In order to evaluate the development of muscle fatigue during drilling operations in the hand-over-head posture,a simulated drilling test was conducted to measure the maximum voluntary contraction (MVC) of muscles before the test,the maximum residual muscle strength (MRF) after the test,the degree of strength output attenuation (ΔF),and ratings of perceived exertion (RPE) of wrists,elbows,and shoulders. MET was recorded. By setting different combinations of three operating surfaces (front,side,and bottom) and three operating heights as experimental variables,the effects of different operating methods on muscle fatigue during drilling operations under hand head posture were compared. Subsequently,the effects of three operating surfaces (front,side,and bottom) and three operating heights on MET,MRF,ΔF,as well as RPE of wrist,elbow,and shoulder were analyzed. Research has shown that using frontal manipulation and reducing arm lift height can effectively alleviate muscle fatigue. Different operating surfaces significantly affect MET,MRF,and ΔF,as well as RPE of the wrist,elbow,and shoulder. Different operating heights significantly affect MET,MRF,and RPE of the elbow. MET prediction model established in this article can reflect the muscle fatigue status of personnel during drilling operations in the hand-head posture.

为评价手过头姿势下钻孔作业过程中人员肌肉疲劳发展情况,先通过模拟手过头姿势下的钻孔试验,测量试验前肌肉的最大随意收缩(MVC)、试验后的最大剩余肌力(MRF)、力量输出衰减程度(ΔF)以及手腕、手肘和肩部的主观疲劳评分(RPE),记录最大耐受时间(MET);通过设定3种操作面(正面、侧面、底面)和3种操作高度的不同操作组合作为试验变量,比较不同操作方式对手过头姿势下钻孔作业的肌肉疲劳影响情况,随后分析3种操作面(正面、侧面、底面)和3种操作高度下对MET、MRF、ΔF以及手腕、手肘和肩部RPE的影响。研究结果表明:采用正面操作并降低手臂上举高度能有效减轻肌肉疲劳;不同操作面显著影响MET、MRF及ΔF,同时,显著影响手腕、手肘和肩部的RPE;不同的操作高度显著影响MET、MRF以及手肘的RPE;所建立的MET预测模型可反映手过头姿势下钻孔作业过程中人员肌肉疲劳状态。

为评价手过头姿势下钻孔作业过程中人员肌肉疲劳发展情况,先通过模拟手过头姿势下的钻孔试验,测量试验前肌肉的最大随意收缩(MVC)、试验后的最大剩余肌力(MRF)、力量输出衰减程度(ΔF)以及手腕、手肘和肩部的主观疲劳评分(RPE),记录最大耐受时间(MET);通过设定3种操作面(正面、侧面、底面)和3种操作高度的不同操作组合作为试验变量,比较不同操作方式对手过头姿势下钻孔作业的肌肉疲劳影响情况,随后分析3种操作面(正面、侧面、底面)和3种操作高度下对MET、MRF、ΔF以及手腕、手肘和肩部RPE的影响。研究结果表明:采用正面操作并降低手臂上举高度能有效减轻肌肉疲劳;不同操作面显著影响MET、MRF及ΔF,同时,显著影响手腕、手肘和肩部的RPE;不同的操作高度显著影响MET、MRF以及手肘的RPE;所建立的MET预测模型可反映手过头姿势下钻孔作业过程中人员肌肉疲劳状态。

In order to evaluate the development of muscle fatigue during drilling operations in the hand-over-head posture,a simulated drilling test was conducted to measure the maximum voluntary contraction (MVC) of muscles before the test,the maximum residual muscle strength (MRF) after the test,the degree of strength output attenuation (ΔF),and ratings of perceived exertion (RPE) of wrists,elbows,and shoulders. MET was recorded. By setting different combinations of three operating surfaces (front,side,and bottom) and three operating heights as experimental variables,the effects of different operating methods on muscle fatigue during drilling operations under hand head posture were compared. Subsequently,the effects of three operating surfaces (front,side,and bottom) and three operating heights on MET,MRF,ΔF,as well as RPE of wrist,elbow,and shoulder were analyzed. Research has shown that using frontal manipulation and reducing arm lift height can effectively alleviate muscle fatigue. Different operating surfaces significantly affect MET,MRF,and ΔF,as well as RPE of the wrist,elbow,and shoulder. Different operating heights significantly affect MET,MRF,and RPE of the elbow. MET prediction model established in this article can reflect the muscle fatigue status of personnel during drilling operations in the hand-head posture.

人体肌肉疲劳作为工效学领域的重要研究内容,不仅显著影响个体的体能水平,还会显著降低个人的工作效率和作业表现,甚至诱发工作相关肌肉骨骼疾病(Work-related Musculoskeletal Disorders,WMSDs)。手过头作业属于重复性、周期性和连续性劳动,广泛出现在汽车、飞机等维修任务[1-2]中。研究表明:长时间在手过头姿势下作业,极易降低工作能力和效率,导致肌肉疲劳并引发WMSDs[3]
现有手过头作业肌肉疲劳主要关注于作业过程中的肌肉活动以及姿势风险。田保珍等[4]通过分析表面肌电图(surface Electromyogrphy,sEMG),研究了汽车底盘维修人员单手上举作业的肌肉疲劳情况;MACIUKIEWICZ等[5]针对打孔作业中的单手和双手上举姿势根据不同作业面(正面、顶面),通过sEMG研究了其肌肉疲劳状态;CHOPP等[6]利用表面肌电图监测了11个上肢肌肉在特定的任务下,包括4种目标角度(垂直角度:-15、0、15、30°)以及5种施力方向(向后拉、向前推、向下、侧面、向上),研究各个肌肉在头顶工作下的疲劳程度;ANTON等[3]研究了手过头姿势下三角肌、肱二头肌和肱三头肌在不同钻孔距离(近距离、中距离、远距离)和不同地面高度的肌电活动,发现近距离和较低高度作业时三角肌和肱二头肌的肌肉疲劳明显减少,并建议工人靠近自己的身体工作,以减少肩部压力。以上研究虽有助于深入了解手过头作业的肌肉疲劳发展情况,但大多采用sEMG方法分析典型任务场景,成本较高,操作较复杂。
目前,对于避免肌肉疲劳以及WMSDs的产生,国内外利用主客观评价方法研究。如易灿南[7]和唐范[8]等根据最大耐受时间(Maximum Endurance Time,MET)分别建立了该作业姿势下静态单手及双手拉车作业的MET预测模型。ZHANG Zhanwu等[9]通过进行男、女性静态单臂推力试验,建立了推力作业下的MET预测模型。GARG等[10]通过不同作业姿势研究了肩关节疲劳情况,并建立了肩部MET预测模型。MANENICA[11]提出了手部和背部MET预测模型。不同试验姿势所对应的疲劳部位不同,以上MET预测模型只能在该模型相同的作业姿势下应用。因此,现有MET模型不适用于分析手过头作业姿势。为分析维修人员手过头作业肌肉疲劳情况,笔者拟通过模拟手过头姿势下钻孔试验,测量被试在作业过程的MET、作业前的最大随意收缩能力(Maximum Voluntary Contractions,MVC)及作业后的最大剩余肌力(Maximum Residual Force,MRF)和力量输出衰减程度(ΔF=MVC-MRF),并记录作业后上肢各部位(手腕、肘部、肩)的RPE疲劳值,分析3种操作面(正面、侧面、底面)和3种操作高度(H1H2H3)对MET、MRF、ΔF和RPE的影响,并基于试验数据建立手过头作业的MET预测方程,以期达到降低WMSDs风险的目的。
严格筛选16名健康且无肌肉骨骼病史的男性大学生作为被试。所有被试在试验前均签署《试验参与同意书》,并在专业人员指导下试验,同时,被试在试验前24 h内无剧烈运动。被试均为右利手,年龄为 (19.13±0.89)岁,身高为 (177.5±5.978)cm,体质量为 (71.19±11.75)kg,身体质量指数为 (22.59±3.3)kg/m2,上臂长为 (24.75±5.31)cm,下臂长为 (26.5±2.22)cm,肩高为 (143.75±6.64)cm。试验温度维持在 (19.6±1.48)℃,相对湿度保持在 78.60%±6.10%。
1) 推力测量装置。将S型压力传感器(0~100 kg)固定到模拟环境中的自制木箱上,并连接测量仪和计算机,其中,压力传感器实时追踪试验中的施力变化,测量仪精确记录推力数据,并将数据导入计算机,记录MVC和MRF数据。
2) 模拟手过头姿势下的钻孔装置。将一个尺寸为300mm×120mm×20mm的木箱悬挂在头顶,木箱的正面、右侧、底面代表不同操作面(正面、侧面、底面),每个操作面的中心有直径为10mm的孔,用来固定压力传感器,木箱固定在自制支架的横梁上,横梁可上下调节,以匹配被试的作业高度,支架尺寸为1 500mm×1 000mm×2 800mm,以及其他工具卷尺、电钻(1 kg)、记号笔、酒精湿巾、身高测量仪、体质量计、秒表。
3) 主观疲劳评价。采用主观疲劳评分(Ratings of Perceived Exertion,RPE)测量,Borg CR-10量表。
被试用惯用手(右手)拿取电钻(1kg)过头顶,双脚站在地面进行作业,自变量包括3种操作面(正面、侧面、底面)以及3种作业高度(H1H2H3),通过3种操作面和3种操作高度的不同搭配进行9种小试验,依次获取试验数据,见表1。3种操作面如图1所示。操作高度的获取方法来自于对惯用手/手臂的2种人体测量方法[12],首先被试将上臂抬高于头顶并保持水平,肘部屈曲90°,测量从地面到电钻握把中心的高度H1;然后被试将手臂在头顶上完全伸直,形成最大头顶高度并再次测量从地面到电钻握把的中心高度H3;最后通过公式H2=H1+0.4×(H3-H1)获取手部高度2个极值之间的中点H2,由此获得3种试验高度,如图2所示。
试验前,告知被试试验目的、流程和潜在风险。所有被试需先进行预试验,模拟钻孔作业操作并适应压力感应,直至熟练掌握工作任务。同时,确定每位被试在不同操作面和高度下的作业姿势,并测量相关人体参数。被试需在试验人员协助与指导下,尽可能持久地维持指定的试验姿势。若因无法维持或感到不适,被试可随时中止保持该姿势,以确保其安全与舒适,具体试验流程如图3所示。
1) 试验准备阶段。在试验前15min被试需要进行5min身体拉伸运动,避免肌肉拉伤。
2) 推力测量阶段。被试在9种试验中(3种操作面和3种操作高度)依次进行推力测试。试验期间,被试在4~6s内手持电钻用最大力量推动压力传感器,测量3次,取最大值,记为被试的 MVC数值。
3) 模拟作业阶段。被试在指定位置,手持1kg电钻以手过头作业姿势,根据不同操作面(正、底、侧)和不同操作高度(H1H2H3)进行钻孔作业。9种试验的作业姿势与推力测试阶段的作业姿势保持一致,被试会持续保持以50%MVC的肌力模拟作业,若高于或低于50%MVC的肌力,研究人员实时观测数据,并通过口头提示引导被试。当被试反馈无法继续时,立即停止并记录MET。随后,被试迅速前往压力测试位置测试,获取数据MRF及ΔF。此外,被试需填写基于BorgCR-10的RPE量表,记录肩、肘、手腕等部位的疲劳感受。试验每天仅进行一次,2次试验间需间隔超过24h,且试验前24h内禁止剧烈运动。
试验共记录MVC值、MRF值、MET值及ΔF值各144个(3种操作面×3种操作高度×16名被试),手腕、手肘以肩部疲劳程度RPE值各144个(3种操作面×3种操作高度×16名被试),分析在不同操作面及不同操作高度下被试MET、MRF、ΔF、RPE的特点,并创建手过头姿势下钻孔作业MET预测模型。
MET数值可反映作业人员的疲劳情况。结果表明:在S1S2S3的3种操作面中,操作面S1(正面)MET最长,其中S1H1的MET为 (4.29±1.91)min大于S1H2的MET (3.84±2.13) min和S1H3的MET为 (3.78±1.40)min;在H1H2H3的3种操作高度中,高度为H1的MET最长,其中,S1H1的MET最长,其次,S2H1的MET为 (1.77±0.60)min和S3H1的MET为 (1.71±0.76)min;在9种操作条件下,S1H1的MET最长为 (4.29±1.91)min,S2H3的MET最短为 (1.06±0.37)min(图4)。
通过分析试验任务的MET,结果显示,操作面S1(正面)最不容易疲劳,这与MACIUKIEWICZ等[5]对手过头打孔作业的研究结论类似,即向前施力比向上施力更不容易疲劳;同时,试验结果表明:操作高度H1最不疲劳,H3最疲劳,此研究结果与SOOD等[12]在头顶作业任务研究中,表示作业高度的增加会更容易产生肌肉疲劳的结论相似。在汽车维修作业试验中,研究人员比较了3种操作面(正面、侧面、顶面)和3种不同作业高度的手臂肌肉sEMG,也得出正面操作不容易疲劳以及随着高度的增加疲劳度也随之增加[4]。因此,作业人员在进行手过头作业时,应尽量采取正面操作,减少侧面和底面的操作姿势,并且在作业时尽量降低手臂的上举高度,避免产生肌肉疲劳,降低患WMSDs的风险。
通过Shapiro-Wilk检验,数据符合正态分布(p>0.05),可以采用方差分析。运用SPSS软件重复测量方差分析数据,探讨3种操作面和3种操作高度对MET、MRF、ΔF的影响。其中,主效应为操作面是指在3种操作面中被试的不同操作高度对MET、MRF、ΔF是否有显著差异;主效应为操作高度是指在3种操作高度中被试的不同操作面对MET、MRF、ΔF是否有显著差异;交互效应是3种操作面和3种操作高度的组合之间是否有显著差异。结果表明:MET、MRF在操作面和操作高度上具有显著性差异(p<0.05),ΔF只在操作面上具有显著差异(p<0.05)。在操作面和操作高度的交互效应中,MRF和ΔF存在显著差异(p<0.05),MET没有显著差异,方差分析见表2
将试验数据两两比较(图5)。在操作面比较中,MET、MRF、ΔF在正面与底面的比较中都存在显著差异(p<0.05); 在正面与侧面比较中,MET和ΔF存在显著差异(p<0.05);在侧面与底面比较中,MRF和ΔF存在显著差异(p<0.05)。在操作高度比较中,H1H2只有MRF存在显著差异(p<0.05)。H1H3的比较结果显示:MRF、ΔF都存在显著差异(p<0.05)。H2H3比较只有MRF存在显著差异(p<0.05)。
在操作面比较中,正面与底面的MET、MRF、ΔF存在显著差异(p<0.05);正面与侧面的MET、ΔF存在显著差异(p<0.05);侧面与底面的MRF、ΔF存在显著差异(p<0.05),见表3
正面MET值((3.97±1.81)min)大于底面MET值((1.60±0.60)min),正面MRF值((10.08±3.47)N)小于底面MRF值 ((12.10±2.88)N),正面ΔF值((2.61±1.05)N)小于底面ΔF值 ((2.91±1.79)N);正面MET值大于侧面MET ((1.40±0.55)min),正面ΔF值大于侧面ΔF值 ((0.86±0.28)N);底面MRF值 ((12.10±2.88)N)大于侧面MRF值 ((6.52±1.16)N),底面ΔF值 ((2.91±1.79)N)大于侧面ΔF值 ((0.86±0.28)N),见表4。此结果的原因为:被试在试验过程中在不同操作面作业时,腕关节角度的变化不同,操作底面时腕关节为中立姿势,操作正面时腕关节尺偏角度较大,操作侧面时腕关节屈曲角度较大。研究发现,与中立状态相比,手腕屈曲 55% 自由度时手腕弯曲MVC下降27%,手腕伸展 55% 自由度时手腕弯曲MVC下降29%。在正面和侧面操作面时,腕关节的角度运动幅度较大,因此,MRF值小于底面[13]
在操作高度比较中,MRF在H1H2H1H3H2H3的比较中存在显著差异(p<0.05)(表3)。其中,H3的MRF值 (10.30±4.27)N大于H1 ((9.69±3.28)N)和H2 ((8.72±2.76)N);ΔFH1H3的比较中存在显著差异(p<0.05),其中,H1的ΔF值 ((1.82±1.37) N)大于H3的ΔF值 ((1.70±1.48)N)(表4)。结果表明:被试在不同操作高度作业时,肩关节和肘关节角度的变化不同,H1的肩关节屈曲角和肘关节屈曲角的角度最大,H3的肩关节屈曲角和肘关节屈曲角的角度最小,因此疲劳程度也有所不同。余群舟等[14]研究表明:关节力矩越大,可用力矩越小,肢体越不舒适,人就越容易疲劳,此结论与本文观点一致。
3种操作面和3种操作高度进行皮尔逊相关分析,结果表明:只有MRF、ΔF 2组数据存在显著正相关关系,相关系数r=0.201(p<0.05),见表5
将手腕、手肘和肩部的PRE数据进行Shapiro-Wilk检验(图6),上肢肌肉疲劳程度主观评分符合正态分布(p<0.05),可重复测量方差分析。结果表明:手肘在操作面和操作高度上具有显著性差异(p<0.05),手腕和肩部在只在操作面上具有显著差异(p<0.05),在操作面和操作高度的交互效应,手腕、手肘和肩部都不存在显著差异。对数据进行两两比较,只有手肘RPE得分在操作高度H1H3的比较中具有显著差异(p<0.05),见表6

MET模型一般都是以负荷强度f构建预测模型,f (f =外部负荷/MVC)为影响肌肉疲劳的重要因素[7]。对f和MET数据进行相关系数分析(Correlation Coefficient,CORR),结果表明:f与MET显著负相关(p<0.05),Pearson相关系数为-0.55。现有的MET模型多数以指数函数、幂函数形式来构建[7-8]。因此,假设模型为:

式(1)和式(2)进行对数变换,可得以下公式:

将试验数据代入式(3)、式(4),通过一元线性回归分析,可得MET的回归方程,见表8
由于因变量MET与自变量f为显著负相关,表8 中式(6)为升函数,此方程不合理;式(7)的决定系数小于0.5(R2<0.5),f对MET的解释程度较低,因此,确定式(5)和式(8)为手过头姿势下钻孔作业的MET预测模型。
验证所建预测模型合理性,通过平均绝对偏差(Mean Absolute Deviation,MAD)和相对偏差(Relative Deviation,RD)比较MET的实际值与预测值之间的偏差,公式为:

式(5)和式(8)的MAD和RD值见表9
目前,在手过头作业姿势的研究中[3612],只研究不同作业姿势的肌肉疲劳分析以及各个操作姿势的疲劳程度,关于建立手过头作业的MET肌肉疲劳预测模型的相关研究较少。因此,选用现有的手部操作相关作业的MET预测模型进行合理性验证。
将手过头姿势下钻孔作业的试验数据代入现有MET模型进行对比,绘制MET-f曲线图,图中显示文中所建模型与现有模型具有相同下降趋势,MET 模型的MET-f曲线如图7所示。
将文中的MAD和RD数值与现有模型的MAD和RD比较,所建模型式(5)的偏差最小(幂函数:MAD=(0.66±0.38)min,RD=29.65%±11.24%,其次是式(8)(指数函数:MAD=(0.68±0.43),RD=29.65%±13.54%,MANENICA[11]提出的一般模型和手部/抓握模型偏差较小(MAD>5.1 min,RD>33%),原因可能是与文中试验的施力部位较为相同。与双手拉车作业模型[8]和AVIN等[16]提出的手部/抓握模型(MAD>9.4 min,RD>57%)以及单手拉车作业指数函数模型[7](MAD=(16.86±2.106) min,RD=72.511%±5.945%的偏差较大,原因可能是与该模型的作业姿势、施力方式差异较大。MA Liang等[15]的疲劳模型(MAD=(17.335±6.183)min,RD=92.12%±5.878%偏差最大,原因可能是此模型的疲劳速率k设定为1 min-1,但文中的k值是由工作任务决定的,所以会有较大偏差(表9)。由此可见:所建模型更加适合反映手过头姿势下钻孔作业过程中的肌肉疲劳发展情况。
为进一步验证文中建立的MET预测模型的合理性,将式(5)和式(8)代入真实试验数据,计算出在不同操作面与不同操作高度的MET实际值与预测值并进行比较。结果表明:在不同操作面中,式(5)和式(8)正面的误差值相比与其他操作面偏差大,原因是正面的MET疲劳程度研究数值最大,因此会有偏差;侧面和底面的误差值较小。在不同操作高度中,H1的误差值相比与其他操作高度偏差大,原因是H1的MET疲劳程度研究数值最大,因此,会有偏差;H2H3的误差值较小,见表10。因此,所建的MET预测模型适合反映手过头作业过程中肌肉疲劳发展情况。
1) 在手过头作业中,不同操作面显著影响MET、MRF以及ΔF,同时,也显著影响手腕、手肘和肩部的RPE;不同的操作高度显著影响MET、MRF以及手肘的RPE,对ΔF、手腕和肩部的RPE没有显著影响。同时,研究发现,采用正面操作并降低手臂上举高度能有效减轻肌肉疲劳。
2) 所建立的以指数函数和幂函数为基础的MET预测模型与其他MET模型相比,其预测值与实际值偏差最小,因此,模型可预测手过头作业过程中的肌肉疲劳情况,为预防产生工作相关肌肉骨骼疾病风险提供支持。
3) 文中根据3种操作面和3种操作高度进行9种手过头姿势下的钻孔试验,目前只建立了一种预测模型,后续会针对不同作业姿势建立不同的MET肌肉疲劳预测模型,使预测模型更具准确性。并且由于试验条件有限,所招募的被试人员均为男性大学生,与长期从事钻孔作业人员的年龄有一定差距,未来研究会进行相应改进。
