Objective

To explore the impact of vision impairment (VI) on the gait of hemiplegic patients, assess their walking ability and fall risks, and provide a basis for developing effective rehabilitation strategies.

Methods

Thirty hemiplegic patients were enrolled and stratified by the severity of visual acuity impairment into three groups (unimpaired, mildly impaired, and severely impaired). The gait data of patients under uncorrected vision were collected using the Qualisys motion capture system and the Kistler three-dimensional force platform, and the balance ability of patients was assessed simultaneously. Subsequently, the gait and assessment data were statistically analyzed to compare inter-group differences.

Results

Compared with the visually unimpaired group, significant differences in step length, symmetry, and walking speed were observed in hemiplegic patients of the mild visual impairment group and severe visual impairment group. As VI increased, gait abnormalities became more pronounced, with a longer double-limb support phase, a longer swing phase of the affected limb, and a shorter single-limb support phase of the affected limb in the gait cycle. Compared with the visually unimpaired group, significant differences in center of pressure (COP) and COP symmetry were found between the mild visual impairment group and severe visual impairment group, with gait abnormalities intensifying. The Berg balance scale (BBS) scores showed that there was a significant difference between the visually unimpaired group and severe visual impairment group, indicating that the group with visual impairment had poorer balance ability.

Conclusions

VI has a significant negative impact on the gait and walking ability of hemiplegic patients. This study emphasizes the importance of focusing on the impact of VI in the rehabilitation of hemiplegic patients, with regular vision assessments and personalized interventions being conducted, which are of great significance in enhancing patients' walking quality.

目的

探讨视力障碍(vision impairment,VI)对偏瘫患者步态的影响,评估其行走能力和跌倒风险,并为制定有效的康复策略提供依据。

方法

纳入30例偏瘫患者,依据视力受损程度分为视力未受损组、轻度视力障碍组和严重视力障碍组。使用Qualisys动作捕捉系统和Kistler三维测力台收集患者未经屈光矫正条件下的步态数据,同时对患者平衡能力进行评估。而后对步态和评估数据进行统计分析,以比较组间差异。

结果

与视力未受损组相比,轻度视力障碍组、严重视力障碍组偏瘫患者的步长、对称性、步速间均存在显著差异。随着VI加重,患者步态异常更为明显,步态周期中双脚支撑相、患肢摆动相占比更长,患肢单支撑相占比更短。与视力未受损组相比,轻度视力障碍组、严重视力障碍组偏瘫患者足底压力(center of pressure,COP)、COP对称性间存在显著性差异,步态异常加剧;伯格平衡量表(Berg balance scale,BBS)评分显示,视力未受损组与严重视力障碍组间存在显著性差异,提示视力受损组平衡能力更差。

结论

VI对偏瘫患者步态和行走能力产生了显著的负面影响。本研究强调在偏瘫患者康复中应重点关注VI的影响,定期进行视力评估及个性化干预,这对于提升患者步行质量具有重大意义。

纳入30例偏瘫患者,依据视力受损程度分为视力未受损组、轻度视力障碍组和严重视力障碍组。使用Qualisys动作捕捉系统和Kistler三维测力台收集患者未经屈光矫正条件下的步态数据,同时对患者平衡能力进行评估。而后对步态和评估数据进行统计分析,以比较组间差异。

结果

与视力未受损组相比,轻度视力障碍组、严重视力障碍组偏瘫患者的步长、对称性、步速间均存在显著差异。随着VI加重,患者步态异常更为明显,步态周期中双脚支撑相、患肢摆动相占比更长,患肢单支撑相占比更短。与视力未受损组相比,轻度视力障碍组、严重视力障碍组偏瘫患者足底压力(center of pressure,COP)、COP对称性间存在显著性差异,步态异常加剧;伯格平衡量表(Berg balance scale,BBS)评分显示,视力未受损组与严重视力障碍组间存在显著性差异,提示视力受损组平衡能力更差。

结论

VI对偏瘫患者步态和行走能力产生了显著的负面影响。本研究强调在偏瘫患者康复中应重点关注VI的影响,定期进行视力评估及个性化干预,这对于提升患者步行质量具有重大意义。

Objective

To explore the impact of vision impairment (VI) on the gait of hemiplegic patients, assess their walking ability and fall risks, and provide a basis for developing effective rehabilitation strategies.

Methods

Thirty hemiplegic patients were enrolled and stratified by the severity of visual acuity impairment into three groups (unimpaired, mildly impaired, and severely impaired). The gait data of patients under uncorrected vision were collected using the Qualisys motion capture system and the Kistler three-dimensional force platform, and the balance ability of patients was assessed simultaneously. Subsequently, the gait and assessment data were statistically analyzed to compare inter-group differences.

Results

Compared with the visually unimpaired group, significant differences in step length, symmetry, and walking speed were observed in hemiplegic patients of the mild visual impairment group and severe visual impairment group. As VI increased, gait abnormalities became more pronounced, with a longer double-limb support phase, a longer swing phase of the affected limb, and a shorter single-limb support phase of the affected limb in the gait cycle. Compared with the visually unimpaired group, significant differences in center of pressure (COP) and COP symmetry were found between the mild visual impairment group and severe visual impairment group, with gait abnormalities intensifying. The Berg balance scale (BBS) scores showed that there was a significant difference between the visually unimpaired group and severe visual impairment group, indicating that the group with visual impairment had poorer balance ability.

Conclusions

VI has a significant negative impact on the gait and walking ability of hemiplegic patients. This study emphasizes the importance of focusing on the impact of VI in the rehabilitation of hemiplegic patients, with regular vision assessments and personalized interventions being conducted, which are of great significance in enhancing patients' walking quality.

脑卒中作为全球重大公共卫生挑战,因其高发病率、高复发率、高致残率、高死亡率和高经济负担而广受关注[1]。在中国,2010~2019年缺血性脑卒中的患病率从1 100/10万增长至1 256/10万[2]。当中风发作时,大脑暂时缺氧,会增加患者患视力障碍(vision impairment,VI)的几率,具体表现为眼睛运动功能和或视力质量的变化[3]。当前约60%脑卒中患者会出现某种形式的视力障碍。从全球视角来看,2020年因屈光不正未矫正导致的失明人数高达370万人[4]。深入研究发现,伴有视力障碍的偏瘫患者在未来1年间跌倒风险是视力正常患者的3.8倍,这无疑加剧了患者行走困境与跌倒隐患[5-7]。此外,一项多中心前瞻性流行病学研究表明,超过50%脑卒中病人的视觉症状被忽视[8]。鉴于多数偏瘫患者属于老年群体,同时跌倒对该群体的伤害极大,应对偏瘫患者视力障碍问题予以特别关注。
尽管先前研究已对偏瘫患者的步态特征展开探讨,但针对视力障碍引发的步态问题研究却相对匮乏。尤其是视力障碍如何重新构建偏瘫患者的步态模式,目前尚未得到充分阐明,这种研究空白直接限制了偏瘫患者个性化康复策略的制定和优化。鉴于此,本文采用Qualisys动作捕捉系统和Kistler三维测力台设备,结合定量视力评估,揭示偏瘫患者步态运动学和动力学参数上的特征。同时,运用伯格平衡量表(Berg balance scale,BBS)评估患者的动态平衡能力,以期为解决此问题提供更完善的数据支持。本文旨在为偏瘫患者的临床康复提供更为精准的指导,力求有效降低跌倒事件的发生频率,提高患者行走安全性和独立生活能力,这对改善偏瘫患者的长期预后和减轻社会、家庭负担具有重要意义。
从中国人民解放军联勤保障部队天津康复疗养中心招募30名偏瘫患者,按照视力障碍程度分为每组10人的等量小组,即视力未受损组、轻度视力障碍组和严重视力障碍组。纳入标准:认知功能正常、Brunnstrom 4期患者、独立行走超过10 m、理解并同意参与实验。排除标准:存在高级脑功能障碍、无法独立行走、有严重的心脏疾病或不稳定的心血管状况。所有参与者均签署了书面知情同意书,且本研究方案已获得天津体育学院伦理委员会的批准(TJUS2024-023)。受试者基本信息及视力测试结果详见表1
采用标准对数视力表对偏瘫患者裸眼视力进行测量,测量范围为4.0~5.3(使用5级记录法),检测≥5.0为视力正常,4.5~5.0为轻度视力障碍,≤4.5为严重视力障碍,记录患者能够识别距视力表5 m特定位置所看到的最小视标对应的数值[9-10]。受试者测试时环境条件保持一致,确保评估的准确性。
在天津体育学院生物力学实验室使用Qualisys动作捕捉系统(采样频率300 Hz,Qualisys公司,瑞典)和Kistler三维测力台(采样频率1 kHz,Kistler公司,瑞典)设备对患者裸眼条件下步态数据进行采集。Qualisys系统通过8个高速摄像机捕捉标记点的三维坐标,受试者统一穿着标准服装和鞋子,同时在受试者骨性标志点粘贴26个反光球,标记点位于髂前上棘、髂后上棘、大转子外侧近端1/3处、股骨内/外侧髁、腓骨头、胫骨粗隆、内/外踝、第一跖趾关节、第二跖骨头表面、第五跖趾关节及足跟处,来获取步长、步速、步态周期指标,受试者反光球具体粘贴位置见图1。Kistler测力台用于采集患者足底压力(center of pressure,COP)和COP对称性的动力学数据。所有步态数据收集均遵循统一的标准操作程序,每位患者重复3次行走测试,每次测试患者步行至少8 m,确保数据的准确性和可重复性。
为确保测量条件的一致性,所有受试者均在平坦且硬质的地面上进行行走测试。除研究采集的运动学和动力学数据外,使用BBS量表对患者的平衡能力进行评估。BBS量表由14项任务组成,评分范围为0~4分,累计最高分为56分,得分越高反映患者平衡能力越强。在本研究中,BBS评估与步态数据采集在同一时段内进行。所有评估均由1名物理治疗师执行,确保评估过程一致性和结果准确性。
为控制可能的顺序效应,将测试、评估顺序随机化,并对每位患者的基线步态参数进行了调整。通过时间序列分析方法评估步态的时序变化,保证研究结果的准确性。
采集的数据通过MATLAB 2023a的信号处理工具箱进行预处理操作,预处理流程涵盖数据同步、使用Butterworth低通滤波器消除高频噪声及低频漂移、数据清洗以排除异常值,轨迹平滑以减少随机波动。基于先前研究参考,使用截止频率为6 Hz的Butterworth低通滤波器,保证数据的准确性和可比性[11]。在数据清洗阶段,通过统计标准差和箱线图方法系统性地识别并排除了异常值,确保数据集不受异常数据点影响。为进一步降低受试者间生理差异的影响,对所有数据均进行了归一化处理。
采用SPSS 27.0软件对偏瘫患者的步态及相关评估数据进行统计分析,结果以(平均值±标准差)表示。采用描述性统计方法概括数据的基本特征,组间比较采用单因素方差分析(ANOVA),以评估3组在步态参数与平衡测试得分方面的差异。为进一步明确组间差异,事后多重比较采用LSD法,所有分析均计算95%置信区间,检验水准设定为α=0.05。
视力测试结果显示,视力未受损组(5.04±0.06)与轻度视力障碍组(4.74±0.22)间存在极为显著差异性(P<0.001),视力未受损组与严重视力障碍组(4.32±0.18)患者视力存在极为显著差异性(P=0.000),轻度视力障碍组与严重视力障碍组患者视力存在极为显著性差异(P=0.000)。
步长结果显示,在患者患侧,视力未受损组与严重视力障碍组间存在显著差异性(P=0.035)。步长对称性结果显示,视力未受损组与轻度视力障碍组间存在显著差异性(P=0.014)、与严重视力障碍组间存在极为显著性差异(P<0.001),同时轻度视力障碍组与严重视力障碍组间存在显著差异性(P=0.029)。步速结果显示,视力未受损组与严重视力障碍组间存在极为显著差异性(P=0.006),见图2
时空参数结果显示,各视力条件下患者步态周期时长差异无统计学意义(P>0.05);步态周期相对值占比结果显示,严重视力障碍组相较于视力未受损组患者双脚支撑相、患肢摆动相占比更长,患肢单支撑相占比更短(见图3)。
COP结果显示,在患者健侧,视力未受损组与严重视力障碍组间存在极为显著差异(P=0.002),轻度视力障碍组与严重视力障碍组间存在显著差异(P=0.044);在患者患侧,视力未受损组与轻度视力障碍组间存在极为显著差异(P=0.004),视力未受损组与严重视力障碍组间存在极为显著差异(P<0.001)。COP对称性结果显示,视力未受损组与轻度视力障碍组间患者存在极为显著性差异(P=0.000),视力未受损组与严重视力障碍组患者间存在极为显著性差异(P=0.000),而轻度视力障碍组与严重视力障碍组间不存在显著差异性(P=0.071),见图4
BBS评分结果显示,视力未受损组与严重视力障碍组间存在显著差异性(P=0.035),视力未受损组与轻度视力障碍组间差异无统计学意义(P=0.151),见图5
本文采用Qualisys动作捕捉系统、Kistler三维测力台及平衡量表,记录偏瘫患者步态运动学、动力学及平衡指标。通过统计分析,研究结果基本支持视力未受损组在关键步态特征方面与视力障碍组患者间存在差异性,同时视力受损患者平衡稳定性更差的假设。尽管在整个步态周期中各组间未表现出显著差异性,但在周期内时相占比上出现了变化,这些改变会直接影响视力受损患者的步长及平衡控制能力,增加跌倒风险。
步长和步速测量结果显示,与视力未受损组相比,严重视力障碍组步长显著缩短、步速显著减慢,表明视力障碍对患者行走能力产生了影响。研究显示,患者患侧步长较健侧普遍更长,推测是患侧肢体运动功能受到损害所致。患者在行走过程中会下意识地延长患肢摆动时长,以补偿肢体功能的不足,从而维持步态的稳定性和行走的流畅性[12]。随着视力障碍严重程度增加,患者患侧下肢支撑能力受到显著影响,导致其在步态周期中患肢支撑相占比更短,这种改变不可避免地影响患者健侧下肢的摆动时相,导致健侧步长相对缩短。这是患者为了适应患肢支撑能力下降而做出的自然步态调整,目的是保持行走过程中的稳定性。Nedergård等[12]研究认为,步长改变是患者为了补偿受影响肢体功能损失而采取的适应性行为。Son等[13]对中风患者实施自我管理的眼保健操干预措施,发现该措施能显著改善患者健侧与患侧肢体的步长。此外,由于周期时长、双脚支撑相时长占比增加而使步速降低,这与既往研究结果相吻合,即视力障碍会使患者采取更谨慎的步态策略,以减少跌倒风险的发生。有研究发现,视觉和认知功能显著影响糖尿病患者的步行速度,其中视觉影响占总效应的17.97%[14]。听力障碍、视力障碍和双重感官功能障碍均与步速减慢相关[15]。尽管患者听力和视力的减退已被证实与步速下降具有相关性,但其潜在的具体机制尚未完全阐明。因此,在偏瘫患者的康复过程中,视觉因素引发步态改变的内在机制,值得展开更为深入且全面的研究。
步长对称性进一步揭示了视力障碍对患者步态协调性的影响。本文发现,相较于视力未受损患者,视力障碍患者在步态对称性上的表现较差,这与患者依赖于其他感官信息来补偿视觉缺失信息有关。不对称的步态模式易造成双侧关节受力不均,不仅会增加患者跌倒风险,还会加速关节的退行性变化[16]。步态的时空对称性对于促进肢体间的协调、提升行走效率以及降低跌倒风险至关重要。在改善步态对称性方面,分带式跑步机训练相较于其他方法更具优势,尤其在步长对称性改善上表现卓越,可使患者步长对称性实现提升27.3%,且该效果未以牺牲步行速度或步长为代价[17]。因此,分带式跑步机训练可被视为一种有效的康复手段,用以改善偏瘫患者的步态对称性。值得注意的是,偏瘫患者“健侧肢体”并非真正健康,后期康复治疗需结合与患侧肢体的对比综合考虑[18]
时间对称性比率是评估患者步态协调性的敏感指标,可以反映患者行走时双下肢的协调性[12]。本文发现,与视力正常患者相比,视力障碍患者在单个步态周期中双脚支撑相及患肢摆动相的占比更长,而患肢单支撑相的占比则更短。这一结果表明,视力障碍对步态周期内各时相占比产生了显著影响。支撑相与摆动相的差异会对偏瘫患者的步态协调性产生不利影响,提示在偏瘫患者的康复干预过程中,需重点关注步态对称性的评估与针对性干预措施的实施。
本文发现,与视力未受损组相比,视力障碍组患者表现出更为显著COP偏移现象,且这一偏移在患侧尤为明显;随着视力受损程度的加重,COP的波动幅度也随之增大,提示患者在动态平衡维持过程中面临的困难程度更高。该结果与既往研究结论一致,即视力受损会迫使患者依赖其他感官信息代偿视觉输入的缺失,进而对步态稳定性产生不利影响[19]
COP对称性进一步揭示了视力受损对患者步态协调性的影响。本文发现,视力障碍组患者COP对称性更差,加剧了对健侧肢体的依赖,导致双下肢关节受力不均,进而增加了跌倒风险。
平衡能力要求个体整合并处理来自不同感官的信息,同时准确地识别这些信号并据此调整对各感官信息的依赖程度。其中,视觉信息对维持姿势稳定性和步态调节至关重要,它提供了身体对环境位置的反馈,帮助个体进行空间定位和深度感知。视力受损会减少视觉线索,迫使患者更多地依赖于其他感官系统来补偿视觉信息的缺失。这种补偿会造成步态调整不精确,增加步态变异性及平衡控制的不稳定性[2,7-8,20]。本文显示,与视力未受损组相比,严重视力障碍组BBS评分更低。虽然视力未受损组与轻度视力障碍组BBS评分并未体现出统计学差异,但评分也较视力未受损组差。该结果与先前研究中感觉障碍与平衡能力下降相关且感觉处理功能受损与增加的跌倒风险极为密切的观点相吻合[20]
研究显示,视力障碍对慢性疾病患者的功能预后有显著的负面影响[21]。忽视视力障碍会导致患者在康复过程中代偿行为增加、恢复期延长、日常生活质量降低,对后续康复进程产生不利影响。研究表明,在脑卒中患者姿势控制中睁眼/稳定支撑面和睁眼/不稳定支撑面条件下,视觉的贡献率分别为2.656%和31.006%,提示视觉在姿势控制中起到了重要作用[22]。有研究认为,通过简单的措施例如早期正式的视觉评估、管理和适应性策略可以改善视力障碍[5]。近年来,通过机器人疗法可明显改善患者的视觉建构,进而提升患者的视觉空间处理能力[23]。便携可穿戴辅助设备利用振动反馈机制向视力受损患者传达周边物体的相对位置与距离信息,可以辅助用户通过触觉感知环境,并执行物体识别与导航等活动[24]。本文强调对偏瘫患者早期及定期评估视觉功能的重要性,建议制定个性化康复方案并及时干预,以提升患者行走能力和生活质量。鉴于视力障碍患者之间的差异性,实施患者分层和个性化治疗策略至关重要[25]。此种举措不仅有助于减少跌倒事件的发生,还可提升患者的独立性和自信心。
本研究存在以下局限性:①样本量限制。研究样本量相对较小,限制了统计分析的功效,并可能影响结果的外推性。②横断面研究设计。采用横断面设计,研究只观察到某一特定时间段内患者的步态特征,无法充分揭示步态特征随时间的长期演变趋势。③视力障碍评估的局限性。尽管采用统一的视力测试流程,但评估还不够全面,未来研究会涉及更广泛的视觉功能评估,如视觉场、视野范围、深度感知和色彩辨识等。
基于对偏瘫患者运动学、动力学和平衡能力评估的综合研究发现,视力障碍会导致患者步态异常加剧,降低步行质量,影响平衡能力,增加跌倒风险。本研究为偏瘫患者步态分析和康复策略的制定提供了强有力的支持,具有重要的临床意义。
视力障碍对偏瘫患者步态和平衡能力存在显著影响:随视力障碍程度的增加,患者的步态异常加剧,具体表现为步态周期内时相占比及步长发生改变,步态对称性和步速降低,同时平衡能力下降,跌倒风险增加。本研究强调,在偏瘫患者康复过程中需充分考虑视力障碍的影响,建议制定个性化干预措施以增强行走安全性,进而提升患者的整体生活质量。
