To prevent and treat the early hearing loss of firefighters caused by occupational noise sources,the occupational noise exposure detection for firefighters was conducted,and the degree of hearing loss in firefighters was evaluated. The ratio of hearing loss in firefighters was quantified,and the relationship between the hearing test results of firefighters and occupational noise exposure was analyzed. The results indicate that the intensity of the noise source for firefighters exceeds the exposure limit of noise levels in the workplace of 85 dB(A). Among the hearing screening results of 50 firefighters,the number of people who falisd the distortion product otoacoustic emission hearing screening in both ears is 21,accounting for 42.0%,and the pass rate is significantly lower than that of normal individuals. The number of people with left/right ear pure tone audiometry hearing thresholds ≥ 26 dB is the greatest at 6 kHz,accounting for 30.0% and 26.0% at n =15 and n =13,respectively,suggesting high-frequency hearing loss in firefighters after occupational noise exposure.

为防治职业噪声源对消防员早期听力损失,开展消防员职业接触噪声检测,评估消防员听力损失程度,量化消防员听力损失比率,进一步分析消防员听力检测结果与职业噪声暴露关系。结果表明:消防员接触的噪声强度超过作业场所噪声等级接触限值85 dB(A);对50名消防员的听力筛查结果中,噪声暴露后畸变耳声发射(DPOAE)听力筛查双耳未通过人数21人,占比为42.0%,通过率较正常人明显降低;纯音测听左/右耳听阈≥26 dB人数在6 kHz最多,人数在15和13时,占比为30.0%、26.0%;消防员职业噪声暴露高频环境中,听力损失明显。

为防治职业噪声源对消防员早期听力损失,开展消防员职业接触噪声检测,评估消防员听力损失程度,量化消防员听力损失比率,进一步分析消防员听力检测结果与职业噪声暴露关系。结果表明:消防员接触的噪声强度超过作业场所噪声等级接触限值85 dB(A);对50名消防员的听力筛查结果中,噪声暴露后畸变耳声发射(DPOAE)听力筛查双耳未通过人数21人,占比为42.0%,通过率较正常人明显降低;纯音测听左/右耳听阈≥26 dB人数在6 kHz最多,人数在15和13时,占比为30.0%、26.0%;消防员职业噪声暴露高频环境中,听力损失明显。

To prevent and treat the early hearing loss of firefighters caused by occupational noise sources,the occupational noise exposure detection for firefighters was conducted,and the degree of hearing loss in firefighters was evaluated. The ratio of hearing loss in firefighters was quantified,and the relationship between the hearing test results of firefighters and occupational noise exposure was analyzed. The results indicate that the intensity of the noise source for firefighters exceeds the exposure limit of noise levels in the workplace of 85 dB(A). Among the hearing screening results of 50 firefighters,the number of people who falisd the distortion product otoacoustic emission hearing screening in both ears is 21,accounting for 42.0%,and the pass rate is significantly lower than that of normal individuals. The number of people with left/right ear pure tone audiometry hearing thresholds ≥ 26 dB is the greatest at 6 kHz,accounting for 30.0% and 26.0% at n =15 and n =13,respectively,suggesting high-frequency hearing loss in firefighters after occupational noise exposure.

噪声性听力损失是全球范围内常见的职业健康问题。目前,我国大约有1 000万职工在噪声超标的环境下工作,其中,约有100万人患有不同程度的职业性耳聋[1]。消防员的职业接触噪声大多超过接触限值85 dB(A)[2],其职业噪声暴露(Occupational Noise Exposure,ONE)问题不容忽视。据统计,近30%的消防员出现了轻至重度听力损失,54%的消防员出现了听力模糊、耳鸣等噪声性耳聋相关症状[3]。当前,受环境、工作模式和突发事件不可预测性等因素影响,消防员的职业噪声暴露数据很难准确获得[4]。因此,研究职业噪声源对消防员听力损失的影响,是亟需开展的课题。
噪声对听觉系统的影响机制包括机械损伤[5]、氧化应激[6]、钙超载[7]等,这些因素会损害耳蜗毛细胞,毛细胞一旦遭受损害将不可再生,因此,高强度噪声造成的听力损失是永久性的、不可逆的。消防员长时间现场救援作业时,各种抢险救援器械产生的摩擦、转动、撞击等机械性噪声可能导致听觉疲劳,甚至听力损失等[8]。有研究结果显示,消防员听力损失患病率为27.95%[9],且部分人员高频听阈升高[10]。长期接触噪声的消防员听力损失患病率远高于普通人群,且消防员听力损失的增加与ONE时间成正比,高频听力损失与ONE相关[11]。噪声性听力损失不仅影响消防员参与救援时的战斗力,对其身心健康也产生负面影响[12-13]。然而,当前开展消防员职业接触噪声暴露对听力损失的研究还较为鲜见。
鉴于此,笔者拟依据《噪声职业病危害风险管理指南》[14],开展消防员职业接触噪声检测,并评估8h等效声级(LEX,8 h)≥80 dB(A)的岗位噪声职业健康风险;同时,检测消防员听力,分析消防员听力阈值与工龄、岗位和职业噪声源接触累积时间的相关性,并分析职业接触噪声对消防员听力损失的影响,以期为消防员听力损失防治提供一定的参考。
依据《噪声职业病危害风险管理指南》[14],使用电气噪声测试仪检测消防员职业接触噪声。选择A计权S慢速档位,测试范围为50~130 dB(A),测试精度为±1.5 dB。检测仪检测点包括:①距噪声设备(高压水枪、无齿锯、电动无齿锯、机动链锯)1m处;②操作者持噪声设备作业,距离操作者耳部10cm处。测试时间为3min,每个测试点测试3次,取平均值。分别按照时间规格为8h及1 h的时间加权强度计算一天8h等效声级(LEX8h)及一天1h等效声级(LEX,1h)。
采用自动耳声发射测试仪,在本底噪声≤45 dB(A)的安静房间内对消防员作畸变耳声发射(Distortion Product Otoacoustic Emission,DPOAE)检查。各频率 DPOAE 信号信噪比、通过情况均由设备自动记录导出。
根据《声学校准测听设备的基准零级第1部分:压耳式耳机纯音基准等效阈声压级》(GB/T4854.1—2004),使用纯音听力计进行纯音测听检查,本底噪声≤45 dB(A),受检者佩戴降噪耳罩,测试频率为 0.5、1.0、2.0、3.0、4.0、6.0和8.0 kHz。以任一频率纯音测听气导听阈≥26 dB为听力下降[15]
采用 SPSS24.0统计软件处理噪声检测的数据和DPOAE听力筛查结果及纯音听力检测获得的阈值,并采用方差分析、卡方检验等进行统计分析,结果为P<0.05,说明有统计学意义。
4种设备(高压水枪、无齿锯、电动无齿锯、机动链锯)产生的噪声测定数据结果见表1。根据噪声作业的暴露时间(全天噪声等效声级LAeq,T、一天8h等效声级LEX,8h、一天1h等效声级LEX,1h)等效声级计数并计算8h超标率及1h超标率,得到无齿锯、电动无齿锯及机动链锯LEX,8h均超作业场所噪声等级接触限值85 dB(A),超标率分别为 17.92%、5.25%、35.30%。无齿锯及机动链锯LEX,1h均超作业场所噪声等级接触限值94 dB(A),超标率分别为7.56%、23.27%。消防站非稳态噪声等效声级及超标占比见表2。消防站3种职业噪声源等效声级及超标率如图1所示。
50名消防员均接受DPOAE听力筛查,左耳未通过人数24人,占比48.0%,右耳未通过人数28人,占比56.0%,双耳未通过人数21人,占比42.0%。
50名消防员均接受标准的纯音测听检查,消防员左/右耳纯音测听听阈≥26 dB人数及占比见表3。消防员纯音测听听阈≥26 dB与各频率关系如图2所示。检测频率为0.5、1、2、3、4、6、8 kHz,可见:在3、4、6、8 kHz频率即高频区双耳听阈提高明显,其中,左耳及右耳听阈≥26 dB人数均在6 kHz最多,人数在15、13时,占比分别为30%及26%。
设消防员人数为n,将消防员按照入职年限时长分为<1 a(n=31)、1~3 a(n=10)和>3 a(n=9)3组。进一步分析各组平均听阈,3组听力阈值中,1~3 a组听阈阈值最高。各组左耳6 kHz听阈均值均≥26 dB,如图3所示。
近年来,我国职业危害因素形式多种多样,职业病的发病率不断升高,所造成的损害也日益严峻,其中,职业性耳鼻喉疾病高居第3大类,呈上升趋势,其中,以职业性噪声聋发病人数最多[16]
消防员在消防应急救援工作及职业训练中暴露于广泛的有害噪声源中,通过量化消防员职业噪声源声压水平,发现职业暴露下3种噪声源均超接触限值85 dB(A)。尽管噪声接触时间有限,消防员应重视其职业噪声暴露风险,加强噪声相关知识宣教,积极采取噪声防护措施。
消防员听力检测结果显示:消防员职业噪声暴露后DPOAE筛查通过率远低于正常检出率,在听力损伤的早期,DPOAE的敏感性优于纯音测听,能早期发现内耳损伤[17]。因此,基于 DPOAE 的敏感性,可将其作为消防员职业噪声性聋的早期监测手段。
纯音测听结果表明:消防员左/右耳听阈≥26 dB人数在6 kHz频率即高频阈值最多,因此,对于已经达到职业病诊断要求时,内耳损伤时间已经较久的消防员来说,仍需通过纯音测听检测确定其真实听力状况,以初步确认听力损失程度。
因此,定期完善消防员DPOAE及纯音测听检测,能够早期发现消防员职业噪声源暴露造成的听力损失,进而早治疗、早预防,从而降低消防员的职业噪声性听力损失。
1) 消防员虽然接触职业噪声源(无齿锯、电动无齿锯和机动链锯)的时间较短,但通过时间加权,消防员仍超过国家噪声等级接触限值,职业暴露不容忽视。
2) 消防员DPOAE筛查通过率明显降低,应采用DPOAE早期筛查消防员听力。
3) 消防员采用纯音测听发现,双耳高频听阈受损,左/右耳听阈≥26 dB在6 kHz人数最多,应用纯音测听是监测职业噪声暴露后消防员听力变化的重要手段之一。
