Essential hypertension, as a common chronic cardiovascular syndrome, shares inherent similarities with rheumatic immune diseases in terms of its multi−system involvement and complex pathological mechanisms. Academician Tong Xiaolin's "State−Target Medicine" provides a precise paradigm for integrated Chinese and Western medicine, emphasizing "macro−level state regulation and micro−level target intervention". Professor Jiang Quan's theory of "Body−Viscera Combined Obstruction" profoundly reveals the patterns of disease transmission and mutual damage between the "body" external physical forms, meridians, and blood vessels and the "viscera" internal organ systems. This article introduces this theory into the prevention and treatment system of EH within the State−Target Medicine framework, systematically explaining its pathogenesis and new diagnostic and therapeutic approaches. It is proposed that the core pathogenesis of EH lies in "meridian obstruction" as the form and "disharmony between the body and viscera" as the root. The key pathological factors involve abnormalities in the metabolism and distribution of qi, blood, water, and fire, forming core pathological states such as "congestion", "water", "stagnation", "cold", and "aging", which persist throughout the entire disease progression of "affecting qi and blood − affecting meridians − affecting viscera". In terms of treatment, the general principle of "harmonizing and balancing" is followed, with "simultaneous treatment of the body and viscera" as the guiding principle. At the macro level, the state is regulated by harmonizing qi and blood and dredging meridians; at the micro level, "target drugs" with proven antihypertensive and vascular function−improving effects are selected based on modern pharmacology. For example, for the "congestion state" characterized by gastrointestinal excess heat and turbid qi ascending, Houpu Sanwu Decoction is used to unblock the fu organs and dispel turbidity, combined with Cassia seed and hawthorn; for the "water state" characterized by internal retention of water−dampness, Danggui Shaoyao Powder is used to strengthen the spleen and promote diuresis, combined with Poria and Leonurus; for the "stagnation state" characterized by liver qi stagnation and transformed fire flaming upward, Sini Powder is used to disperse and relieve stagnation, combined with Prunella and Uncaria; for the "cold state" characterized by cold congealing in the meridians and contraction and spasm, Gegen Decoction is used to dispel cold and unblock collaterals, combined with Pueraria and cinnamon twigs; for the "aging state" characterized by kidney essence deficiency and meridian hardening, Duhuo Jisheng Decoction is used to tonify and unblock collaterals, combined with Eucommia and Achyranthes. By constructing a state−target differentiation and treatment system for EH based on the "Body−Viscera Combined Obstruction" theory, this article provides a theoretical basis and practical path for enhancing the systematic and precise prevention and treatment of hypertension with traditional Chinese medicine.

原发性高血压(essential hypertension,EH)作为一种常见的慢性心血管综合征,其多系统受累、病理机制复杂的特点与风湿免疫病具有内在相通性。在态靶医学框架下,将体脏合痹学说引入EH防治体系,系统阐释其病机与诊疗新思路。研究认为,EH的核心病机在于脉络痹阻为形、体脏失和为根,病理关键在于气、血、水、火的代谢与输布失常,形成壅态、水态、郁态、寒态、老态等核心病态,并贯穿于“病气血—病脉络—病脏腑”的疾病全程。治疗上遵循“和正调衡”总则,以体脏同治为纲,宏观上通过调畅气血、疏通脉络以平态,微观上结合现代药理选用具有明确降压、改善血管功能作用的靶药以击靶。例如,针对壅态之胃肠实热、浊气上冲,用厚朴三物汤通腑泄浊,配伍决明子、山楂;水态之水湿内停、泛溢脉络,用当归芍药散健脾利水,配伍茯苓、茺蔚子;郁态之肝气郁结、化火上炎,用四逆散透达解郁,配伍夏枯草、钩藤;寒态之寒凝经脉、收引挛急,用葛根汤散寒通络,配伍葛根、桂枝;老态之肾精亏虚、脉络硬化,用独活寄生汤补益通络,配伍盐杜仲、怀牛膝。通过构建EH体脏合痹的态靶辨治体系,为提升中医药防治高血压的系统性与精准性提供了理论依据与实践路径。

原发性高血压(essential hypertension,EH)作为一种常见的慢性心血管综合征,其多系统受累、病理机制复杂的特点与风湿免疫病具有内在相通性。在态靶医学框架下,将体脏合痹学说引入EH防治体系,系统阐释其病机与诊疗新思路。研究认为,EH的核心病机在于脉络痹阻为形、体脏失和为根,病理关键在于气、血、水、火的代谢与输布失常,形成壅态、水态、郁态、寒态、老态等核心病态,并贯穿于“病气血—病脉络—病脏腑”的疾病全程。治疗上遵循“和正调衡”总则,以体脏同治为纲,宏观上通过调畅气血、疏通脉络以平态,微观上结合现代药理选用具有明确降压、改善血管功能作用的靶药以击靶。例如,针对壅态之胃肠实热、浊气上冲,用厚朴三物汤通腑泄浊,配伍决明子、山楂;水态之水湿内停、泛溢脉络,用当归芍药散健脾利水,配伍茯苓、茺蔚子;郁态之肝气郁结、化火上炎,用四逆散透达解郁,配伍夏枯草、钩藤;寒态之寒凝经脉、收引挛急,用葛根汤散寒通络,配伍葛根、桂枝;老态之肾精亏虚、脉络硬化,用独活寄生汤补益通络,配伍盐杜仲、怀牛膝。通过构建EH体脏合痹的态靶辨治体系,为提升中医药防治高血压的系统性与精准性提供了理论依据与实践路径。

Essential hypertension, as a common chronic cardiovascular syndrome, shares inherent similarities with rheumatic immune diseases in terms of its multi−system involvement and complex pathological mechanisms. Academician Tong Xiaolin's "State−Target Medicine" provides a precise paradigm for integrated Chinese and Western medicine, emphasizing "macro−level state regulation and micro−level target intervention". Professor Jiang Quan's theory of "Body−Viscera Combined Obstruction" profoundly reveals the patterns of disease transmission and mutual damage between the "body" external physical forms, meridians, and blood vessels and the "viscera" internal organ systems. This article introduces this theory into the prevention and treatment system of EH within the State−Target Medicine framework, systematically explaining its pathogenesis and new diagnostic and therapeutic approaches. It is proposed that the core pathogenesis of EH lies in "meridian obstruction" as the form and "disharmony between the body and viscera" as the root. The key pathological factors involve abnormalities in the metabolism and distribution of qi, blood, water, and fire, forming core pathological states such as "congestion", "water", "stagnation", "cold", and "aging", which persist throughout the entire disease progression of "affecting qi and blood − affecting meridians − affecting viscera". In terms of treatment, the general principle of "harmonizing and balancing" is followed, with "simultaneous treatment of the body and viscera" as the guiding principle. At the macro level, the state is regulated by harmonizing qi and blood and dredging meridians; at the micro level, "target drugs" with proven antihypertensive and vascular function−improving effects are selected based on modern pharmacology. For example, for the "congestion state" characterized by gastrointestinal excess heat and turbid qi ascending, Houpu Sanwu Decoction is used to unblock the fu organs and dispel turbidity, combined with Cassia seed and hawthorn; for the "water state" characterized by internal retention of water−dampness, Danggui Shaoyao Powder is used to strengthen the spleen and promote diuresis, combined with Poria and Leonurus; for the "stagnation state" characterized by liver qi stagnation and transformed fire flaming upward, Sini Powder is used to disperse and relieve stagnation, combined with Prunella and Uncaria; for the "cold state" characterized by cold congealing in the meridians and contraction and spasm, Gegen Decoction is used to dispel cold and unblock collaterals, combined with Pueraria and cinnamon twigs; for the "aging state" characterized by kidney essence deficiency and meridian hardening, Duhuo Jisheng Decoction is used to tonify and unblock collaterals, combined with Eucommia and Achyranthes. By constructing a state−target differentiation and treatment system for EH based on the "Body−Viscera Combined Obstruction" theory, this article provides a theoretical basis and practical path for enhancing the systematic and precise prevention and treatment of hypertension with traditional Chinese medicine.

原发性高血压(essential hypertension,EH)是心脑血管事件最重要的危险因素,其发病机制涉及神经、内分泌、代谢、血管等多系统的复杂交互网络[1]。中医学将其归于“眩晕”“头痛”“肝风”等范畴,传统辨治多从肝、肾、脾三脏功能失调立论[23]。根据临床数据可将高血压分为5大证型,分别为阴阳两虚证、肝肾阴虚证、肝火亢盛证、气虚痰浊证、瘀热互结证,其中肝肾阴虚证和肝火亢盛证临床居多[4]。但从六经辨证来看,临床高血压患者少阳和太阳亦多见[5],部分高血压,尤其是急性、波动性、与外感相关者的深层病机在于表闭,太阳外开不利,阳明内阖不利,少阳枢机不利等[6],治疗上不应见血压高就一味镇肝潜阳,而应开鬼门、洁净府,对于邪气初犯人体,邪在三阳者,则重在“给邪以出路”,以汗、吐、下因势利导,遵循祛邪为第一要务[7],通过解除表邪来恢复人体正常的气血循环,从而达到降低血压的根本目的。《黄帝内经》指出痹病核心病机为风寒湿邪杂合而至、痹阻经脉气血,并具有由表入里、由浅入深、逐渐加重的传变趋势[8],姜泉团队在此经典理论基础上,结合现代风湿免疫病及多种复杂性疾病多系统受累的特点,系统深化并提出了体脏合痹学说[9]。这种由体及脏、脏体同病的传变规律,与《内经》“五体痹”传变为“五脏痹”的理论一脉相承,体脏合痹学说不仅继承了五体合五脏的经典理论框架,更强调“体”与“脏”通过经络络属、气血津液环流构成一个不可分割的有机整体,并系统阐释了体痹及脏、脏痹及体、体脏合痹的动态传变规律与防治一体化诊疗思路,从而将这一理论从单纯的病机传变提升为更具临床指导意义的系统性学说。与此同时,仝小林创立的 “态靶医学”理论[10],主张将中医对疾病整体病理状态的把握与现代医学对特异性指标及靶点的干预相结合,实现了“病−症−期−态−靶”的立体化诊疗,为中西医深度融合树立了典范。
当前,传统医药正逐渐受到越来越多的关注,要实现高质量发展,并真正走向世界,为全人类健康服务,就必须实现科学化、标准化与规范化[11]。本文将体脏合痹学说置于态靶医学的框架下,系统阐述其在EH中的病机内涵、核心病态及体脏同治、态靶结合的诊疗策略,旨在为EH的中医药防治开辟一条立足整体、精准干预的新路径。
《中医临床诊疗术语·证候部分》将EH命名为“风眩”,其证型可归为肝阳上亢、痰湿中阻、瘀血阻窍、气血亏虚及肾精不足等[12]。在体脏合痹理论视角下,筋骨肌肉之体与肝肾心脾之脏通过经络气血相互维系。高血压的进一步发展可导致血管及心、脑等靶器官损伤,此为体痹;继而影响脏腑功能,形成脏痹;二者交互作用,终成体脏合痹之势。
体痹泛指皮、肌、脉、筋、骨五体的痹阻。从EH的病理生理看,外周血管阻力的增加是血压升高的直接力学基础,这与中医脉络痹阻导致气血运行不畅的病机认识高度一致。《素问·痹论》云:“风寒湿三气杂至,合而为痹也。”《说文解字》解释为“痹,湿病也。”湿,《说文解字》进一步解释为“湿,幽湿也。从水,一所以覆也。覆土而有水,故湿也”。湿,不仅仅有水湿之意,还有迟缓、不通畅的意思。故此处的痹不仅指风湿病,更广义地指代邪气阻滞经脉、气血不通的病理状态。当风、寒、湿等邪气乘经脉之虚客入五体,导致脉络壅滞阻闭时,机体为了保障心、脑、肾等重要器官的血液供应,会本能地提高循环系统压力,从而导致血压升高[13]。从病生理机制看,高血压可视为机体为克服外周血管阻力增高而做出的代偿性反应。这一推论在临床可得到印证:使用麻黄、桂枝等解表药或大黄、厚朴等通腑药后,随着表气通、腑气降,患者血压常随之下降,这反向证明了痹阻解除后,机体无需再维持高压力状态。
从六经传变与体痹的关系来看,高血压初起多表现为气机逆乱。三阳经病多为阳气周流障碍的病理反应,三阴经病则多与本体不足相关。太阳主表,邪在太阳则玄府闭塞,与皮痹相合;少阳为枢,枢机不利则阳气出入失序,与筋痹相合;阳明为阖,阖降失常则浊气上逆,与肉痹相合。若失治误治,邪气可由表入里。例如,太阳传阳明,外感寒邪郁而化热,形成胃家实,气逆于上,可致血压升高;少阳传厥阴,郁而化火,风火相煽,可致肝阳上亢,血压波动。这一传变过程清晰地展示了体痹如何逐步影响气血运行,亦外周血管及经络功能失调,并最终推动EH的发生与发展。
脏痹是疾病深入脏腑、病情深重阶段的体现,其本质是体痹不愈,病邪循经络内传于相应脏腑,导致脏腑功能失调。五脏痹理论肇始于《黄帝内经》,其中心、肝、脾、肺、肾五脏的痹阻证候,与EH晚期的多器官功能损害有高度的对应关系[14]
肝痹证候表现为胁痛、筋挛、夜卧多惊,这与肝阳上亢、肝风内动及气机郁结相关,也与EH患者常伴有的焦虑、失眠、多梦等症状相似。心痹以心悸、胸闷、心下鼓痛为主要表现,此为脉络瘀阻,病邪内舍于心之象,是EH最常见、最直接的靶器官损害。脾痹见四肢懈惰、脘腹胀满、呕恶纳呆,提示痰湿内盛,痹阻经络。痰湿阻滞脉道,使其僵硬、阻力增加,为EH提供了重要的病理产物和体质基础。肺痹以咳逆上气、喘息烦满为主,与肺通调水道功能失司相关,可见于EH并发心力衰竭之时。肾痹表现为腰痛骨重、屈伸不利,甚至水肿、遗尿,揭示了EH晚期的肾损害。由此可见,EH并非仅仅是肝阳上亢,而是全身性气血津液运行失调、多脏腑功能痹阻的最终结果。从五脏痹的视角看,EH可被视为一种以肝痹为先导、以肾痹为根基、以心痹为归趋,常兼夹脾痹之痰湿,并可累及肺痹的全身性、系统性的脉络—五脏痹。
体脏合痹学说强调体与脏之间互为因果、动态传变的恶性循环关系,并由此衍生出“以体养脏、调脏安体、体脏同治”的动态诊疗策略[9]。这一学说与EH的病理机制高度吻合。现代医学认为,EH涉及肾素−血管紧张素−醛固酮(renin−angiotensin−aldosterone,RAAS)系统激活、交感神经兴奋及血管内皮功能障碍等多个环节[15]。从体脏合痹视角看,RAAS系统激活导致的血管收缩与水钠潴留,可对应脉络挛急与水湿内停的体痹范畴;而长期高血压导致的心血管重构与靶器官损害,则属于脉络僵硬乃至脏腑受损的脏痹范畴。
基于上述分析,EH的体脏合痹病机可概括为:生理状态下,五脏藏精化气,通过经络系统濡养五体,维持正常血脉运行。高血压的始动因素,如风寒外袭、长期精神紧张、饮食不节及年老体衰等,首伤于体。气机不畅则血行滞涩,形成气滞血瘀,痹阻脉络;脾失健运则痰浊内生,壅塞脉道,此阶段为体痹。若体痹不除,邪气藏伏于脉络,形成伏邪。当脏腑气血亏虚,伏邪沿络脉内传于脏时,则形成脏痹。瘀血痹阻肝络,则肝失条达,阳亢化风,发为眩晕;痰浊痹阻心脉,则发为胸闷心悸;久病及肾,肾络痹阻,则加剧水钠代谢紊乱与血压升高。此即体痹及脏。反之,肝阳亢逆可加重脉络紧张,肾阴亏虚可致脉络失养而脆性增加,此即脏痹及体。最终形成体脏合痹的恶性循环,其核心环节在于脉络痹阻,这与现代医学的血管内皮功能障碍、外周血管阻力增高及动脉硬化等病理改变深度契合。
在EH发病过程中,多由外感内伤等因素引发体痹为先。外界之风、寒、湿邪侵犯人体,可分别导致皮、肌、脉、筋、骨五体痹。随着病邪深入,体痹可内传于相应之脏。具体而言,风寒郁闭肌表,卫气不得宣泄,内郁化热,导致气血上冲,发为表闭阳郁型EH,此为皮痹传肺;寒湿困阻肌肉,脾运不健,水湿内停,发为湿阻肌腠型EH,此为肌痹传脾;寒湿客于脉中,血行涩滞,发为营卫虚弱型或寒湿阻络型EH,此为脉痹传心;寒邪客于筋脉,拘急收引,气血不通,发为寒凝筋脉型EH,此为筋痹传肝;寒湿深侵于骨,肾阳不足,温煦失职,发为阳虚寒凝型EH,此为骨痹传肾。
因此,体脏合痹论治EH的意义在于体系的建立。《素问·痹论篇》云:“逆其气则病,从其气则愈,不与风寒湿气合,故不为痹。”对于EH多系统、多靶点的损害,治疗上需双管齐下,首先基于体痹特点,强调通其络、给邪以出路;其次,基于脏痹特点,强调顺其气、衡脏腑。通过恢复机体气血运行的稳态,达到缓解疾病之目的。
随着EH进入病脉络乃至病脏腑阶段,脏痹一旦形成,会反过来加剧体痹,形成恶性循环。肝阳亢逆是典型的“脏痹及体”表现,长期情志不遂致肝气郁结,化火灼伤肝阴,肝阳上亢形成脏痹;亢阳化风上扰清窍,表现为眩晕、头痛加剧,同时阳亢加剧气血上冲,使脉络更加紧张挛急,表现为血压波动性升高。肾阴亏虚亦是如此,久病及肾,肾精亏耗,阴液无法濡养筋脉,导致血管壁失去弹性、僵硬度升高,表现为脉痹与骨痹,如动脉硬化,进一步推高外周血管阻力。心阳不振见于病程后期,邪气内舍于心,心阳不振,血行无力,导致脉络中气血运行迟滞,瘀血、痰浊进一步痹阻脉络,表现为胸闷、心悸,甚至水肿、心衰,与血压控制不佳、夜间阵发性呼吸困难等临床表现相符。
综上,EH的传变规律完整呈现了“早期为体痹及脏—中期为体脏同病—晚期为脏痹为主、体脏合痹”的动态过程。
态靶辨治理论认为疾病是多因素、多层次的动态过程,主张将“态”与“靶”结合,以动态发展的疾病状态和明确的疾病靶点为基础,制定个体化的诊疗方案,以提高患者的治疗效果和生存质量[16]。仝小林提出EH的“三期五态”对于其防治意义非凡,其中“三期”即“病气血、病脉络、病脏腑”,病气血期以功能性改变为主,对应“体痹”早期,病机为气机郁滞、血脉挛急,如肝郁气滞、寒凝经脉等,尚未造成实质性脏器损害,治疗重在“调气血以安体”。病脉络期以结构性改变为主,对应“体脏同病”阶段,病理产物,如痰、瘀、浊等痹阻脉络,导致脉道失柔、僵硬化,并开始影响脏腑功能,治疗需“通脉络、和体脏”。病脏腑期以器质性损害为主,对应“脏痹”及“体脏合痹”阶段,心、脑、肾等靶器官受损,体脏之间形成恶性循环,治疗重在“补脏腑、调体脏”,延缓靶器官损害。病理特点分别为脉挛急、脉僵硬和脏腑损,与体脏合痹思想异曲同工。“五态”即壅态、水态、郁态、寒态、老态。壅以堵塞言,水以液体言,郁以情志言,寒以紧凝言,老以脉络病变言[11]。壅态为胃肠壅滞,浊痹体络,病机为过食肥甘,脾胃运化不及,浊邪壅滞胃肠,上冲痹阻清窍脉络。宏观调态以通腑泄浊,宣通脉络为主,靶方为厚朴三物汤。水态为水湿内停,泛溢体络,病机为脾肾失调,水液代谢障碍,水湿泛溢,浸淫脉络。宏观调态以健脾利水,化饮通络为主,靶方为当归芍药散/五苓散。郁态为肝气郁结,火扰体络,病机为情志不舒,肝失条达,气郁化火,上扰清空,痹阻脉络。宏观调态以透达郁结,清肝安络为主,靶方为四逆散。寒态为寒凝经脉,收引体络,病机为风寒外袭或阳虚内寒,导致经脉拘挛,气血不通。宏观调态以散寒通络,解痉舒脉为主,靶方为葛根汤/桂枝加葛根汤。老态为肾精亏虚,瘀损体脏,病机为年老久病,肾精亏耗,痰瘀互结,深伏于络,损及体脏。宏观调态以补益肝肾,消癥通络为主,靶方为独活寄生汤。在宏观“调态”以恢复体脏平衡的基础上,精准选用“靶药”以实现微观“打靶”,从而实现调脏安体,顾态打靶(表1)。
态靶结合、体脏同治的模式,尤其适用于EH及其多种并发症的长期综合管理,体现了中医药在复杂慢性病防治中“系统调控与精准打击”并重的独特优势。态靶结合,体脏同治是不同的2个维度,对于疾病而言,态靶结合是空间上宏观与微观的并重,而体脏同治是时间上动态传变的把握;对于人体而言,态是健康的整体状态,靶是当下的核心矛盾,脏是正气的根基,体是气血的通道。本文通过将体脏合痹学说与态靶医学理论相融合,构建了EH的态靶辨治新体系,为提升中医药防治高血压的系统性与精准性提供了坚实的理论依据与实践路径。
