OBJECTIVE To investigate the initial and subsequent treatment strategies for NSCLC with concurrent EGFR mutations and PD-L1 expression, and to summarize the diagnostic and therapeutic approaches so as to provide references for treatment decisions for such patients.
METHODS The treatment plan of a patient with NSCLC harboring EGFR mutations and PD-L1 expression was analyzed.
RESULTS The patient was diagnosed with adenocarcinoma of the right lung with EGFR mutations and expression of PD-L1.The initial treatment plan was osimertinib combined with pemetrexed and carboplatin, and the patient ultimately achieved a progression-free survival (PFS) of 39 months from September 2020 to December 2023. On December 27, 2023, re-examination revealed chronic disease progression, and the regimen was adjusted to osimertinib combined with bevacizumab. On March 12, 2024, the patient experienced disease progression, with no significant changes in genetic testing. Chemotherapy was considered for reinitiation, and we selected the treatment regimen of "pemetrexed + carboplatin + sintilimab + anlotinib". After one cycle, a follow-up CT scan demonstrated a partial response (PR), and PR was maintained after the second cycle.
CONCLUSION This study reports a case of advanced NSCLC with EGFR mutations and PD-L1 expression. In the treatment of this patient, an individualized diagnosis and treatment plan was formulated through literature review and relevant genetic testing. By combining immunotherapy, chemotherapy and anti-angiogenic drugs, the patient's disease was effectiveI resistance are complex and vary based on individual patient differences and tumor locations. After disease progression, re-genetic testing may clarify the resistance mechanisms and guide clinical treatment to achieve better efficacy and precision treatment.

目的 探讨EGFR突变合并PD-L1表达NSCLC的初始及后续治疗方案，总结诊疗思路，以期为该类患者的治疗决策提供参考。
方法 对1例EGFR突变合并PD-L1表达NSCLC患者的治疗方案进行分析。
结果 患者诊断为右肺腺癌合并EGFR突变和PD-L1表达，初始治疗方案为奥希替尼联合培美曲塞和卡铂，患者最终获得了自2020年9月~2023年12月长达39个月的PFS（无进展生存时间）。2023年12月27日复查评估慢性进展，方案调整为奥希替尼联合贝伐珠单抗。2024年3月12日患者出现疾病进展，基因检测无明显变化，考虑重启化疗，本次进展后，选择“培美曲塞+卡铂+信迪利单抗+安罗替尼”治疗方案，1个周期后复查CT达到PR（部分缓解），C2程后持续PR。
结论 本研究报道了EGFR突变并PD-L1表达晚期NSCLC的病例。在该名患者的治疗中，通过文献复习及相关基因检测，制定了个体化诊疗方案，联合免疫、化疗以及抗血管生成药物，使患者疾病得到有效控制，并且疗效显著。TKI耐药的机制复杂，且根据患者的个体差异和肿瘤部位的不同而有所差别，在患者出现进展后可以通过再次进行基因检测明确耐药机制后指导临床治疗可获得更好的疗效，达到精准化治疗。
