Coastal chemical industrial parks have frequently experienced lightning strikes,which seriously threaten production safety. A risk assessment method based on HCL methodology was proposed to assess Natech (Natural-technological) accidents triggered by lightning strikes on storage tanks in these parks. Firstly,ESD and Fault Tree Analysis(FTA) were employed to qualitatively analyze the evolution paths of Natech accidents caused by lightning strikes on storage tanks,providing a visual foundation for blocking the accident transmission process. Secondly,BN were used to quantitatively calculate human error probabilities and assess the hybrid causal relationships of lightning-induced storage tank accidents. Finally,HCl methodology was applied to visually deconstruct the complexity and uncertainty of Natech accidents caused by lightning strikes on storage tanks. The research results indicate that decision-making error is the primary risk source in the human error model. Organizational atmosphere,psychological state,poor working environment and inadequate supervision are identified as the main factors leading to frequent human errors. The lack of effectiveness of lightning protection facilities is recognized as the trigger in the accident chain of lightning strikes on storage tanks. To reduce the severity of risk scenarios,emphasis needs to be placed on strengthening the control of full-surface fires and pool fires.

沿海化工园区的雷击事故时有发生,对园区生产安全构成严重威胁。为评估化工园区雷击储罐引发的Natech事故,提出一种基于混合因果逻辑(HCL)的风险评估方法。首先,采用事件序列图(ESD)和故障树分析法(FTA),定性分析雷击储罐导致Natech事故的演化路径,为阻断事故传递过程提供可视化基础;其次,采用贝叶斯网络(BN)定量解算人因失误概率,评估雷击储罐事故的混合因果关系;最后,采用混合因果逻辑方法,实现可视化解构雷击储罐Natech事故的复杂性和不确定性。研究结果表明:决策失误是人因失误模型的首要风险源;组织氛围、心理状态、工作环境不佳及监管不力是导致人因失误频繁的主要因素;防雷设施有效性缺失是雷击储罐事故链的诱因;降低风险场景的严重性需要重点加强对全液面火灾和池火灾的管控。

Coastal chemical industrial parks have frequently experienced lightning strikes,which seriously threaten production safety. A risk assessment method based on HCL methodology was proposed to assess Natech (Natural-technological) accidents triggered by lightning strikes on storage tanks in these parks. Firstly,ESD and Fault Tree Analysis(FTA) were employed to qualitatively analyze the evolution paths of Natech accidents caused by lightning strikes on storage tanks,providing a visual foundation for blocking the accident transmission process. Secondly,BN were used to quantitatively calculate human error probabilities and assess the hybrid causal relationships of lightning-induced storage tank accidents. Finally,HCl methodology was applied to visually deconstruct the complexity and uncertainty of Natech accidents caused by lightning strikes on storage tanks. The research results indicate that decision-making error is the primary risk source in the human error model. Organizational atmosphere,psychological state,poor working environment and inadequate supervision are identified as the main factors leading to frequent human errors. The lack of effectiveness of lightning protection facilities is recognized as the trigger in the accident chain of lightning strikes on storage tanks. To reduce the severity of risk scenarios,emphasis needs to be placed on strengthening the control of full-surface fires and pool fires.

自然-技术(Natural-Technological,Natech)事故是指由自然灾害引起,并导致加工或储存化学物质的工业设施释放危险物质的技术事故[1]。引发Natech事故的自然灾害包括洪水、地震、火山爆发、海啸、雷电等[2]。通过调查工业事故数据库,发现雷击是各类化工园区储罐Natech事故的主要诱因之一[3]。因此,构建化工园区雷击储罐风险定性和定量分析相结合的评估模型,对深入研究该类事故发展机制,实现风险评估和改进现有管理策略具有重要意义。
以往对化工园区雷击储罐事故的研究主要侧重遭受雷击后设备的脆弱性分析和化工园区雷击事故的风险评估。2013年,NECCI等[4]应用蒙特卡罗法,确定了设备的期望雷击频率和设备损坏概率。次年,NECCI[5]提出一种用于识别和计算雷击对不同类别常压储罐影响结果的条件概率方法,为雷击事故设备脆弱性分析提供了技术支持。在化工园区风险评估模型研究方面,贝叶斯网络(Bayesian Network,BN)应用较为广泛。YANG Yunfeng[6]基于化学罐区雷电触发多米诺效应,采用BN模型辨识出最易受影响的设备。CHENG Yulong等[7]基于贝叶斯网络,构建了Natech事故风险评估模型,评估了雷电诱发衍生事件风险程度。上述研究大多聚焦于雷击事故后化工园区设备损坏的风险程度,而对于事故推演和因果推理等方面的研究相对有限,不利于风险事故全流程的预防决策。
据此,选取化工园区储罐雷击事故场景为研究对象,从情景推演角度出发,综合考虑雷击情景下化工园区储罐的脆弱性,构建雷击储罐混合因果逻辑(Hybrid Causal Logic,HCL)模型,进而分析事故的演化特性,探究斩断次生灾害传播链条的方法,以期优化应急资源部署策略。
基于HCl模型构建化工园区雷击储罐风险分析模型,HCL可为因果关系的识别提供一种工具,主要由3种方法构成[8],包括事件序列图(Event Sequence Diagram,ESD)、故障树分析(Fault Tree Analysis,FTA)以及BN模型,如图1所示。其中,ESD作为一种风险识别的经典图形方法,旨在表征特定场景后果的严重性[9-11]
ESD从初始事件(Initiating Event,IE)开始,识别IE是发展ESD的第一步。系统在确定IE之后的响应称为ESD中的关键事件(Pivotal Event,PE)。ESD中的最后一个事件称为结果事件(End States,ES),往往是事故或者状态,作为ESD的结束[10]。FTA表示导致事故的各因素之间逻辑关系,由逻辑门、基本事件(Basic Event,BE)、中间事件(Intermediate Event,IET)和顶事件(Top Event,TE)组成[12]。BN基于概率论,由节点及连接节点的有向边构成。节点之间依赖关系的类型和强度用条件概率来表征[13]。BN节点概率采用模糊数模型,具体语言变量与模糊数对应关系见表1[14]
雷击储罐事故场景以雷暴天气下雷击中外浮顶储罐顶部(IE1)和雷击中外浮顶储罐侧壁(IE2)展开,随后依次引入PE和ES,分别见表2表3,最终构建雷击储罐事件ESD,如图2所示。
在ESD诸多关键事件中,PE1、PE2和PE6等 3个事件的发生基于一系列可预测且可控的客观条件。在处理这些事件时,FTA展现出其显著的优势。
对于PE1,防雷设施失效通常分为未安装防雷设施和已安装防雷设施失效2种情况,针对后者通常可以从接地装置失效、避雷网/针失效和等电位连接失效3方面构建FTA1,如图3a所示。对于PE2,多数大型储油罐采用外浮顶结构,为保证浮盘的灵活性并防止油气泄漏,浮盘与罐体之间需要安装密封装置防止外部空气进入储罐,同时防止储罐内的气体逸出。参考程玉龙[15]对密封结构的研究,选取基本事件构建FTA2,如图3b所示。对于PE6,储罐区产生火源主要涉及3种情况:雷击火花、电气设备火花与储罐的静电火花。其中,雷击火花主要是雷电击中储罐后导致储罐局部产生热点。电气设备火花主要是由于储罐区的电气设备老化过载或者电气设备维护使用不当而产生热点。静电火花通常来自储罐自身的静电积聚以及人体静电而产生热点。据此构建FTA3,如图3c所示。
采用排序质心法(Rank-Order Centroid method,ROC)为FTA基本事件概率赋值,具体计算过程可以参考前人研究[16]
参考人为因素分析和分类模型(Human Factors Analysis and Classification System,HFACS)[17]选择PE4节点,据化工园区事故统计数据[1518-19]及化工园区储罐实际管理情况,结合雷击储罐事故特征和情景推演结果,得出化工园区HFACS模型,见表4
采用解释结构模型(Interpretative Structure Modeling,ISM)构建BN网络,通过专家经验判断节点之间关系的强弱,生成邻接矩阵B,作为ISM的输入,计算B,得到可达矩阵MM可划分为可达集合R(Si)与先行集合Q (Si),其中,Si代表Mi个要素。当R (Si) = [R (Si) ∩ Q (Si)]时,则Si代表的事件被划分为第一层。此时删除MSi所在行和列,并以此法计算第二层事件,最终确定后续所有事件的层级划分,分层有向图结果如图4所示。其中,圆圈代表节点,箭头代表节点间的关系,从箭头发出端到指向端代表影响和被影响节点。
案例选取沿海某化工园区,该园区有多座固定顶常压柴油储罐、外浮顶常压原油储罐、液化石油气储罐、液化天然气储罐和液氧储罐。其中,外浮顶常压原油储罐型号见表5
采用ROC定量计算FTA1、FTA2和FTA3各基本事件发生概率,以FTA2密封圈内气体到爆炸极限为例说明ROC的计算过程,FTA基本事件概率计算过程见表6,其中, ϕ i表示基本事件分配似然度,Pi为第i个事件发生的概率。最终FTA顶上事件概率见表7
采用模糊数形式表示人因失误模型根节点的先验概率和中间节点的条件概率。BN根节点的先验概率通过模糊数进行概率整合,根节点先验概率见表8。采用模糊数模型对各节点的条件概率进行输入,以BN2节点(组织过程缺陷)为例,条件概率计算见表9。将根节点先验概率和中间节点的条件概率输入GeNie4.0(BN模拟软件)建立化工园区人因失误BN模型,如图5所示。
1) 雷电击中外浮顶储罐的概率。雷电击中外浮顶储罐概率[20]计算过程如下:

式中:PL为雷电击中储罐的发生概率;ng为地面雷电密度,1/(km2·year);Ac为雷电捕获面积,m2;Td为该地区的年雷暴小时数,h/year;D为储罐的外径,m;rcm为地面平均吸引半径,m;H为储罐高度,m。
2) 雷击中储罐外浮顶的条件概率。雷击中储罐外浮顶条件概率[20]计算过程如下:

式中:PT为雷电集中储罐顶部的概率;D1为储罐外浮顶直径,m;rf为储罐外浮顶水平方向上受到防护的最大半径,m;HT为避雷针高度,m;hf为罐内液面高度,m。
最终获得雷电击中外浮顶的概率如下:

3) 固定式灭火系统有效概率。外浮顶储罐固定灭火系统作为一种重要的安全设施,用于保护外浮顶储罐免受火灾的威胁。其中,灭火系统失效概率参照文献[21],取 P P E 3 = 1-8.1⊆10-3 = 0.992,见表10
4) 雷电击穿储罐并发生泄漏的概率。当雷电击中储罐并击穿罐壁时,会导致罐内物质与外界环境直接接触,从而可能引发泄漏事故。雷电的高能量和瞬时性使得其对罐壁的破坏具有极大的不确定性和难以预测性。当雷电击穿罐壁,发生泄漏事故的概率[20]如下:

式中:Pd为雷电击穿概率;SL为暴露在雷电冲击下容器总表面中与介质接触的面积,m2;Stot为暴露在雷击下的容器总表面积,m2;t为储罐壁厚,m(取对数时无量纲化)。
最终计算获得该案例雷击击中外浮顶常压储罐的概率为0.013,其他事件节点发生概率见表11
通过融合ESD、FTA和模糊BN的特点,构建HCL风险评价模型,如图6所示。随后全面评估雷击引发的化工园区储罐的风险情景,并确定各事件链的发生概率,见表12
根据化工园区雷击储罐事故混合因果模型分析,共得出7条事件链。其中,防雷设施的有效性达到0.697,ESD各事件链揭示了防雷设施状态与事故发生存在联系。有效的防雷措施可以降低事故发生的风险,不足或失效的防雷设施则可能成为事故链的触发点。
固定灭火系统的有效概率为0.992,其高效运行是防止火灾蔓延的关键。在极端雷电环境下,灭火系统的电子元件有因雷击失效而无法自启动的可能性,因此增加了人为干预的必要性。人因失误概率为0.04,如果人员因失误未能及时介入,密封圈火灾可能升级为全液面火灾,尽管全液面火灾事件链发生可能性最低为7.3×10-8,但后果在诸多事故中最为严重。
最后,储罐泄漏的风险相对较低,为1.3 × 10-4。这反映在常规运行和维护条件下,储罐能够保持较高的安全性能,进而有效降低了泄漏事故发生的可能性。如果罐区出现不可控的热源引燃泄漏原油,泄漏演变成池火并可能进一步扩大事故后果。尽管这种情况的发生概率较低(2.9 × 10-5),但也不容忽视。
1) 基于ISM和HFACS优化BN模型,发现决策性失误发生概率最高,组织氛围、心理状态工作环境和监管违规是人因失误模型中发生频率较高的因素。
2) 防雷设施有效性在事件序列图中出现频率最高,其失效是诱发后续事故的关键。全液面火灾发生概率最低,其次是池火灾,两者是雷击储罐事故最为严重的风险场景。
3) 雷击储罐混合风险评估模型能够实现多灾种事件链分析,有助于快速理解雷击储罐事故风险状况和演化路径,为决策者提供直观的风险可视化工具。
