忆和反馈处理的影响[12]。该范式的另一个值得注意的优点是,其指数与不同的神经基质和过程相关联,为认知僵化的内在机制提供了深入的见解[13]。认知灵活性是认知控制的一个基本方面,包括调整自己的行为或认知策略以满足不断变化的环境需求[14]。任务转换表现是认知灵活性的一个重要指标。在任务转换范式中,个体需要在执行两项或多项任务之间随机转换,并在预先提示的指引下完成下一次试验。
采用任务转换范式,为了证实精神分裂症患者在任务转换过程中全脑功能连接的差异,通过嵌套谱划分方法(nested-spectral partition, NSP)量化全脑网络的整合和分离程度,解释了精神分裂症患者和健康对照组大脑网络的不同重塑模式。通过观察精神分裂症患者和健康对照组在任务转换过程中的转换反应时间和转换成本对比,探讨精神分裂症患者在任务切换时全脑功能连接模式的变化特征,以及这种变化如何影响其认知控制表现。
本研究使用的数据集来自加州大学洛杉矶分校神经精神病表型组学联合会(UCLA Consortium for Neuropsychiatric Phenomics, https://openneuro.org/datasets/ds000030/versions/1.0.0)共享神经影像数据库,其中包括 130 名健康受试者和 142 名神经精神病患者(精神分裂症 50 人、躁郁症 49 人和多动症 43 人),年龄在 21~50 岁。数据集包括任务态 fMRI、静息态 fMRI 和结构性 MRI,并提供了受试者记忆和认知控制(反应抑制)方面的认知行为数据。
除磁共振成像扫描外,受试者还完成了神经心理学测试。参与者必须是没有西班牙裔或拉丁裔血统的白人,或任何种族的西班牙裔或拉丁裔,至少受过 8 年教育。对于讲英语或西班牙语的受试者,测试语言由口语流利程度测试决定。受试者在接受测试前还需筛查是否有头部外伤、神经系统疾病、精神药物使用情况、过去 6 个月内的药物依赖情况、主要精神疾病或多动症病史以及当前的情绪或焦虑症。此外,还采用尿液分析法筛查药物滥用情况,如果结果呈阳性,则排除测试。根据加州大学洛杉矶分校机构审查委员会批准的程序,所有参与者都签署了知情同意书。
参与者会看到一系列4种可能刺激中的一种,并被要求根据图像之前和上方的任务提示对刺激做出反应。这4个刺激物包括红色三角形、红色圆形、绿色三角形和绿色圆形。参与者可根据图像的颜色(即红色或绿色)或形状(即三角形或圆形)作出反应。在要求参与者对刺激物的形状做出反应的试验中,提示语包括 “SHAPE ”或 “S”;在要求参与者对刺激物的颜色做出反应的试验中,提示语包括 “COLOR ”或 “C”。在 33% 的试验中,指令发生了转换,参与者被指示从对形状做出反应转换到对颜色做出反应,反之亦然。在 67% 的试验中,指令保持不变,但提示发生了变化。参与者总共完成了 96 次测试,总耗时为 6'52″。所有路径的反应时间(reaction time, RT)都被记录下来,以反映个人表现。
本文中使用的磁共振成像数据是在加州大学洛杉矶分校阿曼森-洛夫莱斯脑图谱中心(西门子版 syngo MRB15)和斯塔格林认知神经科学中心(西门子版 syngo MRB17)的两台 3T 西门子 Trio 扫描仪上采集的。功能磁共振成像数据采用 T 2 *加权回声平面成像序列采集,参数如下:切片厚度为4 mm,34片,重复时间(repetition time,TR)为2 s,回波时间(echotime,TE)=30 ms,翻转角度为90°,矩阵为64×64,扫描的视野宽度(field of view,FOV)=192 mm。此外,还采集了T2加权匹配带宽高分辨率解剖扫描(与fMRI扫描切片相同)和磁化准备快速梯度回波成像(magnetization-prepared rapid acquisition gradient echo,MPRAGE),扫描参数为:切片厚度为4 mm,翻转角为90°,TR/TE=5 000/34 ms,矩阵为128×128。MPRAGE 的扫描参数为 TR=1.9 s,TE=2.26 ms,矩阵为256×256,扫描的视野宽度FOV=250 mm,切片厚度=1 mm,176 片。
在 Ubuntu 14.04 系统中使用 FSL(FMRIB’s software library) (http://www.fmrib.ox.ac.uk/fsl/) 和 AFNI(analysis of functional neuroImages) (http://afni.nimh.nih.gov/afni/) 软件处理任务 fMRI 数据。健康对照组的平均帧向位移(framewise displacement, FD)为 0.160 ± 0.159,精神分裂症组为 0.267 ± 0.215。两组之间的 FD有明显差异[双样本 t 检验,t(98) = 2.779,P=0.004],并进行了组间比较的协方差分析。以中间切片为参照基准进行切片时间校正。采用标准的蒙特利尔神经研究所统一分割法对数据进行归一化处理,并使用半最大值全宽为6 mm的高斯核进行空间平滑处理。fMRI 信号在 0.01~0.1 Hz的带通频率范围内进行滤波。由于全脑信号中可能包含与精神分裂症症状相关的临床信息,因此在分析中没有剔除全脑信号。
根据 Power 图集全脑被划分为 264 个感兴趣区。这种网络分区已被证明可为脑网络特性提供更高的测试再测可靠性。Cole 等[15]将最初的 264 个感兴趣区域(regions of interests, ROIs) 划分到 10 个不同的脑网络中。根据 Cole 等[15]的研究,为这 264 个 ROI 中的每一个定义了一个半径为 6 mm的球体。
根据 Schaefer 图谱71 将大脑划分为N=246 个脑区和 10 个脑功能系统。为了获得区域 fMRI 时间序列,对每个区域内的体素的血氧水平依赖性(blood oxygen level-dependent, BOLD)信号进行平均。然后利用皮尔逊相关系数估算区域间的功能连通性(functional connectivity, FC)。利用滑动窗口法将 BOLD 信号分割成片段,并计算每个窗口内的时间动态功能连通性(FC)。根据参考文献[16]的建议,选择的窗口宽度为 60 s(30 个点),滑动步长为 2 s(1个点),因此总共有 132 个窗口。这种方法可以捕捉到 FC 随时间的动态变化。这包括汇总每组所有参与者的 fMRI 时间序列,并在更长的时间尺度上计算 FC。利用每个参与者的整个 fMRI 时间序列,分别构建了单个静态 FC 网络。这种将时间动态 FC 与群体稳定和个体静态 FC 网络相结合的方法有助于捕捉 fMRI 数据中的时变动态和整体连接模式能全面了解大脑在不同时间尺度和个体差异下的功能连通性。
为了评估分离和整合,首先采用基于特征值的嵌套谱划分方法(nested-spectral partition, NSP)检测大脑功能连接(FC)网络中的分层模块。该方法遵循一套程序来识别这些模块。
(1)在第一功能模式中,所有区域具有相同的负或正特征向量值;该模式被视为第一水平,具有一个模块(即全脑网络)。
(2)在第二种功能模式中,特征向量为正的区域被分配到一个模块,而特征向量为负的区域被分配到另一个模块。这种模式代表第二级,包括两个不同的模块。
(3)在随后的步骤中,根据第三种模式中特征向量的负号或正号,将第二层中的每个模块进一步细分为两个子模块。这种迭代过程一直在继续,随着功能模式顺序的增加,FC 网络被模块化地划分为多个层次。这种划分一直持续到达到一个特定水平,即每个模块只由一个区域组成。
分区过程完成后,NSP 方法提供了模块数量Mi(i=1,2,…,N)和模块大小mj(j=1,2,…,Mi),代表每个模块在每个层次上的区域数量。
在分层模块化结构中,每一层支持了模块内部的整合及模块之间的分离,并且随着阶次的增加,模块数量逐渐增加,支持了更高的分离过程;再者,随着阶次的增加,伴随着对功能连接网络贡献度的下降;尤其是具有最高模块数量的层数支持了完全的分离过程,具有较小的贡献度;功能连接网络第i层的分离与整合成分定义为

式(1)中:N 用于将模块数 M 标准化在(0,1] 范围内;pi= j m j - N / M i/N代表模块尺寸校正系数,用于计算第 i 层模块尺寸与优化模块尺寸的偏差; Λ i 2为静态功能连接网络第i层的贡献度。
根据 NSP 方法检测到的分层模块,对大脑 FC 网络中的功能分离和整合进行了表征。第一级由整个 FC 网络的单个模块组成,在全局整合中起着至关重要的作用,表达式为

随着功能模式顺序的增加,后面各层次由更多规模较小的模块组成,从而促进更高层次的分离。因此,分离成分的计算方法是将第二级到第 N 级的结果相加,即

因此,对于单个 FC 网络,得出了两个分量:整合分量和分离分量。分量值越高,表明分离程度越高,而分量值越低,表明整合程度越低。
进一步定义每个区域对整合和分离分量的贡献为

以第 i 个功能模式 j = 1 N U i j 2=1为衡量标准,通过对系统内各区域的相应分量进行平均,确定功能系统的整合和分离程度。
对于动态 FC 网络,计算了每个个体在每个时间窗口的时间分辨分离分量HSe(t)和整合分量HIn(t)。将整合和分离强度分别定义为HSeHIn随时间变化的平均值。此外,采用以下方法计算整合和分离变异值,即

整合和分离变异值分别用 H I n j(t)和 H S e j(t) 时间序列的标准偏差 σ H j I n σ H j S e计算得出。
由于较短的fMRI序列导致明显较高的网络隔离,利用比例校准策略来获得fMRI长度无关的网络测量。假设每组内稳态FC 网络的整合成分记为 H I n S,所有参与者的单个静态 FC 网络的整合成分记为HIn=[HIn(1),HIn(2),…,HIn(n)]。每个个体的校准结果计算公式为

式(6)中:<> 代表所有参与者的群体平均值;n 代表参与者个人。对区域整合分量 H I n j进行校准。对于第 n 位参与者的第 j 个脑区,校准后的脑区整合分量表示为 [ H I n j= H I n j/HIn(n)]×H'In(n),确保每个脑区对网络整合的相对贡献保持一致。
在动态 FC 网络的情况下,采用了校准方法,以确保每个个体的时间整合分量,调整为与其静态整合分量相匹配。代表个体在所有时间窗口的动态整合分量的向量表示为hIn=[ h I n 1, h I n 2,…, h I n 132]。校准后的结果,表示为ht'In= h I n tHi'In/<hIn>,其中<·>表示所有时间窗口的平均值。
校准程序同样适用于静态和动态网络中全脑和局部尺度的分离部分。这些校准步骤在每个组内分别进行。
构建静态脑FC网络,并利用嵌套谱分割方法分别计算了FC网络的整合度和分离度的变异性。定义了时间分辨整合和分离成分的标准偏差,以研究动态整合变异性和分离变异性。精神分裂症患者全脑的整合变异性显著降低[t(98)=2.436, P=0.017,如图1(a)所示],分离变异性显著升高[t(98)=-3.664, P<0.001,如图1(b)所示]。在局部尺度上,整合变异性无明显变化,分离变异性减少的区域主要位于默认模式系统和视觉系统[如图1(c)图1(d) 中颜色条代表两组之间的差异]。而将大脑划分为7个功能系统和皮层下区域:视觉系统(VIS)、 运动系统(MOT)、背侧注意系统(DOR)、突显系统(SAL)、边缘系统(LIM)、控制系统(CON)、默认系统(DMN),而皮层下区域包括海马(Hipp_SC)、基底节(BG_SC)和丘脑(Tha_SC)。此外,在所有测量中,边缘系统和显著系统的变化最大。边缘系统的整合变异性最大,突出系统的分离变异性最大[P < 0.001,如图1(e)图1(f)所示]。因此,在精神分裂症患者中,大脑网络向更高分离度的偏离主要受边缘系统和突显系统的调节。
在任务转换实验中,通过记录反应时间(reaction time, RT)来测量个体的认知能力。反应时间反映了执行任务的速度,研究精神分裂症患者大脑网络与反应时间之间关联的改变。精神分裂症患者的RT明显长于健康对照组[图2(a)]。
在精神分裂症组中,整合与RT没有脑区显著相关[图2(b)],分离与RT有3个脑区显著相关,主要分布在背侧注意系统、视觉系统和丘脑皮层下系统[图2(c)]。 在精神分裂症组中,RT与全脑(ALL)和10个功能系统的整合和分离无明显相关性[图2(d)]。在健康组中,整合与RT 显著相关的 19 个脑区[图2(e)]主要分布在皮层下系统,位于基底节和丘脑;分离与RT 显著相关的 5 个脑区[见图2(f)]主要分布在躯体运动系统。RT 与位于丘脑的皮层下系统的整合呈负相关性[r=-0.291,P=0.040,图2(g)所示]。
总体而言,从精神分裂症组到健康组,位于基底节和丘脑的皮层下系统的整合和分离与反应时间的相关性发生了变化。
长切换成本(long costs, LC),表现为反应时滞较长、准确率较低。 长切换成本表示长延迟条件下任务切换所表现出的成本。可以评估切换任务时长延迟对注意力资源分配的影响。精神分裂症患者LC增加,但差异不显著[t(97)=-1.463,P=0.147,图3(a)所示],表明认知功能明显异常,认知任务切换反应较慢。
在精神分裂症组中,整合与LC有13个脑区显著相关,主要分布在运动系统、背侧注意系统和突显系统[图3(b)];分离与LC56个脑区显著相关,主要是在显著系统中[图3(c)]。 在突显系统(r=0.355,P=0.012)、位于海马的皮质下系统(r=0.291,P=0.043)和位于基底节的皮质下系统[r= 0.320,P = 0.025,如图3(d)所示]。
在健康组中,整合与LC显著相关的有63个脑区[图3(e)],分离与LC显著相关的有26个脑区均分布在视觉系统中[图3(f)]。 具体来说,在视觉系统(r=0.306,P=0.030)和背侧注意系统(r=0.315,P=0.026)中,LC与整合呈正相关,而在视觉系统中,分离呈负相关[r= -0.282,P=0.047,如图3(g)所示]。
总体而言,在精神分裂症组中,全脑和10个功能系统中的分离与LC呈正相关,分离强度越高对应于较长的转换成本。 健康组的整合度与全脑和8个功能系统中的LC呈正相关,整合度越高,对应的长转换成本越长。
短切换成本(short costs, SC)表现为反应时延更短、准确度更高。 短切换成本表示短延迟条件下任务切换所表现出的成本。可以评估切换任务时短延迟对注意力资源分配的影响。在短转换成本中也发现了类似的结果。患者SC增加,但差异不显著[t(97)=-0.722,P=0.472,如图4(a)所示],表明认知功能明显异常,认知任务切换反应较慢。
在精神分裂症组中,整合与SC有59个脑区显着相关,主要集中在运动系统[图4(b)];分离与SC有42个脑区显著相关,主要是在突显系统中[图4(c)]。 具体来说,SC与运动系统(r=-0.283,P=0.049)、突显系统(r=-0.309,P = 0.031)、边缘系统(r=-0.318,P = 0.026),默认模式网络(r=-0.302,P=0.035)的整合呈负相关,并且与突显系统的分离呈正相关[r=0.346,P=0.015,如图4(d)所示]。
在健康组中,整合与SC显著相关的2个脑区是边缘系统和位于丘脑的皮层下核系统[图4(e)]。 分离与SC显著相关的一个大脑区域是位于基底神经节的皮质下系统[图4(f)]。SC与健康组中全脑(ALL)和10个功能系统的整合和分离没有显著相关[图4(g)]。
在短期转换成本的精神分裂症组中,结果更为明显。 在精神分裂症组中,分离强度与全脑及10个功能系统中的相关性呈正相关,且分离强度越高,短切换成本越长。
引入基于特征值的嵌套谱分区方法,用于研究在任务切换任务中精神分裂症患者和健康对照组的大脑功能网络的分层分离和整合,以及将网络特征与反应时间以及任务切换联系起来;接下来,揭示了分离和整合之间的平衡与反应时间以及任务切换之间的联系;得出如下结论。
(1)与健康对照组相比,精神分裂症患者的大脑FC网络更加分离,在任务转换任务中,精神分裂症患者的功能连接网络转向分离性增强,而显著的变化主要发生在边缘系统和突显系统。说明精神分裂症患者全脑功能连接模式受到不同程度的重塑,导致任务切换时的信息传输效率低于健康对照组。
(2)在需要快速反应的情境中,精神分裂症患者的表现更为明显。任务切换范式下精神分裂症患者的反应时长于健康对照组。与精神分裂症患者在执行功能任务时出现的脑激活差异有关。精神分裂症患者的脑网络改变主要集中于特定脑功能网络(如边缘网络、执行网络及突显网络)的功能连接异常。
(3)精神分裂症患者长切换成本与全脑和8个功能系统中的分离呈正相关。正常大脑处于分离与整合的平衡状态,并且在面对认知任务时,大脑能够动态地重新配置其网络结构,以适应当前的认知需求。分离度高的大脑更能够有效地处理需要跨网络协调的任务时,可能导致较长的转换成本。
(4)精神分裂症患者的分离强度与短切换成本呈正相关,即分离程度越高,短切换成本越长。表明在任务切换过程中,精神分裂症患者的大脑网络中分离程度的增加可能导致执行任务所需的时间延长,进而影响切换成本。反映了精神分裂症患者在处理任务切换时面临的认知控制困难,导致其在任务执行方面表现出更长的反应时间。
总之,揭示了精神分裂症患者在任务切换过程中大脑功能网络的特殊重塑模式。精神分裂症患者表现出更高的分离度,这可能导致在任务执行时的信息传输效率下降。特别是在需要快速反应的情境下,精神分裂症患者的表现更为突出,其反应时间延长,可能与特定脑功能网络的异常连接有关。这些发现有助于更深入地理解精神分裂症患者在认知控制方面的困难,为未来的干预和治疗提供了新的视角。
