To address the issues of ambiguous multi-level,multi-link and multi-functional interaction relationships and the adverse coupling effects during operations in confined spaces,FRAM was introduced. Combined with ISM and AHP,the hierarchical structure and judgment methods were optimized and improved. By dividing the risk hierarchical structure,the impact of system function coupling variability was quantified,and the importance of functional units and hierarchical structures was calculated. Through the results of functional variability and coupling loss degree,the input-output phenotype of upstream and downstream functional changes were determined,and the coupling mechanism among system functional elements was clarified. The results show that by applying the risk prevention and control model for operations in confined spaces based on the improved ISM-FRAM-AHP,23 functional units and a 10-layer risk hierarchical functional network are obtained. The maximum values of functional variability and coupling loss degree are 4.36 (external environment F23) and 0.808 4 (formulating operation plans F2),indicating a relatively high operation safety risk degree. The functional changes are mainly manifested in sequence,goal and control. Four effective functional barrier measures,physical,symbolic,functional and invisible,are proposed for 8 failure links.

为解决有限空间作业过程多层次、多环节、多功能交互关系模糊和不良耦合影响的问题,引入功能共振分析模型(FRAM),结合解释结构模型(ISM)和层次分析法(AHP)优化改进层次结构和判断方式,通过划分风险层次结构,将系统功能耦合变异性的影响进行定量化,并计算功能单元和层级结构的重要度,通过功能可变性和耦合损失度结果,确定上下游功能变化输入输出表型,理清系统功能要素间耦合作用影响机制。结果表明:应用基于改进ISM-FRAM-AHP的有限空间作业过程风险防控模型,能够得到23个功能单元和10层风险层级功能网络;功能可变性数值和耦合损失度最大值为4.36(外部环境F23)和0.808 4(制定作业方案F2),作业安全风险程度较高,功能变化主要表现为顺序、目标和控制,针对8条失效链路设置物理、象征、功能、隐形4种有效屏障措施。

为解决有限空间作业过程多层次、多环节、多功能交互关系模糊和不良耦合影响的问题,引入功能共振分析模型(FRAM),结合解释结构模型(ISM)和层次分析法(AHP)优化改进层次结构和判断方式,通过划分风险层次结构,将系统功能耦合变异性的影响进行定量化,并计算功能单元和层级结构的重要度,通过功能可变性和耦合损失度结果,确定上下游功能变化输入输出表型,理清系统功能要素间耦合作用影响机制。结果表明:应用基于改进ISM-FRAM-AHP的有限空间作业过程风险防控模型,能够得到23个功能单元和10层风险层级功能网络;功能可变性数值和耦合损失度最大值为4.36(外部环境F23)和0.808 4(制定作业方案F2),作业安全风险程度较高,功能变化主要表现为顺序、目标和控制,针对8条失效链路设置物理、象征、功能、隐形4种有效屏障措施。

To address the issues of ambiguous multi-level,multi-link and multi-functional interaction relationships and the adverse coupling effects during operations in confined spaces,FRAM was introduced. Combined with ISM and AHP,the hierarchical structure and judgment methods were optimized and improved. By dividing the risk hierarchical structure,the impact of system function coupling variability was quantified,and the importance of functional units and hierarchical structures was calculated. Through the results of functional variability and coupling loss degree,the input-output phenotype of upstream and downstream functional changes were determined,and the coupling mechanism among system functional elements was clarified. The results show that by applying the risk prevention and control model for operations in confined spaces based on the improved ISM-FRAM-AHP,23 functional units and a 10-layer risk hierarchical functional network are obtained. The maximum values of functional variability and coupling loss degree are 4.36 (external environment F23) and 0.808 4 (formulating operation plans F2),indicating a relatively high operation safety risk degree. The functional changes are mainly manifested in sequence,goal and control. Four effective functional barrier measures,physical,symbolic,functional and invisible,are proposed for 8 failure links.

近年来,有限空间作业事故高发频发,给人民群众的生命财产安全造成重大损失[1],据统计,仅2013—2022年,工贸企业就发生有限空间较大事故95起,造成357人死亡[2],安全生产形势异常严峻。由于有限空间作业风险具有隐蔽性和突发性,其作业环境复杂[3]、流程繁琐[4]、环节间耦合影响大[5]、事故防范难度大[6],因此,研究并构建一套有限空间作业全过程风险防控策略,对进一步强化有限空间作业安全管理、有效遏制有限空间作业安全风险具有重要意义。
目前,学者们多从风险评估角度开展研究。汪圣华等[7]构建了以危险气体体积分数为关键指标的有限空间作业动态风险综合评估模型;宋思雨[8]、杨春丽[9]等分别采用解释结构模型(Interpretative Structural Modeling,ISM)和层次分析法(Analytic Hierarchy Process,AHP),从有限空间作业中毒事故和气体危害因素视角分析作业风险的影响因素;LUCIA等[10]设计了一种有效结构化程序,评估和管理有限空间中的工作活动风险;SELMAN等[11]总结出安全进入有限空间开展紧急救援工作的程序框架,提出保护等级和救援等级的概念;DAMIEN等[12]提出基于风险管理标准的有限空间5步风险评估工具,分析并识别了有限空间作业期间内的风险影响因素。近年来,功能共振分析模型(Functional Resonance Analysis Method,FRAM)在复杂社会技术系统分析中起到至关重要的作用,其应用旨在揭示潜在的系统故障或特定工作领域的关键基础元素[13]。HOLLNAGEL等[14]提出以航空事故为例,从安全Ⅱ视角出发结合系统的非线性和可变性分析事故致因;高扬等[15]结合AHP法对公务航空事故进行功能共振量化分析;陈芳等[16]利用蒙特卡罗方法改进功能共振事故模型,分析了航空进近管制系统安全;胡甚平等[17]引入集对分析方法确定了12条不安全行为形成链接的FRAM。现有研究多从事故致因出发,集中于事故类型特征和影响因素分析,对涉及多环节的复杂流程系统关注较少,缺乏对有限空间作业过程风险防控的定量分析研究,难以有效遏制安全生产事故的发生。
鉴于此,笔者拟引入改进FRAM模型,构建有限空间作业流程FRAM系统风险层级功能网络,定量化系统功能耦合变异性影响,理清系统功能要素间耦合作用影响机制,以期改善有限空间作业过程多层次、多环节、多功能交互关系模糊和不良耦合影响的问题。
有限空间作业流程主要划分为4个阶段:作业审批阶段、作业准备阶段、安全作业阶段、作业完成阶段[18]。有限空间作业流程风险防控各阶段如图1 所示。
FRAM基于随机共振理论发展而来,不局限于传统的系统结构解析、致因要素分析以及事故发生过程中的场景和事件识别形式,而是强调系统因素间的紧密耦合作用使系统表现出动态非线性特征,从完整系统的功能特征角度剖析事故或识别动态系统中的风险因素,在众多风险因素中识别出薄弱环节,从而预估和限制系统功能共振。利用图2所示的六角形模型分析,该模型包括输入I、输出O、控制C、资源R、前提P、时间T这 6个系统功能参数。在应用功能共振分析方法时,一般采取下列4个步骤:①识别和描述该系统的基本功能;②分析所有功能单元的潜在变化;③绘制网络图,分析功能共振;④制定功能屏障措施。笔者将重点优化改进第②、③步方法及定量分析研究。
根据分析得到的功能单元F2F21,以作业审批F4为例,其FRAM功能参数描述见表1
考虑完整的有限空间作业流程还包括作业申请(起始)、清场验收及作业资料归档(结束)2个关键环节,因此,补充到该FRAM模型功能单元中,分别命名为F1F22。有限空间作业环境多为封闭或半封闭式,自然通风条件不良,极易产生H2S、CO等有毒有害气体或其他危险源。夏季高温季节,有限空间作业风险明显增加,作业环境温度和湿度直接对作业人员的生理和心理状态造成不良影响,异常的作业环境会导致中毒窒息等事故发生率进一步增加。因此,增加外部环境F23,外部环境影响包括温度、空气湿度、氧含量、有毒气体浓度等。综合全部23个功能单元,构建有限空间作业流程FRAM模型系统功能网络,如图3所示,该功能网络包括作业申请及外部环境、作业审批、作业准备、安全作业、作业完成和作业结束5个阶段,且直观展现了有限空间作业全过程系统多环节、多要素之间的交互关系。
FRAM系统风险路径识别高度依赖专家主观判断,难以清晰表达系统的层次结构和功能相互影响关系。ISM可将复杂系统的要素关系进行层级和结构划分,建立多级递阶层次结构模型。AHP能提供综合判断并能够得出数量分析结果的多目标决策分析方法。引入ISM方法划分FRAM系统功能共振网络图层次结构,计算各层级结构重要度,基于系统功能单元上下游联系和要素间相互影响关系构建多级递阶层次结构模型。在结构模型的基础上,引入AHP法从时间和精度2种表型描述功能单元变化特征,利用AHP法计算输入端和输出端的表型权重,根据重要度排序确定输入和输出的主要表型。改进ISM-FRAM-AHP的风险防控分析方法程序如图4所示。
1) 明确FRAM分析目的,识别系统正常运行所必需的功能要素。确定FRAM功能单元,详细描述基本功能参数,功能单元编号为Fi,功能单元数量为N F i { F 1 F 2 F i F N } 根据功能参数 6个属性的特征描述,分析系统内功能的相互联系并依次连接,绘制功能共振网络图。
2) 建立邻接矩阵A,综合分析FRAM功能共振网络图和各要素间相互影响关系。确定功能单元关系有向图,应用N阶矩阵 A = [ a i j ]描述功能单元间的内在联系。若 a i j取1,则表示上游功能单元Fi输出对下游功能单元Fj有直接影响关系;反之,若 a i j取0,则表示功能单元Fi对功能单元Fj没有影响关系。
3) 计算可达矩阵M,构建多级递阶层次结构模型。利用邻接矩阵A进行迭代布尔运算,可得可达矩阵M,其中,I为单位矩阵,n为迭代运算次数,见下式:

提取可达矩阵M可求解得到可达集 R ( F i )、先行集 Q ( F i )和交集共同集 T ( F i ) = R ( F i ) Q ( F i )。以可达矩阵为准则划分各系统要素所处层级,当 R ( F i ) = T ( F i )时,Fi被确定为第1层要素,然后将第1层要素从要素集中去除,以此类推,在剩余要素集重复上述步骤,直至所有要素归于各层次,将每层要素按照从高到低的顺序排列,得到该系统所有要素的层次结构。最终根据已有层次结构,绘制ISM-FRAM递阶层次结构模型图。
4) 识别系统功能变化,计算功能结构重要度和功能单元可变性。基于ISM-FRAM递阶层次结构模型图可计算各个层级的结构重要度,假设系统层级数量为mL表示每一个层级, L { 1,2 m },则该模型各个层级权重为ML,见下式:

根据层级结构重要度定义功能节点出入度,指向待计算的功能为入度功能,由待计算功能指向其他功能为出度功能,同一个功能单元的所有输入和输出端口具有相同的功能结构重要度。定义待计算功能单元i所处层级权重为 M L i,则 M L j i表示指向i功能单元的j功能所处层级的权重, M L i k表示i功能单元指向k功能端口所处层级的权重,因此,i功能单元结构重要度表示为Ri,见下式:

5) 利用简单方法描述系统功能变化特征,刻画功能输出在时间和精度2方面变化。功能之间如何耦合以及这种耦合关系如何引发不期望的结果直观表现为行为变化的特征,较之功能自身的变化,FRAM更关注功能输出端的变化程度。通过量化评分来表征功能单元的输出变化程度,在时间方面的表征分别是过早、准时、过晚和不发生;在精度方面的表征分别为精确、可接受和不精确,上游输出是否精确直接影响下游是否需要调整。设置功能可变性评分见表2,根据功能单元输出的实际变化情况对输出变化表型评分,得分越高表示上游输出的功能可变性变化程度越高。功能单元i时间方面的评分为 F i T T ( 1,3 3,4 );精度方面的评分为 F i PP∈(1,2,3)。
引入AHP法定量分析正常运行的FRAM系统功能,刻画各功能单元输出变化程度概率。基于AHP法步骤,采用专家打分法计算各因素权重,同样重要、稍微重要、明显重要、强烈重要、极端重要分别用1、3、5、7、9表示。将功能单元输出时间和精度2方面的可变性表征进行两两比较,根据行业专家反馈对重要程度打分,结合判断矩阵意见分别计算出功能单元i时间变化概率 w i t T和精度变化概率 w i p P。因此,定义功能单元功能i的功能可变性为Gi,见下式:

根据功能单元结构重要度Ri和功能可变性Gi,可定义功能单元i的耦合损失度Bi,表达功能单元耦合变异性损失程度,见下式:

6) 利用复杂方法(故障模式的概念)分析上下游功能耦合关系,刻画如何从功能输出或结果的角度分析功能变化表型。利用AHP法分别对上游功能单元输出的4种表型,包括力量/距离/方向(E)、时机/持续时间(D)、顺序(S)、目标(H),以及下游功能输入端口的5种表型进行权重组合计算。在系统输入功能端口方面,权重最高的表型更有可能影响下游的功能耦合变异,若上游功能输出中权重较高的表型与下游功能输入中权重较高的表型相连接,系统就可能发生不期望的后果。因此,通过ISM-FRAM-AHP组合分析,找出连接系统功能输入和输出端口耦合变异性主要表型的链路即为失效链路,进一步确定导致系统发生功能共振的关键影响因素。
7) 分析ISM-FRAM-AHP系统功能共振结果。针对系统中实际发生的功能耦合变化影响制定相应的屏障措施,提出优化改进建议,抑制或消除失效链接影响,确保系统功能正常运行。
根据有限空间作业过程风险防控功能要素关系进行级间划分,得到功能单元递阶层次结构模型,该模型如图5所示。
利用功能共振分析可视化工具(FRAM Model Visualiser,FMV),结合图3图5将有限空间作业流程FRAM模型系统功能网络转化为有向图,通过ISM划分风险层次结构,其中,第1层为风险表象层,第10层为风险根源层,改进ISM-FRAM-AHP系统风险层级功能网络如图6所示。
将改进FRAM递阶层次结构模型代入式(2),得到层级结构重要度,见表3。结合层级结构重要度和功能节点出入度,根据式(3)计算得到功能单元结构重要度Ri,见表4。由图6风险根源层可知:当有限空间作业申请开始并正常运行时,在安全交底之前作业相关人员的安全职责已明确,而有限空间作业位置、空间和环境已存在并确定,这会对有限空间作业风险造成根源性质影响。第9层功能网络中,制定作业方案F2和作业审批F4组成有限空间作业过程的关键上游环节,易引发下游功能单元变异,其工作重点是明确有限空间数量、位置和危险有害因素等基础情况,建立有限空间管理台账。F5F14分别单独构成功能网络第7层和第8层,安全交底和人员防护作为安全作业前的准备阶段关键环节、控制风险层级上下游链接的枢纽位置,对较多功能单元的影响明显,其执行对象是针对实施作业相关的全体人员并要求签字确认。
邀请有限空间作业一线负责人员分别对功能单元输出的时间和精度可变性打分,通过AHP法步骤计算得到功能单元时间和精度变化概率,变化概率如图7图8所示。将 w i t T w i p PRi分别代入式(4)和式(5),得到功能单元可变性Gi和耦合损失度Bi,功能可变性和耦合损失度如图9所示。同理,按照打分标准对不同上游功能的输出和下游功能的输入端打分,根据打分结果结合AHP法计算得到表型权重,确定功能单元输入和输出端的主要表型,见表5表6,其中,输出表型在图6中以输出端口的形式表示。系统功能共振源自上下游功能的耦合,对于输入端而言,权重最大的表型发生变异的可能性最大,最可能引起下游功能的波动,在有限空间作业系统风险层级功能网络传递过程中某些关键链接就可能由正常状态转变为失效状态,从而导致事故发生。
图9可知:上游功能单元F2F3F4F5F6F23和下游功能单元F11F15F18的功能可变性数值和耦合损失度较大,说明上述功能单元发生耦合变异的风险程度较高。通过总结大量有限空间事故案例调查报告,结合功能可变性和耦合损失度结果可得8条可能造成事故风险的失效链路(图6)。其中带有“双斜线”的关键链路表示发生有限空间作业安全生产事故时与该作业过程环节正常功能存在偏差的失效链接,带有“×”的关键链路表示在已经发生事故的情况下导致断裂的功能链接。综合分析可知:
1) 外部环境功能单元F23的变化不易观测,输出时间和精度变化差异较大且主要表型为持续时间,但功能可变性数值最大(4.36),其输出一旦发生变异,会引起全局系统产生耦合变异效应,直接导致功能链接F23(O)→F18(I)发生断裂,作业人员被迫紧急撤离有限空间,面临中毒和窒息等较大事故风险,其中,H2S、CO中毒事故约占事故总量的80%。
2) 功能单元F2耦合损失度最大(0.8084),其安全风险辨识和隐患排查治理不到位,作业方案无法满足有限空间作业安全需要,未经功能单元F4审批擅自开展作业内容,即功能链接F2(O)→F5(I)和F4(O)→F5(I)变异失效,导致下游功能单元F5在安全交底中难以保证作业内容、作业安全要求、作业过程中可能存在的安全风险和应急处置措施等正确信息并做出错误判断,表现为功能参数控制C失效,对下游功能造成较大事故风险。功能链接F5(O)→F6(I)失效,功能单元F6输出主要表型S、H变异,会使安全防护设备的检查顺序混乱甚至遗漏检查目标,对下游功能造成较大不良影响。
3) 功能单元F11作为先通风、再检测、后作业的初始环节,必须保证持续通风时间D,若该环节缺失或不能满足现场通风要求,会使功能链接F11(O)→F15(P)失效,无法满足安全作业前提。
4) 功能单元F14失去功能参数控制C效果,功能链接F14(O)→F15(I)发生变异并失效,导致F15作业人员没有根据作业环境选择并佩戴符合要求的个体防护用品与安全防护设备,表现为面临较大事故风险或中断作业顺序S。受上游功能共振影响,在有限空间作业中发生生产安全事故时,会使功能链接F15(O)→F18(I)失效并断裂,但由于功能链接F17(O)→F18(R)同时断裂,现场监护人员盲目施救或应急救援措施不当,极易引起事故后果扩大。值得注意的是,F17作业监护疏忽,使作业异常信息传递出现偏差,导致F15不能做出准确判断,造成F18资源R失效,不能及时发出撤离警报信息并采取安全措施协助作业人员撤离,形成风险作业—紧急撤离—盲目施救—扩大事故的恶性循环事故链。
根据上述ISM-FRAM-AHP系统功能耦合分析结果发现,应进一步重视有限空间作业系统功能中人为因素较大的薄弱环节,防止由于系统功能共振导致的功能链接变异失效,预防不良影响超出该有限空间作业过程可承受的安全风险而导致的事故。因此,针对功能耦合综合分析的突出问题从最上游功能采取有效功能屏障措施(表7),达到有限空间作业过程风险防控最佳效果。
1) 改进ISM-FRAM-AHP模型能够得到有限空间作业全过程系统23个功能单元和10层风险层级功能网络的耦合交互关系。
2) 功能可变性数值和耦合损失度结果最大值分别为4.36和0.808 4,作业环节安全风险程度较高,功能变化输入输出主要表现为顺序、目标和控制。
3) 通过耦合变异综合分析得出8条功能失效链路,明确可能造成事故风险的功能耦合不良影响,针对有限空间作业事故的关键原因,从最上游功能设置物理、象征、功能、隐形4种有效屏障措施。
