目的

探究脓毒症患者静脉滴注多黏菌素B（polymyxin B, PMB）的血浆药物暴露与临床疗效的关联，并探究药动学目标。

方法

单中心、观察性研究纳入2021年7月~2025年7月接受PMB静脉治疗的脓毒症患者。主要结局为临床治疗成功率，次要结局包括28 d全因死亡率和90 d内生存时间、细菌清除率、28 d无机械通气/无ICU/无住院天数及PMB相关急性肾损伤（acute kidney injury, AKI）发生率。分析药物暴露与临床结局之间的关系，基于ROC曲线计算截断值。并对接受连续性肾脏替代治疗（continuous renal replacement therapy, CRRT）的患者进行亚组分析。

结果

82例脓毒症患者中临床成功率为37.8%。AUC_{ss,0-12 h}截断值为27.10 mg·h^-1·L^-1（C_ss,avg 2.26 mg·L^-1），药物暴露高于该阈值与较长的90 d内生存时间有关（ETR=1.92，P=0.039）。亚组分析显示增加负荷剂量可改善CRRT患者的生存时间（ETR=1.38，P=0.015）。PMB相关AKI发生率为40.54%（15/37），AKI组的AUC_{ss,0-12 h}显著高于非AKI组（AUC_{ss,0-12 h} 43.51 vs． 27.23 mg·h^-1·L^-1，P=0.006）。

结论

PMB浓度与脓毒症患者的临床疗效有关，维持AUC_{ss,0-12 h} > 27.10 mg·h^-1·L^-1（相当于C_ss,avg 2.26 mg·L^-1）可提高临床成功率，但较高的AUC_{ss,0-12 h}是PMB相关AKI发生风险的独立危险因素。临床实践中需进行治疗药物监测以确保疗效和安全性。

OBJECTIVE

To investigate the relationship between intravenous polymyxin B (PMB) exposure and clinical outcome in septic patients, and to explore the pharmacokinetic target.

METHODS

This single-center observational study enrolled septic patients who received PMB therapy between July 2021 and July 2025. Primary outcome was the clinical success rate. Secondary outcomes included 28-day all-cause mortality, 90-day survival time, bacterial clearance, 28-day ventilator/ICU/hospital-free days and incidence of PMB-related acute kidney injury (AKI). The AUC_{ss, 0-12 h} and C_{ss, avg} were analyzed for associations with outcomes, and cut-off value was determined by receiver-operating characteristic (ROC) curves. Subgroup analysis was conducted among patients receiving continuous renal replacement therapy (CRRT).

RESULTS

Of 82 septic patients, clinical success rate was 37.8%. The AUC_{ss, 0-12 h} cut-off value was 27.10 mg·h^-1·L^-1 (C_{ss, avg} 2.26 mg·L^-1). AUC_{ss, 0-12 h} > 27.10 mg·h^-1·L^-1 was associated with longer 90-day survival time (ETR=1.92, P=0.039). In the CRRT subgroup, increased loading dose was associated with longer 90-day survival time (ETR=1.38, P=0.015). The incidence of PMB-related AKI was 40.54% (15/37).. Patients with AKI had higher AUC_{ss, 0-12 h} (43.51 vs. 27.23 mg·h^-1·L^-1, P=0.006).

CONCLUSION

PMB exposure is associated with clinical efficacy in septic patients. Maintaining an AUC_{ss, 0-12 h} > 27.10 mg·h^-1·L^-1 (C_{ss, avg} of 2.26 mg·L^-1) enhances clinical success. Higher AUC_{ss, 0-12 h} is an independent risk factor for PMB-related AKI. Therapeutic drug monitoring is recommended to ensure efficacy and safety.
