OBJECTIVE

To evaluate the effectiveness of clinical pharmacist intervention in standardizing the use of antiemetic drugs in tumor treatment by investigating the use of antiemetic drugs in inpatient medical records at a hospital.

METHODS

Clinical pharmacists reviewed and analyzed the use of antiemetic drugs in the archived medical records of hospitalized patients who received intravenous antitumor treatment from January to April 2023. Following communication with physicians, intervention measures were implemented. The standardization rate and specific usage of antiemetic drugs were assessed at three months and six months post-intervention.

RESULTS

Before intervention, the standardization rate of antiemetic drugs at the hospital was 37.63%. The main issues contributing to non-standard use included inappropriate selection of antiemetic drugs, inappropriate drug selection, inadequate variety, insufficient treatment duration, and unreasonable upgrading of antiemetic regimens. After clinical Pharmacists' intervention, the standardized rate of antiemetic drugs was 63.53% (P<0.001) at three months post-intervention (the intervention group 1), and 86.36% (P<0.001) at six months post-intervention (the intervention group 2). Regarding specific antiemetic drugs, the usage rate of metoclopramide decreased significantly from 44.09% before intervention to 8.24% at three months and 7.95% at six months post-intervention (P<0.05). The usage of olanzapine showed a gradual increase, though the difference was not statistically significant.

CONCLUSION

Clinical pharmacists achieved significant improvements in the standardization of antiemetic drug use through targeted interventions, thereby reducing nausea and vomiting in tumor patients and achieving favorable intervention outcomes.

目的

通过对某院住院归档病历中抗肿瘤治疗相关止吐药的使用情况进行调查,评价临床药师干预在提高止吐药使用规范性方面的作用。

方法

临床药师对2023年1~4月接受静脉抗肿瘤治疗的住院归档病历进行相关止吐药使用情况的点评分析。与医师沟通后,采取了相应的干预措施。在干预后3个月和干预后6个月分别统计了止吐药物的使用规范率及具体使用情况。

结果

干预前,该院止吐药物的使用规范率为37.63%。不规范使用情况主要有止吐药品种选择不当、剂量不足、疗程不足以及无理由升级止吐方案。经临床药师干预后,干预后3个月(干预后一组)止吐药的使用规范率为63.53%(P<0.001),干预后6个月(干预后二组)止吐药的使用规范率为86.36%(P<0.001)。在止吐药品种方面,干预前甲氧氯普胺的使用率为44.09%,干预后3个月和干预后6个月分别降为8.24%和7.95%,差异具有统计学意义(P<0.05)。奥氮平的使用率虽有逐渐增加的趋势,但差异无统计学意义。

结论

临床药师通过干预措施,显著改善止了抗肿瘤治疗中吐药物的使用规范性,减少肿瘤患者的恶心和呕吐症状,取得了良好的干预效果。

目的

通过对某院住院归档病历中抗肿瘤治疗相关止吐药的使用情况进行调查,评价临床药师干预在提高止吐药使用规范性方面的作用。

方法

临床药师对2023年1~4月接受静脉抗肿瘤治疗的住院归档病历进行相关止吐药使用情况的点评分析。与医师沟通后,采取了相应的干预措施。在干预后3个月和干预后6个月分别统计了止吐药物的使用规范率及具体使用情况。

结果

干预前,该院止吐药物的使用规范率为37.63%。不规范使用情况主要有止吐药品种选择不当、剂量不足、疗程不足以及无理由升级止吐方案。经临床药师干预后,干预后3个月(干预后一组)止吐药的使用规范率为63.53%(P<0.001),干预后6个月(干预后二组)止吐药的使用规范率为86.36%(P<0.001)。在止吐药品种方面,干预前甲氧氯普胺的使用率为44.09%,干预后3个月和干预后6个月分别降为8.24%和7.95%,差异具有统计学意义(P<0.05)。奥氮平的使用率虽有逐渐增加的趋势,但差异无统计学意义。

结论

临床药师通过干预措施,显著改善止了抗肿瘤治疗中吐药物的使用规范性,减少肿瘤患者的恶心和呕吐症状,取得了良好的干预效果。

OBJECTIVE

To evaluate the effectiveness of clinical pharmacist intervention in standardizing the use of antiemetic drugs in tumor treatment by investigating the use of antiemetic drugs in inpatient medical records at a hospital.

METHODS

Clinical pharmacists reviewed and analyzed the use of antiemetic drugs in the archived medical records of hospitalized patients who received intravenous antitumor treatment from January to April 2023. Following communication with physicians, intervention measures were implemented. The standardization rate and specific usage of antiemetic drugs were assessed at three months and six months post-intervention.

RESULTS

Before intervention, the standardization rate of antiemetic drugs at the hospital was 37.63%. The main issues contributing to non-standard use included inappropriate selection of antiemetic drugs, inappropriate drug selection, inadequate variety, insufficient treatment duration, and unreasonable upgrading of antiemetic regimens. After clinical Pharmacists' intervention, the standardized rate of antiemetic drugs was 63.53% (P<0.001) at three months post-intervention (the intervention group 1), and 86.36% (P<0.001) at six months post-intervention (the intervention group 2). Regarding specific antiemetic drugs, the usage rate of metoclopramide decreased significantly from 44.09% before intervention to 8.24% at three months and 7.95% at six months post-intervention (P<0.05). The usage of olanzapine showed a gradual increase, though the difference was not statistically significant.

CONCLUSION

Clinical pharmacists achieved significant improvements in the standardization of antiemetic drug use through targeted interventions, thereby reducing nausea and vomiting in tumor patients and achieving favorable intervention outcomes.

肿瘤药物治疗引起的恶心呕吐是最常见的不良反应之一。若无预防性镇吐治疗,70%~80%接受化疗的患者会出现恶心、呕吐症状,其程度受化疗药物致吐强弱等多因素的影响[1-4]。从机制来看,抗肿瘤药物可通过外周途径和中枢途径2条通路引起呕吐反射。外周途径:药物刺激胃肠道黏膜的嗜铬细胞,促使其释放5-羟色胺(5-HT3),并与5-HT3受体结合诱发呕吐反射,通常表现为急性呕吐。中枢途径:P物质与位于呕吐中枢的神经激肽-1(NK-1)受体结合诱发呕吐反射,通常表现为延迟性呕吐[2]
按未给予预防处理时急性呕吐的发生率,抗肿瘤药物的致吐风险可分为4级[3,4]:高度致吐风险,急性呕吐发生率>90%;中度致吐风险,急性呕吐发生率30%~90%;低度致吐风险,急性呕吐发生率10%~29%;轻微致吐风险,急性呕吐发生率<10%。因此,规范使用止吐药物是保障抗肿瘤治疗安全与连续性的关键环节。由于抗肿瘤药物品种多样、致吐风险等级差异显著,临床医师常凭经验给药,易导致止吐不足(恶心呕吐发生率升高,患者依从性下降等情况)或过度止吐(费用增加并可能带来额外的不良反应)。本研究旨在通过临床药师的干预,通过与临床医师的协作、沟通及改进,优化某院抗肿瘤治疗中止吐方案的选择与执行,旨在提升用药合理性,降低抗肿瘤治疗相关恶心呕吐的发生率。
收集某院2023年1~4月接受静脉用抗肿瘤治疗的住院归档病历,随机抽取120份,设为对照组。2023年5月开始进行临床药师干预措施。分别对2023年5~7月和2023年8~10月接受静脉用抗肿瘤治疗的住院归档病历进行统计,根据随机数字法抽取各120份,分别设为干预后一组和干预后二组。纳入标准:接受静脉用抗肿瘤治疗。排除标准:既往有胃肠道疾病史;病史资料不完整。设计“止吐药物使用表”,收集信息包括患者基本信息(病例号、性别、年龄)、风险因素(饮酒史,既往晕车或呕吐情况,焦虑或紧张状态),提取医嘱中肿瘤治疗方案、同期止吐药物使用情况(包括药品名称、剂量、频率、疗程),以及记录患者出现爆发性恶心呕吐的情况[5-8]。本研究经浙江萧山医院医学伦理委员会批准(批件号:K2022077)。
临床药师自2023年5月开始干预止吐药物的合理使用,具体措施如下:(1)参考《中国肿瘤药物治疗相关恶心呕吐防治专家共识》(2022年)、《NCCN临床实践指南:止吐(2022.V1)》,制定该院的抗肿瘤药物致吐等级表(见表1)。(2)在该院逸曜合理用药管理系统中,对肿瘤药物进行致吐风险标注(特别是致吐风险为高、中级及易出错的肿瘤药物),同时备注各药物的止吐治疗建议;保证临床医师在开立抗肿瘤治疗时均有弹框提示。(3)每月由肿瘤科专业临床药师对使用静脉用抗肿瘤治疗的住院归档病历进行止吐药抽查点评,并及时与临床科室沟通;每季度进行总结整理并在院内网公示点评结果。(4)对使用不合理率较高的科室或临床医师由医务科进行行政干预。(5)作为抗肿瘤治疗规范化无呕病房的创建小组成员,参与无呕规范化病房的成立。
参考《中国肿瘤药物治疗相关恶心呕吐防治专家共识》(2022年)、《NCCN临床实践指南:止吐(2022.V1)》,根据肿瘤药物的致吐风险,并结合患者个体因素,判断肿瘤治疗方案的致吐性等级。分别从止吐药的联用情况、品种选择、用法用量和疗程等方面判断止吐方案的规范性。其中,在中-高度致吐性方案中,共识及指南中推荐“帕洛诺司琼+地塞米松+奥氮平”,但结合该院实际,使用“5-HT3受体拮抗剂+地塞米松+奥氮平”均视为规范。在病程中有明确记录患者因个人原因拒绝使用某种止吐药物,视为规范情况。基于止吐药物的使用目的是为尽量减少患者的恶心呕吐,因此疗程超过指南及共识推荐,一般视为规范;而止吐药物使用疗程不足,则视为不规范。该院常见的不规范情况主要有止吐药联合使用不规范、剂量不足、高致吐风险下止吐药品种不足、疗程不足以及无指征升级止吐方案。
分别计算各阶段肿瘤治疗相关止吐药物的使用规范率,并比较干预前后的差异。同时统计各阶段不同类型规范事件的发生比例,及爆发性恶心呕吐的发生率。爆发性恶心呕吐是指已预防性给予止吐药物后,仍出现恶心及呕吐和/或需要给予解救性止吐治疗,可在抗肿瘤用药后的任何时间段。
应用SPSS 20.0统计学软件分析数据,计数资料用n(%)表示,用χ2检验,以P<0.05为差异有统计学意义。
经统计,2023年1~4月、5~7月和8~10月时间段内,接受静脉抗肿瘤治疗的归档病历分别有676、635和659份。采用随机数字法各抽取120份,分别设为对照组、干预后一组、干预后二组。初始检查各组病历后,发现当仅接受免疫治疗、靶向治疗(包括贝伐珠单抗、曲妥珠单抗等)和内分泌治疗(氟维司群)时,均未予使用止吐药物,均符合规范,予以剔除。剔除后,实际纳入分析病例分别为对照组93份、干预后一组85份,干预后二组88份。3组患者的性别、年龄、化疗方案等一般资料差异无统计学意义(P>0.05),详见表2
在临床药师干预前,对照组中止吐药物使用规范的仅有35份,占比37.63%,不规范使用率达到62.37%。经临床药师干预后,干预后一组中止吐药物使用规范的有54份,占比63.53%,不规范使用率为36.47%;干预后二组中止吐药物使用规范的有76份,占比86.36%,不规范使用率降为13.64%。持续干预组中止吐药物的使用规范性显著升高,结果见表3
经过该院用药情况分析,不规范情况主要有止吐药联合使用不规范、剂量不足、高致吐风险下止吐药品种不足、疗程不足以及无指征升级止吐方案。同一个止吐方案常存在多种不规范情况,如联用不规范且疗程不足同时出现,详见表4。在止吐药物品种上,该院主要使用5-HT3受体拮抗剂、甲氧氯普胺、地塞米松、NK-1受体拮抗剂。不规范情形以联合用药不当最为常见,共43例次(46.23%),主要表现为在中-高致吐方案中仍联合使用甲氧氯普胺、中致吐方案使用5-HT3受体拮抗剂+NK-1受体拮抗剂过度联合。其中,甲氧氯普胺的不合理使用占42/43例次;除低致吐方案可单用甲氧氯普胺外,其余均为不规范。在用法用量上,5-HT3受体拮抗剂、NK-1受体拮抗剂基本使用规范,地塞米松片剂量使用规范;而地塞米松针剂存在剂量偏低的问题,特别是含顺铂的高致吐方案中,33次针剂使用里16次剂量偏低(地塞米松针剂的使用以5 mg剂量为主)。此外,还存在止吐强度与致吐等级不匹配的情况,包括中-高致吐风险时用药不足、低致吐风险时用药过度。在疗程上,存在使用时间偏短的情况,如高致吐方案止吐药使用时间<4 d。
经临床药师干预后,干预后一组和干预后二组中含甲氧氯普胺的联合方案显著减少(P<0.05)。干预后二组中,中-高致吐方案使用止吐药不足的情况显著减少(P<0.05)。在药物剂量上,干预后二组中地塞米松针剂剂量偏低的情况与对照组相比显著减少(P<0.05)。在无理由升级止吐方案方面,主要是关于低致吐方案使用2种及以上的止吐药,干预后二组与对照组、干预后一组比较,均有显著性改善,差异具有统计学意义(P<0.05)。
在对照组中,有记录的爆发性恶心呕吐为8份,干预后一组中为7份,干预后二组中为4份,组间比较差异无统计学意义(P>0.05)。
由抗肿瘤药物导致的恶心呕吐通常指化疗相关性恶心呕吐(CINV),是化疗中最常见的不良反应之一[9-11]。止吐药物的使用是抗肿瘤治疗中必不可少的组成部分。因此,规范、有效的止吐方案不仅能直接改善患者的治疗体验和生活质量,更会影响到后续治疗的依从性和疗效。所以,CINV的预防与止吐药物的合理用药应作为肿瘤专科临床药师的关注重点。
在临床药师干预前,抽样检查发现该院肿瘤治疗相关止吐药物的使用规范率仅37.63%,主要存在以下突出问题:经验性用药(习惯选用甲氧氯普胺,而奥氮平使用率不高)、未按致吐等级给药、剂量与疗程不足(地塞米松剂量偏低、止吐疗程偏短等)。在止吐药相关指南及共识中,对于中-高致吐性方案,奥氮平1A类、I级推荐,而甲氧氯普胺未被推荐。低致吐性方案,推荐予单一止吐药物(包括5-HT3受体拮抗剂、甲氧氯普胺等)。对于高致吐方案,建议至少给予3种止吐药物,而在临床中易出现低于指南推荐的3药联合情况,从而增加爆发性恶心呕吐的风险。研究显示[12],奥氮平5 mg联合标准止吐方案(阿瑞匹坦+帕洛诺司琼+地塞米松)可显著提高延迟期呕吐缓解率,仅有1例出现与治疗相关的3级嗜睡;对于使用铂类化疗药物的患者奥氮平联合阿瑞匹坦、帕洛诺司琼、地塞米松可能成为新的标准治疗方案[13-15]。甲氧氯普胺的止吐效果弱于5-HT3受体拮抗剂单药;如增加甲氧氯普胺剂量,不良反应风险随之上升[16]。因此,提高奥氮平的合理应用、限制甲氧氯普胺的不当使用,并严格按照致吐风险分级制定多药联合方案,成为临床药师干预的首要任务。
针对上述问题,临床药师开展干预措施,在合理用药系统中增加肿瘤药物的致吐风险提示及止吐方案建议;日常工作中参与肿瘤科查房,对止吐药的使用进行即时反馈,对初期检查后止吐药物的使用存在问题较多的科室进行科室培训,对多次检查后存在问题仍明显的临床医师进行当面沟通交流。经过上述措施的开展,该院抗肿瘤治疗相关止吐药的使用规范性逐步提高,提示干预有效且质量不断改进。在止吐药品种上,甲氧氯普胺的使用显著下降,基本杜绝了中-高致吐方案使用甲氧氯普胺的情况。同时奥氮平的使用在逐渐增加,但增幅尚未达到统计学意义,这主要是由于奥氮平作为非典型抗精神病药物,临床医师对其安全性仍存在一定顾虑。
因此,临床药师需加强与临床医师的沟通协作,通过采取多种干预途径与措施,提升临床医师对化疗药物致吐风险评估的认可度。根据具体的化疗方案及患者个体特征,综合评估风险因素,制定并执行个体化的止吐方案。确保止吐药物的规范使用,最大限度降低恶心呕吐发生率,同时避免不必要的用药风险。在创建无呕规范化病房的过程中,临床药师作为成员之一,应发挥主观能动性,做好药物信息宣教,提供药物信息支持。研究证明,通过临床药师的持续干预,止吐药物的规范率明显提升,减少了爆发性恶心呕吐的发生,也获得了临床医生的广泛认可。
