Objective

To study the characteristics of gait behavior in a mouse model of chronic ankle instability and provide a reference for the study of the mechanism of chronic ankle instability as well as drug screening and evaluation.

Methods

Thirty C57BL/6J male mice were randomly divided into a control group (n=15) and an injury group (n=15). In the control group, the ankle joint underwent sham operation, and in the injury group, the anterior talofibular ligament and calcaneofibular ligament of the left ankle joint were transected. Gait parameters were analyzed in each group using TreadScan passive gait analysis system.

Results

Compared with the control group, the injury group showed a 28.43% increase (P<0.05) in average standing time and a 23.07% increase (P<0.05) in the percentage of standing time, whereas the average swing time and the percentage of swing time were shortened by 50.63% (P<0.001) and 19.75% (P<0.01), respectively. The average braking time and average stride time in the injury group were also shortened by 18.37% (P<0.01) and 37.86% (P<0.001), respectively. The injury group exhibited a decrease in step length, anterior-posterior step width, and mediolateral step width by 36.96%, 13.66%, and 8.10%, respectively. The total movement speed and instantaneous speed decreased by 8.05% and 11.12%, respectively, while the stride frequency increased by 51.41%. The average footprint area and average maximum standing area decreased by 8.8% and 13.24%, respectively, and foot pressure decreased by only 3%. The plantar pressure distribution in the injury group was uneven, with a more obvious decrease in plantar pressure in the hindfoot, especially a 13.92% decrease in plantar pressure in the right posterior quadrant.

Conclusions

Mice with chronic ankle instability adopt a more conservative walking pattern during the motion, reducing movement volume and amplitude to improve coordination and stability during walking.

目的

探究慢性踝关节不稳小鼠模型步态行为特点,为慢性踝关节不稳的机制研究以及治疗效果评估提供参考。

方法

选取30只C57BL/6J雄性小鼠随机分为对照组(n=15)和损伤组(n=15)。对照组左侧踝关节做假手术,损伤组左侧踝关节同时离断距腓前韧带和跟腓韧带。使用TreadScan被动步态分析仪分析各组步态参数。

结果

与对照组相比,损伤组平均站立时间以及站立时间百分比分别上升28.43%(P<0.05)和23.07%(P<0.05),而平均摇摆时间和摆动时间百分比分别缩短50.63%(P<0.001)和19.75%(P<0.01)。损伤组平均制动时间和平均跨步时间也分别缩短18.37%(P<0.01)和37.86%(P<0.001)。损伤组步长和前、后步宽分别缩短36.96%、13.66%和8.10%。损伤组运动总速度和瞬时速度分别下降8.05%和11.12%,步频反而上升51.41%。损伤组平均足印面积和平均最大站立面积分别下降8.8%和13.24%,足压仅下降3%。损伤组足底压力分布不均匀,后足足底压力下降更为明显,尤其是右后象限下降13.92%。

结论

慢性踝关节不稳小鼠在运动过程中会采取更加保守的步行模式,通过减少运动量和降低运动幅度以提高步行的协调性和稳定性。

目的

探究慢性踝关节不稳小鼠模型步态行为特点,为慢性踝关节不稳的机制研究以及治疗效果评估提供参考。

方法

选取30只C57BL/6J雄性小鼠随机分为对照组(n=15)和损伤组(n=15)。对照组左侧踝关节做假手术,损伤组左侧踝关节同时离断距腓前韧带和跟腓韧带。使用TreadScan被动步态分析仪分析各组步态参数。

结果

与对照组相比,损伤组平均站立时间以及站立时间百分比分别上升28.43%(P<0.05)和23.07%(P<0.05),而平均摇摆时间和摆动时间百分比分别缩短50.63%(P<0.001)和19.75%(P<0.01)。损伤组平均制动时间和平均跨步时间也分别缩短18.37%(P<0.01)和37.86%(P<0.001)。损伤组步长和前、后步宽分别缩短36.96%、13.66%和8.10%。损伤组运动总速度和瞬时速度分别下降8.05%和11.12%,步频反而上升51.41%。损伤组平均足印面积和平均最大站立面积分别下降8.8%和13.24%,足压仅下降3%。损伤组足底压力分布不均匀,后足足底压力下降更为明显,尤其是右后象限下降13.92%。

结论

慢性踝关节不稳小鼠在运动过程中会采取更加保守的步行模式,通过减少运动量和降低运动幅度以提高步行的协调性和稳定性。

Objective

To study the characteristics of gait behavior in a mouse model of chronic ankle instability and provide a reference for the study of the mechanism of chronic ankle instability as well as drug screening and evaluation.

Methods

Thirty C57BL/6J male mice were randomly divided into a control group (n=15) and an injury group (n=15). In the control group, the ankle joint underwent sham operation, and in the injury group, the anterior talofibular ligament and calcaneofibular ligament of the left ankle joint were transected. Gait parameters were analyzed in each group using TreadScan passive gait analysis system.

Results

Compared with the control group, the injury group showed a 28.43% increase (P<0.05) in average standing time and a 23.07% increase (P<0.05) in the percentage of standing time, whereas the average swing time and the percentage of swing time were shortened by 50.63% (P<0.001) and 19.75% (P<0.01), respectively. The average braking time and average stride time in the injury group were also shortened by 18.37% (P<0.01) and 37.86% (P<0.001), respectively. The injury group exhibited a decrease in step length, anterior-posterior step width, and mediolateral step width by 36.96%, 13.66%, and 8.10%, respectively. The total movement speed and instantaneous speed decreased by 8.05% and 11.12%, respectively, while the stride frequency increased by 51.41%. The average footprint area and average maximum standing area decreased by 8.8% and 13.24%, respectively, and foot pressure decreased by only 3%. The plantar pressure distribution in the injury group was uneven, with a more obvious decrease in plantar pressure in the hindfoot, especially a 13.92% decrease in plantar pressure in the right posterior quadrant.

Conclusions

Mice with chronic ankle instability adopt a more conservative walking pattern during the motion, reducing movement volume and amplitude to improve coordination and stability during walking.

慢性踝关节不稳是常见的骨关节损伤,特征为复发性扭伤、慢性疼痛和关节活动受限,患者在运动时会出现姿势平衡控制下降和步态改变[1-4]。因此,步态分析成为评估踝关节不稳严重程度及研究其行为变化的重要工具[5-6]
尽管步态分析最初是为人类开发,啮齿类动物的步态研究在建立动物模型、药物筛选及病理机制探索等领域同样具有重要意义。例如,通过观察药物对模型小鼠步态的改善,可作为药物疗效评估的依据之一,从而有效促进药物研究实验室研究向临床转化[8-10]。由于啮齿类动物往往具有掩盖残疾和疼痛反应的本能特性,尽管其基础步态行为模式与人类存在一定的相似性,但其步态补偿现象仍难以被精准检测[11-12]。目前针对踝关节不稳小鼠模型的步态分析研究较少,难以形成可用于治疗评估的基线参考依据。随着步态分析技术的改进,通过高速相机捕捉步态信息和图像处理技术相结合的方法,使小型动物的步态分析研究得以广泛开展[13-14]
在既往研究中,踝关节不稳造模方法可分为两类:①通过反复机械扭伤踝关节诱导关节不稳,但难以评估踝关节损伤严重程度,且反复扭伤易诱发小鼠下肢骨折[15]。②离断踝关节韧带诱导关节不稳,常选的韧带有距腓前韧带、跟腓韧带、三角韧带和外侧副韧带[16-18]。其中,离断距腓前韧带和跟腓韧带在踝关节不稳模型中使用最广泛,造模效果稳定,并且在本课题组既往研究中也曾被多次使用。因此,本文选择离断距腓前韧带和跟腓韧带诱导踝关节不稳模型。在严格保证实验环境和实验参数均与既往研究保持一致的前提下,通过扩大动物样本数量以进一步提高模型重复性和结果可靠性。
通过离断距腓前韧带和跟腓韧带可诱导小鼠产生慢性踝关节不稳,该模型虽具有病程短、成本低和表型稳定等优点[18],但目前针对其步态分析研究较少,且现有分析方法多依赖传统的墨迹步态足印法,重复性差,测量和分析易受主观性干扰,分析参数少[16,19-20]。Ching等[15]通过反复机械扭伤法构建小鼠踝关节不稳模型,并利用高速相机采集步长、步宽、站立时间、摇摆时间等步态参数进行分析。但该模型存在明显局限性:造模方法的可重复性较差,导致无法有效评估韧带损伤与步态障碍之间的关联;同时,由于小鼠胫腓骨解剖结构细小,反复机械扭伤操作可能显著增加骨折风险,因此该模型在相关研究中的实际应用较为有限。
本文通过离断小鼠距腓前韧带与跟腓韧带构建慢性踝关节不稳模型,借助Treadscan步态分析系统对模型小鼠的步态行为特征进行系统探究,旨在为慢性踝关节不稳的发病机制研究、候选药物筛选及疗效评价提供实验参考依据。
30只6周龄C57BL/6J雄性小鼠购于苏州JOINN实验室(SCXK(Su)2018-0006)。将实验动物(6只小鼠/笼)置于12 h光/暗循环、温度(22±2)℃、湿度(55±10)%的环境中。小鼠可以自由获得食物和水,并每天检查小鼠的健康状况。小鼠术前饲养2周以适应环境。本实验方案已取得苏州大学动物伦理委员会许可(296-2023)。
所有小鼠饲养至8周龄,随机分为对照组和损伤组,每组小鼠各15只,所有小鼠均使用舒泰肌肉注射麻醉(维克公司,法国),麻醉剂量为1~1.5 mg/20 g,充分麻醉后转移至显微操作台。对于损伤组小鼠,在左后肢足踝外侧皮肤做一个斜向下约7 mm纵行切口,在观察到连接于距骨体外侧与腓骨下缘之间的距腓前韧带,手术刀将其切断;之后用显微镊分开腓骨长短肌腱和趾长伸肌腱,暴露出距下关节上方与腓骨相连的跟腓韧带,切断后缝合皮肤[18]。对照组小鼠在左下肢踝关节相应的皮肤位置用手术刀划开皮肤后缝合,不打开踝关节囊。建模成功后,将小鼠转移至饲养房进一步饲养12周,进行步态测量。
采用Treadscan步态分析仪(Clever System公司,美国)对小鼠步态进行录像分析[21-22]。该系统包括一段透明的压力感应履带跑道,跑道下连接摄像机记录小鼠四肢运动情况。检测时小鼠可自由穿行在预设长度的透明跑道上,跑道上安装了一个17 cm×5 cm可调节隔间,确保小鼠始终保持在相机的视野中,跑道下连接100帧/s高速摄像机,实时采集小鼠四肢运动情况。每只小鼠以8 cm/s速度行走10 s,捕获1 000帧画面。高速摄像机将拍摄的图像信息按指令传送到与其连接的带有图像切割分析的计算机上,使用软件内置的脚趾展开工具手动识别每个后肢的脚趾展开,脚趾和脚底在初始接触时被识别出来,以减少变异性(见图1)。
实验数据收集完成后,利用GraphPad prism 9.5.0软件进行统计学分析。首先检验每组数据的分布正态性和方差齐性,对于符合正态分布且方差齐性的数据,使用非配对t检验分析;反之,则改用非参数检验进行分析[22]
Treadscan系统将小鼠步态中每一步分为站立期和摆动期,其中站立期又细分为制动期和推进期。制动期指足部从接触跑道到正常站立的时间,推进期指从足部正常站立到足部抬起离开跑道的时间,站立时间是站立期足部与跑道接触时间的总和。摆动时间是摆动期足部在空中的时间,即从离开跑道到再一次接触跑道的时间。跨步时间是两个连续的站立期之间的时间,即站立时间与摆动时间总和。结果显示,与对照组相比,损伤组平均站立时间以及站立时间百分比分别上升28.43%(P<0.05)和23.07%(P<0.05),而平均摇摆时间和摆动时间百分比分别缩短50.63%(P<0.001)和19.75%(P<0.01)。此外,与对照组相比,损伤组平均制动时间和平均跨步时间也分别缩短18.37%(P<0.01)和37.86%(P<0.001),见图2
步长、步宽和步频也是步态常用的分析参数。与对照组相比,损伤组步长、前步宽和后步宽分别缩短36.96%、13.66%和8.10%。此外,损伤组运动总速度和瞬时速度分别下降8.05%和11.12%,步频反而上升51.41%(见图3)。
平均足印面积是足部在整个站立期所印记的面积,最大站立足印面积为在所有步幅中的足印最大值。与对照组相比,损伤组平均足印面积和平均最大站立面积分别下降8.8%和13.24%,足压仅下降3%(见图3)。
足底压力分布为当小鼠的足掌踩到跑道上时足底各部分的压力分布定量数据,通过软件计算每只足掌的头端/尾端和内侧/外侧方向最大长度的中点来实现[23-24]。其测量原理来自根据算法分析足底压力不同区域的颜色变化。算法先算出整个足底的标准压力,再将整个足底分为4个象限:前左、前右、后左、后右,并分别测量出4个象限的压力与标准压力的差,从而得出各部分的压力分布。本文所分析的足底压力仅涉及对照组与手术损伤组的左后肢足掌。通过对足底压力分布的进一步分析发现,损伤组足底压力分布不均匀,后足足底压力下降更为明显,尤其是右后象限下降13.92%(见图5)。
踝关节不稳小鼠模型具有成本低廉、病程快和表型一致等优势,在药物开发、病情评估和新疗法临床前研究等领域具有重要作用。研究小鼠慢性踝关节不稳的步态行为特点,可以更好评估踝关节不稳患者的治疗和康复效果[25]
本文所使用的步态分析参数大致可分为空间和时间步态参数。空间步态参数描述了足迹模式的几何特征,而时间步态参数描述了脚着地和脚趾离地事件的时间和同步性。空间变量中的步长、步宽、足印面积和足压,时间变量中的站立时间、摆动时间和步幅时间,与踝关节不稳的关系最为密切,在既往研究中使用频率也最高[26-28]
本文发现,损伤组小鼠在步行周期中表现出站立时间延长,而摇摆时间、制动时间和跨步时间均缩短。本文认为,外侧韧带离断导致的踝关节不稳和软骨磨损引发的疼痛刺激,使小鼠采取更为谨慎的步态补偿模式。通过延长站立时间增强对身体姿态的控制,从而提高整体稳定性;选择更小的步伐幅度,以减轻踝关节负荷并降低不适感;同时通过更快的停顿与移动节奏减少对不稳踝关节的依赖,避免意外摔倒或进一步损伤。既往研究发现,无论是人类还是小鼠,在发生踝关节扭伤后均会观察到活动量减少,平衡性下降和本体感知能力降低的现象[29-30]。因此,损伤后患者会以更慢,更稳定的步行策略代偿损伤侧关节不稳定[31-32],这与本文结果比较一致。此外,Treadscan步态分析系统所采用的皮带式跑道会迫使小鼠进行被动运动,同时其绿色的运动腔室也有可能对小鼠的运动行为产生影响,因为小鼠存在红绿色盲特性,会将绿色表面感知为深色环境,这可能导致小鼠在运动过程中表现得更为谨慎[33]
Turner等[34]采用传统步态足印法对踝关节外侧韧带离断小鼠模型的步态进行测量,发现其步长缩短、步宽减小,这与本文的研究结果一致。本文认为,小鼠在踝关节不稳状态下可能会自发采取更小的步长与步宽以降低摔倒风险。这种缩减的步伐模式有助于提高身体重心的稳定性,减少运动过程中的侧向摇摆,进而降低摔倒概率[35];此外,步宽缩小可能也是为了增强对地面的接触控制能力。运动总速度及瞬时速度的下降,可能源于小鼠在运动中选择更小、更稳定的步伐策略,从而导致整体运动速度降低。与此同时,步频的显著升高表明,小鼠通过增加步伐频率来补偿步幅的缩减,以维持一定的移动能力。这种代偿性运动策略可帮助小鼠在踝关节不稳定的情况下仍能保持基本的运动功能。
足印面积及最大站立面积的下降,可能与小鼠缩减步幅及延长站立时间的行为特征相关。在踝关节不稳状态下,小鼠可能通过减小足底与地面的接触面积以增强对地面的控制能力;此外,接触面积的缩减也可能是小鼠为避免对不稳定踝关节施加过度压力而采取的保护性调节。足底压力分布的不均匀性提示,小鼠在站立与行走过程中对后足的压力施加显著减少[36-37],推测这与小鼠运动时为保护不稳定踝关节而主动调整重心及压力分布模式有关。尤其在后足的右后象限,压力下降表现得更为明显,这一现象可能与小鼠的运动模式改变及姿势适应性调整直接相关,类似的代偿性运动模式在踝关节不稳患者的步态中亦有观察到[4,38-39]
慢性踝关节不稳模型小鼠在步态参数上呈现出一系列适应性改变,这些变化的核心目的在于提升运动稳定性并降低二次损伤风险。具体而言,小鼠通过延长站立时间、缩减步幅长度、调整步伐频率以及优化足底压力分布模式等多种方式,主动应对踝关节的不稳定状态。此类生物力学与运动动力学层面的适应性调节机制,为深入理解慢性踝关节不稳对运动功能的影响,以及探索潜在的康复干预策略提供了重要的实验依据和理论基础。
本研究存在以下局限性:①Treadscan步态分析仪通过跑步机使小鼠处于被动运动状态,其采集的步态数据与小鼠主动运动时的步态特征之间可能存在差异;目前尚未与传统步态分析方法及自由步态分析系统开展进一步对比研究,因此结果的普适性有待验证。②通过离断踝关节距腓前韧带与跟腓韧带构建的小鼠踝关节不稳模型,其损伤程度较非侵入性踝关节损伤更为严重,可能伴随更显著的神经肌肉继发性损伤;未来研究需纳入轻度损伤模型(如单独离断距腓前韧带或跟腓韧带),以探讨不同损伤程度对步态特征的影响。
本文基于Treadscan步态分析仪发现,慢性踝关节不稳小鼠在运动过程中会采取更加保守的步行模式,通过减少运动量和降低运动幅度以提高步行的协调性和稳定性。研究结果可为慢性踝关节不稳的机制研究以及药物的筛选和评估提供参考。
