目的

探讨奥希替尼一线治疗表皮生长因子受体（EGFR）突变非小细胞肺癌（NSCLC）的疗效和安全性，并分析无进展生存期（PFS）的影响因素。

方法

回顾性收集2019年1月~2022年12月仅接受奥希替尼单药一线治疗的EGFR突变NSCLC患者临床资料。主要终点为PFS，采用Kaplan-Meier法进行生存分析，采用单因素及多因素Cox回归模型分析PFS的影响因素；同时描述不良事件（AEs）发生情况。

结果

共纳入患者74例，中位PFS为16.3个月（95%CI = 13.2~20.3个月）。亚组分析显示，ⅢA/ⅣA期患者中位PFS较ⅣB期显著延长（25.1个月vs．15.0个月，P=0.035）；预后营养指数（PNI）≥39.4患者中位PFS较PNI<39.4患者显著延长（17.2个月vs．10.7个月，P=0.030）。多因素Cox回归分析显示，疾病分期及PNI为PFS的独立影响因素。常见AEs包括皮疹、口腔溃疡、腹泻、白细胞计数降低及甲沟炎，多为1~2级。

结论

奥希替尼一线治疗的疗效较好，且AEs多数为1~2级。基线的疾病分期和PNI是NSCLC患者接受奥希替尼一线治疗时疾病进展的独立影响因素。

OBJECTIVE

To investigate the efficacy and safety of first-line osimertinib in the treatment of epidermal growth factor receptor (EGFR)-mutated non-small cell lung cancer (NSCLC), and to analyze the influencing factors of progression-free survival (PFS).

METHODS

The clinical data of patients with EGFR-mutated NSCLC who received first-line osimertinib monotherapy only from January 2019 to December 2022 were collected retrospectively. The primary endpoint was PFS. Survival analysis was performed using the Kaplan-Meier method, and univariate and multivariate Cox regression models were used to analyze the influencing factors of PFS. Meanwhile, the occurrence of adverse events (AEs) was described.

RESULTS

A total of 74 patients were included, with a median PFS of 16.3 months (95%CI=13.2-20.3 months). Subgroup analysis showed that the median PFS of patients with stage ⅢA/ⅣA was significantly longer than that of patients with stage ⅣB (25.1 months vs. 15.0 months, P=0.035). The median PFS of patients with prognostic nutritional index (PNI)≥39.4 was significantly longer than that of patients with PNI <39.4 (17.2 months vs. 10.7 months, P=0.030). Multivariate Cox regression analysis showed that disease stage and PNI were independent influencing factors of PFS. Common AEs included rash, oral ulcer, diarrhea, decreased white blood cell count, and paronychia, mostly grades 1-2.

CONCLUSION

First-line osimertinib shows favorable efficacy with manageable safety in patients with EGFR-mutated NSCLC. Baseline disease stage and PNI are independent factors for disease progression in NSCLC patients receiving first-line osimertinib.
